3.A Case fo Keratosis Palmaris et Plantaris Thost-Unna Type.

A case of 17 years old male with KPP of Tbost-Unna Type is reported. The akin lesions were characterized by the presence of confluent, sharply demarcated hyperk eratotic plaques on the palmar and plantar surfaces. Hypercontraction of palms and soles and hyperhidro is of scalp, face, palms and soles were also noted. Roentogenologic study revealed secondary changes of osteoporosis, flexion deformity and osteoarthritis due to soft tissue contracture and phalangeal and metatarsal bones of hands and feet showed bony resorption with tapering appearance. Histalogic findings of palmar lesion showed excessive hyperkeratosis, parakeratosis and acantosis in the epidermis and no pathology was noted in the dermis and skin appendages, He was treated with hot water compress, 5% Salicylic acid ointment and vit. A and D for 2 months and favorable effect was noted. Authors reviewed relevant literature and made discussions also.

4.Anticardiolipin Antibody in Graves' Disease.

BACKGROUNDS: Antiphospholipid antibodies which are frquently found in systemic lupus erythematosus and primary antiphospholipid syndrome are associated with recurrent abortions and thromboembolism. In this study the authors investigated whether antiphospholipid antibodies are found in Graves disease, a representative organ-specific autoimmune disease and what is the clinical implication of the antiphospholipid antibodies if they appear in Graves disease. METHODS: Anticardiolipin antibody and lupus anticoagulant activity were measured in 57 untreated hyperthyroid Graves patients. 42 euthyroid patients with thyroid nodules served as controls. RESULTS: Eight of the 57 patients with Graves disease had anticardiolipin antibody which was significantly more frequent than in control group. Six of the eight patients who had anticardiolipin antibody had IgM type antibody and two had IgG type antibody. All their antibody activity declined with several months of antithyroid drug therapy and finally disappeared when the patients became euthyroid. Presence of anticardiolipin antibody had no relationship with clinical events such as spontaneous abrtion and thromboembolism. CONCLUSION: Anticardiolipin antibody is frequently found in patients with Graves disease. They seem to appear as an epiphenomenon of autoimmunity and they seem not to have any clinical implications.

10.Acute Respiratory Failure Derived From Subglottic Stenosis in a Patient with Relapsing Polychondritis.

Relapsing polychondritis (RP) is rare, chronic, relapsing, and multisystemic inflammatory disease targeting the cartilaginous structures. Respiratory track involvement occurs in approximately half of the cases. Subglottic stenosis is a rare manifestation of RP. Here, we report a case of RP with a subglottic stenosis, resulting in acute respiratory failure. A 63-year old man was admitted complaining of multiple joint pain, general weakness, weight loss, throat pain, hoarseness, exertional dyspnea, and hearing difficulties. A laryngoscopy and neck CT revealed a subglottic stenosis. Four days after admission, he complained severe dyspnea resulting in acute respiratory failure. Immediately, a tracheostomy was done for airway preservation. After high dose steroid therapy, the general symptoms were improved. However, the subglottic stenosis was sustained. Thus, a laryngotracheal augumentation and stent insertion was performed. The speech valve was then replaced. The subglottic stenosis was managed with low dose steroid and monthly cyclophosphamide pulse therapy, and the patients has been followed up regularly.
