Glutathione S-Tanaferase (GST) is tripeptide, which plays a central role in the detoxification of electrophilic xenobioticas, including cytotoxic drugs and carcinogens, by conjugation with redueed glutathione. There are four major claases af human GST : pi, alpha, mu and rnicrosomal, They are propoaed as a marker for human and experimental neoplasia iincluding liver, kidney, lung,colon, uterine cervix, etc. Expression of pi, alphn, mu form of GST in control (15 cases), CIN (14 cases), invasive carcinoma (28 cases) of human uterine cervix was investigated immunohistochemically. They were selected fram September, 1992, to,June, 1992, in the department of Obstetrics and Gynecology, Yonsei university College of medicine. In nucleus, the expresaion of GST pi was increased statistically significant in case of CIN and invasive Ca. of Cx. compared to control(p=0.018, p=0.002). But expression of GST alpha, mu has no significant difference in each case. In cytoplasms, the expvession of GST pi was increased statistically significani, in case of CIN and invasive Ca. of Cx. (p=0.001, p<0.001). But expreasion of GST alpha, mu has no significant differenrce in each case. There was no statistically significant diHerence in expression of GST isoenzymes oC each nucleua and cytoplasm according to stage and cell type in Ca. oC Cx. The further evaluation of survival and expression of GST pi in Ca. of Cx. in order to establish new prognostic factor in Ca. of Cx.

PURPOSE: To evaluate the effects of uncinectomy for osteotomy method during endonasal dacryocystorhinostomy (DCR). METHODS: This retrospective review study comprised 102 nasolacrimal duct obstruction patients that underwent endonasal dacryocystorhinostomy with uncinectomy at our hospital between 2003 and 2006. The patients were classified into two groups based on records of preoperative endoscopic examination. Group A consisted of patients who had no nasal cavity abnormality, and Group B comprised patients who had a nasal cavity abnormality but received no treatment. RESULTS: Sixty-four patients were included in Group B. The success rates of endonasal DCR were 89.5% in Group A and 90.6% in Group B, with no statistically significant difference between the two groups (p>0.99). There were no cases of mucosal burn, orbital fat prolapse, spinal fluid leak, maxillary sinusitis, or delayed bleeding. CONCLUSIONS: Anterior resection of the uncinate process is the most important surgical step to expose the medial aspect of the lacrimal fossa and to form the precise location of osteotomy during endonasal dacryocystorhinostomy.

PURPOSE: The objective of this study was to evaluate fracture strength of collarless metal-ceramic FPDs according to their metal coping designs. MATERIALS AND METHODS: Four different facial margin design groups were investigated. Group A was a coping with a thin facial metal collar, group B was a collarless coping with its facial metal to the shoulder, group C was a collarless coping with its facial metal 1 mm short of the shoulder, and group D was a collarless coping with its facial metal 2 mm short of the shoulder. Fifteen 3-unit collarless metal-ceramic FPDs were fabricated in each group. Finished FPDs were cemented to PBT (Polybutylene terephthalate) dies with resin cement. The fracture strength test was carried out using universal testing machine (Instron 4465, Instron Co., Norwood MA, USA) at a cross head speed of 0.5 mm/min. Aluminum foil folded to about 1 mm of thickness was inserted between the plunger tip and the incisal edge of the pontic. Vertical load was applied until catastrophic porcelain fracture occurred. RESULTS: The greater the bulk of unsupported facial shoulder porcelain was, the lower the fracture strength became. However, there were no significant differences between experimental groups (P > .05). CONCLUSION: All groups of collarless metal-ceramic FPDs had higher fracture strength than maximum incisive biting force. Modified collarless metal-ceramic FPD can be an alternative to all-ceramic FPDs in clinical situations.

Deep skin and soft tissue defects with exposed bone and tendon is difficult to treat, because skin graft rarely survives and flap surgery is sacrifice of donor site. Since "Stage I" membrane was developed by Yannas and Bruke in 1980, numerous kinds of artificial skin have been developed. The adaptability of "Terudermis", developed by the Terumo Co., as an artificial skin composed of sponge made of a fibrillar atelocollagen and a heat-denatured atelocollagen, was clinically evaluated on application to 13 cases presenting deep skin and soft tissue defect with exposed bones and tendons from October 1997 to march 1998. Terudermis has the advantage of allowing early incorporation of fibroblasts and capillaries into its collagen sponge due to very weak dehydrothermal cross-linking. Before Terudermis graft, several days of wet dressing and debridement were required to prepare healthy well-vascularized bed because Terudermis was weak on unsanitary wounds. After bed preparation, Terudermis was grafted like usual skin graft. Tie-over bolster dressing or compressive dressing was used case by case. The dressing was opened 2~3 days after Terudermis grafting. Wet dressing was done daily until the skin graft was done. Autologous skin graft was done 2-3 weeks after Terudermis graft. Our clinical results indicated that Terudermis was beneficial in treating 77% of our patients. Through the use of this new method, treatment of severe skin and soft tissue defects that are usually treated by musculocutaneous or other conventional skin flaps can be replaced by Terudermis as an new artificial dermis.

Syphilis is a communicahble disease caused by the motile microaerophilic spirochete Treponema pallidum, which is only a netural pathogen for human. Prevalence of syphilis and other sexually transmitted diseases has traditioially fluctuated with changes in sccia, conditions and sexual behavior. Although sexual contact is the main route of transmission, T. pallidum may also be infected through direct contact with syphilitic lesions, blood transfusion, ingestion of menstrual blood or vaginal secretions, or trsnsplacental transemission. Fomites as the means of transfer is only hypothetical and account for very few, if any, infection. Prosectors, blood handlers and laboratory technicians are at risk for accidental inoculation with infected materials. In extremely unusual circumstances, infection by means of contact with a skin lesion and human bite have been reported. BACKGROUND: Acromioclavicular (AC) stability is maintained through a complex combination of soft-tissue restraints that include coracoclavicular (CC), AC ligament and overlying muscles. Among these structures, the role of the CC ligament has continued to be studied because of its importance on shoulder kinematics, especially after AC injury. This study was designed to determine the geometric change of conoid and trapezoid ligaments and resulting stresses on these ligaments according to various scapular motions. METHODS: The scapuloclavicular (SC) complex was isolated from a fresh-frozen cadaver by removing all soft tissues except the AC and CC ligaments. The anatomically aligned SC complex was then scanned with a high-resolution computed tomography scanner into 0.6-mm slices. The Finite element model of the SC complex was obtained and used for calculating the stress on different parts of the CC ligaments with simulated movements of the scapula. RESULTS: Average stress on the conoid ligament during anterior tilt, internal rotation, and scapular protraction was higher, whereas the stress on the trapezoid ligament was more prominent during posterior tilt, external rotation, and retraction. CONCLUSIONS: We conclude that CC ligament plays an integral role in regulating horizontal SC motion as well as complex motions indicated by increased stress over the ligament with an incremental scapular position change. The conoid ligament is the key structure restraining scapular protraction that might occur in high-grade AC dislocation. Hence in CC ligament reconstructions involving only single bundle, every attempt must be made to reconstruct conoid part of CC ligament as anatomically as possible.

PURPOSE: We report the clinical features of paranasal sinus mucoceles with orbital extension and compare the results of external and transnasal approaches based on the rates of complications and recurrence. METHODS: Thirty-three cases of paranasal sinus mucoceles with orbital extension diagnosed at our hospital from 2003 to 2007 were retrospectively reviewed. RESULTS: The mean age of patients was 48.6 years. The common sites of origin were the frontal, ethmoidal, frontoethmoidal sinuses, and proptosis was the most common presenting feature. Among the mucoceles of frontal and frontoethmoid sinuses, there was no difference in the rates of recurrence or complications between the two different methods. CONCLUSIONS: Mucoceles with orbital involvement generally present with a noninfiltrating mass resulting in many ophthalmic signs and symptoms. Obliteration of the involved sinus is not recommended if there is erosion of the sinus bony wall with extension of the mucocele into the orbit. The mucosa lining the mucocele become adhered to the orbital periosteum and cannot be removed during surgery without significant risk of injury to the adjacent structures. Endoscopic sinus surgery is considered effective for paranasal sinus mucoceles with orbital involvement.

PURPOSE: To evaluate the surgical results and clinical effectiveness of silicone tube intubation through Hasner's valve under intranasal endoscopic guidance in children with congenital nasolacrimal duct obstruction (CNDO). METHODS: The present study enrolled a total of 58 eyes of 46 children over 12 months of age diagnosed with CNDO in the Department of Ophthalmology, Inje University College of Medicine from 2003 to 2010. Patients who had previously undergone ineffective probing or had a combination of an anatomical abnormality such as secondary nasolacrimal duct obstruction were also included. The silicone tube was intubated through the Hasner's valve and observed with intranasal endoscopy; tube retention time was planned for at least 12 weeks. RESULTS: A total of 20 eyes were previously probed preoperatively (mean 1.7 times). After the average follow-up period of 13.5 months, the overall success of silicone intubation was 57 of 58 eyes (98%). In one failed eye, nasolacrimal obstruction was already combined preoperatively, and unintentional tube removal by the patient occurred within one week. Silicone tube reintubation was performed after four months, and CNDO symptoms improved. CONCLUSIONS: Intranasal endoscopic observation to ensure the passage of a silicone tube through Hasner's valve decreases anatomical injury. When considering patient age and former treatment, silicone tube intubation with intranasal endoscopic observation could produce desirable results with a higher success rate.
