2.The Incidence of Microsatellite Instability, p53 Point Mutations and p53 Protein Expressions in Transitional Cell Carcinoma of the Bladder.

PURPROSE: The objectives of this study is to evaluate the role of microsatellite instability(MI) and p53 gene inactivation in transitional cell carcinoma(TCC) of the bladder. MATERIALS AND METHODS: Total 31 cases of TCC patients, not treated with radiation or anticancer chemotherapy, were included in this study. Fifteen out of 31 patients were superficial and 16 were invasive TCC according to pathologic stage, while 16 were low grade and 15 were high grade TCC according to Ash grading system. The presence of MI was investigated in 3 loci of 9p site(D9S162, IFNA, D9S1747), and 6 loci of 17p site(D17S261, D17S513, D17S654, D17S695, D17S796, TP53) using a method based on the polymerase chain reaction(PCR). And the p53 point mutations were detected using polymerase chain reaction-single stranded conformation polymorphism (PCR-SSCP) analysis. The p53 protein overexpression was determined by immunohistochemical stain using DO-7 primary antibody on formalin fixed, paraffin embedded tissue sections. RESULTS: On MI analysis, altered band pattern were detected in 16 of 31 case(51.6%) in 9p region and 15 of 31 cases(48.4%) in 17p region. MI of the 9p region showed no association with tumor grade or stage. MI of the 17p site also showed no difference between superficial and invasive tumor group, but were higher in high grade(73.3%) than low grade tumor group(25.0%). On PCR-SSCP analysis, total 8 mutations were found in 6 cases(19.4%) and 4 of 8 mutations(50%) were at exon 8. On immunohistochemical staining, p53 nuclear overexpressions were found in 18 cases(58.1%) and more common in high grade and invasive tumor group(p<0.05). Fifteen out of 18 cases(83.3%) showed MI of the 9p or 17p regions. CONCLUSIONS: These results suggest that the p16 alterations may occur in the early stage of tumorigenesis and p53 alterations are more associated with high grade/invasive tumor. And DNA mismatch repair system may play an important role in tumorigenesis of bladder tumor. MI analysis can be a useful method to detect early genetic alterations in transitional cell carcinoma of the bladder.

5.Radiologic Findings of Retroanastomotic Hernia after Gastrojejunostomy.

PURPOSE: To review the radiological findings of retroanastomotic hernia and to derive the useful US and CT criteria to assist in the diagnosis of the condition in patients who had previously undergone gastrojejunostomy. METHODS: During a recent period, 8 consecutive cases of retroanastomotic hernia were encountered. Of the patients involved, seven underwent US and CT imaging. The US and CT scans were reviewed retrospectively to determine the abnormal findings. Surgical confirmation was available in all cases. RESULTS: The efferent loop was herniated through the defect created behind the anastomosis in seven cases, and the afferent loop in one case. Retroanastomotic hernia was suggested prospectively in all cases. Among the seven cases of efferent loop herniation, the US and CT signs of retroanastomotic hernia included whirling of the mesenteric vessels, jejunal loops, and mesentery in the periumbilical abdomen (7/7); mural thickening of the herniated bowel loops (5/7); dilatation of the herniated bowel loops (2/7); and US showed decreased peristalsis of the herniated bowel loops (2/6). In one case, the US and CT signs of retroanastomotic hernia of the afferent loop included dilatation and whirling of a short length of the afferent loop behind the anastomosis. One out of the eight patients had reVersible bowel ischemia, and one had bowel necrosis. CONCLUSION: Retroanastomoic hernia is an important condition, and the US and CT findings might be used for its diagnosis.

6.The Small Rice Bowl-Based Meal Plan was Effective at Reducing Dietary Energy Intake, Body Weight, and Blood Glucose Levels in Korean Women with Type 2 Diabetes Mellitus.

BACKGROUND: The typical Korean diet includes rice, which is usually served in a rice bowl. We investigated the effects of a meal plan using rice bowls of varying sizes on dietary energy intake (EI), body weight (BW), and blood glucose levels. METHODS: Forty-two obese women with type 2 diabetes mellitus were randomly assigned to use either a 200 mL small rice bowl (SB), a 380 mL regular rice bowl (RB), or to a control group (C). Both intervention groups were asked to reduce their EI by 500 kcal/day for 12 weeks and simple instructions for using the assigned bowl were provided. Dietary EI and proportion of macronutrients (PMN) were estimated from 3-day dietary records. RESULTS: Reduction of EI was more prominent in the SB group compared to the RB and C group, although EI decreased significantly from baseline in all groups. Carbohydrate and fat intakes of the SB group were decreased greater than those of the RB and C group. However, changes in PMN were not significant across the 3 groups. Reduction of BW and HbA1c levels in the SB group was more prominent compared to the C group. Although, BW and HbA1c were decreased significantly from baseline in both bowl groups. There was no statistical difference between the two groups. CONCLUSION: The small rice bowl-based meal plan was effective at reducing EI, BW, and blood glucose levels, and the observed reductions in EI, carbohydrate, and fat intake were greater than those of the regular rice bowl-based meal plan.

7.Immunolocalization of AQP5 in Salivary Glands of Rat.

Exocrine glands secrete large volumes of fluid in response to neural or hormonal stimulation. In the present studies, we examined the immunolocalization of AQP1-5 in salivary glands of rat to clarify the potential roles of these for fluid secretion. Immunohistochemistry using cryosections of rat submandibular glands revealed that AQP1 was located in the capillary endothelia between secretory gland cells. Secretory gland cells and ductal cells were not labeled with anti-AQP1, AQP2 and AQP3 were not present in either salivary gland cells, intercalated ductal cells or striated ducts. AQP4 was localized in the basolateral plasma membrane domains of the intercalated ductal cells in rat submandibular glands whereas either secretory gland cells or striated ducts were not labeled. AQP5 was abundant in the apical domains of the secretory gland cells as well as of the secretory canaliculi and intercalated ductal cells, consistent with RT-PCR analyses as well as previous data (Nielsen et al., 1997). Importantly, AQP5 labeling was associated with the apical part of the secretory gland cells, raising the possibility for trafficking of AQP5 from intracellular vesicles to the apical plasma membranes following neuronal or hormonal stimulation. In parotid and sublingual glands, the immunolabeling pattern of AQPs was identical with that seen in submandibular glands, while the AQP5 immunolabeling was sparse in the sublingual glands which is mainly composed of mucinous glands. In conclusion, the basolateral localization of AQP4 in the intercalated ductal cells and presence of AQP5 in the apical plasma membrane of secretory acinar cells, canaliculi and intercalated ductal cells of submandibular gland suggest that both AQP4 and AQP5 may provide a critical pathway for osmotic water flow into the secreted fluid.

8.Clinical Studies on the Etiology and Clinical Course of Pericardial Effusions.

To assess the etiology and clinical course of moderate and severe pericardial effusions, we reviewed 118 consecutive patients seen in the cardiology department of the Kyunghee Medical Center from January, 1984 to July, 1988. 1) The common etiologies of pericardial effusions were tuberculosis(25.4%), malignancy(18.8%), uremia(16.2%), heart failure(14.4%), idiopathic(7.7%) and collagen vascular disease(5.1%). The lung cacer(77.2%) and breast cancer(9.1%) comprised most of the etiology of malignant pericardial effusion. The adenocarcinoma was the most common histologic entity of lung cancer. 2) Collapses of right atrium, right ventricle or both were seen in 17 case(14.4%), and the causes of collapses were malignancy(35.5%), idiopathic(11.8%) and uremia(11.8%). 3) The causes of constrictive pericarditis were : tuberculosis(42.9%), idiopathic(28.6%), malignancy(14.3%) and uremia(14.3%). 4) The percentages of improved cases were determined in 55 patients by echocardiography twice, 30th day after treatment and the last follow up day. It revealed 84%/96% in tuberculosis, 85.7%/0% in malignancy, 66.7%/83.3% in uremia, 83.3%/83.3% in heart failure, 100%/100% in both myxedema and myocardial infarction, and 33.3%/33.3% in collagen vascular disease at both follow up day. We conclude that the most common etiology of the pericardial effusions is still tuberculosis in Korea and the most common etiology of cardiac tamponade is malignancy, even though the suspected patients without echocardiography have been excluded can be a limitation of this study. The pericardial effusions by tuberculosis, uremia or heart failure responded well to the adequate treatment and response in malignancy or collagen vascular disease was not good.

9.Arterial Stiffness by Aerobic Exercise Is Related with Aerobic Capacity, Physical Activity Energy Expenditure and Total Fat but not with Insulin Sensitivity in Obese Female Patients with Type 2 Diabetes.

BACKGROUND: Arterial stiffness is an important factor in atherosclerosis. Thus we examined whether aerobic exercise could reduce arterial stiffness in obese women with type 2 diabetes without diabetic complication. METHODS: A total of 35 women with type 2 diabetes (body mass index, 26.6+/-2.8 kg/m2; age, 56.4+/-1.9 years; duration of diabetes, 4.7+/-4.8 years) were assigned to aerobic exercise group (AEG) or control group (CG). AEG completed a 12-week exercise program (3.6 to 5.2 metabolic equivalents, 3 day/week, 60 min/day), with their exercise activities monitored by accelerometers. We measured abdominal total fat area (TFA), visceral fat area (VFA), and subcutaneous fat area (SFA) by computed tomography, insulin sensitivity by insulin tolerance test (K(ITT)), and augmentation index (AIx) by SphygmoCor at baseline and at the end of the 12-week program. RESULTS: The AIx was improved in the AEG compared with the CG (P<0.001). The percent change of AIx had significant correlation with the improvement of physical activity energy expenditure (PAEE), aerobic capacity, TFA, and SFA (r=-0.416, P=0.013; r=0.560, P<0.001; r=0.489, P=0.003; r=0.531, P=0.001, respectively), but not with insulin sensitivity, energy intake, or VFA. CONCLUSION: Improvement in aortic stiffness by aerobic exercise is related with the improvement of aerobic capacity, PAEE, and total fat but not with insulin sensitivity or energy intake in obese women with type 2 diabetes.
