2.Three-Weekly Gemcitabine Plus Cisplatin Chemotherapy in Patients with Locally Advanced or Metastatic Non-small-cell Lung Cancer: Phase II Study of the Korean Association for the Study of Lung Cancer.

PURPOSE: As one of the new chemotherapeutic agents, gemcitabine is widely used as a four-week schedule in combination with cisplatin in the treatment of advanced non-small-cell lung cancer (NSCLC). In this study, we evaluated the efficacy, tolerance, and survival effect of this combination in a three-week schedule in patients with locally advanced inoperable or metastatic NSCLC. MATERIALS AND METHODS: Between January 2000 and September 2002, previously untreated 124 NSCLC patients were enrolled and 118 patients, who completed at least two cycles of chemotherapy, were evaluated. Patients received gemcitabine 1200 mg/m2 on days 1 and 8, cisplatin 75 mg/m2 on day 1 of a 21-day cycle, for a maximum of 6 cycles. RESULTS: They were 81 men and 37 women. Clinical stage IIIB was present in 56 (47.5%), and stage IV in 62 (52.4%) patients. Sixty-seven patients (56.8%) had a performance status of ECOG grade 0 or 1 and fifty-one patients (43.2%) of grade 2. During the period of chemotherapy, grade 3/4 leukopenia and neutropenia were observed in 19.5% and 31.4%, respectively and grade 3/4 thrombocytopenia in 7.6%. The overall response rate was 52.5% among the 118 patients. Overall median survival time was 12.2 months, and one-year and two-year survival rates were 50.2% and 20.4% respectively. The presence of response to chemotherapy, ECOG performance status of grade 0~1, and women showed better survival by the univariate analysis (p=0.010, 0.001 and 0.015, respectively). CONCLUSION: A three weekly gemcitabine/cisplatin doublet was relatively well tolerated, with an acceptable response rate and a reasonable median survival in locally advanced inoperable or metastatic NSCLC.

3.Impact of COVID-19 infection during the postoperative period in patients who underwent gastrointestinal surgery: a retrospective study

Purpose:
The coronavirus disease 2019 (COVID-19) pandemic has led to significant global casualties. This study examines the postoperative impact of COVID-19 on patients who underwent gastrointestinal surgery, considering their heightened vulnerability to infections and increased morbidity and mortality risk.
Methods:
This retrospective observational study was conducted at a tertiary center and patients who underwent gastrointestinal surgery between January 2022 and February 2023 were included. Postoperative COVID-19 infection was defined as the detection of severe acute respiratory syndrome coronavirus 2 RNA by RT-PCR within 14 days after surgery. Propensity score matching was performed including age, sex, American Society of Anesthesiology physical status classification, and emergency operation between the COVID-19-negative (–) and -positive (+) groups.
Results:
Following 1:2 propensity score matching, 21 COVID-19(+) and 42 COVID-19(–) patients were included in the study. In the COVID-19(+) group, the postoperative complication rate was significantly higher (52.4% vs. 23.8%, P = 0.023).Mechanical ventilator requirement, intensive care unit (ICU) admission, and readmission rate did not significantly differ between the 2 groups. The median length of ICU (19 days vs. 4 days, P < 0.001) and hospital stay (18 vs. 8 days, P = 0.015) were significantly longer in the COVID-19(+) group. Patients with COVID-19 had a 2.4 times higher relative risk (RR) of major complications than patients without COVID-19 (RR, 2.37; 95% confidence interval, 1.254–4.467; P = 0.015).
Conclusion
COVID-19 infection during the postoperative period in gastrointestinal surgery may have adverse outcomes which may increase the risk of major complications. Preoperative COVID-19 screening and protocols for COVID-19 prevention in surgical patients should be maintained.
4.Pulmonary Lymphangioleiomyomatosis with Chylous Pleural Effusion.

Pulmonary lymphangioleiomyomatosis is a disease involving the proliferation of atypical smooth muscle cells trom the perilymphatics, peribronchial and perivascular region of the lung and the retroperitneum. The disease usually affects women of child-bearing age. We recently experienced a case of pulmonary lymphangioleiomyomatosis in a 31-year-old women who had suffered from a chylous pleural effusion. Histologic confirmation of lymphangioleiomyomatosis was made upon a video-associated thoracoscopic lung biopsy. Here we report this case with a brief review of the literature.

5.Comparison of Face-to-Face Interview Questionnaires and Medical Records Data for Smoking Habits in Lung Cancer Patients.

BACKGROUND: This study evaluated the accuracy of smoking habit from the data obtained from the medical records of lung cancer patients against the data obtained form face-to-face interview questionnaires METHODS: The smoking habits of 225 lung cancer patients were categorized into never smoked, ex-smoker and current smoker in face-to-face interview questionnaire and medical record taken at the time of admission for a diagnosis. The overall agreement between two sources was evaluated. The factors affecting the disagreement between two sources and the level of data omission of the smoking habits in medical records were analyzed suing multiple logistic regression. RESULTS: The smoking habit between two sources showed moderate overall agreement(Kappa (kappa)=0.60). The lowest agreement was observed in the ex-smokers(kappa=0.49). Multivariate analysis revealed an age of 65 or older to be a statistically significant factor associated with the increasing disagreement risk compared with those 64 or younger (OR 3.02; 95% CI 1.58-5.80). The omission rate of smoking habits in the medical records was 18.2%. Adenocarcinoma was shown to be a statistically significant factor of associated with an increasing omission rate compared with squamous cell carcinoma (OR 3.00; 95% CI 1.19-7.59). CONCLUSION: The smoking habits obtained from medical record moderately reflect their true behavior. However, the smoking habit data from medical record should be used with caution when being used in a clinical study or cohort study of lung cancer.

Migraine equivalents include cyclic vomiting, acute confusional states, and benign paroxysmal vertigo. Cyclic vomiting is characterized by recurrent, sometimes monthly, boults of severe vomiting that may be so intense dehydration and electrolyte abnormalities occur, particularly in an infact. We experienced a 3-year-old girl who showed manifestations of cyclic vomiting and abdominal pain. We report this case with a review of related literatures.

7.A Case of Leptomeningeal Carcinomatosis Presenting as a Neurological Complication of Stomach Cancer.

Leptomeningeal carcinomatosis, an important and serious neurological complication of malignant cancers, is asso- ciated with severe disability and high mortality. It is most commonly seen in patients with breast cancer, lung cancer and malignant melanoma. However, leptomeningeal carcinomatosis induced by gastric cancer has been rarely reported, although gastric cancer is one of the most common causes of cancer-related deaths in Korea. We report a case of a 76- year-old woman with leptomeningeal carcinomatosis presenting as a neurological complication of gastric cancer and con- firmed by cytology examination of the cerebrospinal fluid and brain magnetic resonance imaging with gadolinium enhan-cement.

8.Achievement of Anterior Teeth Arrangement Using a Single Stage of Clear Aligner

Purpose:
The aim of this study was to evaluate the degree of achievement of anterior tooth movement using a thermoplastic clear aligner by superimposing pretreatment, setup, and post-treatment models and to analyze the factors that influence it.
Materials and Methods:
Forty patients who underwent clear aligner treatment at the Department of Orthodontics were included in the study. The pretreatment, setup, and post-treatment models were superimposed, and the achievement rates were evaluated. We analyzed the factors that caused changes in achievement rate on each condition.Result: The average achievement rate of total tooth movement was 57.65%. Mesiodistal and anteroposterior displacements were achieved better than vertical displacement and rotation. In each displacement, posterior, mesial, and extrusive movements were better achieved than the rotation. Spacing was better corrected than crowding, especially in the mesiodistal direction. Most of the tooth movements were achieved better when using 3-step thicknesses of aligners were used.
Conclusion
The treatment results of clear aligners may differ depending on the type of tooth movement, age and other treatment conditions. Clinicians should consider unfavorable conditions based on the results of this study.
9.Decline in Hospital Visits by Patients with Schizophrenia Early in the COVID-19 Outbreak in Korea

Objective:
This study investigated trends in hospital utilization by patients with schizophrenia during the first wave of the COVID-19 outbreak in Korea.
Methods:
The Prophet algorithm was used to predict the monthly number of patients with schizophrenia in 2020 based on medical insurance data between 2010 and 2019. The projected expectations were compared with the actual number of patients receiving outpatient and inpatient treatment each month in the first half of 2020. We conduct interrupted time series analyses of short-term data to determine the significance of recent changes in the trend of hospital visits by patients with schizophrenia.
Results:
The prediction model showed that the actual number of patients receiving treatment each month during the early COVID-19 outbreak decreased by up to 3.6% compared to the projected expectations. The interrupted time series model also revealed a significant change in hospital utilization compared to the year before the onset of COVID-19 in Korea (F = 8.961, p = 0.010).
Conclusion
This suggests that many patients with schizophrenia were not receiving adequate treatment during the COVID-19 outbreak. A strategy should be developed to keep treating patients with schizophrenia during the COVID-19 pandemic.

Dermatomyositis is a disorder of unknown etiology characterized by an inflammatory myopathy involving striated skeletal muscle and associated characteristic skin lesions. In recent years, there has been increased the awareness of the association of malignant tumor with this disorder. But the relations between dermatomyositis and malignant tumors remains controversial. Comparing to other countries, the incidence rate may be said to be low in Korea. However, we suggest that our concerning makes it's associated malignant tumors discovered. The cutaneous necrosis, elevated erythrocyte sedimentation rate, older age, or acute onset of symptoms proved to have the predictive value of cancer in dermatomyositis. Therefore, we think that more detailed diagnostic workups for malignant neoplasms are recommended in the patient with this criterion. Here we reported a case.of dermatomyositis accompaning by lung cancer with brief review of literatures.
