1.Analysis of etiology of bloodstream infection in children with hematologic disease by using PCR

Objective To explore the value of PCR for analyzing the pathogenic spectrum of bloodstream infection(BSI)in chil-dren with blood disease,and to provide a new thought for epidemiological survey of BSI.Methods A total of 80 children with blood disease in hematology department with suspected bacterial infections were recruited in the study,whose blood samples were collect-ed and detected by using blood cultivation(BC)and 16 S rDNA-PCR.The results obtained by the two methods were compared.Re-sults Among the 80 children,the positive rate of BC and PCR were 8.8% and 20.0%,respectively.There was significant differ-ence between the two methods(χ2 =5.82,P <0.05).Gram-positive bacteria and Gram-negative bacteria accounted for 68.7% and 31.3% respectively in 16 bacteria.The predominant pathogen was Staphylococcus epidermidis (31.3%).Conclusion Compared with BC,16 S rDNA-PCR and sequencing provide a new way for analyzing the pathogenic spectrum of BSI,which might be effective for epidemiological investigation.The pathogens of BSI are mainly Gram-positive bacteria in children of hematology department,and Staphylococcus epidermidis is the most common pathogen.
2.Role of neural stem cells in treating hypoxic-ischemic encephalopathy

BACKGROUND: Recently, researchers separate neural stem cells (NSCs) from neural system. As the progenitor of nerve cell and glia cells, NSCs have strong ability of self-renewal and proliferation, and play a very important role in repair process after nerve injury. OBJECTIVE: To review the repairing effect of NSC transplantation on hypoxic-ischemic encephalopathy. RETRIEVAL STRATEGY: Using the "Neural stem cells, brain, hypoxic-ischemic", we searched PubMed database for related articles published in English between 1993 and 2007. 131 articles were collected, and after the first trial, only articles ① related to neural stem cell transplantation for brain injury, and ②published in recently or in authoritative journals were selected. Repetitive researches were excluded. LITERATURE EVALUATION: The articles were about the studies of NSCs and randomized and controlled experiment for hypoxic-ischemic brain injury. Thirty-three English articles were included involving the biological characteristics of NSC, distribution, separation and culture in vitro, and the basic experiment or clinical research about the repair of hypoxic-ischemic brain injury by NSC. Of these articles, there are 7 review articles, and the others were about clinic or basic experiment researches. DATA SYNTHESIS: Researches show that after transplantation NSC can survive in host, furthermore, it proliferates and differentiates to repair the configuration and function of injured brain area. Even though we have achieved much in NSC transplantation for brain injury, yet there are many problems to be resolved, such as NSC source and conservation, the control mechanism of differentiation and proliferation when NSC is cultured in vitro. In addition, the amount or functional activity of NSCs acquired is lower, and the molecular biological features of cells in repaired tissue are uncertain. CONCLUSION: As a kind of ideal seed cell, NSC provides an approach for the repair of hypoxic-ischemic brain injury, and starts a new era of clinic therapy by stem cells.

Objective:To investigate the inhibitory effect of Epibueropyridinium A on aldose reductase.Methods: Aldose reductase was extracted from cattle crystalline lens.Besides Epibueropyridinium A,the reactive system also contained DL-glyceraldehyde,aldose reductase,and NADPH.The activity changes of aldose reductase were detected at 340 nm.Epalrestat was taken as the positive control.The inhibitory type,Ki and IC_(50) were determined by double reciprocal plot,quadratic drawing,and drawing of inhibitor's concentration to inhibitory ratio,respectively.Results: Epibueropyridinium A significantly inhibited the activity of aldose reductase in a competitive manner,with IC_(50) being 4.2 ?g/ml and Ki being 4.88 ?g/ml.Conclusion: Epibueropyridinium A is competitive inhibitor of aldose reductase.

Objective To study the morphology of the persistent foramen of Huschke (PHF)and the weakness of the remains of foramen of Huschke (WRHF)in the external auditory canal (EAC)by high resolution computer tomography (HRCT)in vivo. Methods The thin HRCT images of 525 cases elder than 5 years were retrospectively reviewed by two radiologists,to detect the PHF or WRHF,and to depict the precise location,size and the distance to the tympanic membrane.The incidence,sex differences, and morphologic data of PHF and WRHF were statistical analyzed.Results ①PHFs were found in 146 EACs of 40 males and 77 fe-males with the prevalence of 13.91%,and were found on both sides in 29 cases.Mean maximum axial diameter was 2.35 mm± 1.1 6 mm (0.6-5.8 mm)and sagittal diameter was 2.09 mm±1.2 mm (0.5-6.2 mm).Mean distance between the inner edge of PFH and the sulcus tympanicus was 1.61 mm±2.4 mm (0-10 mm).②WRHFs were found in 233 EACs of 58 males and 122 fe-males with the prevalence of 22.1 9%,and were found on both sides in 53 cases.Mean maximum axial diameter was 2.43 mm± 0.89 mm (0.8-5.4 mm)and sagittal diameter was 2.08 mm±0.73 mm (0.6-4.3 mm).Mean distance between the inner edge of WRPFH and the sulcus tympanicus was 1.55 mm±1.2 mm (0 - 9.9 mm).③The distance to the sulcus tympanicus was within 3 mm in 137 cases (90.73%)of PFHs and 221 cases (94.43%)of WRFHs.Conclusion Both PHF and WRFH are common by the CT morphological study in vivo.The medial part of EAC is predominant and female is predominant.The incidence and the location of PHF and WRFH are no significant difference.
8.Study on Poliomyelitis Eradication Strategy of in Hunan Province

Objective Through oral vaccination activity on polio,the epidemiological and virological surveillance of acute flaccid paralysis (AFP) case finds its way to develop, so that the realization of eradication on polio may be verified.Method Various kinds of immunization activities such as regular basic immunization, booster vaccination, intensified vaccination and mopping-up operation were adopted among children of the right age in the whole province. Zero case report and active surveillance on AFP were carried out to those population under 15 who had had consultation in medical unit above county level. Each reported case or found case was treated by individual case investigation, follow-up study and sample collection. All the samples were virus-isolated ad identified by RD, Hep-2 and L20B cells. Result Since 1991, an AFP case surveillance system has been built up. Up to the end of 2000, a total of 1,749 AFP cases had been reported in hunan province, from which 1,404 cases samples were collected, and another 218 cases samples of contacts were also collected. A sum of 7 polio wild virus strains was examined out. Since July 1993, no wild strain has been found (isolated). Conclusion Hunan Province has successfully intervened in the circus (spread) of polio wild virus, and hence realized the goal of polio eradication.
9.Influence and significance of atorvastatin on serum C-reactive protein level in acute cerebral infarction pa- tients.

Objective It is to study the influence of atorvatatin on serum C-reactive protein(CRP)level in acute cer- ebral infarction patients and to analyze its clinical significance.Methods 62 patients with acute cerebral infarction di- agnosed by CT or MRI were chosen and divided into two groups randomly.Both the groups were treated with normal method and the treatment group was given atorvastatin 20mg per day.Serum CRP levels in the two groups were measured by immunnturbidimetry within24h after final diagnosis and at two weeks the level decreased obviously in treatment.Re- slllts Serum CRP levels all increased in the patients.After treatment for two weeks the level decreased obviously in treatment group.The difference was significant compared with control group.Conclusion Atorvastatin can markedly reduce the serum CRP level in acute cerebral infarction patients and has important significance for the pathogenetic condi- tion change and prognosis of cerebral infarction.
10.Influence of some herbal medicines and foods on the INR of patients under warfarin therapy

Warfarin is a well-accepted drug used for preventing thromboembolic diseases. It is important to monitor the dosage of warfarin. Prothrombin time is one of the screening tests of extrinsic coagulation path,which is the first choice to monitor the oral use of warfarin. Standardized prothrombin time, namely international normalized ratio ( INR ) is used to adjust warfarin dosage in clinical practice. Many herbal medicines and foods may enhance the effect of warfarin therapy,and some of them may weaken this effect,which can result in severe clinical complications.
