1.Effects of Cytokines on Proliferation Responses of Th1 Cells to Mitogen.

Thl cloned cell line 28-4 which is an I-A + KLH - specific Th1 type clone of (C57BU6xC 3H) F1 origin was kindly provided by professor Tomio Tada. In these studies, employing these cloned cells, the author investigated both proliferation responses of Thl cells in the presence of various concentrations of cytokines, such as IL-2, IL-4 or IL-6 and proliferation of Thl cells to various concentration of mitogens such as PHA, ConA or PWM. In addition, the author also investigated the proliferation response of Th1 cells to the optimal dose of PHA, ConA or PWM in the presence or absence of above mentioned cytokines. It was found that IL-2, IL-4 or IL-6 alone their growth stimulation degree was dependent on cytokine concentration and that PHA, ConA or PWM stimulated Thl cell proliferation and optimal dose of PHA ConA and PWM was 3 g, 4 g and 2 g per ml, respectively. In addition, proliferation response of Th1 cells to ConA or PWM in the presence of IL-2 was significantly enhanced, but the proliferation response to PHA was not increased significantly. However, IL-4 did not significantly modulate mitogen-activated Thl cell proliferation response. Interestingly, IL-6 decreased PHA- or ConA-activated proliferation of Thl cells, but did not change PWM-activated proliferation. Taken together, these studies strongly suggested that IL-2, IL-4 or IL-6 itself clone stimulated the Thl cell proliferation and that PHA, ConA or PWM also stimulated Thl cell proliferation. In addition, these studies also indicated that IL-2 increased ConA- or PWM-activated Thl cell proliferation, but IL6 inhibited PHA- or ConA-activated Th1 cell proliferation and that IL-4 did not significantly change the mitogen-activated Th1 cell proliferation.

2.Operative Treatment Of Burst Fracture On The Thoracolmbar Junction

We analyzed 41 cases of burst fractures on the thoracolumbar junction which were operated with Kaneda and Cotrel-Dubousset implant at Chosun University Hospital between 1989 and 1993. The purpose of this study was to evaluate the radiologic sign, the amount of reduction, complications, and functional results. The results were as follows: 1. According to McGrorys Criteria to evaluate the posterior superior vertebral body angle of burst fractures, 33 out of 41 cases(80.5%) were positive and the average angle degree was 107.6°. 2. The average postoperative kyphotic correction was 15.4° in the Kaneda group and 13.8° in the C-D group. The average loss of correction at follow-up examination was 5° in the Kaneda group and 4.8° in the C-D group. 3. Indirect reduction was achieved in 10 cases(50%) and we obtained a good indirect reduction even though canal compromise was over 50%. 4. The pain at operative site was much more severe in the Kaneda group(6 cases) than in the C-D group(2 cases) and both groups experienced 2 cases each of implant failure. 5. According to Denis' pain and work scale, 28 cases(63.8%) had good and excellent, 8 cases had fair and 5 cases had poor results. In summary we recommend doing 1) a posterior instrumentation first for stability and indirect reduction, if it is not a severe neurologic symptom and 2) anterior decompression if it is a severe or progressive neurologic symptom.

Solid pseudopapillary tumor of the pancreas is a rare tumor that affects young females with low malignant potential and good prognosis with more than 90% survival at 5 years. Metastasis is very rare. We report the case of a 74-year-old female who had pancreatic solid-pseudopapillary tumor and synchronous hepatic metastasis.

4.Vacuum Arthrography of the Hip Joint

It is possible to create a diastasis in the hip by traction. The space thus created between articular surfaces can be visualized radiographically and is known as “a vacuum phenomenon”. Twenty one cases of unilateral hip joint disease were studied with vacuum arthrography from Oct. 1983 to July 1984 in Catholic Medical College and Center. The patients ages ranged from a month to 53 years of age but most of them(18 out of 21 patients) were below 10. There were 5 female patient's and 16 male patients in this series. The results were as follows: 1. In the absence of an effusion in all normal hip joints the vacuum phenomenon was demonstrated. 2. In the absence of an effusion in the hips of a transient synovitis, 4 Legg-Calve-Perthes disease and 3 sprain the size of vacuum was normal. 3. The size of vacuum of 3 transient synovitis and a pyohip of healing stage which had only mild effusion was smaller than those of normal hips. 4. In the presence of large amount of effusion in the hips of 5 transient synovitis, 2 pyogenic arthritis and 2 tuberculous arthritis it has not been possible to produce the vacuum phenomenon. 5. This study, in summary, shows that the size of vacuum represents the amount of effusion. 6. Non-ossified femoral head in the new born, articular surface of acetabulum and thickness & local changes in the cartilage of the femoral head could be demonstrated using the vacuum arthrography. 7. It has not been possible to demonstrate the articular surface lateral to the limbus, ligamentum teres and limbus in the vacuum arthrogram.

5.A Case of Central Serous Chorioretinopathy Associated with Systemic Corticosteroid Treatment of Bell's Palsy.

Central serous chorioretinopathy (CSCR) is potential ophthalmologic sequelae of steroid use characterized by an idiopathic serous detachment of neurosensory retina within the macula. Its etiology and pathophysiology are still unknown, but what has been implicated to cause CSCR is the the use of corticosteroid via multiple administration routes including oral, intravenous, inhaled, intranasal, epidural and intraarticular as well as topical forms. We report, with a review of the literature, an additional case of CSCR, which developed during systemic corticosteroid treatment in a Bell's palsy patient.

6.Treatment of Torsion Dystonia with Motor Point Block Using Phenol : A case report.

Dystonia is an abnormal movement characterized by sustained muscle contractions, frequently causing twisting and repetitive movements or abnormal postures. Torsion dystonia is characterized by torsion spasms of muscle contraction, which distorts the limbs and trunk into dystonic postures. We present a case of a patient with torsion dystonia who was recalcitrant to oral medication or even surgical operations. This patient was treated with motor point block using 5% phenol solution. Using electromyographical guidance, phenol was injected into the paraspinal and upper extremity muscles, respectively. He showed reduction of dystonia and improvement of functional abilities. Motor point block using phenol can be considered as a tool of the management for patients with torsion dystonia. MDVAQ@yumc.yonsei.ac.kr

9.The Comparison between Particle Agglutination Assay and Enzyme Immunoassay in the Detection of Mycoplasma pneumoniae-Specific Antibody.

BACKGROUND: The two common serological test methods used for initial diagnosis of acute Mycoplasma pneumoniae (MP) pneumonia are particle agglutination assay (PA) and enzyme immunoassay (EIA). We compared the differences between the two methods and suggest a test method more suitable for clinical laboratories. METHODS: A total of 35 patients (18 adult and 17 pediatric) performed MP specific antibody test using PA (Serodia-Myco II, Fujirebio, Japan) and EIA (Ani Labsystems, Finland) methods. IgM and IgG antibodies were measured separately by EIA method. PA and both IgM and IgG EIA were tested in 26 patients and PA and IgG-EIA were tested in 9 patients. RESULTS: The concordance rates between PA and EIA were 57.7% for IgM and 65.7% for IgG antibodies. Positive PA results showed better agreement with IgG (77.8%) than IgM (38.9%), while negative PA results showed better agreement with IgM (100%) than IgG EIA results (25%). In adult patients, the correlation between PA titers and IgM (r=0.852, P <0.01) and IgG values (r=0.517, P <0.05) were statistically significant. In pediatric patients, the correlation between PA titers and IgG values (r=0.842, P <0.01) was statistically significant. CONCLUSIONS: In this study, we observed that PA and EIA may not be used alternatively. Therefore, we suggest that use of both PA and IgM-EIA will be the optimal choice for laboratories. However, when laboratories are required to select one from PA or EIA, PA may be more useful to diagnose MP infection.

10.Ultrasonographic findings in Fitz-Hugh-Curtis syndrome: a thickened or three-layer hepatic capsule

Fitz-Hugh-Curtis syndrome (FHCS) is characterized by inflammation of the perihepatic capsules associated with the pelvic inflammatory disease (PID). FHCS is not a serious disease, but if not treated properly, it can result in increased medical costs, prolonged treatment, and dissatisfaction with treatment. However, early recognition of FHCS in the emergency department can be difficult because its symptoms or physical findings may mimic many other diseases. Although contrast-enhanced computed tomography (CECT) is the useful imaging modality for recognition of FHCS, it is available only when a high suspicion is established. We performed point-of-care ultrasonography in an 18-year-old woman who had a sharp right upper quadrant (RUQ) abdominal pain without PID symptoms and found a thickened or three-layer hepatic capsule. These findings coincided with areas showing increased hepatic capsular enhancement in the arterial phase of CECT. These results show that if the thickened or three-layer hepatic capsule without evidence of a common cause of RUQ pain is observed on ultrasonography in women of childbearing age with RUQ abdominal pain, the physician can consider the possibility of FHCS.
