An unusual case of glroependymal cyst involving the lateral ventricle, 3rd ventricle, cerebellopontine angle cistern and prepontine cistern, simultaneously, is reported. The glroependymal cyst is benign developmental cyst that mainly occurs in the cerebrospinal axis in relation to ventricle, but occasionally occurs in relation to extraventricular system, such as subarachnoid space, brain stem and cerebral parenchyme. The histological characteristics of ependymal cyst wall are ependyma-like epithelium and neuroglial tissue.

3.Healthcare Decision Support System for Administration of Chronic Diseases.

OBJECTIVES: A healthcare decision-making support model and rule management system is proposed based on a personalized rule-based intelligent concept, to effectively manage chronic diseases. METHODS: A Web service was built using a standard message transfer protocol for interoperability of personal health records among healthcare institutions. An intelligent decision service is provided that analyzes data using a service-oriented healthcare rule inference function and machine-learning platform; the rules are extensively compiled by physicians through a developmental user interface that enables knowledge base construction, modification, and integration. Further, screening results are visualized for the self-intuitive understanding of personal health status by patients. RESULTS: A recommendation message is output through the Web service by receiving patient information from the hospital information recording system and object attribute values as input factors. The proposed system can verify patient behavior by acting as an intellectualized backbone of chronic diseases management; further, it supports self-management and scheduling of screening. CONCLUSIONS: Chronic patients can continuously receive active recommendations related to their healthcare through the rule management system, and they can model the system by acting as decision makers in diseases management; secondary diseases can be prevented and health management can be performed by reference to patient-specific lifestyle guidelines.

4.A Case of Sarcoidosis Associated with Non-tuberculous Mycobacteria

Sarcoidosis is a multisystem granulomatous disease of unknown etiology. A range of factors including tuberculous infection, beryllium exposure, and cold climate have been implicated in the pathogenesis of the condition. A 45-year-old woman presented with a 4-month history of an asymptomatic eyelid swelling and multiple erythematous papules on the neck. Histological examination of the neck, orbital soft tissue, and lacrimal gland was consistent with sarcoidosis. Ziehl−Neelsen stains and polymerase chain reactions (PCRs) for tuberculosis on the skin biopsy specimens were negative. However, the orbital soft tissue specimen was positive for non-tuberculous mycobacteria (NTM) PCR. The patient was finally diagnosed with sarcoidosis associated with NTM. Treatment with systemic steroid and hydroxychloroquine was started, resulting in an improvement of skin lesions. We herein report a case of sarcoidosis associated with NTM infection with review of the literature, as only little is known regarding the role of mycobacteria in sarcoidosis.

6.Expression of Multidrug Resistance (MDR) Associated P-glycoprotein of Acute Lymphoblastic Leukemia in Children.

PURPOSE: To evaluate the association between multidrug resistance (MDR) gene and prognosis of acute lymphoblastic leukemia (ALL), several parameters were compared according to the expression status of MDR associated P-glycoprotein. METHODS: 40 bone marrow samples from 36 children of acute lymphoblastic leukemia were analyzed with immunohistochemical stain by C219 monoclonal antibody. RESULTS: 1) The expression of MDR associated P-glycoprotein was positive in 47% at the time of initial diagnosis & 66.7% at relapse. 2) There are no stastical difference between two groups in complete remission rate, relapse rate, mean 2 years survival rate. 3) Event free survival duration was 11.3 months (+/-8.5 months) in P-glycoprotein positive group, while 20 months (+/-7.3months) in P-glycoprotein negative group (P<0.05). 4) There are no stastical difference between two groups in the mean age, sex ratio, initial WBC and immunophenotype of subjects. CONCLUSION: These results suggested the possibility of adopting MDR associated P-glycoprotein in the design of therapeutic regimen and prognostification of childhood acute lymphoblastic leukemia. However, a prospective, randomized study incorporating a larger population should precede before a firm conclusion of significance would draw regarding the prognostic and the therapeutic implication of P-glycoprotein in childhood acute lymphoblastic leukemia.

7.Hyaluronan Oligosaccharides Improve Rosacea-Like Phenotype through Anti-Inflammatory and Epidermal Barrier-Improving Effects

Background:
Rosacea is a common skin disease associated with increased expression of cathelicidin, kallikrein 5 (KLK5), toll-like receptor (TLR) 2, and abnormal barrier function. Recently, it was reported that hyaluronan (HA) could influence immune function via various receptors and HA oligosaccharides (oligo-HAs) could suppress TLR-dependent cytokine expression.
Objective:
We investigated if oligo-HAs could influence on inflammation and epidermal barrier induced by LL-37, which had a major role in rosacea.
Methods:
We cultured normal human keratinocytes and treated them with LL-37 and oligo-HAs or the LL-37 alone. A rosacea-like BALB/c mouse model injected with LL-37 was used to determine the role of oligo-HAs in rosacea in vivo.
Results:
Interleukin-8 (IL-8) and tumor necrosis factor (TNF)-α release was suppressed when keratinocytes were co-treated with oligo- HAs and LL-37 compared with keratinocytes treated with LL-37 only. Treatment with oligo-HAs resulted in decreased transepidermal water loss as well as improved redness. Decreased inflammatory cell infiltration, IL-17A and KLK5 expression and increased CD44 and filaggrin expression were also noted.
Conclusion
Our findings suggest that oligo-HA improves rosacea-like phenotype through anti-inflammatory and epidermal barrier improving effect.
8.Characteristics of Atopic Dermatitis in a Post-childhood Atopic March Group.

BACKGROUND: Little knowledge is available on the characteristic differences between patients with atopic dermatitis (AD) with and without atopic march after childhood. OBJECTIVE: To observe and compare the phenotypes of patients with AD in regards to atopic march tendency at a single point. METHODS: We enrolled patients with AD aged between 10 and 30 years. The patients were divided into the atopic march and non-atopic march groups on the basis of an investigator-designed survey questionnaire, and their serum-specific immunoglobulin E (IgE) levels or results of the skin prick test were compared. RESULTS: In a total of 182 patients enrolled in the study, 93 patients with atopic march and 89 patients with non-atopic march were observed. When their serum-specific IgE levels or results of the skin prick test were compared between the two groups, there was no significant difference, except for a in the atopic march group. Analysis of AD severity, family history of allergic diseases, and total IgE levels between the two groups showed no statistically significant differences. CONCLUSION: Our findings suggest that although no apparent phenotype characteristics could differentiate the presence of atopic march, the history of the patient's allergic diseases should be revalidated, and clinicians should watch out for future developments of atopic march when a patient shows a high-class sensitization rate to dust mite.

9.Efficacy of Biodegradable Microneedle Patches on Periorbital Wrinkles.

BACKGROUND: Biodegradable microneedle technology is a recently developed method to deliver medical and pharmaceutical medications into the skin, and is expected to yield better treatment results than topical application methods. OBJECTIVE: To evaluate the efficacy of hyaluronic acid (HA)-based microneedle patches and epidermal growth factor (EGF)-containing microneedle patches on periorbital wrinkle improvement. METHODS: A 20-week randomized, double-blind study was performed. Twenty-five Korean patients with periorbital wrinkles and a wrinkle severity rating scale (WSRS) score above 2 were enrolled into the study. The patients completed the study using the two different types of patches on each side of the designated periorbital wrinkles area every other day for 8 weeks. Patients were requested for an additional 12-week follow up. Wrinkle improvements were assessed by WSRS score, subjective patient satisfaction score, and imaging analysis using the visiometer, corneometer, cutometer, and mexameter, respectively (Courage&Khazaka, Cologne, Germany). RESULTS: Both the HA-based microneedle patch and EGF-containing HA-based microneedle patch had positive effects on WSRS score, patient satisfaction levels, and corneometer result with statistically significant differences. No significant side effects were noticed. CONCLUSION: With respect to efficacy, no statistical difference between the two groups were noted, indicating that the anti-wrinkle effects of the microneedle patch may solely be due to the HA rather than the EGF.

10.Clinical Characteristics and Antibiotic Susceptibility of Staphylococcus lugdunensis Isolated from Skin Diseases: A Retrospective Single-Center Study

Background:
Staphylococcus lugdunensis is an emerging pathogen in skin and soft tissue infections. However, until recently, the role of S. lugdunensis as a skin pathogen remained underrated.
Objective:
The purpose of this study was to investigate the clinical characteristics and antibiotic susceptibility of S.lugdunensis isolated from skin diseases.
Methods:
This is a retrospective chart review of 48 cases in which S. lugdunensis was isolated from bacterial skin cultures between 2016 and 2021.
Results:
Among the 48 patients, the mean age was 41.0 years, 54.2% were male, and 12.5% of the patients had immunosuppressive diseases. Nearly 87.4% of infections occurred below the waist and the dominant sources of isolation were epidermal cyst (31.3%) and hidradenitis suppurativa (29.2%). The rate of resistance of S. lugdunensis against β-lactam antibiotics were as follows: penicillin, 83.3%; oxacillin, 37.5%; and methicillin, 29.2%.
Conclusion
S. lugdunensis is emerging as one of the primary pathogens of skin infections. When S. lugdunensis is isolated, dermatologists should consider that it is not just a component of the skin flora but can also be a pathogen. If S. lugdunensis is identified as the cause of infection, it should be managed with appropriate antibiotic therapy.
