2.The positional relationship between the mandible and the hyoid bone in mandibular protrusion after orthognathic surgery evaluated with 3-d ct.

PURPOSE: This study was intended to evaluate the positional relationship between the hyoid bone and the mandible in patients with mandibular protrusion after mandibular set-back surgery by means of 3D-CT. MATERIALS AND METHODS: Preoperative(3 weeks before) and postoperative (6 weeks after) 3D-CT and cephalogram were taken on 32 patients(12 male, 20 female, mean age of 23.2) treated by bilateral sagittal split osteotomy with rigid fixation. The angular measurement on 3D-CT basilar view were deviation of Me and H, long axis angle of left and right cornu majus. The lineal measurement on 3D-CT basilar view were composed of intercondylar line and coordinates(x,y) of Me and H. The angular and lineal measurement of lateral cephalogram were composed of mandibular plane angle, SNA, SNB, ANB, FH-NA and FH-NB, and coordinates(x,y) of B, Pog, Me and H, PAS, Lpw, MPH and IAS. On the frontal cephalogram, deviation of Me were evaluated. RESULTS: The mean mandibular set-back was 8.0mm horizontally and mandibular plane angle was slightly increased. The hyoid bone was displaced postero-inferiorly, the distance between MP(mandibular plane) and H(hyoid bone) was increased and the posterior airway space values (PAS, Lpw, IAS) were decreased. The coordinates Me(x,y), H(x,y) and deviation angle Me' and H'were revealed the strong positive correlation. CONCLUSION: The results revealed that the horizontal, vertical and transverse relationship of the mandibular and the hyoid bone movements were significantly correlated in patients performed mandibular set-back surgery.

5.Kinetic Analysis of Intracellular ionized Calcium Level from Human Peripheral Blood Lymphocytes Using Flow Cytometry.

BACKGROUND: Intracellular ionized calcium plays a central role in the transduction of external stimuli as a critical second messenger. The spectral properties of fluo-3 allows the analysis of intracellular ionized calcium level by flow cytometers. The aim of this study is to assess the performance of flow cytometer for measuring intracellular ionized calcium level using fluo-3 and to define the reference interval of intracellular ionized calcium level of lymphocytes from healthy people, and to find out the clinical implications according to various disorders. METHODS: For the analytical performance of flow cytometer on determining the concentration of intracellular ionized calcium, precision study, lowest limit of detection, analytical range, and the loading stability of fluo-3 were per foamed. Fifty-four cases of healthy people, 52 cases of renal transplant patients, and 20 cases of diabetes mellitus patients were included in this study. RESULTS: Loading effect of fluo-3 at room temperature was stable upto 5 hours. Lowest limit of detection of ionized calcium concentration was 4.34 nM at in-situ calibration procedure. Within-run and among-day intraindividual CVs of in-situ calibration procedure were 6.67% and 13.99% respectively, and of optical calibration procedure were 13.86% and 16.12% respectively. The reference interval of cytosolic free calcium level for healthy people ranged 73.54 - 155.09 nM without sexual differences. The level of intracellular ionized calcium was lowered by 36.9% on renal transplant group in comparison with healthy control group. But, level of cytosolic free calcium was Increased upto 276.0% on acute rejection group and 159.1% on diabetes mellitus group compared to control group. CONCLUSIONS: These results reveal that in-situ calibration method for intra cellular ionized calcium using flow cytometry with flue-3 can be regarded as an accurate and standardized method. Quantitation of intracellular ionized calcium level might be used as the monitoring test for early detection of acute rejection after renal transplantation.

6.The Simple Excision of Ossicles for Osgood-Schlatter Lesion

Of 35 patients with 38 knees treated using simple excision method for Osgood-Schlatter lesion, had a distinct and separate ossicle at the tibial tubercle. All ossicles were attached to the distal part of the undersurface of the ligamentum patellae and were separated from the tuberosity by a bursa or scar tissue. Surgical excision of ossicle along with adjacent bursa was satisfactory and simplest procedure to relieve the symptoms, did not respond to conservative treatment.

7.Atypical Autoerotic Death Due to Insertion of a Foreign Body into the Urethra and Rectum

Autoerotic death is an accidental death during sexual pleasure, usually caused by a device or equipment used to enhance pleasure. Autoerotic deaths can be classified as typical or atypical. Typical autoerotic deaths occur due to asphyxiation by mechanical compression, while atypical autoerotic deaths occur by means other than asphyxiation. Narcissism-associated accidental deaths during tube and wire insertion into the urethra and rectum are extremely rare. The victim, a man in his 40s, was found dead at his home. Several tools for masturbation and wires for urethral and rectal insertion were observed around the victim and many pornographic videos were also identified. Family accounts and medical records indicated that he masturbated often by inserting plastic tubes and wires into the urethra and rectum. Moreover, the victim had a history of surgeries to remove foreign objects from the urethra and rectum as well as a history of psychiatric treatment. The autopsy results revealed no specific findings indicative of the cause of death. In conclusion, the investigators determined the cause of death as an atypical autoerotic death based on the death scene investigation, autoerotic sexual behavior, personal history, and autopsy results.
8.The Effect of Unilateral Lateral Rectus Recession for the Treatment of Moderate-Angle Exotropia.

PURPOSE: To evaluate the surgical results and the amount of correction per millimeter after unilateral lateral rectus recession for moderate-angle exotropia in a pediatric population. METHODS: This study included 30 subjects who underwent 9-mm unilateral lateral rectus recession for basic type intermittent exotropia with deviation of 23 to 28 prism diopters (PD) and were followed-up for 6 months or more after surgery. RESULTS: Seventeen subjects were male and 13 were female. The mean age of subjects was 9.7 years. Defining satisfactory postoperative ocular alignment as 8PD or less, 25 of 30 subjects (83.3%) were satisfactorily aligned at 2 months postoperatively. The success rates at 4, and 6 months postoperatively were 76.7% respectively. No overcorrected subjects were found after 2 months postoperatively. A postoperative limitation of abduction was not found in all subjects after 2 months postoperatively. The average amount of correction per millimeter of recession at 2, 4, and 6 months was 2.83+/-0.26PD, 2.75+/-0.37PD, and 2.72+/-0.44PD respectively (range, 1.56 to 3.18PD). There was no statistically significant difference in the amount of correction per millimeter throughout the follow-up period (ANOVA, p=0.05). CONCLUSIONS: Unilateral lateral rectus recession appears to be an effective surgical procedure for the treatment of moderate-angle exotropia. However to reduce the rate of undercorrection, which is one of disadvantages of this procedure, proper patient selection is required.

9.A Case of Adenocarcinoma arising from a Mature Cystic Teratoma of the Ovary.

Malignant transformation of mature cystic teratoma is a rare complication. Most com-mon malignant transformation of mature cystic teratoma is squamous cell carcinoma, which represents about 75 to 80% followed by adenocarcinoma in 6 to 7%. It is generally consi-dered that prognosis is extremely poor in case of adenocarcinoma associated with cystic te-ratoma. In this paper, we describe an adenocarcinoma arising from a mature cystic teratoma of left ovary in 47 year old woman. Left ovary was measured 18x15x7cm, 1,295gm in size. Uterus and right ovary were normal appearance. Left ovarian cyst was adhered to omentum and peritoneum. But neither rupture of cyst nor ascites were found at laparotomy. A brief review of concerned literatures was presented.

10.Community Care for Cancer Patients in Rural Areas: An Integrated Regional Cancer Center and Public Health Center Partnership Model

Purpose:
The accessibility of medical facilities for cancer patients affects both their comfort and survival. Patients in rural areas have a higher socioeconomic burden and are more vulnerable to emergency situations than urban dwellers. This study examined the feasibility and effectiveness of a cancer care model integrating a regional cancer center (RCC) and public health center (PHC).
Methods:
This study analyzed the construction of a safety care network for cancer patients that integrated an RCC and PHC. Two public health institutions (an RCC in Gyeongnam and a PHC in Geochang County) collaborated on the development of the community care model. The study lasted 13 months beginning in February 2019 to February 2020.
Results:
The RCC developed the protocol for evaluating and measuring 27 cancer-related symptoms, conducted education for PHC nurses, and administered case counseling. The staff at the PHC registered, evaluated, and routinely monitored patients through home visits. A smartphone application and regular video conferences were incorporated to facilitate mutual communication. In total, 177 patients (mean age: 70.9 years; men: 59%) were enrolled from February 2019 to February 2020. Patients’ greatest unmet need was the presence of a nearby cancer treatment hospital (83%). In total, 28 (33%) and 44 (52%) participants answered that the care model was very helpful or helpful, respectively.
Conclusion
We confirmed that a combined RCC-PHC program for cancer patients in rural areas is feasible and can bring satisfaction to patients as a safety care network. This program could mitigate health inequalities caused by accessibility issues.
