2.A Case of Short Stature with 46, XXp+(Xqter-->Xp22::Xp22-->Xqter).

We experienced a case of Turner variant in 17 year old girl. She showed normal appearance except short stature. The patient was born at term with a birth weight of 2200g and her height was below the 3 percentile at admission. Thelarche appeared at the age of 13 years. FSH and LH levels were elevated, estradiol was within normal range, but progesterone level was decreased, bone age was 14 to 15 year old, and the small sized uterus was detected by abdominal ultrasonogram. Karyotype analysis revealed 46,XXp+(Xqter-->Xp22::Xp22-->Xqter). Her height didn't grow properly despite serial growth hormone therapy, because her epiphyseal growth plate was nearly closed.

3.Effects of Growth Hormone Therapy in Prader-Willi Syndrome.

PURPOSE: Growth hormone(GH) has not only growth promoting effect but also various metabolic effects. We evaluated GH effects by anthrometric data, biochmical data, electrolytes and simple CT in patients with Prader-Willi syndrome. METHODS: Nine children with Prader-Willi syndrome(PWS) were studied. The children were treated with GH(0.6U/kg/week) for 6 months. Before and after therapy we measured height, weight, waist, hip, and thigh. Blood sampling for eletrolytes, HgA1C, lipid profiles and other biochemistry were done in all patients before and after therapy. We also compared fat distribution with scan. RESULTS: Height standard deviation (SD) score increased from -0.7 to -0.5 and weight SD score decreased from 5.3 to 4.9. Body mass index(BMI) decreased from 28.2kg/m2 to 27.2kg/m2. But the changes in height, weight and BMI were not significant statistically. The waist/hip ratio decreased from 1.04 to 0.97(P<0.05), Thigh circumference had been decreased from 58.2+/-21.7cm to 49.9+/-6.9cm insignificantly. The visceral fat were decreased from 7,613+/-1,760 to 5,022+/-1,533 after GH therapy, and thigh muscle mass was increased from 6,358+/-1,616 to 7,175+/-2,155 (P<0.05). Total cholesterol and triglyceride decreased and HDL cholesterol increased after therapy although they were insignificant statistically. There were no differences in electrolytes, HgA1C, other biochemistry(Ca, P, protein, albumin, BUN, Cr) before and after therapy. CONCLUSION: In children with PWS, waist/hip ratio and fat mass were reduced and muscle mass was increased after GH therapy. There was tendency that total cholesterol and triglyceride decreased and HDL cholesterol increased after therapy. We confirmed that GH therapy had not only growth promoting effect but also metabolic effect on lipid and protein metabolism in children with PWS.

4.A Clinical Study of Accidental Pediatric Patients.

The propose of this study was to propose for protection of childhood injury or accident and to provide safety measures. Data was collected from 2,052 who visited emergency room of E University Hospital during 1 year period form January to December, 1996. The results were as follows : 1. The most common type of accident was fall down (53.4%). 2. Male patients outnumbered female by the ratio of 1.68 : 1. 3. The highest incidence rate of accidents were observed in children between 1-3 years age group(34.9%). 4. Accidents were seasonally more frequent in spring(28.2%). 5. Accidents mainly took place at home(63%). 6. The time of accidental occurrence appeared to be predominantly high during a day 8:00 PM to 0:00 AM(35.1%). 7. The most frequent involved part of body was head and face(74%). 8. Most patients discharged from the hospital in good condition(85.4%). Though clinical analysis, in the relation to accident, there were statistically significant difference in sex, age, season, injury place(all=0.000).

5.Treatment of Squamous Cell Carcinoma in Extremity & Trunk.

PURPOSE: The purpose of this study is to compare general survival rate and survival rate according to expectable prognostic factors by analyzing the result of treating a patient of squamous cell carcinoma. MATERIALS AND METHODS: From Mar. 1999 to Feb. 2011, 151 patients were pathologically confirmed as squamous cell carcinoma of limbs and body in our hospital, and among those patients, 51 patients underwent the surgical treatment. This study included 41 patients who underwent the surgical treatment and were followed-up for more than 12 months. The mean age of population was 64.4 years. 31 males and 10 females were included. Wide excision with following skin grafts or flaps for reconstruction (29 cases) was mostly performed, but amputation (12 cases) was also performed for cases with extremities where resection margin was difficult to obtain and cases with neural or vascular invasion. 8 patients underwent chemotherapy or radiotherapy after resection, and 33 underwent the operation only. Stages were classified by AJCC Classification, survival rate was calculated by Kaplan-Meier method and survival rate of groups was compared by Log-rank test. For the expectable prognostic factors related to survival rate, location of primary lesion, cause of disease, pathologic grade, staging, surgical method, additional anticancer therapy were examined and each survival rate was compared. RESULTS: The average follow-up period was 65.2 (12-132) months. Thirty patients survived out of 41 patients till last follow up. The overall survival rate in 5 years was 77%. Three cases (7.3%) had local recurrence, and 7 cases (17.0%) had metastasis. The average period of recurrence from operation was 27 (18-43) months. Possible prognostic factors such as location of primary lesion, cause of disease, pathologic grade, staging, additional anticancer therapy showed no significant difference in survival rates. However, patients with amputation showed significantly lower survival rate than those with wide excision. CONCLUSION: In analysis the results of treating 41 cases of squamous cell carcinoma, the overall 5-year survival rate was 77%. And, among the several prognostic factors, only the surgical method was significant statistically.

6.Operative Methods of Chronic Anteromedial Rotatory Instability of Knee: Advancement and Bony Reinsertion Operation of Tibial Collateral Ligament

Chronic anteromedial rotatory instability has not been demonstrated to have a satisfactory solution and we have experienced recurrence of the instability after operations. So the authors devised a new operative technique for the TCL (Tibial collateral ligament), that is, burying of the torn end of the ligament beneath the “ㄷ” shaped bony flap. It would better be called advancement and bony reinsertion operation. And we performed modifed MacIntosh tenodesis for the torn anterior cruciate ligament. The results were as follows: l. Of six knees, four (66.6%) rated “good”, two (33.3%) “fair”, and none (O%) “poor” (follow-up period; at least 6 months). 2. The technical problem of the advancement and bony reinsertion operation was discussed.

7.Ocular Dominance Determined by Near Point of Convergence Test in Intermittent Exotropia.

This study was designed to validate the usefulness of the near point of convergence(NPC)test in determination of dominant & non-domnant eyes in intermittent exotropia patients. We performed NPC test in 36 intermittent exotrpic patients, and then, determined the non-dominant eye which deviated outward beyond the NPC. The dominant eyes determined by the NPC were compared to those that were determined by amblyopia, sensory fusion and photophobia. Best corrected visual acuity was used to divide patients into two groups : 26 patients without amblyopia, and 10 with amblyopia. Among the amblyopia group, if the eyes with better visual acuity were taken as dominant eyes, then eight(80%)cases were identical with the NPC test and two(20%)cases were different. Ten patients had sensory anomaly, and among then, nine(90%)cases were identical with the NPC test and one(10%)case was different. Twenty six(64%)patients had photophobia in one eye and all(100%)cases were identical with the NPC test. In intermittent exotropia, determination of dominant eye by the NPC test showed high coincidence with other tests. Therefore, we suggest that the NPC test is an easy and accurate method in determining either the dominant or non-dominant eye for surgery.

8.Mutagenicity of Human Urine Excreted after Ingestion of Roast Beef.

This study was undertaken to observe the mutagenic occurrence in urine excreted after the ingestion of roast beef. Two healthy nonsmoker persons of both sex were selected for this test, employing two strains (TA98, TA100) of Salmonella typhimurium according to Ames' method. The mutagenic activity began to appear in urine of both sex three hours after ingestion of 300 g of roast beef, gradually increasing until 6 hours and declining thereafter.

9.A Study on the Pre-hospital Emergency Care in workplace through the Analysis of Fatal Work-place Injuries.

OBJECTIVES: This study was designed to evaluate the problems of pre-hospital Emergency medical care system (EMS) in workplace. We analysed 25 fatal work-place injuries during the recent 4 years and the work-place EMS of 8 enterprises located in Masan-city, Changwon-city and Kuje-island. METHODS: The safety managers and the members of Dept. of safety in the labor unions were interviewed about the work-place EMS. And we investigated on the injury reports, the work-place medical-room records and the medical records of emergency center for fatal 25 cases. RESULTS: The enterprises had the at-risk machines and processes, volatile materials and high-altitude working processes. There were duty doctors in only 3 enterprises but a few duty nurses or health-care providers in the others. The time spent for the education to the workers on safety was 24hrs/yr in 3 enterprises in 1998 but less than Bhrs in the others. There were medical service center in all enterprises but the ambulances in three. The time for activation of the ambulance was ranging from 5 minutes to 10 minutes in 6 enterprises, and from 10 minutes to 30 minutes in two. The patient transportation to the emergency center was possible within 30 minutes in all enterprises but there were no equipments for airway maintenance and shock management in all enterprises. The 15 (60%) fatal injuries were occurred at one enterprise. The 64% of casualties had the duration of job-employment more than 10yrs and the 68% were suffered the typical type of work-place injury as descending injuries, collisions and falls. Most of all primary calls for rescue were concentrated on the fire-service agencies. But in 85% of fatal injtories, the tome for the activation of ambulance was more than 10 minutes and no emergency care was taken in the field in 48 percent of casualtles. The transportation time to the emergency center was more than 30 minutes in 50 percent. It toolk from injury to death was less than one hour in the 40 percent of all cases, and from one hour to four in the 50 percent. The causes of death in the 68 percent were the head-and-neck injuries or thoracic injuries. CONCLUSIONS: We found that there was Insufficiency of the education associated with work-place injury for workers, manpower and facilities, equipments related to the work-place EMS, the problems of the delay in transportation system.

10.A Case of Deep Vein Thrombosis Associated with Pulmonary and Renal Thromboembolism.

The deep vein thrombosis had been described as the reaction of vascular inflammatory change by John Hunter in 1773, and Virchow described the pathophysiology of etiologic factors for the first time in 1856. Deep Vein thrombosis and pulmonary embolism is common disease in the North American and European countries. It was reported that deep Vein thrombosis and pulmonary embolism were associated with 300,000 to 600,000 hospitalizations for each year and that as many as 50,000 individuals die each year as a result of pulmonary embolism in the United States. In Korea, deep vein thrombosis is uncommon, complicating pulmonary embolism & renal thromboembolism are also rare, and many cases had not specific history & precipitating factor. The report is for one case of deep vein thrombosis associated with pulmonary & renal thromboembolism, who was 34-years-old, admitted to Masan Koryo hospital because of pain and edema in left lower extremity, had not specific history & precipitating factor. The thrombosis(or thromboembolism) involed his left lung & kidney, and the patient present with severe swelling of affected limb and oliguria. He underwent leg elevation under absolute bed rest and anticoagulant therapy. The significant improvement was done under medical treatment. We report one case of deep vein thrombosis with a brief of literatures.
