1.Hypertension in the elderly: insights from recent research
Journal of Geriatric Cardiology 2010;07(2):116-125
Hypertension is a leading cause of mortality and morbidity around the world and, prevalence of hypertension is increasing with aging.Hypertension in the elderly is associated with increased occurrence rates of sodium sensitivity, isolated systolic hypertension, and 'white coat effect'.Arterial stiffness and endothelial dysfunction also increase with age. These factors should be considered in selecting antihypertensive therapy. The prime objective of this therapy is to prevent stroke. The fmdings of controlled trials show that there should be no cut-off age for treatment.A holistic program for controlling cardiovascular risks should be fully discussed with the patient, including evaluation to exclude underlying causes of secondary hypertension, and implementation of lifestyle measures. The choice of antihypertensive drug therapy is influenced by concomitant disease and previous medication history, but will typically include a thiazide diuretic as the first-line agent;to this will be added an angiotensin inhibitor and/or a calcium channel blocker.Beta blockers are not generally recommended,in part because they do not combat the effects of increased arterial stiffness. The hypertension-hypoten-sion syndrome requires case-specific management.Drug-resistant hypertension is important to differentiate from faulty compliance with medication.Patients resistant to the third-line drug therapy may benefit from treatment with extended-release isosorbide mononitrate.A trial of spironolactone may also be worthwhile.

Objectives: To investigate the occlusion features in thre e dimensions in patients with temporomandibular disorders(TMD). Methods: Occlusal casts were obtained from 23 asymptom volunteers and 101 TMD pati ents and were analyzed in three dimensions by scores vertically, mesio-distally and labial-or buccal-lingually. Bilateral TMJs of each TMD patient was examin ed with Schuller's radiograph and tomograph, the sounding TMJs were examined wit h arthrography. The relationship between occlusion scores and the changes shown in X-ray images were analyzed. Results: 66.3% TMD patients had abnormal occlusion in vertical direction.Abnormal occlusion in vertical directio n was correlated with TMD symptoms(P 0.05 ). Conclusion: Abnormal occlus ion in vertical direction is correlated with TMD symptoms.

Objective: To investigate the morbidity of climacteric depression in Beijing area. Method: Among 18 districts and counties of Beijing area, using the stratified clustering and probability proportional method sampled 7232 women aged 40 to 65. The subjects completed Zung's self-rating depression scale, possible risk factors were also collected. Result: 6786 valid questionnaires were analyzed, among them, 223 suffered from climacteric depression (3.65%). The depressive symptoms had a significant positive correlation with the climacteric syndromes. Risk factors of depression included history of abortion or premature birth, duration of menopause, hypertension, bone fracture, smoking and incompatible with their husband. Family history of psychiatric illness had no influence in this study. Conclusion:Depression in climacteric women is not only a mental health problem, but also a biopsychosocial problem.

Chinese Medical Equipment Journal 1993;0(06):-
This paper designs a new distributing structure of nozzles in medical micro-jet device on the bases of emulating results. This new structure can improve the output coefficient and flux of drug delivery. Then, a micro-jet device is fabricated by MEMS technology based on this structure. The experiment results indicate that the number of output droplet is approximated to normal distribution on droplet diameter and velocity. Moreover, the droplet velocity and the flux are both improved in this new micro-jet device, whose values are 2.93m/s and 11.77?l/s respectively.

Journal of Interventional Radiology 2001;0(05):-
Objective To observe the clinical curative effect of transcatheter hepatic arterial chemoembolization (TACE) in treatment of gastrointestinal stromal tumors(GIST) with metastatic liver cancer. Methods Four GIST patients with 19 lesions of liver metastasis were treated with 2 ~ 4 times of TACE. The diameters of the metastatic tumor were from 2 ~ 5 cm. FuDR, CDDP, EPI-ADM, together with lipiodol ultrafluid and gelatin sponge to from a mirture was prepared for arterial embolization. Results DSA showed abundant tumor vasculature and tumor stain on hepatoarterial angiography. The size of tumor showed no decrease after TACE and lipiodol was cleared away in a short period. No patient achieved CR or PR until two of them received imatinib. Conclusions TACE is not effective in treating hepatic metastasis from GIST shown in limited cases. In order to obtain more experience of endoscopic retrograde cholangiopancreatography (ERCP) ,and in this way, more correct diagnosis and much more effective treartment were received ,we studied 1 257 cases of ERCP we had performed in last two years . The results were: In 1 25 7 cases of ERCP,1 229 cases(97. 77% ) were successful and 435 cases (34. 61% ) received treatment. Ninety-four cases received papillotomy and extraction, 309 cases received biliary drainage in therapeutic ERCP. lt in- dicated that the importance of gallbladder visualization and biliary drainage should be emphasised.

AIM To study the effects of A 3 site (components), extracted from Angelica naphtha (total oil),on physiological properties of isolated rat atrium and action potential of isolated guinea pig ventricular papillary muscles. METHODS Spontaneous beats, contractile force and functional refractory period(FRP) of isolated atrial muscle were recorded according to the regular methods. Standard microelectrode intracellular recording technique was used to record action potential(AP) of ventricular papillary muscles. RESULTS A 3 site(10~160 mg?L -1 ) dramatically inhibited spontaneous beats of isolated rat right atrium and made beats stop with 160 mg?L -1 . A 3 site concentration dependently reduced contractile force of isolated rat left atrium induced by electrostimulating, with the IC 50 value of 52 3 mg?L -1 . A 3 site(40~100 mg?L -1 ) obviously prolonged FRP. The FRP was prolonged from 106 ms (as control) to 130 ms at final concentration of 100 mg?L -1 . A 3 site decreased action potential amplitude (APA), shortened 20%APD(APD 20 ) and 90%APD(APD 90 ), but had no significant effect on RP of AP. CONCLUSION The mechanism of these effects of A 3 site on myocardial physiological properties and action potential was probably associated with blocking the influx of Ca 2+ and Na +, promoting K + outflux.

Objective To develop a sandwich enzyme-linked immunosorbent assay ( ELISA) for the determination of a humanized antibody MIL50.Methods RiVax, a mutant of RTA (a chain of ricin) expressed in E.coli, was used as the coating protein.Horseradish peroxidase (HRP) labeled IgG of goats against humans was used to determine MIL 50 captured by RiVax coated on the plate. Results Compared with ricin, RiVax could bind well with MIL50,indicating it could be used as a coating protein to determine MIL50 instead of holotoxin.Using this method, the detective limit of MIL50 in PBST (PBS containing 0.1%Tween 20) could be as low as 0.030 mg/L.The absorbance value of ELISA for MIL50 in the ser-um and homogenate of the liver , spleen, lung, kidney, muscle of rats was lower than that in PBST .Conclusion The sandwich ELISA is a sensitive method for the analysis of distribution of MIL 50 in rat tissues.
