1.Intestinal Malrotation with Concurrent Portal Vein and Superior Mesenteric Vein Thromboses.

Intestinal malrotation with volvulus is generally presented as a bilious vomiting and acute intestinal obstruction in the newborn period. It could compromise vascular supply of the small bowel secondary to torsion of superior mesenteric artery (SMA) and without urgent surgical management, it could lead to detrimental outcomes such as transmural bowel infarction and sepsis. However, in chronic cases, it is rarely obstructs the vascular supply and propagates to an acute bowel infarction. Rarely, chronic malrotation with midgut volvulus may not reduce the mesenteric blood supply because of collateral vessels, and the chronically stagnated blood flow of the superior mesenteric vein (SMV) favors thrombus formation within the lumen. The recommended treatment is Ladd's procedure and anticoagulation therapy. The authors present an unusual case of intestinal malrotation with chronic volvulus resulting in superior mesenteric vein and portal vein thrombosis in a 28-year-old patient.

2.Effect of Maternal Anxiety on Parenting Stress of Fathers of Children With ADHD

Background:
Hyperactivity, inattention, and impulsivity of children with attention deficit hyperactivity disorder (ADHD) increase parenting stress and familial conflict. Among parentrelated factors, maternal mental health has been studied in-depth, but studies on paternal factors in this context are scarce. This cross-sectional study was conducted of children with ADHD and their parents in South Korea. We investigated the relationships between ADHD symptom severity of children and the mental health of their mothers and fathers.
Methods:
The study included 70 children with ADHD and their 140 married heterosexual parents (70 fathers and 70 mothers). Children completed the Child Depression Inventory and State-Anxiety Inventory for children, and their parents completed the Korean ADHD rating scale-IV, Adult ADHD self-report scale, State-Anxiety Inventory, Patient Health Questionnaire-9, and Parental Stress Scale.
Results:
There was a significant positive correlation between children’s ADHD symptoms and maternal anxiety symptom severity, whereby more severe ADHD symptoms were associated with more severe maternal anxiety symptoms. There was also a significant positive correlation between maternal anxiety symptom severity and paternal parenting stress severity, whereby more severe maternal anxiety was associated with more severe paternal parenting stress. A mediation model showed that paternal parenting stress severity was not directly related to children’s ADHD symptoms, but the severity of maternal anxiety mediated this relationship.
Conclusion
The present study found the importance of mental health in mothers of children with ADHD and the interrelatedness of mental health within families. Future assessments and treatment of children with ADHD should include both the children and their parents.
3.Rhabdomyolysis induced by venomous snake bite

Purpose:
Despite previous studies reporting the development of rhabdomyolysis (RM), this affliction tends to be neglected as an envenomation sign in South Korea. The current retrospective study investigates the prevalence and prognosis of RM after a snakebite. We further searched for predictors of snakebite-induced RM, which can be observed at presentation.
Methods:
This study included 231 patients who presented to the ED within 24 hours after a snakebite. The patients were classified according to the severity of RM, and the data, comprising baseline characteristics and clinical course including the level of creatine kinase (CK), were collected and compared according to the severity of RM.
Results:
The prevalence of RM and severe RM were determined to be 39% and 18.5%, respectively. Compared to the group without RM or with mild RM, the group with severe RM had a higher grade of local swelling, a higher frequency of acute kidney injury and neurotoxicity, and a greater need for renal replacement therapy and vasopressor administration. However, the incidence of acute renal injury in the RM group was 7.7%, with two patients needing renal replacement therapy. No mortalities were reported at discharge. Results of the multinomial logistic regression model revealed that the WBC levels are significantly associated with the risk of severe RM.
Conclusion
RM should be considered the primary clinical sign of snake envenomation in South Korea, although it does not seem to worsen the clinical course. In particular, physicians should pay attention to patients who present with leukocytosis after a snakebite, which indicates the risk of developing RM, regardless of the CK level at presentation.
4.Effects of Basic Psychological Needs and Support of Health Professionals on Self Care Agency in Inpatients with Schizophrenia: Based on the Self-determination Theory

Purpose:
The purpose of this study was to identify the effects of basic psychological needs and support of health professionals on the self care agency of inpatients with schizophrenia.
Methods:
This descriptive correlational study was based on the basic psychological needs theory, a sub-theory of the self-determination theory (SDT). Participants were 172 inpatients with schizophrenia from four mental hospitals in two areas. Data were collected from self-report questionnaires administered June-September 2019. Data were analyzed using the t-test, one-way ANOVAs with Scheffé’s test, Pearson’s correlation coefficients, multiple regression and PROCESS macro mediation analysis with IBM SPSS 22.0.
Results:
Basic psychological needs and support of health professionals showed a positive correlation with the self care agency. The results of the regression indicate the basic psychological needs and support of health professionals were predictors of self care agency.
Conclusion
The results suggest that basic psychological needs and support of health professionals are needed to improve the self care agency of inpatients with schizophrenia. Thus, the mental nursing practice needs a program for improving the self-care agency of inpatients with schizophrenia.
5.Qualitative Evaluation of YouTube Videos on Conservative Treatment after Ankle Sprain and Analysis of Correlation with Preference

Purpose:
Ankle sprains are among the most common musculoskeletal injuries, and many videos have been shared with patients through YouTube. YouTube allows the easy sharing of information, but there are accuracy and reliability issues due to the lack of verification processes for video producers. This study evaluated ankle sprain videos on YouTube and the correlation between the video scores and viewer preferences using “the number of likes and views”.
Materials and Methods:
For analysis, the words “ankle sprains”, “ankle exercise”, and “ankle rehabilitation” were searched on YouTube. Only videos taken in Korean were analyzed, and videos with less than 10,000 views and for commercial purposes were excluded. For 31 videos, two orthopedic surgeons and one orthopedic resident used one evaluation tool to evaluate the video quality with up to 30 points for subcategories. The scores between each evaluator were then averaged to evaluate the video quality. To reflect the viewer preferences, “the number of likes and views” were analyzed, and the relationship between the video score and “the number of likes and views” of the video was analyzed statistically.
Results:
The average score for the video was 9.33. The average scores for subcategories were anatomy and symptoms, 0.90; biomechanics and trauma-mechanism, 0.61; imaging quality, 0.95; clinical evaluation and physical examination, 0.61; treatment, 1.41; rehabilitation, 2.39; prevention and after effects, 0.89; and overall video quality, 1.61. The average of “the number of views” was 50,218.35 times, and “the number of likes” was 893.77 times. No correlation was observed between the video scores and “the number of likes and views”.
Conclusion
Most videos posted on YouTube did not provide high-quality medical information on ankle sprain, and there was no correlation between the video scores and viewer preferences. This suggests that incorrect medical information is being transmitted to patients, which can be harmful. Therefore, it is necessary to present the latest trend treatment and rehabilitation protocols for ankle sprains in an expert council and promote it through various channels.
6.Penile neoplasm associated with Equus caballus papillomavirus type 2 infection in a miniature Appaloosa: a case report

An 18-year-old miniature Appaloosa stallion presented with 6 months of history of sanguineous crusts on medial hind limbs and discomfort of micturition. Cauliflower-like and small masses were treated with cryotherapy for 6 months, but the regrowth of masses occurred. Subsequently, local excision via laser and topical treatment with 5% 5-fluorouracil for 5 months were followed. However, the horse was euthanized 4 months later due to regrowth of the masses. The mass was diagnosed as penile papilloma with cellular atypia and Equus caballus papillomavirus type 2 (EcPV-2) DNA was detected. This is the first report of equine penile neoplasm with EcPV-2 infection in Asia.
7.A Study of Thyroid Function in Partial Thyroxine-Binding Globulin Deficiency.

OBJECTIVE: It is generally thought that thyroxine-binding globulin (TBG)-deficient individuals are euthyroid and do not require treatment. However, there have been case reports of TBG deficiency combined with hypothyroidism. The purpose of this study was to investigate the relationship between TBG deficiency and thyroid function. METHODS: We reviewed the medical records of 32 patients diagnosed with TBG deficiency between 1997 and 2008 in Soonchunhyang University Seoul Hospital. All were partial TBG deficiency. Eighteen patients had combined hypothyroidism, and 14 patients had normal thyroid function. We compared the TBG, thyroid-stimulating hormone, free thyroxine, and total triiodothyronine levels between these 2 groups. Eighteen patients with TBG deficiency with hypothyroidism started thyroxine medication and continued for 2-3 years. After, they were followed up with thyroid function tests after discontinuing medication for 4 weeks at 2-3 years of age. RESULTS: The TBG level in TBG deficiency with hypothyroidism patients was significantly lower than that in TBG deficiency with normal thyroid function (4.43+/-2.22 mg/L vs. 6.23+/-1.81 mg/L; P=0.02). The percent TBG compared with normal mean TBG level according to age in the hypothyroidism patients was also significantly lower than that of patients with normal thyroid function (13.42%+/-6.92% vs. 19.08%+/-4.87%; P=0.014). Sixteen of 18 patients diagnosed with TBG deficiency with hypothyroidism showed persistent hypothyroidism at 2-3 years of age. CONCLUSION: We conclude that TBG-deficient patients should be observed closely and undergo thyroid function testing in order not to miss hypothyroidism. More investigations of TBG deficiency and thyroid function are needed in the future.

METHODS: A total of 113 patients with type 2 diabetes and 123 patients with diabetic nephropathy were enrolled in this study. The Pro12Ala polymorphism of the PPAR-gamma2 gene and the Gly482Ser polymorphism in the PGC-1alpha gene were determined with the polymerase chain reaction amplification, BstU-I and Msp I enzyme digestion, and gel electrophoresis. RESULTS: The genotype and allelic frequency of PPAR-gamma2 Pro12Ala gene were not different statistically between the diabetic nephropathy and the control. The genotype of PGC-1alpha Gly482Ser in diabetic nephropathy was also not different from those in control. The allelic frequency and carriage rate of Ser allele in PGC-1alpha Gly482Ser were significantly higher in patients with diabetic nephropathy than those in control (respectively, p<0.05). CONCLUSION: The polymorphisms of the PGC-1alpha Gly482Ser gene are significantly associated with the nephropathy in Korean patients with type 2 diabetes mellitus.

10.Indolent B-Cell Lymphoid Malignancy in the Spleen of a Man Who Handled Benzene: Splenic Marginal Zone Lymphoma.

We present the case of a 45-year-old man with a history of benzene exposure who developed splenic marginal zone lymphoma. For 6 years, he had worked in an enclosed space cleaning instruments with benzene. He was diagnosed with splenic marginal zone lymphoma 19 years after retirement. During his time of working in the laboratory in the 1980s, working environments were not monitored for hazardous materials. We indirectly estimated the cumulative level of past benzene exposure using job-exposure matrices and technical assumptions. Care must be taken in investigating the relevance of occupational benzene exposure in the occurrence of indolent B-cell lymphoma. Because of the long latency period and because occupational measurement data do not exist for the period during the patient's exposure, the epidemiological impact of benzene exposure may be underestimated.
