2.Establishment of a mouse model of acid aspiration-induced lung injury

Objective To establish a non-traumatic mouse model of acid aspiration-induced lung injury which al-lows longitudinal studies.Method C57BL/6 mice were anesthetized and orotracheally intubated with a 20 gauge angio-catheter guided by optical fiber.The mice were subsequently placed in the right lateral decubitus position and external com-pression to the left lung was manually applied.A polyethylene catheter was advanced into the right lung and used to instill either hydrochloric acid (2.5μL/g, 0.1 mol/L, pH 1.5) or saline as control.Then the mice were recovered with supple-mental oxygen for 4 hours.The pulmonary physiological function and survival of mice within 2 weeks after surgery were as-sessed.Results Methylene blue instillation showed that the staining fluid went into the right lung of the non-traumatically intubated mice.The survival rate of the mice with non-traumatic instillation was 80%, statistically significantly higher than those with tracheostomy instillation.Histological examination and lung function ( wet/dry ratio, elastance and arterial oxy-gen saturation) assay demonstrated that acid instillation caused a profound pathological changes and functional impairment of the lung.Besides, acid aspiration into the mouse lung caused a significant increase in neutrophil infiltration in mouse pulmonary alveoli and high concentrations of inflammatory factors (TNF-α, IL-6, CXCL1 and CXCL2) in the bronchoalve-olar lavage fluid.Conclusions We successfully established a mouse model of acid aspiration-induced lung injury, which may serve as a reliable model for longitudinally studying pulmonary immune-inflammatory mechanism in humans.
3.Clinical Study of Yifumin Capsule in Treating Type 2 Diabetic Insulin Resistance

Objective To study the clinical therapeutic effect of Yifumin Capsule in treating type 2 diabetic insulin resistance and its effect on relevance indexes.Methods Two hundred and forty cases of type 2 diabetic insulin resistance with deficiency of qi-yin and stagnation of phlegm and toxin were divided randomly into two groups,120 cases in each group.The treatment group was given orally Yifumin Capsule,and the control group was given orally pioglitazone.The treatment course of the two groups was 12 weeks.Clinical symptoms,blood glucose,blood lipid,CRP,ISI,FINS,HOMA-IR,liver and kidney function were detected respectively.Results Chinese medicine symptoms and signs were all improved.The total effective rate of clinical effect was 88.1% in treatment group and 52.9% in control group.The clinical therapeutic effect of the treatment group was obviously better than the control group.Both groups had a notable seducement in HOMA-IR,FINS,FPG,2 h PG,HbA1c,TG and LDL-C with an obviously improvement in ISI after the treatment.Compared with the control group,the treatment group had a significant decrease in TC and CRP with notable difference.Conclusion Yifumin Capsule can effectively improve the insulin resistance of type 2 diabetic patients.

Objective: To investigate the expression difference of TRAIL and its receptors between resting and activated peripheral blood lymphocytes, and to study their role in cellular growth and proliferation. Methods: Expression of TRAIL and its receptors were detected by RT PCR and DNA sequencing. Results: Neither the expression of TRAIL nor its receptors were detected in resting peripheral blood lymphocyte. However, both expression were found in activated peripheral blood lymphocytes of various degree. Conclusion: TRAIL and its receptors may play a role in regulation and control of the activated lymphocytes in immune reaction.

Objectives:To explore the influence of nutritional support treatment on patients with the injured pancreatitis. Methods:The twenty five cases with injured pancreatitis were divided into research group (13 cases of 1997-2001, with nutrition support) and control group (12 cases of 1992-1996,without nutritional support). Blood composition, biochemistry, complications, hospitalization time and mortality were compared between two groups. Objective:To observe and analyze the therapeutic effect and safety of tirofiban combined large dose statin in patients with acute myocardial infarction (AMI),and evaluate its influence on myocardial perfusion and cardiac function.Methods:A total of 97 consecutive AMI patients visited to our hospital from May 2011 to January 2013, who were beyond the emergency PCI time window (≥12h),their chest pain was remissive or not further aggravated were studied.All patients were pumped with tirofiban for 48h continuously;according to combined rosuvastatin dose,they were divided into large dose group (n = 52,20mg,once/d,until one month after infarction,then changed to routine dose of 10mg,once/d)and routine dose group (n=45,10mg,once/d).All patients received se-lective PCI after 7 ~ 10d conservative treatment.Myocardial perfusion level,left ventricular ejection fraction (LVEF)on one week after PCI and 30d after AMI,enzymology changes [creatine (CK),lactate dehydrogenase (LDH)]and major adverse cardiovascular events (MACE)during hospitalization were compared between two groups.Results:Compared with routine dose group,there were significant reductions in thrombus scores in infarct related artery (IRA)[(1.32±1.01)scores vs.(0.81±0.78)scores]and corrected TIMI frame [(32.4±4.73)vs. (26.8±2.34)]in large dose group (P =0.021,P <0.001);after selective PCI,TIMI flow of large dose group was significantly better than that of routine dose group (P =0.024).On one week after PCI,LVEF:(51.4±8.9)% of large dose group was significantly higher than that of routine dose group (47.7±8.7)%,P =0.021;there were no significant difference in levels of CK and LDH between two groups on 7d and 30d after PCI (P >0.05).There was no MACE in both groups during hospitalization and 30d after PCI.Conclusion:Tirofiban combined large dose statin is safe and effective in patients with acute myocardial infarction,it can reduce intra-coronary thrombus burden,im-prove myocardial tissue perfusion and cardiac function without increasing MACE.

Physical training induces beneficial adaptations,but long excessive exercise may lead to severe damage to the skeletal muscles,liver,heart,kidneys and immune functions. Over the past few decades,health scholars have been searching for natural components that can prevent or improve the damage induced by hyperkinesis. The mechanisms of organ damage induced by long overtraining include immunosuppression,metabolism disorder,hormone disturbance,oxidative damage, etc. Natural poly-saccharides have interventional effects on these injuries,possibly by improving immunity,regulating metabolism and ameliorating free radical damage.

Objective To explore medical equipment management to decrease medical risks.Methods A medical equipment risk management model was established to deploy medical equipment rationally,evaluate the risk value of medical equipment,calculate the total risk value of medical equipment of the department and to propose the requirements for medical equipment utilization and management.Results The risks of medical equipment could be decreased by improved medical equipment management in deployment,utilization and maintenance as well as emphases on personnel and regulation.Conclusion Risk minimization is of great significance for medical equipment users and management department.
