1.Prevalence of Hospital infection

OBJECTIVE To investigate the infection condition and associated causative factors in our hospital in order to improve the methods of monitoring,preventing and controlling infection.METHODS All patients admitted in 24 hours were investigated.including record review and patient examination.Participants mainly consisted of the specialized staff,with assistant administrative staff for hospital infection.The investigating table was filled for record review and patient examination respectively.Statistical analysis was performed.RESULTS 119 of the 123 hospitalized patients were investigated(96.8%).The current rate for hospital infection was 6.7%.The most infected site was respiratory tract.Antibiotics was used in 39.5% of the patients.CONCLUSIONS The current method can be used for investigating the present state of hospital infection.Based on the investigation,objective monitoring,intensified specimens detection,and reasonable antibiotics administration can be performed to decrease the incidence of hospital infection.
2.Advances and problems in Western Pacific Region Index Medicus

The history of Western Pacific Region Index Medicus (WPRIM), and advances in its journal selection, retrieval service platform construction and data acquisition were described.The daily record of users showed that WPRIM is of certain international influencing power, but is not a matured regional international index medicus. Suggestions were put forward for its popularization, project development funds, journal review and data acquisition.
3.Determination and clinical significance analysis of ANF、PRA、AT-Ⅱ and ALD in the heart failure patients

Objective:To investigate the effect of ANF, PRA, AT-? and ALD on the occurrence of Heart failure patients and Congestive heart failure(CHF) .Methods:The plasma were measured by radioimmunoassay(RIA) in 163 patients(112 on heart failure patients,51 congestive heart failure)before and after taking Angiotensiin converting enzyme. The results were as foron: ll. Results:The concentrations of ANF, PRA,AT-? and ALD in acute myocardid infarction, Rheumatic valvuar disease, cor pulmonale and Hypertension patients were higher significantly than the contorl group(heart failure)( P 0.05) .After taking ACE inhibitor,AT an extent, the values were decreased-ANF 32.5%, AT-? 25.3 % and ALD 27.1 % , A positive correlation was shown between ANF system and RSSA system. Conclusion: The results suggested that monitoring of ANF,PRA,AT-? and AID with RIA technique showed a beneficial to heart's disease and heart's dysfunction.
4.Observations on the Efficacy of Wrist Three-needle Acupuncture in Treating Pre-adhesive Stage Scapulohumeral Periarthritis

Objective To investigate the efficacy of wrist three-needle acupuncture in treating pre-adhesive stage scapulohumeral periarthritis. Method Seventy-eight patients with pre-adhesive stage scapulohumeral periarthritis were randomly allocated to two groups. The treatment group of 39 patients received wrist three-needle acupuncture and the control group of 39 patients, conventional acupuncture. Result After one course of treatment, the cure rate and the total efficacy rate were significantly higher in the treatment group than in the control group and there was a statistically significant difference (P<0.05). There was a statistically significant pre-/post-treatment difference in the VAS score in the treatment group (P<0.05). There was a statistically significant difference in the pre-/post-treatment difference value between the treatment and control groups (P<0.05). Post-treatment pain relief rate was significantly higher in the treatment group than in the control group (P<0.05). Conclusion Wrist three-needle acupuncture is even more effective than conventional acupuncture in treating pre-adhesive stage scapulohumeral periarthritis.
5.Observations on the Therapeutic Effect of Electrothermic Acupuncture on Knee Osteoarthritis with Yang Deficiency and Cold Congealment

ObjectiveTo investigate the clinical efficacy of electrothermic acupuncture in treating knee osteoarthritis.Method One hundred and twenty patients were randomly allocated to treatment and control groups, 60 cases each. The treatment group received electrothermic acupuncture and the control group took Xianlinggubao capsules orally. The clinical therapeutic effects and changes in the WOMAC score were observed in the two groups after four weeks of treatment.ResultThe total efficacy rate was 95% in the treatment group and 85% in the control group. There was a statistically significant difference in clinical therapeutic effect between the two groups (P<0.05). There was a statistically significant difference in pre-/post-treatment WOMAC score difference value between the two groups (P<0.05).ConclusionThe clinical efficacy of electrothermic acupuncture is better than that of Xianlinggubao capsules in treating knee osteoarthritis with yang deficiency and cold congealment.
6.Prevention and management of in-hospital cornplications in the hip replacement in elderly patients

Objective To investigate the prevention and management strategies of in-hospital complications in the hip replacement in elderly patients(≥80 years old) Methods The data of 42elderly patients with hip replacement between Oct.2004 and Dec.2006 were collected and analyzed.Their age ranged from 80 to 96 years with an average of 84.6 years.Twenty-four cases were associated with severe osteoporosis(Dorr Ⅲ),13 with coronary heard disease,10 with arrhythmia,7with diabetes,5 with cerebral paralysis.Twenty five cases had a cemented stem bipolar femoral head replacement,2 had uncemented stem bipolar femoral head replacement,7 had cemented total hip replacement,3 had revision of total hip replacement. Results No death occurred during operations,1 died postoperatively due to dislocation at the 5th day postoperation.One case had coma for 1 week after general anaesthesia.5 had temporary cognitive handicap,38(90.5%)were able to walk at the aid of walker with easement of pain before discharging. Conclusions To gain good results in elderly patients with hip arthroplasty,less invasive and quick anaesthetic technique and operative procedures are encouraged.Meticulous preoperative preparation.active preventing and treating the intraoperative and postoperative complications should be emphasized.
7.The study and application of intraoperative neurophysiological monitoring in oblique lateral interbody fusion combined with posterior transpedicle internal fixation

Objective To investigate a reliable and practicable model of intraoperative neurophysiological monitoring (IONM) in oblique lateral interbody fusion (OLIF) combined with posterior transpedicle internal fixation.Methods A total of 34 consecutive patients who underwent OLIF combined with posterior transpedicle internal fixation with IONM at the SRRSH from November 2014 to July 2016 were collected and follow-up at least for 3 months postoperative.The intraoperative somatosensory evoked potentials (SEP),motor evoked potentials (MEP),dermatomal somatosensory evoked potential (DSEP) and spontaneous electromyography (sEMG) were used and evaluated,besides,triggered electromyography (tEMG) was used to monitor the implantation of pedicle screws,the possible reasons and treating procedures of monitoring alerts were summarized.Results A total of 34 cases of patients were effectively monitored during OLIF combined with posterior transpedicle internal fixation.SEP monitoring was successful in all patients,and no abnormal SEPs were observed in all cases.DSEP monitoring was successful in 20 cases (58.8%),and no abnormal DSEPs were observed in these patients.MEP was successfully monitored in all cases,eliminating the anesthesia factor,no abnormal MEPs were observed in all cases.The sensitivity and positive predictive value of SEP,MEP,DSEP were 0%.3 cases had abnormal sEMG reactions during the channel establishment which disappeared after adjustment of channel,and 2 cases which appeared abnormal wave recovered after brief observation.Among the 5 patients with abnormal wave,one case got numbness,one patient felt pain in front of the thigh postoperative.Among the other 29 patients without abnormal wave during channel establishment,one case appeared numbness.These 3 patients with neurologic symptoms recovered in 3 months after conservative treatment.The sensitivity and specificity of sEMG was 66.7% and 90.3% respectively,negative predictive value and positive predictive value were 96.6% and 40% respectively.6 screws got tEMG response after tapping even the intensity was less than 10mA,which showed broken codex of pedicle according to postoperative CT scan.Cornclusion Intraoperative application of sEMG monitoring in OLIF can obviously reduce the incidence of the neurological deficit,which was an effective method,tEMG monitoring during lumbar pedicle screw fixation can detect the mal-positioned screws and protect the nerve roots.SEP,MEP and DSEP did not seem well suited for OLIF.
8.Clinical Observation of Deeply Needling Lianquan (CV23) plus Acupuncture at the Ten Nape Points for Post-stroke Pseudobulbar Palsy

Objective By observing the effect of deeply needling Lianquan (CV23) plus acupuncture at the ten nape points on the deglutition function in post-stroke pseudobulbar palsy, to objectively evaluate the efficacy of deeply needling Lianquanplus acupuncture at the ten nape points in treating post-stroke pseudobulbar palsy.Method A total of 141 patients diagnosed with post-stroke pseudobulbar palsy were divided into a treatment group (71 cases) and a control group (70 cases) by following asimple-randomized design (random number table). The treatment group was intervened by deeply needling Lianquan plus acupuncture at the ten nape points, while the control group was intervened by conventional acupuncture. A month later, the general therapeutic efficacy, Kubota's water drinking test, Toshima Ichiro's swallowing assessment, and Standardized Swallowing Assessment (SSA) were evaluated, and the therapeutic efficacies of the two groups were compared.Result The general therapeutic efficacy, waterdrinking test result, Toshima Ichiro's swallowing assessment, and SSA score were significantly improved in both groups after the treatment (P<0.05). After the treatment, the general therapeutic efficacy, water drinking test result, Toshima Ichiro's swallowing assessment, and SSA score in the treatment group were significantly different from those in the control group (P<0.05), and the treatmentwas superior to the control group. The total effective rate was 91.5% in the treatment group, versus 70.0% in the control group, and the between-group difference was statistically significant (P<0.05).Conclusion Deeply needling Lianquan plus acupunctureat the ten nape points is effective in treating post-stroke pseudobulbar palsy, superior to the conventional needling method.

Objective To observe the therapeutic efficacy and action mechanism of muscle-region alignment electroacupuncture in treating post-stroke shoulder pain. Method Eighty patients were randomized into a muscle-region alignment needling group and a conventional acupuncture group. The Short-form McGill Pain Questionnaire (SF-MPQ), and serum levels of IL-6, TNF-?, and NO were majorly observed before and after the treatment. Result The muscle-region alignment electroacupuncture and conventional acupuncture both obviously reduced the SF-MPQ score and down-regulated the serum levels of IL-6, TNF-?, and NO, and the decreases by the muscle-region alignment electroacupuncture were more significant than that by the conventional acupuncture. Conclusion The action of muscle-region alignment electroacupuncture in treating post-stroke shoulder pain is plausibly by down-regulating serum levels of IL-6, TNF-?, and NO, reducing or inhibiting the production of inflammatory factors and restraining inflammation.
