2.Socio-Demographic and Clinical Variables Associated with Chronic Homelessness among the Street Homeless People.

OBJECTIVES: This exploratory study was designed to estimate the socio-demographic characteristics, course of homelessness, adverse childhood experiences, and prevalence of mental disorders among the street homeless people in Seoul, Korea. In addition, we examined the associations between chronic homelessness and study variables. METHODS: The study subjects were 75 homeless people dwelling in the street around Seoul Station. Questionnaires related to socio-demographic data and the course of homelessness were provided to each subject. The subjects' Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV) mental disorder diagnoses were made by using Mini International Neuropsychiatric Interview-Plus interviews. RESULTS: Most subjects exhibited chronic homelessness and the prevalence of DSM-IV mental disorders among the homeless subjects was higher than that among the Korean general population. There was a statistically significant association between chronic homelessness and adverse childhood experiences. Compared to those with no history of childhood adversity, homeless people with a history of childhood adversity had a younger age of first homeless episode and a higher prevalence of DSM-IV major mental disorders. The association between DSM-IV diagnosis and chronic homelessness was not significant, but the group with mental disorders had more prevalent re-housing and homeless recurrent histories. CONCLUSION: Street homeless people were often involved in chronic homelessness. Results of this study suggest that homeless people who were never married and had adverse childhood experiences were more vulnerable to chronic homelessness. In addition, we detected a possible relationship between the recurrence of homelessness and the presence of mental disorders.

3.A Case of Suprasellar Germinoma Metastasized to the Epidural Space of the Lumar Spinal Canal: Report of a Case.

We have experienced a case of suprasellar germinoma metastasized to the lumbar spinal canal. A 22-year-old man was admitted to St. Mary's Hospital May, 1975 with complaints of low back pain, progressive weakness of both lower extremities and voiding difficulty for about one month. About 7 months ago he received craniotomy and irradiation therapy for suprasellar germinoma. Neurological examination revealed foot drop on the left side with sensory change and atonic neurogenic bladder. Pantopaque myelogram showed a total block at the level of the 3 rd lumbar vertebra suggestive an epidural mass compressing dural sac posteriolaterally. Total laminectomy of lumbar first to fifth vertebrae disclosed a firm, rubbery hard and vascular mass in the epidural space, which was not adherent to the dura. Histological examination of the tumor revealed typical germinoma. Pathogenesis of geminoma and possible mode of epidural metastasis are discussed.

4.The Relationship between Reduced Lung Function and High Sensitive C-reactive Protein in Healthy Adult Men.

BACKGROUND: Increased levels of systemic markers of inflammation have been reported in patients with impaired lung function with obstructive and restrictive lung diseases. We hypothesized that forced expiratory volume in 1 second (FEV(1)) within the normal range is inversely associated with high sensitive C-reactive protein (hs-CRP), as a marker of chronic inflammation in healthy Korean adult men. METHODS: We analyzed the association of hs-CRP with pulmonary function test including FEV(1), components of metabolic syndrome among male participants (n=3,736), more than 20 years old who had visited a health promotion center at a university hospital between January and December, 2006. RESULTS: In this cross-sectional study, there was an inverse association between hs-CRP levels and quartiles of FEV(1) (P for trend<0.0001) after adjusting for age, smoking status, waist circumference, glucose, triglyceride, and HDL-cholesterol. A similar association was present in non-smoking subjects, but there was no significant trend (P for trend=0.115) whereas it was significant in smoking subjects (P for trend<0.0001), and in ex-smoking subjects (P for trend=0.006). The odds ratios of having elevated hs-CRP (>2.2 mg/L) across FEV(1) quartiles, with the lowest quartile (Q1) as the reference group was 1.01 (0.75~1.35) in Q2, 1.24 (0.93~1.65) in Q3, 1.52 (1.15~2.01) in Q4 after adjusting for the confounding variables. CONCLUSION: The decline of FEV(1) within the normal range was inversely associated with low grade inflammation as measured by hs-CRP. This result showed that systemic inflammation may be linked to early impaired pulmonary function in healthy adult men.

Collected data were analyzed by simple frequency analysis, N-gram analysis, keyword network analysis, and seasonality analysis. RESULTS: The results show that exercise had the highest frequency in simple frequency analysis in both years. However, weight reduction in 2010 and diet menu in 2015 appeared most frequently in N-gram analysis. In addition, keyword network analysis was categorized into three groups in 2010 (diet group, exercise group, and commercial weight control group) and four groups in 2015 (diet group, exercise group, commercial program for weight control group, and commercial food for weight control group). Analysis of seasonality showed that subjects' interests in diets increased steadily from February to July, although subjects were most interested in diets in July in both years. CONCLUSIONS: In this study, the number of data in 2015 steadily increased compared with 2010, and diet grouping could be further subdivided. In addition, it can be confirmed that a similar pattern appeared over a one-year cycle in 2010 and 2015. Therefore, dietary method is reflected in society, and it changes according to trends.

8.Reconstruction Of Achilles Tendon Using Fascia Using Fascia Lata Allogrft: A Case Report

We had a female patient with soft tissue tumor involving large portion of Achilles tendon. After resection of tumor, Achilles tendon was reconstructed using an allogenic human fascia lata manufactured by Pfrimmer-Viggo. EMG findings on postoperative 8 months show marked improvement of function of medial and lateral gastrocnemius muscles. Biopsy specimen of fascia lata allograft, which was acquired during TAL 8 months postoperatively, shows myxoid degeneration, a few lymphocytic infiltration and some foreign body giant cells. Functional, histologic and EMG findings suggest that human fascia lata allograft could be used to bridge defect of Achilles tendon being resected during tumor surgery.

9.Comparative Analysis of Perception Volume, Intake Volume, and Satiety Rate of Rice, Soup and Side Dishes Using Tableware and Food Tray

The perception volume, intake volume, and satiety rate of rice, soup, and side dishes using tableware and food trays were analyzed. Rice, beef, soup, and side dishes in tableware (1st week) and the same menu in food trays (2nd week) were served to twenty-nine female college students over two consecutive weeks (BMI 22.1 kg/㎡ , Age 22.9 yr). The results showed that the perceived volume of soup served in tableware was significantly higher than when served in a food tray. On the other hand, except for the perception volume of soup (tableware: 174.80±19.40 g, food tray: 136.14±12.77 g, P<0.05), there was no significant difference in the food perception volume and food intake volume using tableware and food trays between the two groups.The satiety rate of the food tray group (6.68) was significantly higher than that of the tableware group (7.42) after one hour (P<0.05). However, except after one hour, the two groups’ satiety rate showed a similar pattern. In most of the comparisons, the results showed no difference between table ware and food trays.Besides, the use of food trays has advantages in terms of easy to manage and convenience. Therefore, food trays might be a better option instead of a tableware.
10.Comparison between Users of Community Mental Health Services and Psychiatric Patients in Mental Health Related Facilities in Korea.

OBJECTIVES: This study was conducted to compare the characteristics of psychiatric inpatients in mental health related facilities and community mental health services-utilizing patients by diverse factors in Korea. METHODS: Questionnaires were sent via mail to 140 standard mental health centers from October to December of 2008. 64 facilities responded and data of 461 mentally disabled who are registered at and utilize the services of these mental health centers were analyzed. T-test and cross-analysis were performed to determine the differences in the averages of the variables and the discrepancy in ratios, respectively. RESULTS: Differences between long-term psychiatric inpatients and community mental health services-utilizing patients were seen in characteristics such as gender, age, type of medical coverage, family support systems, diagnosis, age of onset, duration of hospitalization, community-dwelling period, and clinical symptoms and functions. It was significantly short in length of stay in facilities in users of community mental health services with comparison to institutionalized patients. CONCLUSION: The results of this study suggest that invigoration of community mental health services is an alternative that can help deter long-term hospitalization.
