2.Various appearances of rib companion shadow mimicking a pathologic condition.

We have observed that the companion shadow of the upper rib may be misinterpreted as a small pnemothorax or pleural plaque associated with asbestosis. To observe the radiographic characteristics of the normal companion shadow, we analyzed, on the posteroanterior(PA) chest radiographs, the companion shadow of 50 normal cases. Factors such as occurrence on each rib, the sharpness of the margin, the relative position to the rib, the shape and the thickness were observed. Also, we analyzed the displaced pleura of 4 pneumothorax cases to differentiate their frndings from the findings of normal companion shadows. On 50 normal chest radiographs, 192 compaion shadows were observed on the first to fourth ribs. In 173 of those shadows, the visceral margin of the companion shadow on the second rib simulated pneumothorax more closely than those on any othe rivs due to its apical location and thinness. In six of 50 normal cases, the companion shadow on the first or second rib showed an inw rdly convex lower margin, mimicking pleural plaque. The compaion shadow was suggested on the plain chest radiograph by the following characteristics imultiplicity(47/50), thicker than normal pleura(3/4), persistent on serial filma with the same shape and specific location(4/4).

3.Study on the trematode parasites of the birds in Korea.

Authors carried out an investigation of the helminthic parasites of the wild birds in Korea. Most wild birds were collected from Kyoung Sang Do, Kyoung Kee Do and Seoul area from September 1970 to March 1973. A total of 70 wild birds, 19 species were examined, which comprised as follows; 10 Anas platyrhynchus, 10 Anas fulicata, 4 Anas poeciolorhyncha zonorhyncha, 8 Anser albifrone albifrone, 5 Anser cygonopsis cygnoid, 10 Aythya fuligula, 6 Gallicrex cinerea, 2 Porgana fusca, 2 Phisianum calichius korponri, 4 Sterptopilla orientalis, Podiceps cristatus, Larus crasirotatus, Malantta fusca, Garis artcus, Buetes buteo, Egretta alba, Starmus philpiens, Bombycilla granulus. Trematode parasites found from six species of the wild birds. Echinostomatidae has been found from intestine of the Anas platyrhynchus platyrhynchus, Anas poeciolorhyncha zonorhyncha, Anas fulcata. Notocatylus attenuatis found from intestine of the Podiceps cristatus, Anas fulcata. Catylurus japonicus was found from rectum of the Anas platyrhynchus platyrhynchus, Anas poeciolorhyncha zonorhyncha.

5.Effects of Toically Applied Autologous Serum on Aqueous Bichemistry and Endothelial Healing Following Experimental Corneal Alkali Wounds.

The effect of topically instilled autologous serum on the heang of endothelium and change of aqueous chemical properties following alkali wounds were evaluated, New Zealand white rabbits, weighing 2.5kg. total of 64(64 eyes), were used in this experiment, Standardized corneal alkali wounds were produced by applying a round filter paper of 5.5mm in diameter, soaked in 1N NaOH, to the central cornea for 60 seconds. Autologous serum in the treated group and balanced salt solution(BBS) in the control group were given 4 times per day for 3 week. Endothelial defect areas and the concentrations of ascorbic acid, glucose and minerals in the aqueous humor were measured. Endothelial defect area was not significantly different between autologous serum-treated and control groups. The concentrations of ascorbic acid in the aqueous humor were significantly increased n the autologous serum-treated groups compared to the control groups. However, the concentrations of glucose, Na+, K+, Ca++ and Mg++ in the aqueous humor were not statistically different between two groups

7.A Clinical Study of Herpes Zoster with Generalized Varicelliform Eruptions.

BACKGROUND: Herpes zoster with generalized varicelliform eruptions occurs in 2 to 10% of patients with herpes zoster. It occurs mainly in old or debilitated persons especially those who have immunologic defects such as lymphoproliferative diseases, AIDS, or recipients of immunosuppressive therapy. The reported incidence of herpes zoster with generalized varicelliform eruptions is variable. OBJECTIVE: The purpose of this study was to elucidate the incidence and clinical features of herpes zoster with generalized varicelliform eruptions. METHODS: We reviewed the clinical data of 962 patients with herpes zoster by retrospective methods. The annual incidence, age, sex, seasonal variation, predilection sites, and associated conditions of herpes zoster with generalized varicelliform eruptions were analyzed from January 1990 to December 1996 (7 years). RESULTS: 1. Among 962 patients, 8 patients with herpes zoster revealed generalixed varicelliform eruptions (0.83%). 2. The age ranged from 20 to 85 and the majority of cases occurred in the 6th decade. There were 4 females and 4 males. 3. Past histories of malignancy were observed in 2 patients. However, there were no signs of malignancy at the time of diagnosis of herpes zoster with generalized varicelliform eruptions. 4. The most common site of initial lesion was the thoracic dermatome,followed by the lumbar and the cervical ones. CONCLUSION: From our observation, it is suggested that herpes zoster with generalized varicelliform eruptions may occur in patients without underlying malignancy or immunosuppressive disorders. Sudden incidental uprising of herpes zoster with generalized varicelliform eruptions was observed in 1996.

8.A clinical study on febrile convulsion.

Although febrile convulsion (FC) is a common clinical entity with a high recurrence rate, no specific factors have been identified to be highly predictive of its recurrence. A total of 114 cases of FC identified during a 28 month period (January, 1989-May, 1991) was analysed. A slight predominance of boys over girls (1.3 to 1.0) was observed. the majority of cases (89.4%) were under 3 years of age and the most common age range for the onset of FC was 1~2 years. The most common form of FC was tonic type (54.0%), followed by tonic-clonic (32.5%), clonic (5.3%), and complex partial type (5.3%). Most FC episodes lasted less than 5 minutes (81.6%), and common clinical diagnosis underlying FC were as follows: pharyngotonsillitis (64.0%), bronchitis and/or pneumonia(16.7%), and gastroenteritis (9.7%). The family history for seizure disorder was present in 27.2% of the cases: FC in 21.1%, epiepsy in 4.4%, and both in 1.8%. Among 78 cases of the first FC, 43 cases were followed for 5 to 29 months without prophylactic anticonvulsant therapy. The overall recurrence rate of FC in this group was 67.4% and the highest incidence was observed in patients less than one year old (86.7%). A higher recurrence rate (80%) was observed in patients whose body temperature was lower than 39degrees C, as opposed to higher than 39degrees C (50%). It is concluded that the recurrence of FC is positively correlated with factors such as younger age (less than one year old), and lower body temperature at the time of onset of FC.

9.The Role of Lipocortin-1 in the Corticosteroid Action in Collagen Induced Arthritis.

OBJECTIVES: Lipocortin-1 (LC-1), a member of annexin family of calcium-binding proteins induced by corticosteroid, originally evoked interest as one of the secondary messengers in the antiinflammatory action of corticosteroid, But the exact mechanism of LC-1 responsible for antiinflammatory effect is still unclear. We investigated the potential role of LC-1 in the effect of corticosteroid on amelioration of collagen induced arthritis (CIA) in mice. METHODS: Four groups of DBA/1j mice were immunized by intradermal injection of 5mg/kg of type 2 collagen with complete Freunds adjuvant which was boostered on day 21 and 42. Group 1 received no treatment and group 2 received 1mg/kg dexamethasone intraperitoneally twice weekly from day 21. Group 3 and 4 were treated with 50 and 0.5microgram/kg of anti LC-1 monoclonal antibody subcutaneously and dexamethasone from day 21 twice weekly, respectively. The prevalence of arthritis and arthritis score were assessed twice weekly. At week 10, we measured serum anticollagen antibody levels and splenic mononuclear cell stimulation indices (SI) to collagen. RESULT: CIA started to develop after 4 weeks of collagen treatment in all groups. All mice of group 1 developed arthritis by the 9 week. Treatment with dexamethasone markedly inhibited arthritis development (P<0.05). Cotreatment of anti LC-1 monoclonal antibody and dexamethasone abolished the antiinflammatory effect of dexamethasone (P<0.05). But there was no significant difference in the serum levels of anticollagen antibody or splenic mononuclear cell SI among the groups. CONCLUSION: These findings support the hypothesis that LC-1 is involved, at least in part, in the antiinflammatory actions of corticosteroid in chronic inflammation, although the mechanism of which is unclear.

10.Trichoblastic Fibroma: A Pathologic Analysis of 4 Cases.

Trichoblastic fibroma is a benign trichogenic tumor that has both epithelial and mesenchymal components and exhibits partial follicular induction. We studied 4 cases of trichoblastic fibroma and reviewed their clinical and histologic features. Two tumors were present in the face. The remaining two were in the vulva and perianal area, respectively. The age of the patients ranged from 53 to 68 years, with an average age of 62. All were female. Histologically, the lesions showed a well circumscribed mass, located at dermo-subcutaneous junction in three patients and subcutaneous in one. They demonstrated mesenchymal induction evidenced by hair germ-like structure and perifollicular sheath. There was no connection between the tumor and epidermis. Differentiation toward hair structure led to the formation of the infundibulum through inner root sheath. Trichoblastic fibroma may be confused clinically and/or histologically with basal cell carcinoma. Identification of the mixed epithelial and mesenchymal components, and the absence of epidermal connection and cleft within the stroma are important in differentiating this benign neoplasm from basal cell carcinoma.
