1.Erythema Nodosum Probably Induced by Kerion Celsi.

A 6-year-old girl visited us with a 4-week history of inflammatory, pustular, tender patches and plaques on the scalp, and a 3-day history of multiple, erythematous, indurated, tender, subcutaneous nodules on both pretibial areas. A swab was taken from the scalp lesion and colonies of Tiichophyton mentagrophytes grew on culture. The histopathological findings of the leg nodule were consistent with erythema nodosum. The patient was treated with oral itraconazole and deflazacort, combined with topical potassium permanganate solution. Erythema nodosum regressed two weeks later and the kerion of the scalp regressed six weeks after starting the treatment, leaving residual scarring alopecia. The patient was diagnosed as erythema nodosum probably induced by kerion celsi, that has not been reported in the Korean literature.

2.Content Analysis on University Students who Witnessed School Violence during Their Formative Years.

PURPOSE: The purpose of this study is to explore the experience of university students who witnessed of school violence during their formative years. METHODS: The participants in this study included 30 university students who had witnessed school violence. Analysis of the data was performed using content analysis according to Downe-Wamboldt. Data were collected from September 2015 to February 2016. RESULTS: Based on this study, there were four domains of school violence including witnessing, type of reaction, aftermath and future measures. According to the analysis, these four domains were classified into 13 variables, 32 categories and 73 significant statements. CONCLUSION: The results of this study suggest that further research is needed to develop measures to address problems that have developed for students who have witnessed school violence.

3.The Effect of Glycopyrrolate (Robinul) on Scopolamine Induced Bradycardia .

Glycopyrrolate, a synthetic quarternary ammonium compound, has a similar pharmacologic property to that of scopolamine, a belladonna alkaloid. Since glycopyrrolate, a quarternary ammonium compound, dose not readily cross the blood brain barrier, it has less central effect than that of scopoiamine a tertiary-amine compound. The authors administered a small dose of scopolamine and glycopyrrolate to human volunteers, and examined the effect on the heart rate ofr both drugs. The results were as follows: 1) A small dose of scopolamine(0.1mg) showed significnat decrease in the heart rate. 2) A small dose of gylcopyrrolate(0.1mg) indeced little change in the heart rate. 3) A small dose of gylcopyrrolate(0.1mg), 10minutes after glycopyrrolate(0.1mg), showed marked increase in the heart rate. 4) A small dose of scopolamine(0.1mg), given 10 minutes after glycopyrrolate (0.1mg), showed tendency for decrease in heart kate. 5) A small dose of gylcopyrrolate(0.1mg), 10 minutes afterscopolamine(0.1mg), showed significant increases in the heart rate. 6) Scopolamine induced bradycardia was suppressed by an additional dose of glycopyrrolate and a preceding dose of glycopyrrolate.

4.Sleep Disorder Experience in Older Patients with Depression.

PURPOSE: The purpose of this study was to identify sleep disorder experience in older patients living in the community and acknowledging being depressed. METHODS: For this study, 11 older patients with depression were purposively chosen. Data were collected through in-depth individual interviews from July 2013 to January 2014 and analyzed in terms of by Giorgi's phenomenological methodology. RESULTS: The study results showed that sleep disorder experience in older patients with depression consisted of sixteen themes and five themes-clusters: 1) exposure of the causes of sleep disorders; 2) life is painful; 3) harassing themselves and their family; 4) difficulty in controlling themselves; 5) trying to deal with the sleep disorder to overcome the situation. CONCLUSION: It is necessary to develop educational guidelines for patients with sleep disorders or nursing interventions that anyone can easily provide for elders in the communities. The first priority should be given to efforts to apply diverse methods to improve sleep hygiene and minimize the period of exposure to medication before starting medication for patients with sleep disorders.

6.Factors related to Suicidal Ideation in Patients with Chronic Schizophrenia, Depressive Disorder, or Bipolar Disorder.

PURPOSE: The purpose of this study was to determine the factors that affect suicidal ideation in patients with chronic mental illnesses, such as schizophrenia, depressive disorder, and bipolar disorder. METHODS: The research participants were 160 patients with mental illnesses residing in B City and G Province. They responded to questionnaires about their suicidal ideation, self-esteem, depression, family support, social support, and internalized stigma of mental illness. The collected data were analyzed using t-test, ANOVA, Pearson correlation coefficients, and STEPWISE multiple regression with the SPSS/WIN 18.0 program. RESULTS: History of self-esteem, Depression, Social support, Psychiatric diagnosis of MDD and suicide attempt showed an explanatory power for 64% of suicidal ideation in patients with chronic mental illnesses. CONCLUSION: Specialized health and medical service personnel providing services to patients with chronic mental illnesses must determine the risk factors and protective factors for suicidal ideation associated with each illness, and establish a more individualized and systemized case management system based on customized approach.

7.Carcinosarcoma and Multiple Early Gastric Carcinomas of the Stomach.

We report a rare case of carcinosarcoma arising in the stomach. The tumor is presented in the posterior wall of the gastric lower body and antrum of a 56-year-old man. Grossly the tumor has polypoid appearance with diffuse surface ulceration and measures 5.5 cm in diameter. The tumor is accompanied with three separate well differentiated early gastric adenocarcinomas (two EGC type IIc & IIa). Microscopically, the tumor consists of moderately well differentiated adenocarcinoma in the periphery, and lobulated sarcomatous areas in the center, which shows chondroid differentiation. Transitional areas between adenocarcinoma and chondrosarcoma are evident. Immunohistochemical studies show positivity for cytokeratin and carcinoembryonic antigen in the epithelial component, and for vimentin and S-100 in the sarcomatous component. The transitional areas are positive in carcinoembryonic antigen, vimentin, S-100, and cytokeratin. The tumor extended to the subserosa and showed metastasis of only adenocarcinomatous component in six out of 47 dissected perigastric lymph nodes.

9.New Approach of Mandibular Angle Resection.

The mandibular contour determines the shape of the lower part of face and thus influences the appearance of head and neck. A prominent mandibular angle produce characteristic quadriangular, coarse and muscular appearance. To correct this facial contour to oval shape, the prominent angle can be contoured. the cutaneous approach has been used, but it leaves noticeble scar and has risk of facial nerve injury. Thus the intraoral approach has been used commonly. But because of restricition of operation field, the intraoral approach disadvantages of long operation time, high risk of complications. So, we have performed mandibular contouring procedure by new approach which uses reciprocating saw through stab incision under the mandibular body to correct the 9 patients of mandibular angle protrusion who visited our clinics recently. And we found that mandibular angle contouring by this new approach is easy, and makes the operation and recovery time shorter. There were no complications and cosmetic results were quite satisfactory.
