1.Combination of catheter directed thrombolysis and artery angioplasty for the treatment of chronic long superficial femoral artery occlusion

Objective To evaluate intra-arterial catheter directed thrombolysis in combination with artery angioplasty for chronic long superficial femoral artery occlusion.Methods Clinical data were reviewed on patients of chronic long superficial femoral artery occlusion treated by endovascular therapy from September 2010 to September 2012.In 19 patients,catheter-directed thrombolysis was initially carried out,which was followed by endovascular angioplasty balloon dilatation or stent placement.The other 37 patients were treated by upfront endovascular angioplasty balloon dilatation or stent placement.Results In the thrombolysis group,the occluded length of the artery before and after thrombolysis was (16 ± 5) mm and (10 ± 6) mm(t =3.598,P < 0.05).The length and the number of stents used in thrombolysis group was lower than stent group [(10±5) cm、(1.6±0.8) vs.(12±4) cm、(2.1 ±0.7),t=2.102,P<0.05].There were no significant difference in primary patency rate assisted,primary patency rate and secondary patency rate between thrombolysis group and stent group.Conclusions For patients of chronic long superficial femoral artery occlusion,a successful catheter directed thrombolysis with artery angioplasty reduces the number of stents for angioplasty.

3.Ultrasound-guided interstitial ~(125)I seed implantation in the treatment of locally advanced pancreatic carcinoma

Objective To investigate the feasibility and efficacy of ultrasound-guided interstitial implantation of radioactive ~(125)I seed for the treatment of unresectable pancreatic carcinoma.Methods Twenty-one patients with unresectable primary pancreatic carcinoma were treated with interstitial ~(125)I seed implantation under laparotomy.The radioactive activity and the number of implanted seed were determined according to preoperative CT scanning outcomes by using the Treatment Planning System(TPS).The matched peripheral dose(MPD) was 65~110 Gy.The radioactive activity ranged 0.4~0.5 mCi per seed.A total of 10~75 ~(125)I seed were implanted with an 18-gauge seed needle and the Mick applicator under direct visualization or ultrasound guidance.Gastroenterostomy or cholangiojejunostomy were performed in 8 patients before or after the implantation.Postoperative external radiation therapy combined with chemotherapy with gemcitabine was carried out in 2 patients. Stent placement was conducted in 2 patients preoperatively and postoperatively, respectively.Results Out of 15 patients with abdominal pain,complete and partial pain relief were obtained in 7 patients and 7 patients,respectively,at 1~3 postoperative days,the response rate being 93.3%(14/15). Except for 2 cases of loss of follow-up,there were 5 cases of complete response(CR),7 cases of partial response(PR),5 cases of progressive disease(PD),and 2 cases of no change(NC).Of 19 patients,the median survival time was 5 months and the 1-year survival rate was 26.3%.Chylous leakage occurred in 1 patient and seed dislodgement to the liver occurred in 3 patients.No pancreatic fistula or pancreatitis were encountered. Conclusions Radioactive ~(125)I seed implantation for the treatment of pancreatic carcinoma is safe,effective,and micro-invasive,being a good option of remedy.
4.Preparation of a novel AAV-ITR gene expression mini vector in Sf9 insect cells via baculovirus.

AAV-ITR gene expression mini vector is a double-strand or single-strand DNA that only contains inverted terminal repeats of adeno-associated virus, cis-elements and gene of interest and does not contain any other foreign DNA sequences. We prepared Bac-ITR-EGFP and Bac-inrep. Spodoptera frugiperda cells were infected with Bac-ITR-EGFP (P3) and Bac-inrep (P3). Up to 100 μg of AAV-ITR-EGFP gene expression mini vectors were extracted from 2 x 10(7) cells of Sf9 72 h after infection. The gel electrophoresis analysis shows that most forms of AAV-ITR-EGFP gene expression mini vector were monomer and dimer. The mini vector expression efficacy was examined in vitro with HEK 293T cells. The EGFP expression was observed at 24 h after transfection, and the positive ratio reached 65% at 48 h after transfection.

5.Protective effect of hydrogen sulfide on mitochondrial damage of smooth muscle cells of rats with high pulmonary blood flow pulmonary hypertension and its mechanism

Objective To study the influence of hydrogen sulfide (H2 S)on the mitochondrial function of pulmonary artery smooth muscle cells of the rats with high pulmonary blood flow pulmonary hypertension, and to clarify the function and its mechanism in the occurrence of high pulmonary blood flow pulmonary hypertension.Methods A total of 27 rats were randomly divided sham operation group,operation group,and operation+sodium hydrosulfide (NaHS)group (n=9).The pulmonary hypertension rat model was built by left lung resection.After fed for 35 d, the mean pulmonary artery pressure (mPAP),ratio of right ventricle/body weight (RV/BW),and ratio of right ventricle/(left ventricle+septum)[RV/(LV + S)]of the rats in three groups were measured.The plasma H2 S levels and the CSE activities in pulmonary artery tissue of the rats were detected;the activities of total mitochondrial ATP enzyme,superoxide dismutase (SOD),glutathione peroxidase (GSH-Px)and malondialdehyde (MDA)levels were determined;the ultrastructure of pulmonary artery smooth muscle mitochondria was observed through transmission electron microscope.Results Compared with sham operation group,the plasma H2 S level and the CSE activity in pulmonary artery tissue of the rats in operation group were decreased(P<0.01);the mitochondrial membrane of pulmonary tissue was swelling, and the mitochondrial activity was decreased (P<0.01);the mitochondrial ATP enzyme,SOD and GSH-Px activities were significantly decreased,and the MDA level was significantly increased (P<0.01).Compared with operation group,the H2S level in plasma and the activity of CSE in pulmonary tissue of the rats in operation + NaHS group were increased (P<0.01 );the mitochondrial membrane swelling was reduced, and the vitality was restored;the ATP enzyme, GSH-Px, and SOD level in pulmonary tissue were significantly increased (P<0.01),and the MDA level was significantly reduced (P<0.01).Conclusion H2 S can enhance the activities of mitochondrial ATP enzyme,GSH-Px,and SOD,and decrease the mitochondrial lipid peroxidation level,thus it plays a protective effect on rat pulmonary artery smooth muscle.
6.Research of Detecting Depth for Near-infrared Fluorescence Imaging in Vivo

Objective To obtain the near-infrared fluorescence image in vivo and find the relationship between the detecting depth and fluorescence probe concentration. Methods Signals of fluorescence probe in various depths of vivo and tissue phantom with cooled CCD camera were acquired. Results The fluorescence intensity informations with different fluorescence probe concentrations and depths in vivo and liquid phantom were obtained. Conclusion Relationship between fluorescence intensity,location and depth of detecting probe in vivo is found. The linear relation of fluorescence probe concentration and detecting intensity is simulated,which will be used as a reference for the experiment system.
7.Effects of multiple-trough sampling design and algorithm on the estimation of population and individual pharmacokinetic parameters.

The purpose of this study is to investigate the effects of multiple-trough sampling design and nonlinear mixed effect modeling (NONMEM) algorithm on the estimation of population and individual pharmacokinetic parameters. Oxcarbazepine and tacrolimus were used as one-compartment and two-compartment model drugs, respectively. Seven sampling designs were investigated using various number of trough concentrations per individual ranging from 1-4. Monte Carlo simulations were performed to produce state-steady trough concentrations. One-compartment model was used to fit simulated data from oxcarbazepine and tacrolimus. The accuracy and precision of the estimated parameters were evaluated using the median prediction error (PE), the median absolute PE and boxplot. The results indicated that trough concentrations could yield reliable estimates of apparent clearance (CL/F). For oxcarbazepine, as the number of trough concentrations per subject increased, the accuracy and precision of CL/F, between-subject variability (BSV) of CL/F and residual variability (RUV) tended to be improved. For tacrolimus, however, although no improvement were observed in the accuracy of CL/F and BSV of CL/F, the PE distribution ranges were significantly narrowed and the RUV estimates were less bias and imprecise. In terms of algorithm, Monte Carlo importance sampling (IMP) and IMP assisted by mode a posteriori estimation (IMPMAP) were consistently better than other methods. Additionally, the sampling design had no significant effects on the individual parameter estimates, which were only depended on the interaction between BSV and RUV in various algorithms. Decreased in BSV and RUV levels can improve the accuracy and precision of the estimation for both population and individual pharmacokinetic parameter estimates.
8.Associations of polymorphism of leptin receptor gene Gln223Arg with asthma and metabolic syndrome

Objective:To investigate the relationship of polymorphism of leptin receptor gene Gln223Arg with asthma and metabolic syndrome.Methods: Collected 120 asthma patients,92 metabolic syndrome patients,54 asthma combined metabolic syndrome patients and 81 normal controls.According to the severity,the asthma patients were divided into mild-medium group and severe group.The serum leptin level was measured by ELISA,the genotypes of leptin receptor were analyzed by the method of polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),and statistcs of each subject′s MBI,blood pressure,lung function and fasting blood glucose were collected.Results: ①There were significant differences in genotype and allele frequency in leptin receptor gene Gln223Arg between the metabolic syndrome group,asthma combined metabolic syndrome group and normal control group(P<0.05).②The allele frequency and genotype in leptin receptor gene Gln223Arg were significant different between the severe asthma group and normal control group(P<0.05).③The serum leptin level,BMI and systolic blood pressure of AA+AG genotype group were significiant higher than GG genotype group(P<0.05),while the value of FEV1% and FEV1/FVC of lung function were lower than GG genotype group(P<0.05).Conclusion: Leptin receptor gene Gln223Arg polymorphism is correlated with asthma and metabolic syndrome,and by causing leptin resistance,the A allele might be the genetic factor that contribute to individual susceptibility for asthma and metabolic syndrome.
9.3-D Image of Near-infrared Fluorescent Probe in Tissue-like Solution.

Objective To develop the fluorescence molecular imaging system and obtain the 3D images of near-infrared fluorescent probe in tissue-like solution.Methods Weak optical signals were acquired with hypothermal cooled CCD camera.The system captured the fluorescent images of different projections at rotating the tissue phantom.The photon propagation in tissue followed the diffusion approximation theory.The 3D image of probe was calculated with a fast reconstruction algorithm based on finite element approach.Results The imaging system could obtain the full-angle dense raw dataset.The reconstruction algorithm had capacity to recover the position and contour of the fluorescent target included in diffusive medium.Conclusion The results from phantom experiments are promising and show the potentials to act as a platform for in vivo fluorescence tomography.
10.Application of Double Flash Scanning in Patients with Coronary Artery Stent

Purpose To explore the image quality, radiation dose and diagnostic accuracy of double flash scanning mode in patients with coronary artery stent. Materials and Methods Sixty-three patients with coronary stents underwent CT coronary artery imaging using conventional coronary artery angiography as the gold standard. Double flash mode was used in patients with heart rate ≤ 70 beats per minute. Spiral scanning was used in patients with heart rate between 70-100 beats per minute. The image quality, vascular CT value, noise, SNR and CNR were compared. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy were determined, and the radiation dose was calculated. Results There were 59 stents in Double flash group (57 were diagnostic) and 58 stents in spiral group (all diagnostic). There was no statistically significant difference in image quality, CT value, noise, SNR or CNR for the first scan between two groups (P>0.05). Image quality for the second scan in Double flash group showed statistical difference with the first scan and spiral group (P<0.05). The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of Double flash and spiral group were 90.0%, 89.8%, 64.3%, 97.8%, 89.8% and 83.3%, 95.7%, 83.3%, 95.7% 93.1%, respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy for stents with diameter ≥ 3.0 mm were significantly higher than stents with diameter <3.0 mm. The radiation doses of Double flash group were significantly lower than the spiral group (P<0.05). Conclusion Double Flash mode scanning provides good image quality and diagnostic accuracy with lower radiation dose in patients with coronary artery stents. Imaging of stents with diameter ≥ 3.0 mm is superior to stents with diameter <3.0 mm.
