1.MUSCLE OXYGENATION HETEROGENEITY IN A SINGLE MUSCLE AT REST AND DURING BICYCLE EXERCISE

We evaluated regional differences of muscle O2 dynamics between distal and proximal sites in the vastus lateralis (VL) muscle using near infrared spatial resolved spectroscopy (NIRSRS). forty-one male subjects performed a 30 W ramp incremental bicycle exercise test until exhaustion. The NIRSRS probes were attached on each distal and proximal site in the VL. The pulmonary O2 uptake and heart rate were monitored continuously during the experiment. The TOI at rest was significantly higher in proximal than distal sites (65.0±5.2 vs. 69.7±4.6%, p<0.001). The TOI at exhaustion was also significantly higher in proximal than distal sites (39.5±6.7 vs. 47.5±7.6%, p<0.001). Moreover, a significant correlation was found between VO2max and the TOI at exhaustion in each proximal and distal site in the VL. Half time reoxygenation, the time to reach a value of half-maximal recovery, was significantly slower in distal sites than proximal sites (27.1±5.6 vs. 25.0±6.1 sec, p<0.01). In conclusion, lower muscle oxygenation at exhaustion in higher VO2max may be due to enhanced O2 extraction in high oxidative capacity muscle. In addition, slower reoxygenation and lower muscle deoxygenation at the distal site in the VL may be explained by differences in O2 supply and/or muscle fiber composition between distal and proximal sites.

Based on two years working of “the enlarged working group for the curriculum of chemistry for medical education”, a provisional plan of the curriculum of chemistry for medical students is proposed. The article is consist of four sections; namely A: general problems, B: “chemistry” as a general education, C: purpose and specific problems of the general education for medical students, D: a provisional plan of the curriculum of chemistry for medical students.
The main part D is consist of three subsections: namely (1) a plan of the curriculum of physical and inorganic chemistry, (2) a plan of the curriculum of organic chemistry, (3) a plan of the curriculum of experiments.

PURPOSE: Pain from osteoarthritis (OA) is generally classified as nociceptive (inflammatory). Animal models of knee OA have shown that sensory nerve fibers innervating the knee are significantly damaged with destruction of subchondral bone junction, and induce neuropathic pain (NP). Our objective was to examine NP in the knees of OA patients using painDETECT (an NP questionnaire) and to evaluate the relationship between NP, pain intensity, and stage of OA. MATERIALS AND METHODS: Ninety-two knee OA patients were evaluated in this study. Pain scores using Visual Analogue Scales (VAS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), painDETECT, duration of symptoms, severity of OA using the Kellgren-Lawrence (KL) system, and amount of joint fluid were evaluated and compared using a Spearman's correlation coefficient by rank test. RESULTS: Our study identified at least 5.4% of our knee OA patients as likely to have NP and 15.2% as possibly having NP. The painDETECT score was significantly correlated with the VAS and WOMAC pain severity. Compared with the painDETECT score, there was a tendency for positive correlation with the KL grade, and tendency for negative correlation with the existence and amount of joint fluid, but these correlations were not significant. CONCLUSION: PainDETECT scores classified 5.4% of pain from knee OA as NP. NP tended to be seen in patients with less joint fluid and increased KL grade, both of which corresponded to late stages of OA. It is important to consider the existence of NP in the treatment of knee OA pain.

People with high health knowledge tend to use information and communication technology (ICT) to obtain, maintain, and promote information necessary for health management. Therefore, experts hypothesize that improving health knowledge will increase the use of online medication guidance via ICT. In this study, we investigated the relationship between health knowledge and willingness to use online medication guidance among Iwaki Health Promotion Project health checkup subjects. The 736 examinees participating in this project in 2022 were surveyed on age, gender, number of people living with them, whether they exercise, drink alcohol, or smoke, whether they use smartphones, whether they use medication registers, their health knowledge score, and whether they take any medications. The presence/absence of desire for online medication guidance (1/0) was used as the objective variable to identify influencing factors. Univariate and multivariate analyses showed that independence was found for four factors: age (OR: 0.975, P<0.001), gender (male) (OR: 1.914, P<0.001), smartphone use (OR: 2.156, P=0.004) and health knowledge score (OR: 1.405, P<0.001). This study found that health knowledge plays a role in the intention to use online medication guidance. In order to improve health knowledge, it will be necessary to enhance skills to obtain and use high-quality health information and to improve the environment for using ICT-based communication devices.
