The medicine of critical disease is a clinical discipline that gradually sprang up in the late 1960s. Yet at present controversy still hats as to whether the discipline should be regarded as a separate one and it has not been ranked among the separate disciplines listed by the State Educational Committee. The authors, however, give an account of the customers and the current situation of disciplinary development of the discipline of critical disease ho the perspective of a discighne. They also offer an account of the organizational structure and the form of staff of ICUs, pointing out that ICUs have 3 subjects structurally and are divided into CategoriesⅠ,Ⅱ and Ⅲ. Except for ICUs in a few hospitals that are up to the level of Category n, ICUs in most hospitels in China are lower than or just up to the level of CategoryⅠ. ho the authorss give an introduction to the ho formsn of ICU management open, semi-opened-cpc ana closed.

3.Cytogenetic diagnosis on single lymphocyte of DMD patient with exon 50 deletion by two-time duplex PCR

Objective To perform the cytogenetic diagnosis on single lymphocyte of DMD patient with dystrophin gene exon 50 deletion.Methods Single lymphocytes of a DMD patient with dystrophin gene exon 50 deletion and normal volunteers were picked out and prepared for two-time duplex PCR.Results The rate of precise positive was 92%,91% and 93% in specimens of the patient(SRY positive,exon 50 negative),the male volunteer(SRY positive,exon 50 positive)and the female volunteer(SRY negative,exon 50 positive),respectively.Conclusion Two-time duplex PCR is fit for the genetic diagnosis of single lymphocyte from DMD patient with dystrophin gene exon 50 deletion.

5.Effect of gender matching on the outcomes of living-donor renal transplantation

Objective To evaluate the effect of gender matching on the outcomes of livingdonor renal transplantation.Methods A total of 419 cases of living-donor renal transplantation in our center were divided into male-donor-male-recipient (MDMR) group,male-donor-female-recipient (MDFR) group,female-donor-male-recipient (FDMR) group,female-donor-female-recipient (FDFR)group.The outcomes including graft and patient survival,acute rejection and renal function were analyzed retrospectively.Results Compared to MDMR group,MDFR group and FDFR group had lower Scr [(80.7±17.9),(87.4±21.9) μmol/L vs (120.3±72.5) μmol/L,all P < 0.05] and uric acid (UA) [(318.1 ± 86.4),(303.5 ± 66.9) μmol/L vs (358.4 ± 77.8) μmol/L,P < 0.05] 6 months after operation.Compared to MDFR group,FDMR group had higher Scr[(117.7±27.4) μmol/L vs (80.7±17.9) μmol/L,P < 0.01],UA [(371.0±92.4) μmol/L vs (318.1±86.4) μmol/L,P < 0.05] and lower glomerular filtration rate (GFR) [(70.4± 17.8) ml/min vs (79.6± 18.9) ml/min,P < 0.05].Compared to FDMR group,FDFR group had lower Scr [(87.4±21.9) μmol/L vs (117.7±27.4) μmol/L,P < 0.01] and UA [(303.5±66.9)μmol/L vs (371.092.4) μmol/L,P< 0.01].Compared to MDFR group,FDFR group showed lower GFR [(72.4±25.3) ml/min vs (82.7± 18.7) ml/min,P < 0.05] 1 year after operation.Compared to MDMR group,FDFR group showed lower UA [(322.9±69.7) μmol/L vs (376.0±66.2) μmol/L,P < 0.05] 2 years after operation.Compared to FDMR group,FDFR group showed lower Scr [(88.7 ±27.0) μmol/L vs (112.7±27.8) μmol/L,P < 0.05] and UA [(318.3 ±61.2) μmol/L vs (396.2± 100.3) μmol/L,P < 0.05] 3 years after operation.5 years after operation,there were no significant differences in above indexes,the incidence of slow graft function,acute rejection and survival of graft and patient among groups.Conclusions Male recipients of female donors have the worst renal function while female recipients have better outcomes after operation.
6.The significance of anatomical features of popliteal artery in popliteal angioplasty

Objective To define the significance of the anatomical division and features of popliteal artery in popliteal angioplasty.Methods This study involved 118 lower limbs in 113 cases of femoropoliteal occlusive disease undergoing angioplasty in our center between August 2011 and July 2013.A bent-knee angiogram was taken before recanalization of the lesion.The flexion pattern and the location of flexion points of popliteal artery were collected to study their relationship with the division of popliteal artery.A post-procedural bent-knee angiogram was taken in case of popliteal stenting to assess any change in flexion angulation and distal flow.Results There are diverse flexion patterns in popliteal artery,75.4% of the flexion points locate in P2 segment,18.6% in P1,and 6.0% in P3.On post-procedural bent-knee angiogram in 73 cases of popliteal stenting,10 had a more acute distal flexion angulation,distal artery kinking was observed in another 6 cases of which 5 had compromised distal flow.Artery kinking only occurred in cross-flexion-point stenting.Conclusions A better understanding of the division and anatomical features of popliteal artery can be obtained via bent-knee angiogram.This is of value to angioplasty for lesions in different popliteal divisions.
7.Killing K562 cells by RNA interference compared with imatinib

Objective To compare RNA interference (RNAi) with imatinib in killing K562 cells. Methods Design effective shRNA sequences special for bcr-abl silencing and insert them into the eukaryotic expression vector for RNAi by gene engineering. The recombinant plasmi(ts were then transfected into K562 cells. 48 hours later, the efficiency of transfection was identified by fluorescent microscope, bcr-abl mRNA level was detected by RT-PCR. Another group of K562 cells were treated respectively by imatinib with different concentration. All groups of K562 cells were finally analyzed in apoptosis, cell proliferation and phosphotyrosine-containing proteins. Results Both RNAi and imatinib induced apoptosis, decreased proliferation and reduced phosphotyrosine-containing proteins. Conclusion BNAi can kill K562 cells successfully as imatinib, and it may be a promising way to treat CML patients in clinic, especially for those who fail in imatinib or other chemotherapy.
8.Prevalence of Hepatitis B Virus Infection in Systemic Lupus Erythematosus Patients

Objective To investigate the prevalence of Hepatitis B virus(HBV)infection in systemic lupus erythematosus (SLE)patients.Methods HBV and HCV serological tests performed in the Gulou Hospital Affiliated to Medical College of Nanjing University from January 2010 to March 2015 were retrospectively investigated for analysis HBV and HCV infection rate.The clinical testing data of 866 SLE inpatients (SLE group)from January 2010 to March 2015 were retrospectively in-vestigated for analysis HBV and HCV infection rate.The serological tests performed in 1 795 health examination people (Control group)to estimate the HBV and HCV infection ratein general population using ELISA.Compare the difference of HBV/HCV infection rate between SLE group and Control group.Results In the SLE group,17 patients were HBsAg posi-tive,the positive rate was 1.96%.In the control group HBsAg postive 204 patients,the positive rate was 11.4%,there were significant differences between these two groups (χ2 =67.81,P <0.0001).The HBsAg positive rate was lower in male SLE patients compared with controls (5.26% VS 13.9%,χ2 =4.58,P <0.05).For the female SLE patients,the HBsAg positive rate was significantly lower than the control (1.64% VS 8.12%,χ2 = 35.65,P <0.0001).The HBsAg positive rate was lower in SLE group compared with control group among different age groups,and the difference was significant in 21~30, 31~40 and 41~50 age group (χ2 =21.86,22.78,20.36;all P <0.001).There had no statistical difference between SLE and control group for the HBsAb positive rate(Total,χ2 =0.50,P =0.48;Male,χ2 =0.12,P =0.73;Female,χ2 =2.00,P =0.16).Conclusion The prevalence of HBV infection in SLE patients was lower than general population.
9.Ribosomal protein S7 affects apoptosis of HeLa cervical cancer cells

Objective To preliminarily investigate the effect of ribosomal protein S7 on apoptosis of HeLa cervical cancer cells.Methods The previously constructed recombinant plasmid pIRES2-EGFP-RPS7 was transfected into HeLa cells,the empty vector pIRES2-EGFP transfected cells as control.Enhanced green fluorescent protein(EGFP)expressing cells were quantified by flow cytometry,and RPS7 protein level was also determined by Western blotting.Cell apoptosis of both RPS7 over-expression cells and knockdown cells were evaluated by flow cytometry after staining using allophycocyanin labeled Annexin-V.Results Apoptotic cell level in the obtained RPS7 transient over-expression HeLa cells was significantly higher than that of vector con-trol cells [(1 0.00 ±0.60)% vs (5.73 ±0.61 )%],with a statistic difference (t =8.63,P =0.001 ). Moreover,the apoptotic level in RPS7 knockdown cells was lower than that in control cells [(3.08 ± 0.49)% vs (5.97 ±0.63)%],with a statistic difference (t =6.40,P =0.003).Conclusion Up-regula-tion of RPS7 may promote apoptosis,while down-regulation of RPS7 may inhibit apoptosis of HeLa cells,indi-cating that RPS7 may play roles in regulating cell apoptosis.

OBJECTIVE:To study the development of community pharmaceutical care.METHODS:The significance of community pharmaceutical care was elaborated based on its status quo analysis,and the way to improve the pharmaceutical care was discussed theoretically.RESULTS&CONCLUSIONS:Patient-centered community pharmaceutical care is the primary responsibility of community pharmacists,the community pharmacists should strengthen their study,update their knowledge structure and play an important role in community pharmaceutical care.
