1.Influence of Job Stress and Professional Self-concept on Job Satisfaction among Nurses in Rehabilitation Units.

PURPOSE: The purpose of this study was to define the relationships between job stress, professional self-concept, and job satisfaction among nurses in rehabilitation units and to identify factors that influence job satisfaction. METHODS: This study surveyed 240 nurses working at the rehabilitation units of 10 hospitals located in G City. To collect the data, nurses' job stress, the professional self-concept of nurses instrument (PSCNI), and the Korean-Minnesota Satisfaction Questionnaire (K-MSQ) were used. The data were analyzed through t-tests, ANOVA, Pearson's correlation coefficient, and multiple regression using the SPSS/WIN 18.0 program. RESULTS: The scores for job stress, professional self-concept, and job satisfaction were 3.34+/-0.61, 72.36+/-8.64 and 3.17+/-0.39, respectively. Job satisfaction and job stress showed a low-level negative correlation with statistical significance (r=-.13, p=.042), whereas a medium-level positive correlation was found between job satisfaction and professional self-concept (r=.46, p<.001). Job stress, professional self-concept, total career length and size of working hospital explained approximately 25.8% of job satisfaction (F=17.52, p<.001). CONCLUSION: Considering the results in this study, it is necessary to develop a practical educational program to establish positive professional self-concept for nurses in rehabilitation units. Such efforts must be supported by clinical nursing administrations through policies aimed at reducing job stress and increasing job satisfaction.

2.Epstein-Barr Virus in Korean Malignant Lymphomas.

To determine the prevalence of Epstein-Barr virus infection in lymphoid neoplasms of various histology and location, the paraffin tissues of 74 non-Hodgkin's lymphomas and 13 Hodgkin's diseases were studied by EBER and BHLF RNA in situ hybridization as well as immunostaining using LMP-1, EBNA-2, and ZEBRA. As a control, non-neoplastic lymphoid tissues from the nasal cavity(10), lymph node(38) and Waldeyer's ring(12) were investigated. In non-neoplastic control, EBV genome was detected in none of 10 nasal mucosa, 6 of 38 lymph node, and 1 of 12 Waldeyer's ring. EBV-positive non-neoplastic lymphocytes expressed CD45RO in 2 cases and CD20 in 4 cases. Non-Hodgkin's lymphoma was positive for EBV in 37.8% of the cases in which T-cell lymphoma showed higher rate(56%) than B-cell lymphoma(15%), especially in nasal lymphoma(80%) and angiocentric lymphoma(63.6%). Hodgkin's disease was EBV positive in 38.4% of the cases. EBV genome in tumor tissue existed in latent form as well as in lytic form. LMP-1 was positive in 80% of Hodgkin's disease and 39% of non-Hodgkin's lymphoma in which EBV genome was detected. EBNA-2 was expressed in 3 cases of non-Hodgkin's lymphoma. On the basis of protein expression, most lymphomas belonged to type II latency. These results support that EBV is associated with pathogenesis of malignant lymphoma although its mechanism still awaits to be clarified.

5.Cultural Characteristics of Vibrio vulnificus on Various Media.

Using the 15 strains of Vibrio eulnificus isolated from the blood and tissue fluid of the patients with primary septicemia, cultural characteristics were studied on various media. The media to be used in culturing Vibrio vulnificus should contain sodium chloride, because it could not survive on the media without NaCl. The organism grew poorly or not at all on the media with eosin-y, methylene blue, crystal violet, or azide, because these substances suppress its growth. Vibrio vulnificus grew well in the enrichment media such as brain heart infusion, tryptic soy broth, and nutrient broth with 0. 5% NaCl.

Fulminant hepatic failure (FHF) is often accompanied by a myriad of neurologic complications, which are associated with high morbidity and mortality. Although appropriate neuromonitoring is recommended for early diagnosis and to minimize secondary brain injury, individuals with FHF usually have a high chance of coagulopathy, which limits the ability to use invasive neuromonitoring. Jugular bulb venous oxygen saturation (JvO₂) monitoring is well known as a surrogate direct measures of global brain oxygen use. We report the case of a patient with increased intracranial pressure due to FHF, in which JvO₂ was used for appropriate brain oxygen monitoring.

8.Prevalence of Cognitive Impairment and Related Factors Among the Elderly in Rural Communities of Jeju Province.

PURPOSE: The purpose of this study was to survey the prevalence rate of cognitive impairments and to identify the factors influencing cognitive impairment in the elderly in rural communities of Jeju Province. METHODS: 590 elderly in 6 rural communities of Jeju Province were interviewed, using a questionnaire consisting of sociodemographic characteristics, health behavior, quality of life, and MMSE-K RESULTS: Prevalence of cognitive impairment was 33.1% (39.1% of females, 16.76% of males). Prevalence of dementia was 12.4% (16.3% of females, 2.87% of males). Factors related to cognitive impairment were age, sex, education, standard of living, employment status, and subjective health state. CONCLUSIONS: In community health care for the elderly, factors relating to cognitive impairment have to be considered. When planning community health care, priority should be given to the elderly; who need care but live alone; who lack social support; who have a low standard of living; who experience discomfort in the activities of daily living; who believe they are not in a good state of health; or whose life satisfaction is low.

Using the 8 strains of Vibrio vulnificus isolated from the patients with primary sepsis, the viability of Vibrio vulnificus were studied in the various range of hydrogen ion concentration. The opitimal pH range of growth was 7-9, and survived between pH 3. 6 and 12. 5 when incubated at 37C for one hour.

10.A Case of Myocardial Infarction caused by a Variant Angina during Treatment with beta-blocker of Intramural Hematoma.

Variant angina is characterized by repeated attack at rest associated with ST-segment elevation on ECG and caused by the spasm of coronary artery. But, the pathogenesis of spasm is not well known. A 44-year old man was transferred for the management of intramural hematoma at descending thoracic aorta and uncontrolled hypertension. We started to control hypertension with nitroprusside, propranolol, amlodipine, and doxazocin. At 4th hospital day, severe chest pain, dizziness, and diaphoresis were developed, and ECG showed not only ST-segment elevation on lead II, III, aVF but also 2 degree AV block(Mobitz type II). CK-MB revealed 52.3 ng/dl. When coronary angiography performed emergently, it showed total occlusion of right coronary artery (RCA) and diffuse minimal narrowing of left anterior descending coronary artery (LAD). After nitroglycerin was infused via right coronary catheter, the RCA was opened completely, and reperfusion arrhythmia was developed. Medication were changed to nifedipine, diltiazem, nicorandil, isosorbide mononitrate and he had no more chest pain.
