1.Nutrients Intake, Zinc Status and Health Risk Factors in Elderly Korean Women as Evaluated by the Nutrition Screening Inistiative (NSI) Checklist.

The purpose of this study was to investigate whether the health status as determined by a self-assessment checklist was related to the nutritional status of elderly Korean women. We interviewed 59 female subjects living in the Daegu area aged over 55 years. Information on their dietary intake on two consecutive days was collected by the 24-hour Recall Method. Their body compositions were measured using a Bio-electrical Impedence Fatness Analyzer. Their zinc status was evaluated according to their dietary zinc intake and urinary zinc excretion in urine samples collected for 24 hours. Their heath status was determined by a NSI Checklist. While 50.9% of the total subjects belonged to a moderate nutritional risk group, 23.7% of the subjects were at a high nutritional risk. It appeared that only 25.4% of the subjects were evaluated as being healthy. Therefore, the overall nutritional health status of the elderly females belonged to the moderate nutritional risk group. Except for vitamin C, the average daily nutrient intake of the subjects was below the level of the recommended dietary allowances (RDA) for Koreans. The lower the nutritional health score, the better was their dietary intake of energy (p < .05), protein (p < .001), and zinc (p < .01). A positive relationship between their nutritional health score and body fat ratio was also observed (p < .05). Therefore, we suggest that the NSI Checklist could be used as a practical screening tool at the beginning of nutritional surveillance of the elderly in the community.

2.Zinc Status and Taste Acuity of Old and Young Women.

In an attempt to figure out the relationship between zinc status and taste acuity of old and young women, dietary zinc intake, urinary zinc excretion, and taste acuity were determined for 118 women. Zinc intake was measured by 2-day food records and food frequency method. Urinary zinc excretion was measured from urine samples collected for twenty four hours. Body fat, lean body mass (LBM), and total body water were measured by bio-impedence. Average dietary zinc intake by food record was 4.15+/-1.33mg (=35% if Korean RDA) for the old women and 5.41+/-2.76mg (=25% of RDA) for young women. When zinc intake was measured by a frequency method, the average intakes of the old and young women were 3.5+/-1.7mg 4.5+/-1.9mg, respectively. It appears that dietary zinc intake of young women was significantly higher than that of the old women. Average urinary zinc excretion of the subjects was 0.27+/-0.16mg in the elderly and 0.24+/-0.13mg in young women, which indicated a marginal zinc status. However, zinc status was not significantly different between old and young women. Correlation analysis indicated that zinc intake and urinary zinc excretion were positively related to BMI and LBM in young women. The old women (m=49) showed significantly higher taste detection thresholds than young subjects (n=47) for both sweet and salty tastes. Recognition thresholds for sodium chloride and sucrose were not significantly different between old and young women. The lower the taste thresholds for salty taste, the higher the average dietary zinc intake. However, taste perception concentration was not related to the urinary zinc excretion level.

4.Factors Associated with Health Behaviors in Thyroid Cancer Survivors

In this study, we compared health behaviors, including current smoking, drinking, and physical inactivity, in thyroid cancer survivors and non-cancer controls and investigated the factors associated with unhealthy behaviors among survivors. Baseline data from the Health Examinees study, collected from 2004 to 2013, were used. Thyroid cancer survivors (n = 942), defined as those who had received a clinical diagnosis of thyroid cancer, and 9,420 matched non-cancer controls without past history of any cancer were included in the analysis. The prevalence of smoking, alcohol consumption, and physical inactivity in thyroid cancer survivors were 2.3%, 26.6%, and 52.0%, respectively, with adjusted OR (aOR) and 95% CI between survivors and non-cancer controls of 0.35 (95% CI = 0.08-1.56), 0.46 (95% CI = 0.29-0.74), and 0.65 (95% CI = 0.44-0.96). The prevalence of unhealthy behaviors was higher among male thyroid cancer survivors than female thyroid cancer survivors. In thyroid cancer survivors, those who smoked were more likely to drink (aOR = 4.55 [95% CI = 1.61-12.85]) and those who were physically inactive were less likely to drink (aOR = 0.58 [95% CI = 0.42-0.82]). Current drinking and physical inactivity contributed to a higher likelihood of smoking (aOR = 4.31 [95% CI = 1.35-13.73] and 6.34 [95% CI = 1.65-24.34]). Thyroid cancer survivors had better health behaviors than the non-cancer controls.However, some survivors still had unhealthy lifestyles, especially displaying a clustering of unhealthy behaviors. Thus, health behavior promotion through medical intervention is important for thyroid cancer survivors.
5.Secondary Malignancies in Multiple Myeloma in Korean Patients: A Nationwide Population-Based Study

Purpose:
This study investigated the incidence of secondary malignancy in multiple myeloma (MM) patients compared with that in the general population using a population-based database covering all residents in Korea.
Materials and Methods:
Based on the national health insurance system in Korea, all people primarily diagnosed with MM between January 1, 2010 to December 31, 2018 were identified. A total of 9,985 MM patients aged ≥ 20 years in Korea were included.
Results:
Among them, 237 (2.4%) developed secondary malignancies by 2018. The standardized incidence rates (SIRs) of all secondary malignancies in MM patients were 0.87 (95% confidence interval [CI], 0.76 to 0.98), with a higher incidence of hematologic malignancies than in the general population with an SIR of 3.80 (95% CI, 2.61 to 5.00). The incidence rates of both lymphoid malignancy (SIR, 3.56; 95% CI, 2.31 to 4.82) and myeloid malignancy (SIR, 3.78; 95% CI, 1.16 to 6.39) were higher in MM patients than in the general population. In contrast, a lower incidence of solid cancer was observed in MM patients than in the general population (SIR, 0.76, 95% CI, 0.65 to 0.86). There was no significant difference in survival in MM patients without secondary malignancies, with hematologic malignancy, and with solid cancer (p=0.413).
Conclusion
MM patients had a greater risk of secondary malignancies, especially hematologic malignancies, than the general population. Future studies with a focus on analyzing patients’ history, treatment details, and genetic information in various stages of MM patients are needed to better understand the mechanism behind this increased risk.
6.Associations between Serum 25-hydroxyvitamin D and Consumption Frequencies of Vitamin D Rich Foods in Korean Adults and Older Adults.

OBJECTIVES: This study was conducted to investigate the vitamin D status and to determine the association between serum 25-hydroxyvitamin D [25(OH)D] concentrations and consumption frequencies of vitamin D rich foods in Korean adults and older adults. METHODS: Subjects were 10,374 adults and 2,792 older adults participating in the 2008-2009 Korea National Health and Nutrition Examination Survey (KNHANES). Consumption frequencies of vitamin D rich foods were estimated by using a qualitative food frequency questionnaire (FFQ). Eleven food items such as beef, egg, mackerel, tuna, yellow corvina, pollack, anchovy, mushroom, milk, yogurt, and ice cream were selected as vitamin D rich foods based on previous research. RESULTS: The proportions of deficiency (< 12 ng/mL), inadequacy (12-20 ng/mL) and sufficiency (> or = 20 ng/mL) of serum 25(OH)D concentrations from June to November and December to May in adults were 8.8%, 42.3%, 48.8%, and 28.2%, 52.8%, 19.1%, respectively. The proportions of deficiency, inadequacy and sufficiency of serum 25 (OH)D concentrations from June to November and December to May in older adults were 10.1%, 32.4%, 57.5%, and 24.1%, 45.4%, 30.5%, respectively. The mean serum 25(OH)D concentrations in adults were positively related to the consumption frequencies of mackerel, anchovy, all fish, milk and milk.dairy products. The mean serum 25(OH)D concentrations in older adults were positively related to the consumption frequencies of yellow corvina and negatively related to the consumption frequencies of ice cream. CONCLUSIONS: Our results suggest that Korean adults were more deficient in serum 25(OH)D concentrations than older adults. The consumption of vitamin D rich foods may affect vitamin D status in Korean adults. Further studies are required to confirm these findings.

The objectives of this study were to investigate the status of vitamin D in Korean adolescents and to determine the association between serum 25-hydroxyvitamin D (25(OH)D) concentration and consumption frequencies of vitamin D food sources by season (June to November and December to May). The subjects were 1,579 adolescents aged 12-18 years participating in the 2008-2009 Korean National Health and Nutrition Examination Survey (KNHANES). Consumption frequencies of vitamin D food sources were estimated by using a qualitative food frequency questionnaire (FFQ). Thirteen food items were selected as vitamin D food sources including beef, egg, mackerel, tuna, yellow corvine, pollack, anchovy, mushroom, milk, yoghurt, ice cream, all fish and dairy products from the FFQ based on previous research. The data was analyzed using proc survey procedures. The deficiency (5.25-12 ng/mL), inadequacy (12-20 ng/mL) and sufficiency (> 20 ng/mL) proportions of serum 25(OH)D from June to November and December to May were 9.9%, 51.4%, 38.7%, and 39.4%, 51.4%, 9.2%, respectively. Mean serum 25(OH)D concentration was positively related to the consumption frequencies of mackerel, anchovy, all fish and milk. These results suggest high proportion (> 61%) of Korean adolescents were vitamin D deficiency or inadequacy, and serum 25(OH)D was associated with the consumption of vitamin D food sources including fish and milk.

8.Implementation of ISO45001 Considering Strengthened Demands for OHSMS in South Korea: Based on Comparing Surveys Conducted in 2004 and 2018

Background:
According to the previous studies, the work-related accident rate decreased in Korea after the introduction of occupational health and safety management system (OHSMS), but there were several disasters in Korea such as subway worker's death at Guui station in 2016 and the Taean thermal power plant accident in 2018, which escalated the social demand for safety. In 2018, OHSMS became an international standard, as ISO45001 was announced.
Methods:
A survey was conducted to research the implementation status of OHSMS and changes in people's perception, and the results were compared with those of a past survey.
Results:
Enhanced social demand and various stakeholders' (not only buyer) needs, and social responsibility are perceived as the motivation for the introduction of OHSMS rather than legal compliance or customer demand. In the questionnaire about problems with the implementation of OHSMS, the factors with higher response rate in 2018 than 2004 were “excessive cost” and “complicated documentation management.” In the questionnaire about how to promote OHSMS in organizations, most people answered “reduction of workers' compensation insurance rate” in 2004, but most people answered “exemption from health and safety supervision” in 2018.
Conclusion
For the effective implementation of ISO45001, emphasis is placed on social demand, training to recognize health and safety as a part of management, and the reduction of certification and consulting costs to promote the introduction of OHSMS. Incentives such as insurance premium cuts and exemptions from health and safety supervision are needed.

10.Coronavirus Disease 2019 and Diabetes: The Epidemic and the Korean Diabetes Association Perspective

Diabetes has been associated with more severe outcomes and higher mortality in coronavirus disease 2019 (COVID-19) patients compare to morbidity and mortality in patients without diabetes. Several mechanisms may play a role in this greater morbidity and mortality, especially uncontrolled hyperglycemia, an impaired immune system, pre-existing proinflammatory states, multiple comorbidities, and dysregulated angiotensin-converting enzyme 2 signaling. Thus, the diabetes medical community emergently needs to know about COVID-19 and its effects on patients with diabetes, as they must take precautions to carefully manage these patients during the COVID-19 pandemic. The Korean Diabetes Association provides some guidance and practical recommendations for the management of diabetes during the pandemic. This report provides insight into the association between diabetes and COVID-19, proper management of diabetes in patients with COVID-19 and an official suggestion by the Korean Diabetes Association for managing the COVID-19 outbreak.
