3.Loss of Best Corrected Visual Acuity after LASIK.

Loss of best corrected visual acuity(BCVA)is a landmark of safety, as one of important complications of refractive surgery. To evaluate causes of 2 lines or more loss of BCVA after laser in situ keratomileusis(LASIK), 206 eyes of 139 patients, who had undergone LASIK and had been followed up for 6 months or more, were included in this study. During the follow-up, nineteen eyes(9.2%, 19/206)showed 2 lines or more loss of BCVA postoperatively. The causes of BCVA were irregular astigmatism(15 eyes, 7.3%), retinal complications(3 eyes, 1.5%), and infectious keratitis(1 eye, 0.5%). All irregular astigmatisms had occured within 1 month postoperatively and 13 eyes of all 15 eyes spontaneously recovered after postoperative 3 months. The persistent loss of BCVA occurred in 5 eyes(2.4%, 5/206). Three cases of all 5 persistent losses of BCVA were due to retinal complications. In conclusion, our results suggest that the most common cause of loss of BCVA after LASIK is irregular astigmatism. Irregular astigmatism is a temporary situation which has a tendancy of spontaneous recovery. Thus careful observation is recommended in irregular astigmatism after LASIK. The retinal complications of LASIK may lead to permanent loss of BCVA. Therefore it is needed to examine preoperative fundus closely and to explain possible postoperative complicaitons to the patients and their family.

4.Ultrastructural Study on Basement Membrane Thickening of Iris Capillaries in Diabetic Patients.

The early ultrastructural changes of basement membrane thickening and degeneration of the pericyte were noted in retinal capillaries in diabetic patients. However no early ultrastructural changes were reported in diabetic iris capillary. The authors studied the ultrastructural features of basement membrane of iris capillaries which were obtained during cataract surgery in five diabetic patients and compared with those obtained in five nondiabetic patients. The endothelium basal lamina, pericyte basal lamina and membrane-like pericyte matrix in didabetic iris capillaries were thicker than those in nondiabetic Iris capillaries. The thickening of pericyte basal lamina is more prominant than the endothelium basal lamina in diabetes. The duration of diabetes had no relationship to endothelium basal lamina but, showed statistically significant relationship to thickness of pericyte bascal lamina and pericyte matrix.

5.Isolation of Enterotoxin - positive Strains of Clostridium perfringens Type A in Korea.

Clostridium perfringens is an anaerobe responsible for a wide range of diseases in animals and humans. Symptoms associated with C. perfringens food poisoning are caused by enterotoxin expressed only during sporulation of C. perfringens. It has been known that only 6% of global C. perfringens isolates carry the enterotoxin gene. We found 2 strains of enterotoxigenic C. perfringens out of 33 strains isolated from various sources in Korea using PCR. It was also found that these two strains were both type A that were strongly associated with food poisoning by checking the presence of four major lethal toxins (a-, B-, e-, l-toxin) using PCR. These results suggest that foodborne illness caused by C. perfringens may be common in Korea and that public education is necessary to prevent contamination of foods by this organism.

6.Analysis of the Shoulder and Elbow Section of the Korean Orthopedic In-training Examination.

BACKGROUND: The aim of this study was to analyze the questions in the shoulder and elbow section of the Korean Orthopaedic In-Training Examination (KOITE) and compare them with those of the US Orthopaedic In-Training Examination (US OITE). METHODS: Twenty-nine questions in the shoulder and elbow section of the KOITE from 2010 to 2014 were analyzed and compared with those of the US OITE (80 questions) by literature review. A thorough analysis of the contents was performed after categorizing as topics, diagnostic tools, treatment modalities, taxonomic classification, and references. RESULTS: The shoulder and elbow section of the KOITE was 5.8% weight which was similar to the US OITE (5.9%). The most commonly appearing topic was anterior labral injury (17.2%) on the KOITE compared to instability and arthritis (21.3%, each) on the US OITE. Magnetic resonance imaging was most frequently appeared imaging modality on the KOITE (41.0%) compared to the radiograph on the US OITE (43.0%). The Latarjet procedure was the most commonly asked treatment modality (22.2%) on the KOITE, whereas arthroplasty (33.3%) on the US OITE. The KOITE showed an even taxonomic classification distribution compared to the US OITE. Campbell's operative orthopaedics covered 96.6% questions as a reference on the KOITE compared to the Journal of Bone and Joint Surgery, American Volume on the US OITE, which covered 45.0%. CONCLUSIONS: This specific analysis shows us current trends of the shoulder and elbow section of the KOITE and it might be developed for use in the educational curricula for the trainee.

7.Lesions in the splenium of the corpus callosum: Clinical and radiological implications

Background: Brain MRI may unexpectedly display abnormalities in splenium of the corpus callosum
(SCC). However, the clinical implications of this lesion are unclear and are not always consistent
with ischemic infarctions. We performed this study to clarify the clinical and radiological implications
in patients with SCC lesions. Methods: We retrospectively reviewed consecutive patients with MRIreported
SCC changes between 2009 and 2012. We analyzed clinical and radiological findings,
etiologies, cognitive impairment, and clinical outcomes. Results: We found 30 patients (16 females;
mean 50.5 years) who had SCC lesions on MRI. Confusion was the most common clinical finding
in 50% of cases. Cerebral infarction was the most common etiology (50%). The most consistent
SCC changes on MRI were low signal in T1WI, high signal on T2WI and FLAIR, and high signal
on DWI. We classified SCC lesions into in situ SCC lesions (SCC only) and multiple (SCC plus)
lesions for patients with multiple lesions. The clinical symptoms of SCC only lesions were relatively
mild. Cognitive functions were evaluated by Mini Mental State Examination (MMSE) and clinical
dementia rating (CDR) scale at the time of discharge and patients with SCC only lesions showed less
impaired cognition compared with those with SCC plus lesions. Clinical outcomes were evaluated
by the modified Rankin scale at 1 month and patients with SCC only lesions revealed good clinical
outcomes compared with those with SCC plus lesions.
Conclusions: MRI-reported SCC lesions may have heterogeneous etiologies and present with various
symptoms. The clinical course and outcome are relatively good, particularly in small isolated and
oval shaped SCC lesions.
8.Lesional location of intractable hiccups in acute pure lateral medullary infarction

Background & Objective: Hiccups is a disabling condition of lateral medullary infarction (LMI).
Unlike other symptoms of LMI, the anatomical lesions of hiccups are not well known. Few studies
have evaluated the relationship between the lesional location of LMI and hiccups. We performed this
study to correlate hiccups and magnetic resonance imaging (MRI)-based lesional location in pure
LMI. Methods: Between January 1997 and February 2013, we identified 24 patients with pure LMI
who presented with hiccups in addition to typical lateral medullary syndrome. Sixty six pure LMI
patients without hiccups were included as a control group. Clinical and radiologic findings were
compared between the two groups. MRI-identified lesions were classified rostrocaudally as rostral,
middle and caudal, and horizontally as typical, ventral, large, lateral and dorsal. Results: The pure
LMI patients with hiccups had significantly more frequent aspiration pneumonia (P = 0.001) and
longer hospital stay (P = 0.03). The patients with hiccups significantly more often had dorsal rather
than ventral lesion at horizontal levels (P = 0.012). But, there were no rostro-caudal differences at
vertical levels (P = 0.162).
Conclusions: We suggest that pure LMI associated with hiccups often locates in the dorsal medulla
at horizontal correlation. This MRI-based comparative study has advanced the understanding of the
neural substrate for hiccups in LMI, and indicates that hiccups become predictable when specific
lesional locations in the lateral medulla are considered.

10.Expression of p73 in Non-small Cell Lung Carcinomas.

BACKGROUND: The p73 is a recently identified homologue of the tumor suppressor gene, p53, and it has been found to induce apoptosis and inhibit cell proliferation. However, its role in the development of tumors is unclear. This study examined the expression of p73 in patients with non-small cell lung carcinomas (NSCLCs) to determine its clinical significance and association with the expressions of p53, pRb, and mdm2. METHODS: A total of 183 NSCLCs were analyzed immunohistochemically using a tissue microarray. RESULTS: The p73 protein was expressed in the cell nuclei in 156 (85.2%) out of the 183 cases. There was no correlation between the p73 expression and the clinicopathological variables. However, there was a correlation between the p73 expression and the mdm2 and pRb expressions. Multivariate Cox survival analysis identified tumor size and lymph node metastasis to be independent prognostic factors, but the p73 expression was not found to be associated with the patients' survival. CONCLUSIONS: p73 is commonly expressed in NSCLC and it might, in conjunction with pRb and mdm2, be involved in the development of these tumors.
