1.The development and assessment of Knee Osteoarthritis Self-Care Questionnaire for Elderly

Objective To develop Knee Osteoarthritis Self-Care Questionnaire for Elderly (KOSCQE), and test reliability and validity of the scale. Methods Based on self care theory, with reference to International Knee Documentation Committee Subjective Knee Form, combined with literature analysis and through semi-structured interviews, the item pool of the scale was established. A total of 148 elderly were investigated by questionnaire survey. Results KOSCQE extracted 5 common factors with 26 items, and the total variance contribution rate was 68.972%;the item load factor was 0.576 to 1.872, item level content validity index was 0.901-0.928, factor in overall content validity index of 0.916;total Cronbach αcoefficient was 0.898, test-retest reliability was 0.864, and the confirmatory factor analysis showed that:χ2/df=1.09, goodness of fit index =0.942, root mean square error of approximation=0.020, adjusted goodness of fit index=0.964, normed fit index=0.960, comparative fit index=0.948, relative fit index=0.931, incremental fit index=0.918, parsimony goodness of fit index=0.891. Conclusions The reliability and validity of the KOSCQE of the elderly accord with the requirements of the measurement of the scale, it can improve the elderly's awareness of self care and quality of life of the knee osteoarthritis.
2.Propolis decreases the expression of MMP-9 in fibroblasts and monocyte-derived macrophage in vitro

Objective To explore the potential mechanisms of therapeutic effect of propolis in the treatment of diabetic wound. Methods Human fibroblasts(Fb) and monocytic THP-1 cells were routinely cultured at 37℃ in an environment of 95% air and 5% CO2 in RPMI medium containing 11mmol/l glucose, 15mmol/l HEPES and 5% fetal calf serum. To a portion of culturing THP-1 monocytes phorbol myristate actetate (PMA; 2?10 -7mol/L) was added. After 24 hours when approximately 75% of the monocytes became macrophage which attached to the plate. Propolis was added to a culture system of human fibroblasts(Fb), THP-1 monocyte and THP-1 monocyte-derived macrophage(Mac) with different concentration of propolis(10, 50, 100, 200?g/ml) and different glucose(5mmol/L; 25mmol/L), For each experiment the cells were cultured for 48 hours in the presence of Propolis. Then the media were collected and the protein expression of matrix metalloproternase-9 (MMP-9) were determined by gelatin zymography using a MDPF- labeled gelatin as substrate. The media were loaded onto an 8% SDS PAGE gel containing MDPF labeled gelatin (1%) after electrophoresis, the gels were placed on a long wave ultra violet light box and photographed. The area degraded by the MMPs was observed as dark bands on a light background. The MMPs were quantified by measurement of the band area using the Phoretix_1D_Advanced Digital Science Electrophoresis Documentation and Analysis System. 3.Nursing of patients with monochorionic twins undergoing selective feticide with bipolar electrocoagulation

Objective To summarize the key nursing points in patients with monochorionic twins undergoing selective feticide with bipolar electrocoagulation.Methods Selective feticide with bipolar electrocoagulation were performed in 68 monochorionic twins with one twin anomaly.And the perioperative care was performed on the patients.Results Operations were accomplished successfully in 68 cases.No complications,such as infections,premature rupture of membrane and so on,were found in all cases seven days after operation.Conclusions The nursing key points include preoperative psychological nursing by interpretation of the operations,monitoring fetal heart sounds during operations,and close observation of body temperature,uterine contractions and fetal heart rate in pregnant women after operations.Careful perioperative nursing is helpful for improvement of operative success and to ensure normal development of fetus.
4.Comparison between transabdominal and transcervical chorionic villus sampling in clinical application for prenatal diagnosis

Objective To compare transabdominal and transcervical chorionic villus sampling (TC-CVS) in application for prenatal diagnosis, and to summarize the experience of transabdominal chorionlc villus sampling (TA-CVS). Methods One hundred and nine TA-CVS between April 2005 and November 2007 and 69 TC-CVS between August 1999 and March 2005 were retrospectively analyzed. Results ( 1 ) The mean gestational age was (12.4±1.9) weeks at TA-CVS and (8.8±1.2) weeks at TC-CVS (P< 0.01). (2) Compared with TC-CVS, punctures were fewer (1.4±0.5) and (1.1±0.4) and specimen amount was more (9±5) mg and (17±5) mg in TA-CVS, and the success rate of one puncture (62.3% vs 87.2% ) was higher in TA-CVS. (3)Among the cases followed up, the incidence of vaginal bleeding(TA-CVS: 2 eases, 2.0% ; TC-CVS: 2 cases, 6.1% ) and spontaneous abortion (TA-CVS: 4 cases, 4.0%; TC-CVS: 1 cases, 3.0%) were not significantly different between the two groups(P>0.05).Conclusion TA-CVS appears to be a good method for early prenatal diagnosis with a wide range of indications, high success rate and sufficient specimen.
5.The discussion of the heat preservation performance monitoring of the blood transport case for daily use

Objective To discuss the methods of the heat preservation performance monitoring of the blood transport case and to provide the technical support for the safety of blood transportation.Methods At the different environment temperature,the amount of the cold resource was decided by the mass ratio of cold resource to blood and the temperature was automatically recorded by the intelligent temperature chip continuously,to monitor the changes of each monitoring point in the blood transport case.Results When the mass ratio of cold resource to blood was fixed at 1∶6,the cold chain of the blood transport case could keep the tempera-ture of 2-10 ℃ for 8 hours at the environment temperature of 12 ℃,It could keep the temperature of 2-10 ℃ for 4.5 hours at the environment temperature of 25 ℃,and it could keep the temperature of 2-10 ℃ for 2 hours at the environment temperature of 44℃.Conclusion When the mass ratio of cold resource to blood is fixed,as the environment temperature changes,the available time that the blood transport case keeps with the cold-chain requirement varies according to the results of the heat preservation per-formance monitoring of the blood transport case.
6.Analysis on results of blood bacterial contamination quality monitoring related indicators in a blood center

Objective To find the risk and hidden danger of blood bacterial contamination existence at present and to investigate the method for carrying out the bacterial contamination related quality monitoring.Methods According to the requirements of the blood collection and supplying related laws and regulations standards,the blood bacterial contamination situation was monitored by supervising the personnel during the blood collection and supplying,key equipments,key materials,environment monitoring and sterility test of blood products.Results The total eligible rate of quality monitoring indicators of blood bacterial contamination reached 99.8%,the eligible rate of blood products bacterial contamination monitoring was 100 %.Conclusion The hygiene quality of blood collection process and blood products all are in good condition.

Objective To evaluate the quality of pooled platelets leukocytes reduced after filtering out leukocytes using two man-ufacturers of leucocyte filters for pooled platelets and improving the preparation method.Methods Pooled platelets was prepared from 400 mL fresh whole blood by buffy coats(BC)method,after 1 6 h,(22±2)℃ holding period,pooled six bags of ABO-matched buffy coats.and then filtered with two manufacturers of leucocyte filter,divided into the control group and the experimental group. Before and after filtering,the numbers of platelet and leukocyte,pH,hypotonic shock response(HSR),platelet aggregation and CD62p expression were detected.Results Before filtering leukocytes,the platelet quality of two groups achieved the requirements of Chinese standards.The numbers of platelet and leukocyte,pH,CD62p expression(%)and platelet aggregation showed no signifi-cant difference between two groups(P >0.05).However,After filtering,the pH,platelet aggregation and the platelet recovery,the experimental group and the control group,were (6.53±0.60)vs(7.00±0.06)、(5.5±3.8)% vs (77.4±14.7)%,(86.8±4.3)%vs (90.6±2.7)%,showed significant differences (P <0.05);but the residual leukocytes,HSR and CD62p expression respectively were (3.00±4.00)×10 6/ bag vs (2.00±3.00)×10 6/ bag,(2.40±0.90)% vs (2.00±0.80)%,(7.30±5.90)% vs (5.60 ±3. 70)%,there were no significant differences(P >0.05).Conclusion The quality of pooled platelets leukocytes is reduced,after filte-ring out leukocytes with two manufacturers of leucocyte filters and improving the preparation method,achieves the requirements of Chinese standards.However,the leukocyte filters of experimental group might have influence on platelet aggregation and pH.
8.Nursing of patients undergoing amnioreduction by fast and negative pressure drainage

[Objective] To summarize the perioperative nursing points during the amnioreduction by fast and negative pressure drainage.[Methods] Amniodrainage and associated nursing care were performed in 93 hydramnios cases of pregnant women from January 2006 to December 2010,and the nursing key points were summarized.[Results] Operations were performed successfully in 93 hydramnios cases of pregnant women.No complications occurred in 92 eases 3 d after operation.Bellyache and uterine contraction occurred in one case 2h after operation,which indicated placental abruption,two dead fetuses were got out by cesarean section.[Conclusions] The nursing key points included preoperative psychological nursing by interpretation of the operation,monitoring fetal heart sounds and close observation of contrac-tions in pregnant women.Careful perioperative nursing for patients with hydramnios is important to improve the success rate and reduce postoperative complications.
9.Effect of dexamethasone on expression of interleukin-21 and phospho-STAT3 in a murine model of chronic asthma.

OBJECTIVETo investigate the effects of dexamethasone on the expression of interleukin-21 (IL-21) and phospho- STAT3 (p-STAT3) in a murine model of chronic asthma.
METHODSThirty-three female BALB/C mice were randomly divided into control group, asthmatic group and dexamethasone-treated group (n=11). In the latter two groups, asthmatic models were established by ovalbumin administration. The cells in the broncho-alveolar lavage fluid (BALF) were counted and classified. The airway inflammation was evaluated by HE staining, and the expressions of IL-21 and p-STAT3 in the lung tissue were detected by immunohistochemistry and Western blotting.
RESULTSThe total cell number and lymphocyte number in the BALF were significantly higher in asthmatic group than in the control group and dexamethasone group (P/0.01). Compared with the control group, the asthmatic group showed significantly increased protein expressions of IL-21 and p-STAT3 (P<0.05), which were reduced by dexamethasone intervention (P/0.05).
CONCLUSIONDexamethasone can inhibit the expression of IL-21 and p-STAT3 in the murine model of chronic asthma, suggesting the importance of IL-21/STAT3 signaling pathway in the therapeutic mechanisms of dexamethasone for asthma.

10.Myocardin Reverses Hypoxia-Inducible Factor-1α Mediated Phenotypic Modulation of Corpus Cavernosum Smooth Muscle Cells in Hypoxia Induced by Cobalt Chloride

Purpose:
We aimed to investigate the mechanism of phenotypic transformation of corporal cavernosum smooth muscle cells (CCSMCs) under hypoxic conditions in vitro.
Materials and Methods:
In this study, a hypoxia model was established using cobalt chloride (CoCl2). CCSMCs were treated with different concentrations of CoCl2 for varying time periods, and cell viability was assessed. Hypoxia-inducible factor-1α (HIF-1α), myocardin (Myocd) and phenotypic markers were detected in the CCSMCs. We also transfected the CCSMCs with si-HIF-1α and Ad-Myocd and evaluated the effects on phenotypic modulation of CCSMCs and the relationship between HIF-1α and Myocd was evaluated.
Results:
CoCl2 inhibited the viability of CCSMCs in a dose- and time-dependent manner, and treatment with 300 µM CoCl2 for 48 hours were the optimal conditions for establishing the hypoxia model. The results showed increased expression levels of HIF-1α and osteopontin and decreased Myocd, alpha-smooth muscle actin, and calponin levels in CCSMCs under hypoxia. HIF-1α knockdown reversed hypoxia-induced phenotypic transformation with elevated Myocd expression. Overexpression of Myocd also reversed the effect of hypoxia on the phenotypic switch, but did not affect HIF-1α expression.
Conclusions
Our findings showed that HIF-1α was involved in the effect of hypoxia induced by CoCl2 on CCSMC phenotypic modulation, and Myocd overexpression could inhibit this process. Thus, Myocd might be a potential therapeutic target for erectile dysfunction under hypoxia or HIF-1α activation.
