Objective: To study the effects of octylpheno(lOP)and genistein(GEN)on the expressions of estrogen receptors(ER),amplified in breast cancer 1(AIB1) and proliferating cell nuclear antigen(PCNA) in rat breast cancer.Method:Female SD rats were randomly divided into control,model,GEN treated,OP treated,GEN and OP combined treated groups.RT-PCR and immunohistochemical methods were used to detect the expressions of AIB1,ER,PCNA on normal mammary gland and mammary cancer.Results: Compared with control mammary gland,AIB1 mRNA,ER mRNA,PCNA and ER expressions were up-regulated in mammary cancer.Compared with mammary cancer in model group,the level of AIB1mRNA,ER mRNA and ER expressions were significantly decreased in GEN treated group,while they were significantly increased in OP treated group and PCNA expression was significantly increased too.Compared with OP treated group,the level of AIB1mRNA,ER mRNA,ER and PCNA expressions were partly decreased in GEN and OP combined treated group.Conclusion: Octylphenol can up-regulate the levels of AIB1,ER and PCNA expressions in mammary cancer and may increase the incidence of 7,12-dinmethylbenz[a]anthracene(DMBA)-induced mammary cancer.Genistin or genistein combined with octylphenol can downregulate the levels of AIB1,ER and PCNA expressions in mammary cancer,and inhibit theiincidences.

Objective To observe the effect of genistein,octylphenol,and their combination on proliferation of MCF-7 cells and explore the molecular mechanisms.Methods The MCF-7 cells were divided into four groups: control,5?10~(-5)mol/L genistein treated,8?10~(-6)mol/L octylphenol treated,genistein and octylphenol treated.The cell proliferation,cell cycle,phospho-L-tyrosine protein(PTPr),ER?,ER? mRNA and AIB1 mRNA expression of MCF-7 cells was observed by MTT test,flow cytometry,immunocytochemistry and RT-PCR.Results When MCF-7 cells were treated with 8?10~(-6) mol/L octylphenol or 5?10~(-5) mol/L genistein,or both for 72 h,the proliferation ratio was 12.98%,46.16% and 36.44% respectively;the percentage of MCF-7 cells at G_(2)/M were 12.98%,46.16% and 36.44% respectively;the apoptosis ratio of MCF-7 cells were 3.57%,11.41% and 8.24% respectively;the expression of PTPr was(62.84?9.80),(26.75?5.09),(39.15?7.83) respectively.Octylphenol increased the expression of AIB1 mRNA,but genistein and its combination with octylphenol inhibited the expression of ER? and decreased the expression of AIB1 mRNA in nucleus.Conclusion Octylphenol can promote the proliferation of MCF-7 cells,while genistein and its combination with octylphenol inhibit,which mechanism may be related to regulation of PTPr,AIB1 and ER expression.

Objective: To discuss the procedure and clinical effect of retroperitoneal laparoscopic nephropexy (RLN).Methods: From August 2001 to June 2006, RLN was performed on 28 female patients aged 26-45 years old (mean, 34±2.5) with symptomatic nephroptosis, including 15 with the right kidney, 12 with the left, and 1 with both. The preoperative complaint of patients included subjective symptoms (constant and recurring pain in 28 patients) and objective symptoms (upper urinary infections in 16, hematuria in 12, and upper tract obstruction in 12). One patient underwent nephropexy via the transperitoneal approach and the others underwent nephropexy via the retroperitoneal approach. A retroperitoneoscopic procedure was performed after positioning the patients in the flank position. Digital preparation of the retroperitoneal space was made and standardized trocar was placed. The key step of the surgery was complete exposure of the kidney within Gerota' fascia, which was aimed to separate the potential adhesions between the colon and kidney or between the inferior blood vessels of the kidney. Nephropexy was performed between the fibrous capsule at the lower pole of the kidney and the dissected psoas muscle, using three sutures placed by intracorporeal technique or the percutaneous needle both for introduction and removal of the suture; the sutures were separately tied over the sacrospinalis fascia. Results: The mean operative time was (125±9) min (ranging 115-240 min); the mean postoperative hospital stay was (9±1.2) days, largely owing to the required 5-12 days' bed rest. During a mean follow-up of (24±4.2) months(ranging 3 to 70 months), 3 patients had paresthesia, 5 had constant and recurrent ache, 20 were completely free of pain, and 4 had micro-hematuria. One patient had further episodes of pyelonephritis and upper tract obstruction after operation. Intravenous pyelogram(IVP) revealed that the ptosis incorporated into more than one vertebral body in 2 patients. Postoperative renal function test showed an improvement in renal function. Conclusion: RLN is mini-invasive and has less complication. The procedure should be considered as one of the optimal therapy for nephroptosis.

Objective:To compare the clinical outcomes of laparoscopic nephrectomy and open nephrectomy for radical treatment of renal cancer.Methods: Fifty-three patients with renal cell carcinoma(due to receive radical renal tumor resection) were randomly allocated into 2 groups: retroperitoneal laparocopic(RL) group(n=27) and open approach(OA) group(n=26).The operation time,hospital stay,use of painkiller,fasting period after operation,and blood loss during operation were analyzed and compared.All the cases were followed up for 6-12 months and the survival rates,wound healing,and carcinoma metastasis were recorded.Results: Patients in both groups were all successfully treated and their sexes,clinical stages,ages,and tumor sizes were all comparable.The (operation) time was similar in the 2 groups,with that of RL group being(66.66?10.37) min and of OA group being((69.08?)(11.22) min.)The fasting period in RL group([1?0.36] d) was significantly shorter than that in the OA group([2?0.68] d,P

OBJECTIVE:To fathom the overseas research method for the evaluation of pharmacoeconomics and to improve the development of pharmacoeconomics in China and provide scientific bases for clinical rational use of medicine.METHODS:The research paper abroad introducing the latest therapeutic scheme for breast cancer was taken as an example to expound the method of applying Markov model in the evaluation of pharmacoeconomics.RESULTS & CONCLUSION:Drawing useful experience from abroad about the analytical method and using decision analytic model for the study of pharmacoeconomics are conducive to the development of pharmacoeconomics in China.

Objective To study the expression of P16、Ki-67 gene protein in esophageal squamous cell carcinoma,and its relation with lymph node metastasis、 prognostis.Methods The SP immunohistochemical method was used to detect the expression of P16 、Ki-67 gene protein in 56 cases with esophageal squamous cell carcinoma,and its normal paraneoplastic rectal mucous tissues.Results The positive expression rate of P16 in normal paraneoplastic rectal mucous tissues was 89.29%(50/56),and 42.86%(24/56) in esophageal squamous cell carcinoma,there was significant difference between the two groups(all P＜0.05);the positive expression rate of Ki-67 in normal paraneoplastic rectal mucous tissues was 8.93%(5/56),and 33.93%(19/56) in esophageal squamous cell carcinoma,there was also significant difference between the two groups(all P＜0.05).In well differentiated groups of esophageal carcinoma the positive expression rate of P16 was significantly higher than that in moderately differentiated group、poorly differentiated group(P＜0.05);in bad progressed group of esophageal carcinoma the positive expression rate of Ki-67 was higher than that in good progressed group(P＞0.05);The positive expression rate of P16、Ki-67 in lymph node metastasis was significantly higher than that in without lymph node metastasis(P＞0.05).In the exist period after operation over three years groups,the positive expression rate of P16 57.69%(15/26) was significantly higher than that in the groups of the exist period after operation non-over three years 30.00%(9/30)(χ2 =4.36,P＜0.05),in the exist period after operation non-over three years groups the positive expression rate of Ki-67 46.67%(14/30) was higher than that in the exist period after operation over three years group 19.23%(5/26)(χ2 =4.68,P＜0.05).Conclusion P16、Ki-67 were objective markers to estimate the behaviors of esophageal squamous cell carcinoma and to evaluate the development and predict the mettastasis of tumors of patiens.

Objective To investigate the surgical procedure of restoring stenosed hilar bile duct by gallbladder lamella with cystic artery.Methods Based on extensive incising and hilar cholangioplasty,stenosed hilar bile duct was restored by gallbladder lamella with cystic artery.Results Among all the 12 pa tients,including 10 intrahepatic bile duct calculi patients,1 right hepatic duct injury and 1 left intrahepatic chaolangiocarcinoma,no cystic fistula,reflexed cholangitis and recurrent stone were noticed by post operative follow-up.Conclusion This procedure is a ideal method by remaining the normal function of Oddis sphincter while removing stenosis.

Objective To develop a sensitive and specific HPLC method to simultaneously determine the contents of puerarin, daidzin and daidzein in Puerariae Lobatae Radix. Methods The three compounds were obtained by ionic liquid based on dispersive liquid phase microextraction. The determination was carried out on a Phenomenex C18 column (250 mm×4.6 mm, 5μm) with a mixture of methanol-0.2%acetic acid (volume ration 45∶55) as mobile phase at a flow rate of 0.8 mL/min. The UV detective wavelength was 250 nm, and the column temperature was set at 35 ℃. Results The linear response ranged from 6.24×10-6-37.44 μg for puerarin (r=0.999 71), 5.44×10-6-27.20μg for daidzin (r=0.999 85), and 5.60×10-6-28.00μg for daidzein (r=0.999 94), respectively. Conclusion The method is quick, simple and repeatable for simultaneous determination of the contents of puerarin, daidzin and daidzein in Puerariae Lobatae Radix.

Objective To investigate the blood transfusion paths in elderly patients in order to ensure the blood transfusion safety in the elderly.Methods Clinical data of elderly patients (aged 60 and over) receiving blood transfusion were selected from January 2008 to December 2013.Data of blood transfusion in different clinical department were analyzed.Results From 2008 to 2013,the number of elderly patients receiving blood transfusion and transfusion volume showed the rising trends.Patients in Department of Cardiothoracic Surgery had the most blood transfusion volume among all surgical departments (9.3U/person),and plasma transfusion accounted for the most with plasma and red blood cells in a 3 ∶ 1 ratio.Patients in Department of Internal Medicine had the most blood transfusion volume among all non surgical departments (15.9U/person),and platelets transfusion accounted for the most with platelets and red blood cells in a 3 ∶ 1 ratio.The oldest patients on average receiving blood transfusion in the top 10 was from Department of Emergency (aged 77 years),and the most common cause was gastrointestinal hemorrhage.The youngest patients receiving blood transfusion in the top 10 was from Department of Cardiothoracic Surgery and Department of Gynecology (aged 69 years),and the most common cause was huge pleural mass and ovarian cancer.Conclusions Blood transfusion for elderly patients should be restrict the indications for transfusion strictly,choose the reasonable transfusion time,blood components and volume.Physiological and functional changes and clinical features of elderly patients should be paid attention to at the same time in order to reduce unnecessary transfusions,particularly the dependence on plasma transfusion and unnecessary collocated blood transfusion,to avoid the overload and adverse transfusion reactions.

Objective To compare the safety and short-term efficacy of thulium laser resection of the prostate (TMLRP) and bipolar transurethral plasmakinetic prostatectomy (TUPKP) for the treatment of benign prostatic hyperplasia (BPH) patients.Methods A total of 100 patients diagnosed with BPH were randomly divided into 2 groups:TMLRP group (50 cases) and TUPKP group (50 cases).There was no significant difference of preoperative variables such as age,prostate volume,PSA,IPSS,Qmax and PVR between the two groups (P＞0.05).The perioperative parameters and therapeutic effects were recorded and compared between the two groups.Results Comparison between TMLRP group and TUPKP group included:operating time ((61.2±24.2) min versus (30.1±15.9) min),catheterization time ((1.8±0.4) d versus (3.2±0.6) d)and postoperative hospital stay ((3.3±0.8) d versus (4.1±1.3) d).Significant differences in these parameters were found between the two groups(P＜0.05).Compared with TUPKP group,the blood loss and postoperative bladder irrigation were significantly less in TMLRP group.One month postoperatively,there were 4 cases of urethral stricture in TUPKP group.Three months postoperatively,IPSS,QOL,Qmax and PVR were significantly improved in both groups (P＜0.01),but no significant difference detected between the 2 groups (P＜0.05).Conclusions TMLRP is superior to TUPKP in term of safety and tolerability (decreased blood loss,complication rate and short recovery time),and as efficacious as TUPKP in efficacy.Compared with TUPKP,operating time were significantly longer in TMLRP group
