2.Implant Folding Phenomenon in Cohesive Gel; Diagnosis of Suspected Implant Rupture in MRI Finding.

Since the introduction of the 4th generation silicone gel implants, number of breast augmentation using so-called 'cohesive gel' implants has increased rapidly. Still, implant rupture remains the most annoying complication of breast implant surgery. Recently, magnetic resonance image (MRI) is used as the gold standard in diagnosis of implant rupture. We experienced patients diagnosed as implant rupture with MRI finding, whose implants were found not to be ruptured, but only folded. These patients underwent breast augmentation with cohesive gel and presented to our center with complaint of breast asymmetry and texture-related discomfort. We recommended MRI imaging for diagnosis and the result of radiological diagnosis were 'rupture of implant'. After surgical exploration, we found intact implants without rupture. They were severely folded and fixed with folded form. With our experience, we conclude that cohesive gel implants, due to their more cohesive and form-stable characteristics, sometimes tend to fold and fix in stable folded form and in such cases, MRI image can be similar with the image of ruptured implant. Therefore, in diagnosis of cohesive gel rupture, surgeons should consider both radiologic finding and physical examination carefully before the decision of exploration.

4.Formulation of the Scope and Key Questions of the Guideline Recommendations for Immunosuppressive Treatment in Kidney Transplantation

BACKGROUND: Although a growing number of guidelines and clinical researches are available for immunosuppressive treatment of post-transplantation, there is no clinical practice guideline for the care of kidney transplant recipients in Korea. Selection of a researchable question is the most important step in conducting qualified guideline development. Thus, we aimed to formulate key questions for Korean guideline to aid clinical decision-making for immunosuppressive treatment. METHODS: Based on previous published guidelines review, a first survey was constructed with 29 questions in the range of immunosuppressive treatments. The experts were asked to rate the clinical importance of the question using a 5-point Likert scale. The questions reached 60% or more from the first survey and additional new questions were included in the second survey. In analyzing the responses to items rated on the 9-point scale, consensus agreement on each question was defined as 75% or more of experts rating 7 to 9. RESULTS: In the first survey, 50 experts were included. Among the 29 questions, 27 were derived to get 60% or more importance and 3 new questions were additionally identified. Through the second survey, 9 questions were selected that experts reached consensus on 75% and over of the options. Finally, we developed key questions using PICO (patient, intervention, comparison, and outcome) methodology. CONCLUSION: The experts reached a high level of consensus on many of key questions in the survey. Final key questions provide direction for developing clinical practice guideline in the immunosuppressive treatment of transplantation.

6.Cutaneous Müllerian Cyst on the Ankle: A Case Report

Cutaneous Müllerian cyst (CMC) is a rare benign cystic lesion that typically occurs on the lower extremity of young females shortly after puberty. They have been widely regarded as Müllerian heterotopias due to the morphological similarities of the cyst lining cells to the epithelium of the fallopian tubes. A 16-year-old female presented with a two-year history of a solitary, subcutaneous mass involving the left ankle. She had no specific symptoms associated with the skin lesion. However, the lesion had been growing in size. Histopathological findings revealed cystic structures lined by pseudostratified ciliated cuboidal to columnar cells, accompanying intraluminal papillary projection. The immunohistochemical staining profiles, including positive staining for estrogen and progesterone receptors, were characteristic of the fallopian tube epithelium. After being surgically removed, the lesion was cured without recurrence. Based on her typical presentation and histopathological findings, we present this patient as a case of CMC.
7.Discordance in Secular Trends of Bone Mineral Density Measurements in Different Ages of Postmenopausal Women

Background:
Age-adjusted bone mineral density (BMD) in postmenopausal women decreases in developed countries whereas incidence of osteoporotic fracture decreases or remains stable. We investigated secular trends of bone density from 2008 to 2017 among different age groups of postmenopausal women.
Methods:
We analyzed BMD data obtained from health check-ups of 4,905 postmenopausal women during three survey cycles from 2008 to 2017. We divided them into 3 groups by age (50–59 years, 60–69 years, and 70 years or more) and observed the transition of lumbar and femoral BMD in each group, before and after adjusting for variables that may affect BMD.
Results:
Age-adjusted BMD, bone mineral content (BMC), and T-score demonstrated a declining trend over the survey period at lumbar spine (−2.8%), femur neck (−3.5%) and total femur (−4.3%), respectively. In the analysis for the age groups, the BMD, BMC, and T-score presented linear declining trend (−6.1%) in younger postmenopausal women while women aged over 70 or more showed linear increasing trends (+6.3%) at lumbar spine during the survey period. Femoral neck and total femur BMD demonstrated a declining linear trend only in the 50–59 and 60–69 years groups (−5.5%, −5.2%, respectively), but not in the 70 years or more group.
Conclusion
BMD in younger postmenopausal women has decreased considerably but has increased or plateaued in elderly women. This discordance of BMD trends among different age groups may contribute to decreased incidence of osteoporotic fracture despite a recent declining BMD trend in postmenopausal women.
8.Exploring the Relationship between Psychosocial Risk Factors and Sudden Unexplained Infant Death: A Study of Autopsy Cases from a Perspective of Child Welfare

Infants who are born between 24 hours and 1 year of age require extreme caution in their care due to their fragility. However, if there are multiple risk factors associated with the infant, caretaker, household, and surrounding circumstances, providing proper and appropriate care becomes problematic. This difficulty in caring for infants can contribute to abuse, neglect, or even death. This study investigates unexplained infant deaths that occur in the presence of multiple psychosocial risk factors. This study aimed to explore the relationship between various psychosocial risk factors and sudden, unexplained infant deaths. We examined nine cases from the autopsy archive of the National Forensic Service using a statistical approach. Among these cases, three were subject to legal action, while six were closed without further legal process. Although it was difficult to establish a clear relationship between death and abuse or neglect in the six cases, all were found to have experienced a harsh environment that was similar to abuse or neglect. We discuss the implications of our findings for understanding infant deaths and legal outcomes and propose a new framework to understand the deaths of infants.
9.A Split-Face, Single-Blinded, Randomized Controlled Comparison of 532 nm Picosecond Neodymium-Doped Yttrium Aluminum Garnet Laser versus 532 nm Q-Switched Neodymium-Doped Yttrium Aluminum Garnet Laser in the Treatment of Solar Lentigines

BACKGROUND:
Solar lentigines are the most common form of benign epidermal pigmentation and one of the major cosmetic concerns in Korea. A 532 nm Q-switched neodymium: yttrium-aluminum-garnet (QSND) laser is typically used for the treatment, but the occurrence of post-inflammatory hyperpigmentation (PIH) is not rare. Recently, the use of picosecond (PS) lasers has emerged in pursuit of better outcomes.
OBJECTIVE:
To objectively compare the efficacy and safety of 532 nm PS and QSND lasers for the treatment of solar lentigines.
METHODS:
Twenty patients with solar lentigines were enrolled in a prospective, randomized split-face, single-blind study. One side of each face was treated using a 532 nm PS laser, and the other side using a 532 nm QSND laser. After one treatment, all patients were followed up for evaluation after 2, 4, 8, and 12 weeks. The clinical clearance was assessed by three blinded dermatologists using a 5-point quartile improvement scale (QIS). Subjective satisfaction, development of PIH, pain scale during treatment, and adverse problems were also recorded.
RESULTS:
Clinical clearance measured by QIS showed that the PS laser was more effective than the QSND laser. Subjective satisfaction and pain scale did not significantly differ between the two groups. The incidence of PIH was 5% in sides treated with the PS laser, and 30% with the QSND laser.
CONCLUSION
Both 532 nm PS laser and QSND laser were effective for the treatment of solar lentigines, but the PS laser was more effective with less PIH development.
10.Prevalence of and factors associated with anxiety and depression in Korean patients with newly diagnosed advanced gastrointestinal cancer.

BACKGROUND/AIMS: The purpose of the present study was to assess the prevalence of and factors associated with anxiety and depression in Korean patients with advanced gastrointestinal cancer. METHODS: One hundred and twenty consecutive patients with newly diagnosed, advanced gastrointestinal cancer who were scheduled to receive palliative chemotherapy between July 2012 and June 2014 were enrolled in this observational prospective study. Anxiety and depression were assessed using the Hospital Anxiety and Depression Scale (HADS) and Patient Health Questionnaire-9 (PHQ-9). RESULTS: Thirty-seven patients (30.8%) had anxiety or depression with clinical significance according to HADS or PHQ-9. Multivariate analysis identified lower performance status (odds ratio [OR], 4.19; 95% confidence interval [CI], 1.22 to 14.35; p = 0.023), gastric cancer (OR, 5.39; 95% CI, 0.37 to 78.23; p = 0.018), and knowledge of advanced cancer (OR, 15.07; 95% CI, 1.80 to 125.90; p = 0.012) as significantly associated with anxiety or depression. Twenty-one patients with anxiety or depression visited the psycho-oncologic clinic. In these patients, PHQ-9 score (p = 0.008), global health status (p = 0.023), fatigue (p = 0.047), and appetite loss (p = 0.006) improved from baseline to 3 months after study enrollment. CONCLUSIONS: Approximately 30% of Korean patients with advanced gastrointestinal cancer had anxiety or depression. The prevalence of anxiety or depression was higher in patients with poor performance status, gastric cancer, or knowledge of advanced cancer. Psychiatric interventions may be effective in reducing depression and improving quality of life in cancer patients with anxiety or depression.
