1.Biological properties of gradient bioactive coatings on titanium surface in the skull repair

BACKGROUND:Titanium mesh has good clinical effect in repairing skul defects, but due to the lack of bone induction ability, the titanium mesh has a poor integration with the bone tissue. OBJECTIVE:To observe the biological properties of the gradient bioactive coating materials on the titanium surface in the skul repair. METHODS: Osteoblasts were co-cultured with the titanium mesh with or without gradient bioactive coatings for 14 days, and then cel proliferation was detected using MTT method. Seventy-one patients with skul defects were enroled, including 43 males and 28 females, aged 15-60 years, and were subjected to skul repair using the titanium mesh with (observation group, n=3) or without (control group, n=38) gradient bioactive coatings. During the postoperative folow-up of 12 months, the repairing effects and adverse reactions were observed in the two groups. RESULTS AND CONCLUSION:(1)In vitrocel culture: the cel proliferative ability was increased significantly in the observation group as compared with the control group at 8, 10, 12 and 14 days after cel culture. (2)In vivo repair: the hospital stay and wound healing time in the observation group were significantly lower than those in the control group (P< 0.05), and at the final folow-up, the postoperative recovery effect was significantly higher in the observation group than the control group (P < 0.05). The titanium meshes were fixed firmly in the two groups, with no floating, infection and exposure. These results show that the titanium mesh with gradient bioactive coating has good biocompatibility and osteoinduction capacity.
2.Influence of Telmisartan on peripheral calcineurin of rat with two kidneys one clip hypertension

Aim To study the effects of telmisartan upon serum calcineurin.Methods 92 male SD rats with the same age were randomly divided into control group (N), sham operation group (S), 2K1C+distilled water group (K) and 2K1C+telmisartan group (T).S rats were performed the open-abdomen surgery without being restricted any renal artery, but the K and the T rats were restricted their left renal artery. Beginning from the third week after the surgery, the K rats started to be treated with the intragastric infusion of distilled water 10 ml·(kg·d)~(-1) , while the T rats with telmisartan 10 mg·(kg·d)~(-1) .And after being treated for 2, 4 and 8 weeks, rats were respectively measured the systolic blood pressure (SBP), the diastolic blood pressure (DBP) of the abdominal aorta. Before and after the operation, ultrasonography with probe of 7.5 MHz was used to obtain the structure and functional indexes, such as IVSd, IVSs, LVPWd, and serum calcineurin were evaluated by ELISA and colorimetric assay kit.Result Compared with the S group and the N group, ① the results of blood pressure (SBP, DBP) were significantly higher in K group (all P <0.01), after use of telmisartan, blood pressure was significantly reduced(P <0.01);② the thickness of interventricular septal and left ventricular posterior wall at the end of diastolic and systolic were significantly higher in K group (all P <0.01), after use of telmisartan, the thickness of those declined(all P <0.01);③ the level and activity of serum calcineurin were significantly higher in K group (all P <0.01), after use of telmisartan, the level and activity of calcineurin significantly fell(P <0.01).Conclusion The serum calcineurin of artery was also raised in the left ventricular remodeling. Telmisartan ameliorates ventricular remodeling effectively, which may be associated with decreasing the expression of artery serum calcineurin.
3.Focal cerebral ischemia induces Alzheimer's disease-like pathological change in rats.

The changes in the tau protein phosphorylation and expression of bcl-2, and bax in rat parietal cortex neurons after focal cerebral ischemia-reperfusion (I/R) were explored, and the relationship between the tau protein phosphorylation and the expression of bax or apoptosis was clarified in order to elucidate the relationship between cerebral infarction and Alzheimer's disease. The rat focal cerebral I/R model was induced by occlusion of the right middle cerebral artery using the intraluminal suture method. The level of tau protein phosphorylation at Ser396, Ser404, Tyr231, Ser199/202 sites and the expression of bcl-2, bax and total tau 5 in rat parietal cortex during focal cerebral ischemia/reperfusion were detected by Western blot. The relationship between the tau protein phosphorylation and the expression of bax, or apoptosis was examined by TUNEL method and double-labeling immunofluorenscence method. The results showed that the level of tau hyperphosphorylation at Ser199 / 202, Ser396, Ser404, Tyr231 sites and the expression levels of bcl-2, and bax were significantly higher in I/R group than in the sham group, but the ratio of bcl-2/bax was decreased. Neuronal apoptosis, bax expression and the tau protein hyperphosphorylation were co-localized. It is suggested that Alzheimer's disease-like pathological changes occur after cerebral I/R. The highly abnormal phosphorylation of tau protein plays a key role in cerebral I/R-induced apoptosis. The cerebral infarction may contribute to Alzheimer's disease occurrence and development.
4.Levels and clinical significance of pentraxin 3 and C-reactive protein in patients with chronic obstructive pulmonary disease

Objective To investigate the levels and clinical significance of pentraxin 3 (PTX3) and high sensitive C-reactive protein(Hs-CRP) in patients with chronic obstructive pulmonary disease (COPD).Methods Seventy COPD patients at acute exacerbation stage were selected as the acute exacerbation group,70 COPD patients at disease remission stage were selected as remission group,and 70 healthy subjects were selected as control group.According to pulmonary function,70 COPD patients at acute exacerbation stage were divided into mild-to-moderate group(37 cases) and severe-to-very severe group (33 cases).The serum levels of PTX3,Hs-CRP,white blood cell count (WBC),neutrophils (NE) were measured.Results The serum levels of PTX3,Hs-CRP,WBC and NE of the acute exacerbation group and remission group [(0.89 ± 0.38) μg/L,(52.14 ± 4.25) mg/L,(12.13 ± 4.78) ×109/L,(79.68 ± 9.02)%;(0.47 ± 0.12) μg/L,(10.28 ± 3.42) mg/L,(8.02 ± 3.24) × 109/L,(67.24 ± 8.02) %] were higher than those of the control group [(0.18 ± 0.04) μg/L,(4.42 ± 2.02) mg/L,(4.97 ± 2.57) × 109/L,(63.38 ± 8.79) %] (t =213.252,245.168,48.647,53.241;121.426,124.352,21.425,24.352,all P <0.05).The serum levels of PTX3,Hs-CRP,WBC and NE of the acute exacerbation group were higher than those of the remission group (t =89.435,93.214,18.325,16.325,all P < 0.05).In COPD patients at acute exacerbation stage,the serum levels of PTX3,Hs-CRP,WBC and NE of the mild-to-moderate group and severe-to-very severe group [(0.77 ±0.32) μg/L,(41.23 ±3.74) mg/L,(8.24 ±4.48) × 109/L,(71.54 ±8.73)%;(1.13 ±0.43) μg/L,(59.32 ± 4.65) mg/L,(14.21 ± 5.02) × 109/L,(87.96 ± 9.36) %] were significantly higher than those of the control group [(0.18 ± 0.04) μg/L,(4.42 ± 2.02) mg/L,(4.97 ± 2.57) × 109/L,(63.38 ± 8.79) %](t =43.253,314.265,53.241,67.324;47.315,289.253,46.572,63.241,all P < 0.05).The serum levels of PTX3,Hs-CRP,WBC and NE of the severe-to-very severe group were higher than those of the mild-to-moderate group (t =38.942,267.425,46.537,58.932,all P < 0.05).There was positive correlation between serum PTX3 and Hs-CRP,WBC,NE in COPD patients at acute exacerbation stage (r =0.532,0.498,0.697,all P < 0.05).The serum Hs-CRP was positively correlated with WBC and NE (r =0.542,0.713,P < 0.05).Conclusion Serum levels of PTX3 and Hs-CRP are elevated in patients with COPD.The serum levels of PTX3 and Hs-CRP are associated with disease severity.
5.Is glaucoma suitable for community screening?

Glaucoma is the leading cause of irreversible blindness worldwide.Screening for glaucoma is a critical intervention to identify individuals at an earlier stage of the disease.However,there is controversy about the necessity of glaucoma screening.Screening is a critical intervention for primary prevention of disease.The screening which suited to the level of our economic development has important benefits for our country,society,families and individuals.Glaucoma has fulfils the criteria of WHO for population screening:(1) Glaucoma is a major public health problem in the world.(2) Glaucoma can be detected in its earlier stage.(3) There are suitable and acceptable examinations for glaucoma screening.(4) Early detection and subsequent treatment could delay or retard glaucoma disease progression.(5) With appropriate strategy design,glaucoma screening can creat good economic benefit.The screening strategies depend on the level of social economy development,the health policies of developed areas tend to prevent disease in its early stages.In poor areas,the limited medical resources are used to treat patients in the advanced stages.In the relatively developed area with complete health care facilities and incomplete referral mechanisms,screening for glaucoma is a critical intervention to identify individuals at an earlier stage of the disease.Glaucoma Screening project in Wenzhou suggested that screening should be implement in relatively developed regions of high density of population to improve efficiency;screening should aim for multiple eye diseases to lower the cost per unit benefit.
6.Focal Cerebral Ischemia Induces Alzheimer's Disease-like Pathological Change in Rats

7.Investigation on the current situation and influencing factors of mood disorders in patients with thoracic and lumbar osteoporotic fractures

Objective:To understand the current situation of mood disorders in patients with thoracic and lumbar osteoporotic fractures and to preliminarily explore its influencing factors.Methods:A cross-sectional survey method was used to select 212 patients with thoracic and lumbar osteoporotic fractures from Department of Orthopedics, West China Hospital, Sichuan University, from January 2018 to September 2020 as the research subjects. The general information questionnaire, Huaxi Emotional-distress Index, Pittsburgh Sleep Quality Index (PSQI) and pain Visual Analogue Scale (VAS) were used to investigate them.Results:The incidence of mood disorders in patients with thoracic and lumbar osteoporotic fractures was 10.38% (22/212). The results of multivariate regression analysis showed that age, days of injury, PSQI score and pain VAS score were the influencing factors of emotional disturbance in patients with thoracic and lumbar osteoporotic fractures ( t values were -3.72-7.66, all P<0.05). Conclusions:Patients with thoracic and lumbar osteoporotic fractures have mood disorders. Age, injury days, PSQI score and VAS score are related to the occurrence of mood disorders in patients with thoracic and lumbar osteoporotic fractures. More attention should be payed to the influencing factors of mood disorders in patients,and timely measures should be taken for individualized intervention.
8.Formula compatibility regulation for prevention and treatment of infantile recurrent respiratory infection based on data mining

Objective To explore the compound compatibility regulation of oral traditional Chinese medicines (TCM) for the prevention and treatment of infantile repeated respiratory infection.Methods The oral TCM compound literatures for the prevention and treatment of infantile repeated respiratory infection were collected from four databases,including China Biology Medicine disc,CNKI,VIP Periodical and Wanfanf Data.The retrieval time was from January 2006 to July 2006.Then the TCM oral compound prescriptions meeting the requirements were obtained by the literature arrangement and performed the terms standardized processing for extracting the information and establishing Excel table.Then the Kaiyuan software package R i386 3.3.0 was used to conduct the drug frequency,association rules analysis and clustering analysis.Results A total of 311 prescriptions were screened out and 208 kinds of traditional Chinese medicine were analyzed.It was found that the higher compatibility frequencies of TCM were atractylodes,radix glycyrrhizae,windproof,radix astragali,radix pseudostellariae,poria cocos,dried tangerine or orange peel,etc.;the strong association rules prompted that modified Yupinfeng Powder composed of core medicines such as astragalus,atractylodes and windproof was the basic formula for prevention and treatment of infantile repeated respiratory infection;the clusters analysis found some TCM societies such as emembranous Astragalus mongholicus-keel-oysters,Atractylodes-Poria cocos-licorice-Astragalus mongholicus-dangshen.Conclusion The drug frequency,association rules analysis and clustering analysis can reveal the compatibility rules of TCM prescriptions for preventing and treating infantile repeated respiratory to provide reference basis for optimizing the clinical prescription and improving the curative effect.
9.Study of gonadotropin-releasing hormone antagonist in management of hormonal sensitive prostate cancer

Objective:To explore the value of gonadotropin-releasing hormone (GnRH) antagonist in prostate cancer management.Methods:We retrospectively analyzed the data of 92 consecutive hormonal sensitive prostate cancer (HSPC) patients treated with GnRH antagonist from Jan 2019 to March 2022 in Fudan University Shanghai Cancer Center. The median (IQR) age at diagnosis was 70(65-76)years old. Median(IQR) serum prostate-specific antigen (PSA) level before treatment was 98.30 (32.50-436.75)ng/ml. The median (IQR) testosterone level was 12.30(1.51-18.44)nmol/L. Twenty-six(28.3%)cases were in M 0 stage, while 66(71.7%) were in M 1 stage at diagnosis. There were 67(72.8%)cases in ≥T 3 stage, and 54(58.7%)cases in N 1 stage.The Gleason score of 80(87.0%)cases was ≥8.The second generation androgen inhibitor was used in 58(63.0%)cases, and 21(22.8%)cases had specific gene mutation. Patients received a subcutaneously 240mg Degarelix in the first 28 days and 80 mg Degarelix following every 28 days. The pre-injection and 3 months post injection PSA and testosterone (T) level were collected. According to the proportion of patients with the largest decrease in PSA, the patients were divided into high response group (PSA decrease ≥99% after 3 months of use of Degarelix) and low response group (PSA decrease <99% after 3 months of use of Degarelix). Univariate and multivariate logistic analysis were used to analyze the risk factors affecting the treatment response of Degarelix. Results:Among the 92 prostate cancer patients, after 3 months Degarelix treatment, the median PSA value decreased to 0.64ng/ ml ( P <0.001), and the median testosterone value decreased to 0.45nmol/L ( P <0.001). After treatment, there were 48 cases in the high reaction group and 44 cases in the low reaction group. Before treatment, the median PSA in the high-response group was 100.00(67.11-444.25) ng/ml, higher than 88.50 (9.91-582.25) ng/ml in the low-response group, but not statistically significant ( P=0.077). The median testosterone level in the high response group was 13.82 (7.53-19.43) nmol/L, which was significantly higher than that in the low response group [4.61 (0.75-16.12) nmol/L, P =0.030]. After treatment, the median PSA in the high-response group was 0.22 (0.09-0.82) ng/ml, significantly lower than that in the low-response group [3.22 (0.19-15.88) ng/ ml, P<0.001]. The median testosterone value of the high reaction group was 0.40 (0.09-0.80) nmol/L and that of the low reaction group was 0.45 (0.02-0.65) nmol/L, which showed no significant difference ( P =0.826), and both reached the level of castration (<1.7nmol/L). Univariate analysis showed that age ≤ 65 years old was a good prognostic factor ( OR=0.333, 95% CI 0.119-0.810, P =0.017); T stage ( P =0.540), N stage ( P =0.363), M stage ( P =0.660), Gleason score ( P =0.834), application of second-generation antiandrogens ( P=0.238) and gene mutation ( P =0.525) were not related to Degarelix hyperresponsiveness. In multivariate analysis, age was the only independent favorite prognostic factors( OR=0.913, 95% CI 0.847-0.983, P=0.016). Conclusions:In the real world, GnRH antagonists significantly reduced the levels of testosterone and PSA in HSPC patients after 3 months of treatment regardless of TNM stage, Gleason score, and the second generation androgen inhibitor using.
10.Treating Attention Deficit Hyperactivity Disorder from the Lung Based on Circular Movement and Five Spirits Stored in Corresponding Viscera Theory

It is believed that the main pathogenesis of attention deficit hyperactivity disorder (ADHD) is the abnormality of the ethereal and corporeal soul that dominate perception, behaviour and sensory functions, that is, the left liver ethereal soul ascending too much while the right lung corporeal soul failing to descend, of which lung corporeal soul restlessness and lung failing to astringe and descend is the core. By analyzing the pathogenesis of attention defects due to “heart-lung-kidney-liver four dimensions circuit failure” and hyperactivity and impulsivity due to “liver-heart-lung chief and deputy disharmony” from the perspective of circular movement and five spirits stored in corresponding viscera theory, respectively, it is believed that the lung corporeal soul plays an important role in the onset of this disease, and further summarized that “the five zang (脏) organs are correlated, and the lungs are the pivot” is the pathogenic characteristics, and “the lung corporeal soul is restless, and the lungs fail to astringe and descend” is the core pathogenesis. It is proposed that deficiencies can be treated with self-made Jingning Formula (静宁方) modification to supplement the lungs, calm the corporeal soul, and nourish the source of yin. For deficiency leading to excess, medicinals of clearing the lung, calming the liver, and dissolving phlegm in addition to Jingning Formula can be used to clear metal and calm corporeal soul, inhibit wood and calm ethereal soul, dissolve phlegm and dispel stasis, so as to establish a differentiation and treatment system of “treating syndromes of the five zang organs and focusing on regulating the lungs”, and then provide theoretical reference for clinical diagnosis and treatment.
