3.Changes of Serum Neopterin and Elastase - alpha1 - Antitrypsin.

BACKGROUND: Generalized immune aetivation occurs early in the course of many infectious diseases. Clinical investigations have known that immune activation can be qiantified by the measurement of soluble immune activation products, neopterin and elastase-a-antitypsm in serum. OBJECTIVE: The purpose of this study was to assess macrophage and neutrophil activation in patient with leprosy by measurement of neopterin and elhstase-a-antitrypin. METHODS: The study population consisted of 31 patients with subculoid leprosy and 71 patients with lepromatous leprosy (39 cases of M. leprae positive patients and 32 cases of M. leprce negative patients). Serum samples and clinical and laboratory data were collected form each patient and control. The levels of serum neopterin and elastase-a-antitrypsin were masured by a sandwich enzyme immunoassay. RESULTS: The serum neopterin levels were significantly raised in patients with leprosy and significantly higher in lepromatous leprosy than tuberculoid leprosy. The sejum elastase-a-antitrypsin levels were significantly increased in pat,ients with leprosy, but did not vary significantly between tuberculoid and lepramatous leprosy. There was also no significant correlation between the neopterin and elastase-a-antitrypsin levels and bacterial index in patients with lepromatous prosy. CONCLUSION: These data suggest that non-specific activation of macrophages and neutrophiles occurs in leprosy and high titers of ineopterin and elastase-a-antitrypsin alore, in the absenee of a functioning T cell response, do not appee,r to confer resistance against Mycobacterium leprae.

6.Hypothermia During Prehospital Transportation of Neonates.

BACKGROUND: Hypothermia is a condition that can lead to serious complications and even to death in newborn. Although the temperature control is essential in neonatal care, it is often neglected during urgent transfer from local private hospitals. The purpose of this study is to evaluate the incidence and clinical outcomes of transfer induced neonatal hypothermia. SUBJECTS AND METHOD: In this retrospective study, subject is limited to transferred outborn babies with age less than 24 hours from June 1996 to May 1999. A total of 3,086 patients were admitted in NICU during the study period and inborn and outborn babies were 1,743(56%) and 1,343(44%) respectively. Among the 1,343 outborn babies, 212 babies were transferred from the private hospital within 24 hours of birth and were eligible for the study. Rectal temperature on arrival, transfer time, birth weight, gestational age, initial arterial gas study, and clinical outcome were compared. Statistical analysis has been done with chi-sqaure test and multiple logistic regression analysis. RESULTS: There were 84 cases(39.6%) hypothermia(<36degrees C> among the 212 babies and the rate of hypothermia in transfered babies have not decreased over the study period. There were significantly more hypothermia in lower gestational age (less than 28 weeks) and lower birth weight (less than 2,000gm) caused hypothermia significantly more than normal gestational age or normal range of birth weight. And time interval (less than 12 hour) from birth to arrival at emergency department was also significant factor in hypothermia. The mortality rate was three fold higher in hypothermia than normothermic or hyperthermic babies. CONCLUSION: This study shows that hypothermia during neonate transportation is a major cause of neonatal mortality and morbidity in prehopital care. Body temperature control during transport of neonates under 24 hours of age should be emphasized for the better outcome of treatment.

7.GDC(Guglielmi Detachable Coil) Embolization for Carotid Cavernous Fistula - by Percutaneous Puncture of Superior Ophthalmic Vein -.

OBJECTIVE: For the treatment of carotid cavernous fistula(CCF), transarterial detachable balloon occlusion(DBO) is the method of choice. When it has failed to occlude the fistula, various embolization methods are used to treat the fistula. Transvenous embolization through the superior ophthalmic vein(SOV) is another method of treatment. The venous approach through the SOV after surgical dissection and exposure of this vein has been recommended by some delete, but(here) delete delete(an) alternative treatment method by percutaneous puncture of the SOV without surgical dissection(is described). METHODS: A 19-year-old woman admitted to our hospital two months after accident, presented with proptosis, chemosis, occulomotor and abducens nerve palsies, and bruit of the right eye. The authors tried DBO via transarterial route in initial treatment and the fistula was occluded with subsequent disapearance of bruit. However, 2 weeks later, she complained of recurence of bruit. Transarterial approach was attempted again, but the fistula hole was too small for this approach. The venous approach via SOV by percutaneous puncture was then tried. Puncture was made at the medial one third of the superior orbital rim and the fistula was embolized with Guglielmi detachable coils (GDCs). RESULTS: The fistula was completely occluded and no early and late complications noted. The patient's clinical symptoms were improved within a few days. CONCLUSION: Treatment of CCF by percutaneous puncture of the SOV is an alternative and effective method when other approaches are not feasible.

8.A Review of Pharmacological Strategy for Cognitive Deficits in Schizophrenia.

Cognitive deficit is frequently observed in patients with schizophrenia. It is significantly associated with functional outcome. In the past 20 years, due to significant advances on the concept of schizophrenia, cognitive deficit has been accepted as a core feature. In the DSM-5, cognitive deficit does not introduce diagnostic criteria of schizophrenia, but did one dimension of diagnosis of psychosis. Existing schizophrenia drugs are effective in treatment of positive symptoms of schizophrenia, but lack of effectiveness on improving cognitive function. Led by NIMH (National Institute of Mental Health), the MATRICS (Measurement and Treatment Research to Improve Cognition in Schizophrenia) meeting was conducted in order to achieve consensus on measuring tools and neuropharmacological targets for clinical trials for development of new drugs for improvement of cognitive function in schizophrenia. At the MATRICS consensus meeting, glutamatergic modulators and nicotinic and muscarinic agonists are expected to be promising, but should be proven by a double-blind placebo-controlled multicenter study for patients.

9.Total Hip Arthroplasty in the Fused Hip.

PURPOSE: The purpose of this study is to analyze the clinical and radiological results of performing primary total hip arthroplasty (THA) in patients with a fused hip and to compare the results with that of patients with avascular necrosis of the femur head (AVNF). MATERIALS AND METHODS: 19 patients (21 hips) with a fused hip(s) were retrospectively reviewed. We analyzed them clinically with using the Harris hip score (HHS), and radiologically with performing serial radiograms and we then compared them with the results of the patients in the AVNF group. RESULTS: The mean HHS was improved from 59.2 to 85.8. The VAS improved from 6.5 to 2.2 and the leg length discrepancy (LLD) improved from 27.5 mm to 12.5 mm. As complications, wound infection occurred in 3 hips, heterotopic ossification occurred in 8 hips and recurrent dislocation occurred in one hip. Although a radiolucent line was found in 4 acetabular cups and 6 femoral stems, there was no progression. Osteolysis was found in 1 acetabular cup. CONCLUSION: The results of THA in patients with a fused hip were excellent. Comparing with the AVNF group, improvement of the hip motion and LLD were much better. The improvement of the VAS and HHS showed similar results between the groups. However, the final results of the HHS, VAS, LLD and the range of motion of the hip were worse, and the complication rate was higher.

10.Metastatic Signet Ring Cell Carcinoma to the Breast: A Case Report.

Metastasis of signet ring cell gastric carcinoma to the breast is extremely rare. The common clinical findings are redness, edematous skin and pain, and these findings are similar to those of inflammatory breast cancer. We describe here a case of metastatic signet ring cell gastric carcinoma to the bilateral breasts, and this presented as bilateral palpable breast lumps after the patient had undergone radical total gastrectomy two years previously.
