1.Xeroderma Pigmentosum: The Treatment of Associated Skin Cancer.

Xeroderma pigmentosum is characterized by sunlight sensitivit.y, autosomal recessive inheritance, and multiple cutaneous malignancies, usually basal and squamous cell carcinomas. In this work the effects of bleomycin and topical 5-fluorouracil on basal and squamous cell careinomas appearing in the patient of xeroderma pigmentosum were studied and followed up for two years. The material was a 50 year old female patient of xeroderma pigmentosum associated with basal and squamous cell carrinomas occuring in the exposed area, and its mixed tumor in the nose for thirty years. Duringfirst admission blecmycin was given intravenously in a dosage of 15 mg every other day or twice a week for basal cell carcinomas in the exposed area and mixed tumor in the nose. The total amount of bleomycin given was 315 mg. A year later another squamous cell carcinomas developed on the exposed area. She was treated with topical application of 5% 5-fluorouracil cream daily for 40 days during second admission. The results obtained are summarized as follows: l. The effect of bleomycin is more effective in the squamous cell carcinoma than in the basal cell carcinoma which recurred after 2 years. 2. Topical therapy of 5% 5-fluorouracil cream appeared almost complete effectiveness to the squamous cell carcinoma.

2.Predictive Values of APR Test for Neonatal Infection.

PURPOSE: The study has been done to evaluate the significance of APR test predictive of neonatal infection. METHODS: A study was made on 76 cases of clinically infeected neonates who had been admitted at the NICU of Chungnam National Unoversity Hospital during the period from January, 1996 to July, 1996 in order to know the significance of APR test predictive of neonatal infection. RESULTS: 1)The APR test Of Control group (20 cases) shows negative results on 19 cases (95%) and positive result on 1 case (5%). 2) The APR test of Patient group (76 cases) shows negative results on 17 cases (22.3%) and positive results on 49 cases (77.7%). Of cases with positive result, one points were on 23 cases (30.5%), two points were 25 cases (32.8%), three points were on 11 cases (14.4%) and More than one point were of 59 cases. The sensitivity of APR test, WBC count and CRP is 77.7%, 35% and 62.496, respectivitly. As above result, APR test has a high sensitivity. 3) APR positive values are not significantly different between those of premature and fullterm patients. 4) APR values are more significant than WBC count or CRP value to predict the neonatal infection. 5) The APR test is rnore useful for prognostic correlation than other index. CONCLUSION: APR test is very sensitive and significantly useful predictive of neonatal infection.

4.Reduction Malarplasty through Intraoral Incision: A Now Method.

Utile recently, osteotomy & reposition surgery of prominent zygoma have been performed by means of a coronal incision or intraoral preauricular incision. But penalties are paid, such as scar, the possibility of facial nerve injury and long operative time. Reflecting on our past experiences of facial bone surgery, we developed an alternative approach. In our method, the protrusion in the cheekbone is corrected by performing an osteotomy and reposition method through intraoral incision only. During the past 3 years we have operated on 23 patients of malar prominences. The amount of the bone to be removed is determined on preoperative interview, physical examination and x-rays. Intraoral incision provide access to the zygomatic body and lateral orbital rim. After L-shaped osteotomy, two paralle vertical and one transverse osteotomies, at medical part of the zygomatic body, the midsegment is removed. Posterior portion of zygomatic arch was approached through medical aspect and was outfractured using curved osteotome. After completion of triple osteotomy, the movable zygomatic complex was reduced medially and fixed with miniplates and screws on the zygomaticomaxillary buttress. The patients were followed for 9.5 months with acceptable result and little complication. The author concludes that this technique is effective and safe method in reduction malarpalsty.

5.Disseminated Superficial Actinic Porokeratosis (DSAP): Report of Two Cases.

Disseminated superficial actinic porokeratosis (DSAP) is one of the recently found skin disorderes, which was differentiated from classicaI porokeratosis (Mibelli) and senile keratosis etc. in view of clinical features and hietogenesis by Marvin E. Chernosky on 1967. The one was 60 years old female patient, who had been suffered from multiple dark-brownish keratotic uniform minute papules on dorsa of the hands, forearms and neck with accompanying itching sensation since about 20 year's old of her age. The another one was 38years old housewffe, wha had been suffered from same lesions of the former on the face, neck, extremities especially sun-light exposed area. In both cases, skin lesions were exacerbated during each summer season and increase in numbers of the skin lesions with more severe itching sensation. Chernosky postulated none of a dark-sklnned race affording natural protection against sun-light had skin lesion of D S A P, we were abIe to find out two cases of typical D S A P ih the Korea.

6.Extramammaty Paget's Disease: Electron Microscopic Study for Observation of Bleomycin Effect.

Extramammary Paget's disease is a dermatosis which pathologically and clinically resembles Paget's disease of the breast. The disease occurs in regions with apocrine gIands; most often in the anogenital area, seldom in the axilla. umbilicus, mucous membrane of the mouth, nose, ear and Moll's gland of tne eyelid. In this work the effect of bleomycin for extramammary Paget's disease was studied on ultrastructural aspect. The material was a case of extramammary Paget's disease of 7 years standing occuring in the scrotum and the pubic area by serial sections after IV injection of 60mg, 120mg and 240mg of bleomycin, and examined under the electron microscope. The results obtained are summarized as follows: 1. The size of Paget's cell and its nucleus was gradually decreased. 2. The numbers of lysosome, dense body, autophagic vacuolization and tonofilament was decreased. From these fidings, it is suggested that bleomycin induces retrograde degenerative change of Pagct's cell, and cell necrosis finally.

8.Expression of bcl-2 and c-myc Proteins in Epidermal and Melanocytic Tumors.

bcl-2 and c-myc protein expression were studied in 44 epidermal (8 seborrheic keratoses, 21 squamous cell carcinomas, and 15 basal cell carcinomas), and 26 melanocytic tumors(8 nevi, and malignant melanomas) by immunohistochemistry using the specific anti-bcl-2 and anti-c-myc monoclonal antibodies. 14 out of 15 basal cell carcinomas(BCC) (93.3%) showed expression of bcl-2 protein, 12 of which (85.7%) showed coexpression of c-myc protein. In the melanocytic tumors, 7 out of 8 nevi showed bcl-2 expression (87.5%). Five of these 7 cases (62.5%) also showed c-myc protein expression. Eight of 18 malignant melanomas(MM) (44.4%) showed expression of bcl-2 protein and 7 of these 8 cases (38.9%) also showed c-myc protein expression. All seborrheic keratoses and squamous cell carcinoma(SCC) were negative for bcl-2 proteins. 12 of 15 SCCs(80%) were positive for c-myc protein. In conclusion, bcl-2 and c-myc proteins were coexpressed in BCCs, nevi, and MMs. Coexpression of bcl-2 and c-myc proteins in these tumors was statistically significant(p<0.01), while no considerable differences of bcl-2 and c-myc expression were found between nevi and MMs. These results suggests that bcl-2 may cooperate with c-myc to promote tumorigenesis of BCCs, nevi, and MMs(p<0.01).

9.The Cardiovascular Effects of Epinephrine Used for Hemostasis under Enflurane-N2O Anesthesia during Tonsillectomy.

BACKGROUND: Exogenously administered epinephrine under enflurane anesthesia was known to have mild myocardial sensitizing effect. And N2O activates the sympathetic nervous system mildly. We planed this study to confirm cadiovascular effects of clinically administered epinephrine for hemostasis under the enflurane-N2O anesthesia during tonsillectomy. METHODS: Eighty children scheduled to have tonsillectomy were selected randomly and divided into 2 groups as follows; Group E: 1:100,000 epinephrine 2ug/kg and Group EL: 1:100,000 epinephrine containing 1% lidocaine 2 g/kg. Blood pressure, heart rate, and the occurrence of arrhythmia were evaluated before injection, at injection, 1 min, 2 min, 3 min, 5 min and 10 min after injection and 1 min after operation start. RESULTS: In both groups, systolic and diastolic blood pressure and heart rate are increased. But there are no significant statistical differences in each group and between groups. One min after operation, there are significant increases in systolic and diastolic blood pressure and heart rate in both groups (p<0.05), but there is no significant difference between groups. CONCLUSION: Under the enflurane-N2O anesthesia of children, 1:100,000 epinephrine 2ug/kg used for hemostasis could be used comparatively safe without any significant hemodynamic changes. But because there is always the possibility of myocardial sensitization, careful observation is necessary during epinephrine injection under the enflurane-N2O anesthesia.

10.Erythema Nodosum Probably Induced by Kerion Celsi.

A 6-year-old girl visited us with a 4-week history of inflammatory, pustular, tender patches and plaques on the scalp, and a 3-day history of multiple, erythematous, indurated, tender, subcutaneous nodules on both pretibial areas. A swab was taken from the scalp lesion and colonies of Tiichophyton mentagrophytes grew on culture. The histopathological findings of the leg nodule were consistent with erythema nodosum. The patient was treated with oral itraconazole and deflazacort, combined with topical potassium permanganate solution. Erythema nodosum regressed two weeks later and the kerion of the scalp regressed six weeks after starting the treatment, leaving residual scarring alopecia. The patient was diagnosed as erythema nodosum probably induced by kerion celsi, that has not been reported in the Korean literature.
