1.Mesenchymal stem cells in radiation-induced lung injury treatment

Radiotherapy plays a significant role in the treatment of lung malignant tumor.However,as the most main complication of radiotherapy used to treat lung malignant tumor,radiation-induced lung injury is an important influencing factor in impeding further and higher dose radiotherapy.Previous studies show that mesenchymal stem cells (MSCs) have the characteristics of self-replication,multiple differentiation,immunoregulation and directional migration. MSCs can help to repair many kinds of acute and chronic lung injury,meanwhile stem cell transplantation stimulates production of many cytokines to attenuate acute lung injury.So stem cell therapy provides a new kind of thinking in the prevention and treatment of radiation-induced lung injury.
2.Adverse in utero environment can increase the risk of atherosclerosis in the offspring

Epidemiological and pathological evidence has demonstrated that adverse in utero environment,such as protein restriction,hypercholesterolemia,diabetes and smoking,can increase the risk of vascular disease in the offspring.In utero,the placenta and fetus are exposed to the metabolic,antioxidant and pro-inflammatory and anti-inflammatory signals from the mother and likely to make specific responses,which may lead to permanent changes either in DNA methylation or chromatin modification or both,and these changes,in turn,may result in increased atherosclerosis susceptibility in adulthood.In this review,we briefly summarize the possible signals crossing the placental barrier and discuss the molecular mechanisms of epigenetic programming in the developing fetus that may lead to increased athero-susceptibility of the vessel wall.
3.Clinical study of photodynamic therapy for age-related macular degeneration

Objective To study the clinical results and safty of photodynamic therapy (PDT) after single and multi treatments of patients with subfoveal choroidal neovascularization (CNV) caused by wet age related macular degeneration (AMD). Methods From July, 2000 to July, 2001, 20 wet AMD patients (31 eyes) 47 88 years old (mean 68.2 years old) with best corrected visual acuity from FC/10 cm to 0.6 diagnosed through optic coherence tomography (OCT), fluorescein angiography (FFA) and indocyanine green angiography (ICGA) were treated with PDT. All cases were assigned to benzoporphyrin derivative mono acid (BPD) (6 mg per square meter of body surface area), administered via intravenous infusion of 30 ml over 10 minutes. Fifteen minutes after the start of the infusion, a laser light at 689 nm (Zeiss company, German) delivered 50 J/cm 2 at an intensity of 600 mW/cm 2 over 83 seconds on CNV. Visual acuity, photochrome of ocular fundus, OCT, FFA, ICGA were used to evaluate the effects of photodynamic therapy with BPD. Follow up of these patients was planned 1 2 week and every 3 month after PDT. Once the lesion area progressed, PDT was applied again. Tweenty cases (31 eyes) were followed up from 3 to 18 months (average 12 month).In 1 affected eye, PDT was applied fow 4 times, 4 eye for 2 times, and the other 26 eyes for 1 time. Results The visual acuity in 13 (41 9%) eyes was improved ( increase≥2 lines) after PDT. Stabilized (?1 line) in 17 (54 8%) eyes and decreased 2 lines (attributed to the recur of CNV ) in 1 (3 2%) eye. After PDT, the fundus haemorrhage and fluid leakage reduced. FFA and ICGA showed. cessation and obvious reduction of fluorescein leakage from CNV in all patients 2 weeks after photodynamic therapy, and retreatment decreased the leakage step by step. Fluorescein leakage from at least a portion of the CNV reappeared by 1 3 month after treatment in some cases. OCT also showed the reduction of the size of CNV, moreover, the edema of surrounding retina and choriodal and serous neural epithelial detachment recovered obviously. No side affect during and after PDT was noticed. Conclusions PDT with BPD can achieve short term effect on part or total cessation of fluorescein leakage from CNV without loss of vision or growth of classic CNV in patients with age related macular degeneration, retreatment of PDT was also effective.
4.The treatment of neodymium-ythrium aluminum garnet vitreolysis for anterior vitreous opacity after implantation of intraocular lens

ObjectiveTo observe the effect of the treatment of neodymium-yttrium aluminum garne (Nd:YAG) vitreolysis for the anterior vitreous opacity after implantation of intraocular lens.MethodsForty-nine eyes of 47 patients with the anterior vitreous opacity after implantation of intraocular lens received the slit lamp examination, optical coherence tomography (OCT), and B-scan. The anterior vitreolysis and posterior capsulotomy were performed simultaneously with Nd:YAG laser. The outcomes of visual acuitiy changes and complications were studied. ResultsIn 49 eyes, 46 had anterior vitreous opacity associated with posterior capsule opacification, and the other 3 without obvious posterior capsule opacification. In all patients, the visual acuity improved significantly without any complications after the laser procedure (t=32.50, P=0.007). After Nd:YAG laser treatmen, transparent area was found in anterior opaque vitreous in 21 eyes (42.86%) within 15 minutes, and in 47 eyes (95.92%) within 24 hours. No complication occured in or after the operation. ConclusionsIn the patients with visual deterioration after implantation of intraocular lens, the prescence of anterior vitreous opacity should be concerned. Opening the opaque anterior vitreous with Nd:YAG vitreolysis is effective for the patients with the anterior vitreous opacity after implantation of intraocular lens.

High density lipoprotein(HDL) is an antiatherogenetic lipoprotein and low level of HDL-cholesterol(HDL-C) in plasma is a risk factor for coronary heart disease.Therefore,raising the level of HDL-C by pharmaceuticals,oestrogen replacement and biologic therapy for dyslipidemia exhibit great clinical significance.
6.Primary study of photodynamic therapy for choroidal neovascularization caused by age-related macular degeneration

Purpose To evaluate short term visual acuity effects of a single photodynamic therapy(PDT) treatment with Visudyne (CIBA Vision Corp, Duluth, Ga) for choroidal neovascularization (CNV) in age related macular degeneration (AMD). Methods Definitely diagnostic AMD patients with classic CNV were treated with PDT (5 cases, 7 eyes). The data of visual acuity testing, ophthalmic examination, color photographs, optic coherence tomography, fluorescein angiograms and indocyanine green angiogram before photodynamic therapy and 1 week ,1 month after it were used to evaluate the effects of a single treatment of PDT with Visudyne. Results The visual acuity of all the treated eyes at the follow up examination at 1 month after PDT were not reduced. Distinct reduction of fluorescein leakage from CNV was noted in all patients by 1 week after PDT. Fluorescein leakage from a portion of the CNV reappeared by 1 month after treatment in 2 eyes. Conclusion PDT with Visudyne achieved short term cessation of fluorescein leakage from CNV without loss of vision or growth of classic CNV in some patients with AMD.
7.Comparative Study of Complete and Incomplete Cervical Spinal Cord Injuries Complicated with Venous Thromboembolism

Objective To study the incidence of venous thromboembolism (VTE) between complete and incomplete spinal cord injured patients. Methods 177 cases of acute cervical spinal cord injury were observed from April 2007 to July 2010. According to ASIA classification, 52 cases of grade A involved in observation group, while 125 cases of grades B~D involved in control group. Intervention included low molecular weight heparin and rehabilitation training. Incidence of VTE in two groups was compared. Results There was 11 cases complicated with VTE in the observation group (21.2%) while 6 cases in the control group (4.8%). There was a difference in incidence of VTE between two groups (P<0.05). Conclusion There is a higher incidence of VTE in complete than incomplete cervical spinal cord injury
8.Current application and prospect of three-dimensional laparoscopic radical resection of colorectal cancer

Most colorectal surgeries now are carried out as minimally invasive surgery along with the development and application of laparoscopy.However,the surgical imaging system does not meet the progressing requirement of laparoscopic surgeons.Thanks to the advent of three-dimensional (3D) laparoscopy,a more sophisticated surgical scene with realer images of anatomy is provided to surgeons.It makes identification and manipulation easier,so that to shorten the learning curve.Meanwhile,the 3D laparoscopy is not perfect.But authors believe it will finally be used broadly in colorectal surgery with the improvement of the technology.
9.Value of IVIM-DWI and DCE-MRI in the assessment of benign and malignant mesorectal lymph nodes in rectal cancer

Objective To investigate the diagnostic value of intravoxel incoherent motion diffusion weighted imaging (IVIM-DWI) and dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI)in differentiating non-metastatic from metastatic mesorectal lymph nodes in rectal cancer.Methods IVIM-DWI and DCE-MRI were performed preoperatively in 38 patients with histologically proven rectal carcinoma.The short axis diameter,short-to-long axis diameter ratio,four IVIM-based parameters (ADC,D,D? and f) and six DEC-MRI semi-quantitative parameters (Slope,Maxslope,CER,Washout,TTP,iAUC90 and iAUC180 )were compared between the metastatic (n=28)and non-metastatic (n=27)lymph nodes.Results There were significant statistical significances between the metastatic and non-metastatic lymph nodes in mean short axis diameter (8.87 mm±2.829 mm vs 6.83 mm±1.075 mm),D value[(0.824±0.1 13)× 10 -3 mm2/s vs (1.033±0.244)× 10 -3 mm2/s],CER(1.588 ±0.664 vs 1.054 ±0.41 9),iAUC90 (22.89 ± 9.83 vs 13.59 ± 5.34)and iAUC1 80 (49.38±20.1 9 vs 30.31 ± 1 1.67)(P ≤0.001).The short-to-long axis diameter ratio,ADC,D? ,f,Slope,Maxslope,Washout and TTP values did not show significant differences between the two groups(P >0.05).The respectively optimal cut-off value (area under the curve,sensitivity and specificity)for distinguishing metastatic from non-metastatic lymph nodes were as follows:short axis diameter=7.1 mm(0.744,64.2%,85.1%),D=0.906×10 -3 mm2/s (0.821,81.5%,75.0%),CER=1.05(0.749,85.7%,62.9%), iAUC90 =13.42(0.780,85.7%,62.9%),iAUC180 =49.65 (0.770, 50.0%,100%)respecyively.Conclusion Both IVIM-DWI and DCE-MRI are useful for differentiating non-metastatic from metastatic mesorectal lymph nodes in rectal cancer.
10.Rehabilitation on Cervical Fracture-dislocation Following Spinal Cord Injury

Objective To investigate the effect of rehabilitation on cervical fracture-dislocation following spinal cord injury.Methods126 patients with cervical fracture-dislocation following spinal cord injury from August, 2002 to March, 2009 in our hospital had undergone cervical anterior surgical treatment. 22 cases were picked up as control group, who missed rehabilitation after surgery. 22 cases were chosen randomly from the rest of 104 cases as rehabilitation group, who had undergone rehabilitation. Prognosis was evaluated by Japanese Orthopaedic Association Scores score(JOA).ResultsFollow-up time was 8-32 months,mean time 18 months. The limb function of rehabilitation group improved significantly. JOA score and quality of life were much higher in rehabilitation group than in control group.ConclusionRehabilitation is very beneficial to improve the quality of life and prognosis, and reduce complications of patients with cervical fracture-dislocation following spinal cord injury at the early stage.
