1.Somatic effects of interstitial brachytherapy with permanent seed implants

Objective To explore the effects of interstitial brachytherapy with permanent seed implants on peripheral blood cells, liver functions, renal functions, T cells subgroups and the activity of natural killer cells (NK). Methods Ultrsound-guided or intraoperative implants of radioactive seed 125I were used in 25 patients with pathologically diagnosed and ineradicable malignant tumors. Clinical data of peripheral blood cells, liver functions, renal functions, T cells subgroups and the activity of NK cells prior and after the treatment were compared. Results The white blood cells, hemoglobin and platelet prior the treatment were (6 3?2 2)?10 9/L, (122 7?17 3) g/L and (219 9?117 5)?10 9/L, respectively, while at the 1st and 3rd months after the treatment, these were (6 4?1 8)?10 9/L, (114 5?30 9)g/L, (191 5?59 6)?10 9/L, respectively and (6 1?1 2)?10 9/L, (116 3?16 0) g/L, (191 7?108 2)?10 9/L, respectively, without statistically significant differences ( F =1 893, 0 616, 0 596; P =0 072, 0 779, 0 762). In addition, no significant differences were found in values of ALT, AST, BUN, Cr, CK and CK-MB at different times ( F =1 861, 0 551, 1 740, 1 934, 0 412, 0 052; P =0 118, 0 737, 0 142, 0 105, 0 837, 0 998). The values of TB and DB in patients with pancreatic carcinoma obviously decreased within 1 month after the treatment. Interstitial brachytherapy had little effects on T-cell immunity (CD3, CD4 and CD8) and the activity of NK cells ( F =0 287, 0 520, 0 497, 1 101; P =0 752, 0 598, 0 612, 0 342). Conclusions Interstitial brachytherapy with permanent seed implants has no effects on vital organs and immunologic functions, being a safe and minimally invasive technique for cancer.
2.Valsartan combined amlodipine or hydrochlorothiazide improving blood pressure variability and quality of life in aged patients with hypertension

Objective:To compare influence of valsartan (Val) combined amlodipine (Am) or hydrochlorothiazide (Hyd) on blood pressure variability and quality of life in aged patients with hypertension .Methods:A total of 127 aged patients with hypertension stage 2 and 3 were randomly divided into Val + Am group (n=64) and Val + Hyd group (n=63) .Circadian rhythm and variability of blood pressure and quality of life were observed in both groups before and after treatment .Results:(1) After treatment ,the 24hSBP ,DBP ,SBPV , DBPV and daytime SBP , DBP ,SBPV ,DBPV ,and nighttime ,morning surge of SBP and DBP all significantly reduced in both groups;(2) Compared with Val+ Hyd group ,there were significant reductions in 24hSBP [(120.6 ± 10.2) mmHg vs . (110.9 ± 11.3) mmHg] ,daytime SBP [(120.6 ± 11.3) mmHg vs .(111.6 ± 11.37) mmHg] ,nighttime SBP [(118.5 ± 11.6) mmHg vs .(108.6 ± 11.9) mmHg] ,morning surge of SBP [ (26.2 ± 13.7) mmHg vs .(23.0 ± 10.4) mmHg] (P<0.05 or <0.01);24hSBPV [ (10.7 ± 2.2) mmHg vs .(8.2 ± 2.0) mmHg] ,daytime SBPV [ (10.4 ± 1.9) mmHg vs .(8.1 ± 2.1) mmHg] (P<0.01 all);significant rise in improved percentages of physical health ,mental health and total function in quality of life (P<0.05 or <0.01) in Val + Am group .Conclusion:Valsartan combined am-lodipine or hydrochlorothiazide can both effectively improve blood pressure variability and quality of life in aged pa-tients with hypertension ,and the effect of the former is even better .
3.The effect of long term low dose roxithromycin on the chest CT imaging and clinical symptoms change of bronchiectasis stable patients

Objective To explore the clinical effect and lung CT change of long-term used of low-dose roxithromycin in treatment for bronchial expansion patients in stable phase. Methods 94 cases collected in the Department of Respiration, The Second Hospital Affiliated to Suzhou University from February 2011 to December 2012 were diagnosed as bronchiectasis, 34 cases in control group were given oral treatment for ambroxol 30 mg, three times one day, 60 cases in treatment group were added roxithromycin 75 mg on basis of control group, two times one day. Patients in two groups were both treated for 6 months. The therapeutic effect and the score of life quality and dyspnea scores in two groups were observed, and the changes of CT data were compared before and after treatment. Results After treatment, the life quality score and dyspnea score of two groups were all improved, but the treatment group was signiifcantly higher than the control group (P<0.05). The effective rate in treatment group was 86.67%, which was signiifcantly higher than 70.59%in the control group (P<0.05). After treatment, chest CT imaging score of patients in treatment group were improved, signiifcantly better than that in the control group (P<0.05). Conclusion Long-term low dose administration of roxithromycin can control and stable bronchiectasis symptoms, and improve signs and symptoms .
4.Clinical study on the treatment of ureteral calculi with extracorporeal shock wave lithotripsy combined with urinary stone decoction and ear acupoint pressure

Objective To evaluate the therapeutic effect of ESWL combined with urinary stone decoction and ear acupoint pressure for the patients with renal and ureteral calculi.Methods A total of 110 patients with renal and ureteral calculi were randomly divided into the Chinese medicine (CM) and conventional groups, 55 in each group. Both groups were given ESWL treatment as basic treatment. Chinese medicine group were given the urinary stone decoction and auricular acupressure, and the conventional group used stone granules. Both groups were treated for 2 weeks. According to the photography of ultrasound, the changes of calculi, time of lithagogue, pain relief after operation and the effect rates of pain relief on abdomen and waist, of remission of haematuria, and of urinate pain. ResultsAfter treatment, the successful rates of operation in the CM group was 90.9% (50/55), and conventional group was 76.4% (42/55). There was significant difference between 2 groups (χ2=4.251,P=0.009). The time of stone removal in CM group was significantly shorter than that in the conventional group (7.3 ± 3.8 dvs. 11.2 ± 4.3 d,t=4.032,P<0.001). After 1st, 3rd, 5th, 7th days, the VAS pain scores in the CM group were significantly lower than those in the conventional group (4.1 ± 0.8vs. 5 ± 1.2,t=4.783;4.5 ± 0.9vs. 5.3 ± 1.3,t=4.492; 3.8 ± 0.8vs. 4.5 ± 0.8,t=4.503; 2.2 ± 0.8vs. 2.9 ± 0.9,t=3.691; allP<0.01). After 7th day, the rates of abdominal pain relief was 60% (33/55), pain relief was 79.1% (34/43), the remission of hematuria was 81% (17/21) in the CM group, and the control group were 40% (22/55), 59.6% (28/47), 52% (13/25). There were significant differences between 2 groups on these indices (χ2 values were 4.400, 3.982, 4.217,P values were 0.036, 0.046, 0.040).Conclusions The ESWL combined with urinary stone decoction and ear acupoint pressure could improve the symptoms of patients with kidney ureteral stones.
5.Effects of Acupuncture from Tiaokou to Chenshan and Exercise on Shoulder-hand Syndrome at Stage I after Stroke

Objective To observe the effect of acupuncture penetrated from Tiaokou (ST38) to Chenshan (BL57) combined with exer-cise on affected limbs of shoulder-hand syndrome (SHS) at stage I after stroke. Methods 40 inpatients with SHS at stage I after stroke from January, 2013 to December, 2014 were randomly divided into treatment group (n=20) and control group (n=20). The treatment group accept-ed acupuncture from Tiaokou to Chenshan on unaffected sides and exercise of affected upper limbs, and the control group accetpted routine acupuncture on Triple Shoulder Acupoints on affected sides. They were assessed with Pain Rating Index (PRI), Fugl-Meyer Assessment (FMA) and modified Barthel Index (MBI) before and 4 weeks after intervention. Results The incidence of improvement was 90% in the treatment group, and 80%in the control group (χ2=0.784, P=0.376). The scores of PRI decreased (t>5.37, P<0.001), and the score of FMA and MBI increased (t>4.66, P<0.001) significantly after intervention. The score of FMA and MBI improved more in the treatment group than in the control group (t>2.25, P<0.05). Conclusion Acupuncture from Tiaokou to Chenshan combined with exercise is more effective on SHS at stage I after stroke than conventional Triple Shoulder Acupoints.
6.MSCT is unsuitable for preoperative staging estimation of rectal carcinoma independently

Objective To evaluate the validity and reliability of multi-slice spiral CT (MSCT) in preoperative TNM staging judgment of rectal carcinoma. Methods Three hundred and one patients with rectal carcinoma were diagnosed and treated consecutively in Colorectal and Anal Surgery Department of Shanxi Province Tumor Hospital from January 2009 to December 2009. The clinical data of these patients were analyzed retrospectively. The diagnosis results were compared between the preoperative MSCT staging and the postoperative pathological staging,the ROC curve and the diagnostic concordance test were analyzed by software Medcalc 11.2. Results The sensitivity, specificity, Kappa value, area under the curve of TNM staging were 64.7% ,96. 8% ,0. 667,0. 808 for T2 staging tumors; 93.8% ,75.0% ,0.709,0.844 for T3 staging tumors;87. 8% ,98. 1% ,0. 859,0. 929 for T4 staging tumors; 72. 0% ,91.2% ,0. 619,0. 816 for N staging tumors;92. 7%, 99. 7%, 0. 925,0. 963 for M staging tumors respectively. Conclusion The diagnostic value of independent use of MSCT in estimating the infiltration degree and lymph node metastasis of rectal cancer is very poor and cannot be used in preoperative staging judgment.
7.Psychological stress reduces the plasma NO level and NO-regulating protein expression in the vascular en-dothelial cells of rats

[Abstract ] Objective The mechanism of psychological stressi-nduced damage to vascular endothelial cells (VEC) is not yet clear.This study was to investigate the impact of psychological stress on the NO level and NO-related regulatory protein expression in the plasma of rat models of psychological stress-induced VEC injury. Methods Twenty male SD rats were equally randomized into a con-trol group and a model group, the former raised together, with water regularly fed at 9:00 and 21:00 hours for 10 minutes a day, while the latter raised alone, with water regularly fed at the same hours for 10 minutes a day for the first 7 days and then with water or an empty bottle at the same hours for 10 minutes a day according to the random table.Ninety days later,all the rats were intraperitoneally injected with normal saline at 0.2 ml per 100 g body weight twice a day.Then the behaviors of the animals were observed by open field tests, the contents of cortisol (COR),adrenaline (Adr), and endothelin-1 (ET-1) in the plasma measured by ELISA, the NO level detected by the nitrate reductase method, and the expression of NO-related regulatory proteins in VECs determined by LC-ESI-MS. Results Com-pared with the control group, the model rats showed significant increases in the vertical score (7.00 ±0.70 vs 11.28 ±1.79), horizontal score (37.60 ±4.500 vs 56.25 ±1.71), and total score (44.50 ±5.50 vs 67.25 ±2.98) (P<0.01) as well as in the levels of COR ([65.28 ±2.21] vs [83.85 ±1.95] ng/mL ) and Adr ([3.85 ±0.35] vs [15.14 ±3.97] pg/mL) (P<0.01), but a remarkable de-with vacuoles in the VECs in the model group and structural integrity, ridge clarity, and tight pack in the VEC mitochondria in the con-trol. Conclusion Psychological stress induced by empty bottle stimulation can reduce the NO level and the expression of NO-related regulatory proteins in the plasma, which may cause both functional and structural damages to VECs.
8.The combination of transrectal ultrasonography and serum CEA in preoperative staging of rectal carcinoma

Objective To evaluate the diagnostic accuracy of the combination of endorectal ultrasonography and serum CEA in preoperative diagnosis of rectal wall invasion (T staging) and nodal involvement (N staging) of rectal carcinoma.Methods We retrospectively analyzed clinical records of 310 patients with rectal carcinoma who underwent endorectal ultrasonography and serum CEA evaluation in Shanxi Province Tumor hospital from January 2007 to January 2010.The positive standard of CEA is more than 5 μg/L.The endorectal ultrasonography staging with postoperative pathological staging,and calculated the overall accuracy of T staging and N staging based on TRUS alone or on TRUS combined with serum CEA level were compared.Results The difference in serum CEA level was statistically significant from T1 to T4 (P < 0.05).The accuracy rate of preoperative T staging of rectal carcinoma by TRUS alone was 71% (219/310) and was 82 % (254/310) with TRUS combined with serum CEA level,showing significant statistical difference (x2 =10.92,P < 0.01).The accuracy rate of preoperative N staging of rectal carcinoma was 69 % (211/308)with TRUS alone and was 77 % (238/308) with TRUS combined with serum CEA level,the difference of which was statistically significant (x2 =5.00,P < 0.05).Conclusion Serum CEA level increases with an increasing pathological stage of rectal cancer.The combination of TRUS and serum CEA improves the accuracy of preoperative staging of rectal cancer.
9.Diagnostic value of head-up tilt test with sublingual isosorbide dinitrate in patients with vasovagal syncope

Objective: To explore the diagnostic value of head-up tilt test with sublingual isosorbide dinitrate (HUTSI)in patients with vasovagal syncope (VVS). Methods: A total of fifty-two consecutive patients with clinically vasovagal syncope (VVS group) and thirty-eight control subjects without prior experience of syncope (normal control group) were evaluated by baseline head-up tilt table test (BHUT) , then the BHUT negative subjects underwent HUTSI test. . Results: (1) Positive BHUT test rate of VVS group and normal control group were 25% (13/52) and 0 respectively; from supine to HUT positive, there were significant decrease in heart rate [(73.1±8.5) times/min vs. (56.2±11.2) times/min] and mean arterial pressure [MAP, (81.2± 10.8) mmHg vs. (50.2±10.4) mmHg,P<0.05 both] in BHUT positive subjects. There were 21 HUTSI positive cases(53.9%)among the other 39 VVS patients, and two cases(5.3%)among the 38 subjects of normal control group. From supine to HUTSI positive, there were significant decrease in heart rate [(65.2±7.5) times/min vs. (52.9±10.5) times/min] and MAP [(78.3±10.7) mmHg vs. (48.8±11.2) mmHg, P<0.05 both] in HUTSI positive subjects. Duration from tilt started to positive reaction occurred in HUTSI positive group was significantly shorter than that of BHUT positive group [(10.8±9.3) min vs. (21.1±11.5) min,P<0.05]. All subjects can tolerate the test and only two cases in VVS group and one case in normal control group occurred headache and face red. Conclusion: Head-up tilt test with sublingual isosorbide dinitrate is a practical and easy-to- perform method with high sensitivity, specificity and few side effects for diagnosis of vasovagal syncope.

Objectives:To explore the influence of nutritional support treatment on patients with the injured pancreatitis. Methods:The twenty five cases with injured pancreatitis were divided into research group (13 cases of 1997-2001, with nutrition support) and control group (12 cases of 1992-1996,without nutritional support).
