1.Basaloid-Squamous Carcinoma of the Esophagus: A case report.

Basaloid-squamous carcinoma of the esophagus is rare and similiar to the solid type of adenoid cystic carcinoma of the salivary gland. The origin of this tumor is unknown. The tumor was located in the lower third of the esophagus. The case of basaloid-squamous carcinoma consisted of submucosal tumor showing carcinoma with a basaloid pattern and focal squamous differentiation associated with squamous cell carcinoma or carcinoma in situ of the esophageal mucosa. A few submucosal tumor cells were positive for cytokeratin.

4.Disseminated Superficial Actinic Porokeratosis (DSAP): Report of Two Cases.

Disseminated superficial actinic porokeratosis (DSAP) is one of the recently found skin disorderes, which was differentiated from classicaI porokeratosis (Mibelli) and senile keratosis etc. in view of clinical features and hietogenesis by Marvin E. Chernosky on 1967. The one was 60 years old female patient, who had been suffered from multiple dark-brownish keratotic uniform minute papules on dorsa of the hands, forearms and neck with accompanying itching sensation since about 20 year's old of her age. The another one was 38years old housewffe, wha had been suffered from same lesions of the former on the face, neck, extremities especially sun-light exposed area. In both cases, skin lesions were exacerbated during each summer season and increase in numbers of the skin lesions with more severe itching sensation. Chernosky postulated none of a dark-sklnned race affording natural protection against sun-light had skin lesion of D S A P, we were abIe to find out two cases of typical D S A P ih the Korea.

5.14 Cases of Foreign Bodies in the Bladder.

Foreign bodies in genitourinary tract are not rare and most of them are present in the bladder. Various kinds of object are introduced into bladder through urethra by both male and female patients. Traditionally, they were introduced mainly for the purpose of sexual behavior. But recently like in our cases, many of them are inserted iatrogenically with development of endourologic procedure. We report 14 cases of bladder foreign bodies. Some of them are surprising and beyond common sense in kind of object and aim of insertion. Six cases were inserted by iatrogenic, 5 cases for masturbation, 2 cases by independently of patient`s intention by other person(s). One patient who had history of mental retardation did not remember when, how, why and by whom the foreign body had been introduced. All the foreign bodies could be diagnosed by history, symptom, X-ray and cystoscopy and removed by cystoscopic manipulation or cystostomy with ease and without complication.

7.Rapid Screening of Vancomycin Resistant Enterococci with Chromogenic Agar (ChromID VRE).

BACKGROUND: Rapid screening of vancomycin-resistant enterococci (VRE) is very important for controlling and preventing the spread of VRE in hospitals. We compared the performance characteristics of a chromogenic agar (ChromID VRE, bioMerieux, France: CA) to that of Enterococcosel agar (supplemented with 6 microgram/mL of vancomycin :EA) for direct detection of VRE from stool swabs. METHODS: Total 125 rectal swabs were collected from 57 patients in the intensive care units of an 850-bed university hospital over a period of 3 months. The samples were inoculated on EA, CA and into broth enrichment containing 6 microgram/mL of vancomycin (BE). BE was subcultured on CA after overnight incubation. RESULTS: Eighty two samples from 22 patients were positive for VRE by BE. At 24 h, the sensitivity/specificity of EA and CA were 89%/100% and 72%/100%, respectively. At 48 h, the sensitivity/specificity of EA and CA were 94%/89% and 89%/100%, respectively. CONCLUSION: CA provides equivalent sensitivity comparable to EA for the recovery of VRE at 48 h incubation, and has additional advantage of being able to differentiate between vancomycine resistant E. faecium and E. faecalis.

8.MRI of cervical carcinoma: before and after chemotherapy.

To evaluate usefulness of MR in assessment of tumor response to the chemotheraphy, we prospectively studied cases of cervical carcinoma with more then 2.5cm in diameter or stage IIb or more. Three courses of chemotherapy were performed with cisplatin and 5 F-U. MR images were obtained both before and after chemotherapy. Nine of 13 patients were undertaken radical hysterectomy after chemotherapy and MR examination. MR volumetry, stage and depth of stromal invasion were compared before and after chemotherapy. And in 9 patients who underwent radical hysterectomy, comparison of pathologic and MR imaging findings were also done. @ES The results were following. @EN 1) All tumors dectrased in volume (m=80.5%). 2) Five tumors (38.5%) reduced in stage, IB→CIS(1); IIA→CIS(1), IIA→IB(2), IIB→IB(1). 3) Depth of stromal invasion in MRI correlated well with that of histo-pathologic specimen in 7 of the 9 patients. Conclusively MR imaging is useful in assessement of tumor response to chemotherapy.
