1.Preparation anti biological activity of bivalent IgY against cobra anti viper venom

Objective To prepare the bivalent immunoglobulin yolk (lgY) against eobra and viper venom and to detect its activities as the foundation for production and application of polyvalent . Methods The venom of Naja atra Cantor and Daboia russellii siamensis injected alternately into the leghorn hen. Biva-lent lgY was extraeted by water dilution. The biological activity of bivalent lgY were deteeted in several as-pects, sueh as the potency ( by indireet ELISA assay), the cross immunity ( by double immunodiffusion), the membrane lysis activity ( by experiments of vitelline membrane lysis) and 50% lethal activity ( LD50 ). Results Bivalent IgY was extracted from eggs yolk in 28-42 days after the first immunization. The titers of bivalent lgY against cobra and viper venom were 1:12 800 and 1: 6400. The cross immunologic reactions of bivalent IgY were found obviously with six kinds of snake venoms from Elapinae and Viperinae. There were not immunologic precipitation lines between bivalent IgY and four kinds of snake venoms from Crotalinae. Bi- valent lgY obviously deereased the vitelline membrane lysis activity of cobra and viper venom and prolonged the average survival time of mice with cobra or viper envenomation (P < 0.05). Moreover, with the same dose of bivalent IgY, the survival rate of mice with cobra venom envenomation was higher than those with vi-per venom envenomation. Conclusion Bivalent lgY could signifieantly neutralize biologieal activities of co-bra and viper venom, protect animals with cobra or viper envenomation.

5.Results of a phase Ⅱ study of concurrent 5-fluorouracil/paclitaxel plus radiotherapy in patients ;with carcinoma of the esophagus

Background and purpose:Concurrent radiochemotherapy is the standard modality for locally advanced esophageal squamous cell carcinoma (ESCC) patients. This clinical trial aimed to assess the effectiveness and toxicity of continuous infusion of 5-lfuorouracil (5-FU) and weekly paclitaxel combined with radiotherapy in ESCC patients. Methods:Patients with locally advanced (T2-4N0-1M0-1a) esophageal squamous cell carcinoma were enrolled in a prospective, single-institutional, single-arm study of deifnitive chemoradiotherapy. Patients received 61.2 Gy with IMRT in 34 fractions. Patients had a Karnofsky performance status of 70 or greater, and normal liver, renal, and bone marrow functions. Patients were recommended to receive concurrent 5-FU (300 mg/m2 civ 96 h) for 5 days a week for 5 weeks, plus paclitaxel (50 mg/m2) given during 3 hours every week for 5 weeks. Patients were recommended to receive 2 courses of consolidation chemotherapy after concurrent radio (chemo) therapy (5-FU 1 800 mg/m2 civ 72 h, plus paclitaxel 175 mg/m2 every 28 days). The primary endpoints of the study were 5 year overall survival and acute toxicity. Results:Fifty patients were enrolled in this study, including 38 male patients and 12 female patients;median age:58 years (ranged 26 to 75 years). 72%patients completed all the chemotherapy and 98%patients received the full dose of radiotherapy. 1-, 2-, 3-, and 5-year survival were 75%, 56%, 42%and 28%respectively. Among haematological toxicities, grade 3 leukopenia (16%) was recorded, and no patients experienced any≥grade 2 thrombocytopenia or anaemia. Among non-haematological toxicities, the rates of grade 2 peripheral neurotoxicity, arthralgias and myalgias, nausea, vomiting, and fatigue were 8%, 4%, 4%, 2%and 6%respectively. The rates of≥grade 2 acute radiation-induced esophageal toxicity, radiation pneumonitis and skin toxicity were 32%, 44% and 14% respectively. No treatment-related deaths occurred and no patients experienced any ≥ grade 4 toxicities. Conclusion: Continuous infusion of 5-FU plus paclitaxel given concurrently with radiotherapy may be an effective and tolerable treatment option for ESCC patients.
6.The expression changes of cancer-related pathways genes screened by RT-PCR Array in bladder cancer

Background and purpose:Bladder cancer is the most common urological tumor, and its pathogen-esis is still not fully understood. The study was aimed to observe the expressions of key genes in many tumor-associated signaling pathways in normal bladder tissue and bladder carcinoma, and to provide further evidence for the subsequent study of bladder cancer recurrence and metastasis.Methods:Twenty-seven cases of bladder cancer specimens were col-lected, and normal bladder tissues and bladder cancer tissues were distinguished by frozen section. Then, the expressions of 84 genes of cancer-related signaling pathways in bladder cancer tissues and normal bladder tissues were screened by Cancer Pathway Finder PCR Array produced by QIAGEN company.Results:Compared with the normal bladder tissues, the bladder carcinoma tissues had 8 up-regulated genes and 19 down-regulated genes. In this study, the impact of epithe-lial-mesenchymal transition (EMT) signaling pathway was selected as a research direction in which theGSC,KRT14,DSP were up-regulated,SNAI2,SNAI3 were down-regulated. ThereforeGSC,KRT14,DSP,SNAI2 andSNAI3 were chosen as target genes, and verified by qRT-PCR in many examples. The result showed that the expressions ofGSC gene in bladder cancer tissues were up-regulated, but with no statistical significance;KRT14,DSP expressions in bladder cancer were higher than those in normal bladder tissues (P<0.05);SNAI2,SNAI3 expressions in bladder cancer were lower than those in normal bladder tissues (P<0.05), andSNAI3 showed the most obvious expression differences.Conclusion:KRT14,DSP andSNAI3 may play an important role in bladder cancer’s occurrence, development and metastasis.

Objective To investigate the expression of GFAP in rat hypothalamus after acute heat stress. Methods The rats were caged in a experimental incubator for 60 minutes,the temperature within the incubator was adjusted to 24℃,34℃,38.5℃ or 42℃,the humidity was 60%.Single anti-GFAP immunohistochemical(ABC) method and anti-Fos and GFAP double immunohistochemical method were used to observe the expression of GFAP in hypothalamus in different ambient temperatures after heat stress. Results The GFAP-positive cells were rare in hypothalamus at 24℃,however it was increased in many nuclei(anterior hypothalamic area,paraventricular nucleus,arcuate nucleus,suprachiasmatic nucleus and supraoptic nucleus)at 34℃ and peaked when ambient temperature was 38.5℃,and then decreased.However,Fos/GFAP-IR double labelled astrocytes were observed at 42℃.Conclusion Astrocytes participate in the pathophysiological process of heat stress.
8.Imaging Manifestations of Infectious Atypical Pneumonias

Objective To study the imaging manifestations and characters of infectious atypical pneumonias.Methods The imaging materials of 80 cases of infectious atypical pneumonias which were proved by clinical were collected, the characters of imaging manifestations were analysed.Results The abnormal appearance of lung appeared after fever occured, the average time was 2.3 days. The most appearance was high density patchy and was found in every lung field. The multiple focuses were 43.8% in all the cases. The focuses changed fast. CT could have some help in early diagnosis and prognosis observation of infectious atypical pneumonias. Conclusion Medical imaging can play important part in the diagnosis and determining the treatment effect of infectious atypical pneumonias.
9.A preliminary study on the teaching mode of constructivism in physical diagnostics teaching

The efficacy of constructivism teaching mode in physical diagnostics teaching was evaluated in this study. We built up the constructivism teaching mode in diagnostics teaching taking the clinical symptoms as the theme, and through such aspects as courseware design, teaching plan preparation, SP playing and inquiry, SimMan simulated physical examination, condition analysis, etc. Then questionnaires were conducted to analyze the role of the constructivism teaching mode in diagnostics teaching. The diagnostics constructivism teaching mode can provide students with a platform for self-construction of diagnostics and integrated application of knowledge. Meanwhile, students' sense of participation can be improved and multiple learning skills are enhanced during the course.
10.Compositive imaging diagnosis of adrenal tumor and its clinical treatment: An analysis of 112 cases

Objective: To provide clinicians with diagnostic strategy and techniques for adrenal tumor by evaluating various imaging studies.Methods: The accuracy of localizing and qualitative diagnosis was analyzed by comparing various imaging examination with pathological findings after surgery combined with centesis by BUS.The clinical data of 112 patients with adrenal tumors were analyzed.Results: The pre operation lesion localizing accuracy with BUS,CT and MRI was 85.71%, 95.54%, and 98.14%,respectively.The qualitative accuracy with the same modalities as above were 33.33%,65.69%, 79.63%, respectively.For combining different imaging examination group, the accuracy of localizing and qualitative diagnosis was 100% and 86.92%,respectively.Tolally 102 cases were treated by operation and were surgically cured, of which 9 cases were treated with intervention therapy,30 cases had large tumors(exceeding 10.0 cm? 5.0 cm? 4.0 cm). Conclusion:The diversified diagnostic imaging should be applied orderly and rationality.BUS can be used as a screening method.CT and MRI are accurate in localization and differentiation of benign and malignant.Combination of various examinations has the advantages of different modalities and greatly increase the diagnostic accuracy.We should select right incision. Sufficient preparation during perioperation can reduce the complication.
