1.Malignant Endometrioid Adenofibroma of the Ovary: A case report.

Ovarian endometrioid adenofibroma is rare and characterized by prominent stroma. Its histologic classification is controversial but the malignant counterpart is distinguished from the borderline by the presence of confluent growth pattern of epithelium with invasion of the stroma by the endometrioid cells. A fifty-year-old woman was admitted with one month history of abdominal enlargement. Total abdominal hysterectomy with bilateral salpingo-oophorectomy was performed under the clinical diagnosis of ovarian malignancy. Grossly, the righy ovary had round, encapsulated, solid and whitish gray mass which measured 9 cm in the greatest dimension and showed peripheral small cysts. Microscopic examination revealed that the tumor consisted of endometria type glands set in fibrous stroma. The glands varied from tubules to cysts and the lining cells showed complicated architectural pattern with occasional papillary infoldings, atypical mitosis and malignant nuclear characteristics. Some cysts of glands showed intraluminal mucin products. Stromal invasions by individual epithelial cells showing malignant characteristics were often found. A brief summary of the histopathologic aspect of this tumor is presented together with review of literatures.

3.A Study on the Level of Comfort in Korean Adults: Comparison between Healthy people and in-Patients.

The purpose of this study was to compare the comfort level between healthy people and adult in-patients in Korea. The sample consisted of 248 adults (healthy adults: 124, patients admitted in university hospitals in Seoul and Jinju: 124). The research tool used for the study was General Comfort Questionnaire(GCQ) which was developed by Kolcaba(1992) and was translated by Korean. The research tool consisted of 28 items (6 items on physical dimension, 9 items on psychospritual dimension, 7 items on environmental dimension, 6 items on social dimension). Data were analyzing using the SPSS, yielding t-test, ANOVA. The results are as follows: 1) Mean scores for comfort level in healthy adults were 2.92 on a 4 point scale. 2) Mean scores for comfort level in-patient adult were 2.72 on a 4 point scale. 3) There was a significant difference between the two groups on the comfort level. The healthy adults had higher comfort level than in-patients (t=4.44, p=0.000). 4) There was a significant difference between the two groups on the comfort level of physical and environmental dimension. The healthy adults had higher comfort level than in-patients (t=2.99, p=0.003) and environmental dimension (t=8.81, p=0.000).

4.Peritoneal Diffuse Anaplastic Malignant Mesothelioma Simulating Anaplastic Carcinoma.

A case of diffuse malignant mesothelioma of peritoneum which was difficult to distinguish from metastatic anaplastic carcinoma or malignant melanoma was presented. The patient was a 50 year-old woman and complained of an abdominal pain and ascites for 20 days. Histologically, the tumor cells showed highly anaplastic appearance with sheet-like arrangement without any papillary or tubular structure. They stained Positive for cytokeratin and vimentin and negative for CEA, S-100 protein and melanoma associated antigen. A few cells within the tumor showed Positive reactivity to desmin. Ultrastructurally, the tumor cells had characteristic findings of mesothelioma such as long, exuberant, wavy microvilli with 10 to 12 of length/diameter ratio. The patient died 3 months after the diagnosis in spite of intraperitoneal chemotherapy.

5.The Effects of Preclinical Clinical Performance Examination on Nursing Students' Confidence in Nursing Skills and Critical Thinking Competence.

PURPOSE: The purpose of this study was to investigate the effects of preclinical clinical performance examination (CPX) on nursing students' confidence in their nursing skills and critical thinking competence. METHODS: The design of this research was one-group pretest-posttest, and the participants were 112 nursing students. The preclinical CPX consisted of a clinical examination, patient-nurse relationship, oral test of related knowledge, written test of the nursing process, and debriefing using comprehensive scenarios based on real patient cases. The confidence of nursing skills consisted of an 8-item NRS and the critical thinking competence consisted of a 12-item 4-point scale developed by researchers and measured in both the pretest and posttest. The collected data were analyzed using paired t-tests, ANOVA, and Pearson correlation coefficients. RESULTS: The score for confidence in nursing skills (t=10.60, p<.001) and that for critical thinking competence (t=7.03, p<.001) increased significantly after preclinical CPX. CONCLUSION: This study showed that preclinical CPX was effective in improving nursing students' confidence in their nursing skills and critical thinking competence. Therefore, preclinical CPX is expected to be utilized in nursing practice education. Additional studies including those on control groups are recommended to compare differences between the preclinical CPX group and control group.

6.Professional Self-Concept and Self-Efficacy according to the Need for Advanced Practice Nurse among Korean Nurses.

PURPOSE: The purpose of this study was to identify the need for advanced practice nurse and explain the relationship between self-efficacy and professional self-concept among Korean nurses. METHOD: The participants were 511 Korean nurses who were recruited in hospitals or public health centers in Korea. The instruments used in this study were Sherer et al's self-efficacy instrument, Arthur's professional self-concept instrument, and a structured questionnaire on the need for advanced nurse practitioners. Cronbach alpha, mean, SD, X2, t-tests, ANOVA, Pearson correlation coefficient were used to analyze the data using SPSS PC+ ver 12.0 for Windows. RESULTS: Of the nurses, 57.9% of wanted to be advanced practice nurses. The mean of professional self-concept and self-efficacy scores for the nurses were at the medium level. Professional self-concept and self-efficacy of nurses showed significant differences according to age, working location, marital status, level of education, and length of clinical experience. A significant positive correlation was found between professional self-concept and self-efficacy. CONCLUSION: Before planning to develop the role of Advanced practice nurse, there is a need to measure self-efficacy and professional self-concept among nurses. Politically, strategies to increase self-efficacy and professional self-concept among Korean nurses will be necessary in order to prepare good advanced practice nurse.

7.Fine Needle Aspiration Cytology of Pulmonary Carcinosarcoma.

Carcinosarcoma is an uncommon pulmonary malignancy characterized by carcinomatous parenchyma and sarcomatous stroma. The cytologic, immunohistochemical and ultrastructural features of a case of pulmonary carcinosarcoma suspected by fine needle aspiration cytology is presented. Only bizarre spindle cells arranged in loose groups, in microtissue fragments and in a dissociate fashion were present in the aspiration smears. They were markedly positive for vimentin. The epithelial component was not found, which was probably due to marked paucity of carcinomatous component that was proved by histologic examination of the resected tumor. The diagnosis of pulmonary carcinosarcoma should be considered whenever poorly differentiated epithelial cell groups with a malignant mesenchymal component set in a myxoid background are seen in a pulmonary cytology specimen.

8.Changes of Blood Sugar and Electrolytes According to Maintenance Fluids in General Anesthesia .

Eight patients in the state of ASA classification l, ll were investigate. Blood samples were collected before, just after, 30 minutes after and 60 minutes after induction of anesthesia. Glucose and electrolyte changes in relation to maintenance fluids in balanced and halothane anesthesia were as follows. 1) In both anesthetic techniques blood sugar level showed increasing tendency according to duration of anesthesia. 2) There were no specific changes in serum electrolytes related to type of anesthesia. 3) The administration of D/S and H/D showed a little increase in Na+ and Cl- level compared to the administration of just DsW. 4) The administration of H/d showed less increase in K+ level than the administration of DsW or D/S. 5) Just after induction of anesthesia the K+ level increased a little and decreased gradually thereafter.

9.Analysis of Factors Related to Health Behavior of the ROKAF Pilots.

BACKGROUND: ROKAF (Republic of Korea Air Force) Pilots are required to maintain optimal health status because of their job characteristics and importance as core forces. Their optimal health status can be achived by prac-ticing good health behavior. Therefore, this study tries to analyze the pilots' health behaviors for last 1 month and latest 12 hours before flight respectively, and to examine factors related to health behavior. The goal is to provide data for the development of more efficient nursing interventions. METHODS: In this study, 200 Air Force pilots was interviewed from Oct. 1 to Oct. 15, 2001. by the use of a structured questionnaire. Collected data was analyzed using SPSS Windows Version 10.0 for descriptive statistics, x2-test, Spearman's rho correlation. RESULTS: ROKAF Pilot's have good drinking and smoking habits and their alcohol drinking, smoking, dietary habits, sleeping hours is related to their Self-efficacy, perceived barrier, family support. Also, their before flight health behavior was not different largely their casual health behavior. CONCLUSION: In order to promote ROKAF Pilot's health status, programs should be designed according to age, aircraft types and marriage conditions, and strategies that enable to increase health behavior self-efficacy and to decrease perceived barrier should be employed.
