Journal of Neurosonology and Neuroimaging 2024;16(2):93-99
Background:
Although ultrasound is widely used in clinical practice, educational opportunities for medical students, particularly those in neurology, remain limited. This study aimed to evaluate the impact of neurological US education on first-year medical students and provide insights for future curriculum development.
Methods:
A prospective, single-center study was conducted from 2022 to 2023 involving 231 first-year medical students over two years at the Chungnam National University College of Medicine. The sessions on Transcranial Doppler and carotid Doppler included a 30 minutes lecture, a 30 minutes demonstration by a neurology specialist, and optional hands-on practice, respectively. A post-session survey assessed students’ feedback, preferences, and willingness to engage in US education. For the analysis, the Likert-scale responses were dichotomized into positive and negative responses, and comparisons between years were conducted using chi-square or Fisher’s exact tests.
Results:
Over 80% of students found the sessions helpful in understanding anatomy and physiology. Student interest in US education significantly increases from 2022 to 2023 (80.5% vs. 92.4%, p=0.013), along with overall satisfaction (82.5% vs. 96.2%, p<0.001). Students preferred optional programs and instructions from specialists or residents. The demand for expanded US education showed an increasing trend (68.0–78.1%), although the difference was not statistically significant.
Conclusion
Neurological US education positively affects student learning and motivates students to use US in future practice. This study highlights the importance of neurological US education and provides insights for the development of future specialized curricula in neurology.

Journal of the Korean Surgical Society 1999;57(6):850-857
BACKGROUND: Infantile Hypertrophic Pyloric Stenosis (IHPS) is one of the most common surgical problems of early infancy and one for which an eminently successful surgical treatment has been available since the work of Ramstedt in 1912. A clinical study was begun to access further the accuracy of ultrasonography in identifying hypertrophic pylorus. METHODS: This study is a retrospective clinical analysis of 31 cases of IHPS treated at the Department of Surgery of Pohang St. Mary's Hospital from Jan. 1990 to Dec. 1997. RESULTS: (1) The most prevalent age group was between 3 weeks and 8 weeks in 24 cases (77.4%), and the ratio of males to females was 5.2:1. (2) Among the 31 cases, new born babies were 21 cases (67.7%). (3) In 30 cases (96.8%), the gestational age was between 37 weeks and 42 weeks, and the birth weight was more than 3.5 kg in 21 cases (67.7%). The body weight percentile at admission was lower than the 50 percentile in 31 cases. (4) Among the 31 cases, breast-fed infants were 15 cases (48.4%), milk-fed 13 cases (41.9%), and mixed-fed 3 cases (9.7%). B type blood group was 23 cases (74.2%), and O type was 4 cases (12.9%). (5) In 4 cases (12.9%), an inguinal hernia was noted as an associated anomaly. (6) The onset of symptoms was neonatal (1 week-12 weeks) in all 31 cases, and the duration of the symptoms was between 1 week and 2 weeks in 18 cases (58.1%). (7) Non-bile stained, projectile vomiting was noted in all 31 cases (100.0%), an olive-shaped mass in right upper quadrant of the abdomen was felt in 27 cases (87.1%), and visible peristalsis on the epigastrium was noted in 25 cases (80.6%). (8) On laboratory tests, 17 patients had leukocytosis, and anemia was observed in 2 cases. Hypokalemia was observed in 9 cases (29.0%), hypochloremia in 4 cases (12.9%), and moderate to severe alkalosis (CO2 content > 25 mEq) in 7 cases. (9) Among the 25 cases, for which an the ultrasonographic evaluation was performed, the length of the stenotic canal was from 16 mm to 20 mm in 23 cases (92.0%), and the thickness of the stenotic portion was from 5 mm to 6 mm in 21 cases (84.0%). (10) All 31 cases were surgically treated by using a Fredet-Ramstedt pyloromyotomy, and the mortality was nil. The average hospitalization was 9.4 days. (11) There was 1 case of duodenal perforation and 1 case of intermittent non-projectile vomiting after the operation. CONCLUSIONS: We conclude that early accurate diagnosis, adequate preoperative preparation of the fluid & electrolyte imbalance, immediate surgical correction, and scheduled careful oral feeding are important in treatment of IHPS. Ultrasonographic determination of pyloric muscle length and thickness is the most accurate of the currently available techniques. A Fredet-Ramstedt pyloromyotomy is a safe and successful surgical procedure.

Korean Journal of Occupational and Environmental Medicine 1996;8(3):466-476
Hearing loss is a very common health problem throughout the world, although the number of hearing impaired individuals varies greatly in the countries. It is generally accepted that hearing impairment inevitably leads to a breakdown in communication and physical and mental health problems consequently. Nevertheless, few community based studies have been undertaken to examine both the mental and more general physical state of the hearing impaired patients. This study was conducted to evaluate the anxiety and depression in hearing impaired patients. Self-administered questionnaires including sociodemographic data were given to 336 subjects over the age of 18 years who visited the Health Management Center, Uijongbu St. Mary's Hospital for the routine health check up from January 1996 to April 1996 and three hundred seventeen samples (male: 197, female: 120) were selected. For the assessment of anxiety and depression we used Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) devised by Zung. And hearing impairment was evaluated using a Beltone Model 110 Audiometer. The results were statistically analyzed by ANOVA test and x(2) test. The results obtained were as follow: 1. The mean anxiety scores counted by SAS of 0-15dB HL (hearing loss) group (classified by the average loss in the better ear at 500, 1,000, 2,000Hz frequencies), 16-29dB HL group, 30-44dB HL group, more than 45dB HL group were 33.1+/-6.6 32.2+/-5.9, 41. +/-8.4, 42.8+/-6.0 respectively. And the mean depression Scores counted by SDS of 0-15dB HL group, 16-29dB HL group, 30-44dB HL group, more than 45dB HL group were 37.4+/-7.7, 37.2+/-7.2, 46.2+/-8.4, 48.6+/-8.6, respectively. The more than 30dB HL group showed significant high scores in SAS and SDS than the less than 30dB HL group (p<0.001). 2. The frequency of clinically significant anxiety disorder (SAS > or = 50) was 3.4% in less than 30dB HL group and 11. 1% in more than 30dB HL group. The difference between two groups was statistically significant (p<0.05). 3. The frequency of clinically significant depression disorder (SDS > or = 50) was 5.1% in less than 30dB HL group and 39.5% in more than 30dB HL group. The difference between two groups was statistically significant (p<0.05). 4. When several important interaction variables related to anxiety: and depression (such as age, sex, marital status, education level, economic status, occupation, religion) were stratified and we compared the groups between the more than 30dB HL and less than 30dB HL, most factors about anxiety and depression showed the significant differences in two groups. 5. Multiple logistic regression analysis revealed that hearing loss(p<0.05) predicted anxiety disorder (SAS> or = 50) and hearing loss(p<0.01), age(p<0.05) predicted depression disorder (SDS> or = 50). In conclusion, there was significant difference in anxiety and depression scores, between more than 30dB HL(in the better hearing ear) group and less than 30dB HL group. This study suggested that anxiety and depression should be considered as adverse effects of hearing impairment.

Korean Circulation Journal 1991;21(5):809-820
Since after first report of percutaneous transluminal coronary angioplasty in 1977, improved operator technique and advanced equipment designs have resulted in an increase in primary success rate from 67% to 92% in recent days. Despite these improvement, acute closure and restenosis remain as serious limitations to both the short and long-term success of PTCA. Coronary artery stents have been proposed as a method of treating acute closure and preventing restenosis. We implanted 21 balloon expandable Palmaz-Schatz Stent in selected 21 Patients(mean age 62+/-8 years, M/F : 16/5) with atherosclerotic coronary artery disease. The indications were elective stenting in 17, acute dissection postangioplasty in 2 and restenosis following angioplasty in 2. The clinical diagnosis of the subjects was unstable angina in 12 pts(57%), stable angina in 1, and post infarction angina in 8 including 6 acute myocardial infarction. The target vessel was right coronary artery In 12(57%), left anterior descending artery in 6(29%) and left circumflex in 3(14%). The size of implanted stent was 3mm in 15, 3.5mm in 5 and 4.0mm in 1. The morphology of attempted lesion was AHA/ACC classification type A in 1, type B in 17(B1 ; 1, B2 ; 16) and type C in 3. Angiographic findings were 1-vessel disease in 16 patients, 2-vessel disease in 4 and 3-vessel disease in 1. Coronary artery stenting wast technically successful in 21 all patients(100%) and complications included subacute total occlusion 1 week after stenting in 1, which was recanalized successfully by repeat PTCA without myocardial infarction, prolonged sinus arrest after stenting due to microembolism in 1 and puncture site bleeding requiring transfusion in 1. CONCLUSION: Coronary artery stenting was a safe and effective procedure to obtain adequate coronary blood flow in selected patients but long-term efficacy of primary elective coronary stenting should be evaluated prospectively.

Korean Journal of Dermatology 1997;35(2):249-257
BACKGROUND: Neurofibroma, the hallmark of neurofibromatosis, is a cutaneous or subcutaneous lesion, with a variable clinical presentation. Histologically, neurofibroma consists of proliferation of nerve derived cellular elements, together with an abundant, collagenous extracellular matrix. Specifically, neurofibroma has been shown to contain 30-50% collagen in its matrix. Objective 5. METHODS: We examined the expression of extracellular matrix genes (collagen, fibronectin, laminin), TGF-b mRNA and Ha-ras oncogene mRNA by using Northern and slot-blot hybridization and immunoperoxidase stains. Result: In Northern blot analysis, Ha-ras and TGF-b genes revealed respectively, 8.8kb and 2. 5kb sized mRNA transcripts in neurofibroma. These parameters were normal in the control. The expression of these genes were 1.9, 2.0 fold increased in neurofibroma. In slot-blot analysis, expression of type I collagen showed fibronectin genes to be 2,401+210, 540+43, respectively, in neurofibroma. So there were 3.7 fold, 2.1 fold, differences respectively, compared to the normal control. However, there were no significant changes of type IV collagen and laminin Bl mRNA levels between neurofibroma and normal skin tissues. Irnmunoperoxidase staining by rnonoclonal anti type IV collagen antibody in neurofibroma showed type IV collagen to be diffusely and weakly stained in tissue. On staining by monoclonal anti-laminin antibody, laminin was stained in a matrix and around vessels. CONCLUSION: The increased expression of extracellular matrix genes may suggest that there is a subpopulation of fibroic cells in neurofibroma which are stimulated by TGF-b. Ha-ras genes which might have accumulated with the differentiation of neural tissue may be related to the pathogenesis of neurofibroma tissue formation. Further studies are needed to determine whether the other factors are related to the pathogenesis of neurofibroma.
