1.Comparison of Glomerular Size between Focal Segmental Glomerulosclerosis and Minimal Lesion in Children.

The pathogenetic mechanism of focal segmental glomerulosclerosis (FSGS) is not known. Some authors suggest glomerular hypertrophy may precede the development of FSGS in patients with minimal lesion. It was recently reported that the size of nonsclerotic glomeruli in adults with FSGS is significantly larger than that of cases with minimal lesion. It is not clear whether glomerular hypertrophy observed in adults with FSGS is also seen in children with FSGS. Thus, we have analyzed 37 renal biopsies from children with FSGS by morphometry and the data were compared with 37 renal biopsies from age- and sex-matched patients with minimal lesion. The number of glomeruli submitted for morphometric analysis was 22.6+/-14.2 in cases with FSGS and 30.9+/-11.4 in cases with minimal lesion. Mean glomerular volume (MGV) in FSGS group was significantly larger than that of minimal lesion [(13.1+/-3.9) x10(5) microm3 vs. (10.1+/-1.9) x10(5) microm3, p<0.001]. The relative interstitial volume of renal cortex in patients with FSGS was significantly larger than that of minimal lesion [(0.106+/-0.051) microm3/microm3 vs. (0.029+/-0.012) microm3/microm3, p<0.0001]. In FSGS, the percentage of glomeruli with FSGS was significantly correlated with relative interstitial volume of renal cortex (r=0.79, p<0.0001). As is the case for adult FSGS, MGV of children with FSGS is significantly larger than that of minimal lesion. Thus, the presence of glomerular hypertrophy observed in biopsies with minimal lesion nephropathy seems to be an indication that the coexistent FSGS lesions are undetected due to sampling problems.

3.Plication Method of the Redundant Rectal Mucosa by Longitudinal over and over Suture (overlaping suture).

This report is a review of experience with 45 patients plication methods of redundant rectal mucosa by longitudinal over and over suture who were admitted in department of surgery at Dongsan sacred Heart Hospital during 1997. This new technique was indicated by incomplete rectal prolapse, first degree complete rectal prolapse (internal rectal intussusception), 3rd degree hemorrhoids, redundant rectal mucosa in operation field, anal prolapse and severe external anal skin tag. At first, entire hemorrhoid-bearing area was dissected by excision and ligation methods. Within upward 1cm from ligationpoint, No.3 chromic catgut suture was done including rectal submucosa. Starting from this, linear continueous overlapping locking suture was done until redundant mucosa was tented by Chromic suture traction in range of 1 to 1.5 cm widths. If there is wide redundant mucosa not corrected one time, another linear continuous overlapping locking suture especially not circular may be done. This method is very easy and few complication-bleeding, pain, infection, mucosa loss, etc. The patients who have incomplete defecation sensation, rectocele and skin tag were satisfied with operation. This method may be an easy, effective new technique in patients who have anal prolapse, skin tag, rectocele and constipation etc.

4.Predictive Values of APR Test for Neonatal Infection.

PURPOSE: The study has been done to evaluate the significance of APR test predictive of neonatal infection. METHODS: A study was made on 76 cases of clinically infeected neonates who had been admitted at the NICU of Chungnam National Unoversity Hospital during the period from January, 1996 to July, 1996 in order to know the significance of APR test predictive of neonatal infection. RESULTS: 1)The APR test Of Control group (20 cases) shows negative results on 19 cases (95%) and positive result on 1 case (5%). 2) The APR test of Patient group (76 cases) shows negative results on 17 cases (22.3%) and positive results on 49 cases (77.7%). Of cases with positive result, one points were on 23 cases (30.5%), two points were 25 cases (32.8%), three points were on 11 cases (14.4%) and More than one point were of 59 cases. The sensitivity of APR test, WBC count and CRP is 77.7%, 35% and 62.496, respectivitly. As above result, APR test has a high sensitivity. 3) APR positive values are not significantly different between those of premature and fullterm patients. 4) APR values are more significant than WBC count or CRP value to predict the neonatal infection. 5) The APR test is rnore useful for prognostic correlation than other index. CONCLUSION: APR test is very sensitive and significantly useful predictive of neonatal infection.

5.A Clinical Study of 12 Cases of Sarcoma of the Uterus.

OBJECTIVE: Sarcoma of uterus is originated from uterine muscles and/or connective tissues and rare and most lethal of all primary uterine tumors. The purpose of this study was to evaluate the clinical outcome of 12 patients diagnosed primary uterine sarcoma at the Department of Obstetrics and Gynecology in Kyungpook National University Hospital between 1984 and 1998 for 15 years. METHODS: After review of chart in twelve patients, data including clinical and histologic findings, treatment and outcome were analysed. RESULTS: The most common histologic finding was leiomyosarcoma(6cases, 50%) and the age of patients ranged 24 to 69 years and the average age was 43.25 years and two patients were nulliparous. The most common sign was abnormal uterine or vaginal bleeding(48%), and pelvic cavity or vaginal mass(16%), pelvic pain(16%), no symptom(16%), in order of frequency. The distribution by FIGO clinical stage was four cases(33%) for stage I, one case (8%) for stageII, one case(8%) for stage III, six cases (50%) for stage N. The nine cases received hysterectomy with or without BSO. Two cases received myomectomy to preserve fertility. The mean follow-up duration was 39.6 months. CONCLUSION: Uterine sarcoma is an uncommon and aggressive tumors with a poor prognosis. Leiomyosarcoma is the most frequent histologic type(50%). Stage I uterine sarcoma with or without adjuvant chemotherapy has better prognosis than other stages.

6.TCA chemical peeling.

Tricholroacetic acid (TCA) chemical peeling is an effective, versatle and safe therapeutic method for patients with pigmentary disorders, acne scars and aging skin. With an experienced and skilled operator, the associated complications are rare and tend to be mild. No. ystemic toxicity is known to be induced by TCA. We observed the effect of TCA chemical peeling, evaluated by 242 patients themselves with melasma, freckles, senile lentgines, pigmented nevi, seborrheic keratosis, skin tags or acne scars. The patients evaluated TCA peeling as excellent for the treatment of seboirheic keratosis, pigmented nevi, skin tags and acne scars, and as selectively recommendable for the treatment of melasma, freckles and senile lentigines.

7.Influence of Job Stress and Depression on Suicide Ideation of Women-workers in Service Industries.

PURPOSE: The purpose of this study was to identify the relationship of job stress, depression and to identify factors influencing suicide ideation of women-workers. METHODS: The research was a cross-sectional, descriptive study. Data collection was done using self-report questionnaires with 348 women-workers who agreed to participate in the study. Data were analyzed using descriptive statistics, t-test, chi2-test, logistic regression analysis by using SPSS/WIN 18.0. RESULTS: The results showed, among the independent variables, drinking, job stress, depression are significant factors that influence women-workers' suicide ideation. Women-workers' depression is 3.05 times higher (p<.002), and job stress 1.03 times higher suicide ideation (p<.045), and drinking 2.15 times higher suicidal ideation (p<.006). CONCLUSION: The results indicate that suicide ideation is highly prevalent in women-workers, especially among people with depression and job stress. It is necessary to seek how to prevent and manage womenworkers' suicide ideation considering features.

8.Epidemiological characteristics of patients with drug-resistant tuberculosis.

BACKGROUND: There is increasing concern in many countries about the problem of drug-resistant tuberculosis. Prevalence of primary drug-resistant tuberculosis is the optimal epidemiological indicator for long term monitoring of national tuberculosis control program. Our purpose was The purpose of our study is to assess clinical characteristics and socioeconomic status of patients with drug-resistant tuberculosis. METHODS: We studied 68 cases with durg-resistant Mycobacterium tuberculosis infection diagnosed at the Ewha Womans University Mokdong Hospital from March, 1995 to February, 2000. RESULTS: Patients with primary drug-resistant tuberculosis(PDR) were younger(39.6±16.3 years vs. 48.2±16.5 years; p<0.05), had more population of less than more were under the age of 40 years aged-group(62.9% vs. 36.4%; p<0.05) and were more highly educated than those with acquired drug-resistant tuberculosis(ADR)(38.9% vs. 11.1%; p<0.05). In patients with ADR, the rates of familial history of tuberculosis and living in a rented house residence in a rented house were increased higher than compared with to those of patients with PDR. Patients with ADR had more involved lobes(2.0±0.8 vs. 1.4±0.7; p<0.01) and longer treatment duration than those with PDR(18.3±7.2 months vs. 10.6±6.3 months; p<0.05). Patients with ADR showed larger numbers of resistant were resistant to more number of drugs, lower hospitalization rate and higher rate of self-interruption of medication than those with PDR. In patients with PDR, mono-drug resistance was increased, whereas poly-and multi-drug resistances were decreased compared with those with ADR. Resistance to isoniazid was the highest among antituberculosis drugs, and resistance to isoniazid in patients with ADR was higher than that in patients with PDR(90.9% vs. 71.4%; p<0.05). CONCLUSIONS: Patients with ADR were more likely to include more population be of lower socioeconomic class, and patients with PDR seem seemed to be young and socially active population. For control of drug-resistant Mycobacterium tuberculosis infection, proper isolation and prevention of patient with drug-resistant tuberculosis are needed.

9.The Change of Arterial Blood Gas during High Frequency Jet Ventilation via 14 French Suction Catheter in Microlaryngeal Endoscopic Surgery.

BACKGROUND: It is a routine practice that High Frequency Jet Ventilation (HFJV) is applied through a 14~16 gauge (about 12 French (Fr.)) angiocath. The 14 Fr. suction catheter which is similar to angiocath in its internal diameter is commonly available in the operating room. We evaluated the suction catheter as a carrier of HFJV in point of ventilation, oxygenation and operating field during HFJV at microlaryngeal endoscopic surgery (MES). METHODS: Fifteen adult patients undergoing HFJV via 14 Fr. suction catheter during MES were studied. Time-based arterial blood gas analyses were done before and after HFJV. The movement of operating field was examined using laryngoscopic examination by surgeon and anesthesiologist. We also evaluated complications such as abdominal distension, barotrauma and so on. RESULTS: The mean arterial oxygen tension was maintained above 250 mmHg all the time during HFJV. The mean carbon dioxide tension was less than 51 mmHg. There were no remarkable catheter movement and complications. CONCLUSION: The 14 Fr. suction catheter is a good replacement of angiocath. It provided good operating field, ventilation and oxygenation without complications.
