4.The Role of Multistage Maximal Exercise Test in the Evaluation of Patient for Various Heart Disease.

The multistage maximal exercise test was performed to 135 cases with various heart disease and 207 control group between jan. 1980 and Dec. 1980 in KoreaGeneral Hospital. The change of heart rate, blood pressure and ST segment in various heart disease were compared with those in control group. The results were followings; 1. There were 130 male and 77 female of cases with control group, whose mean age were 50 years old. 2. General check up was the most common underlying disease of control group(67 patients) and the others were gastritis (28), neurosis (24), diabets mellitus (17), hepatitis (4) and peptic ulcer (4) frequency. 3. Change of heart rate, blood pressure and ST segment in control group during GXT were 89~154, 119/80~157/88 mmHg and 0.6~2 mm. 4. There were 81 male and 51 female of cases with various heart disease group, whose mean age were 50 years old. 5. Atherosclerotic heart disease was the most common underlying disease of various heart disease group (49 patients) and the others were hypertension (34), hypertensive cardiovascular disease (22), labile hypertension (14), angina (7), arrythmia (6) and others (3) in frequency. 6. Change of heart rate, blood pressure and ST segment in various heart disease group during GXT were 85~148, 140/93~178/102 mmHg and -0.7~1.3mm. ST segment in atherosclerotic heart disease, hypertension, hypertensive cardiovascular disease, valvular heart disease and myocardial infarction were significantly depressed(p<0.05) compared with ST segment in control group.

5.Clinical Observation on Complications of Transurethral Resection of the Prostate.

A clinical observation was made on 75 cases of TURP from March 1984 to August 1987 had been studied in the Department of Urology, Eul Ji General Hospital. The results were summarized as follows. 1. TURP was performed in 65 cases of benign prostatic hyperplasia and 10 cases of the pro static cancer. Of the 75 cases, blood loss was calculated in 32 cases and change of serum electrolyte was in 42 cases. 2. Weight of resected tissue was 7.1+/-5.8 grams(Mean+/-S.D.). 3. Blood loss during TURP was 317+/-199.2ml. There was a correlation between the total loss and the weight of resected tissue, but the blood loss per gram of resected tissue decreased with the larger glands. 4. Serum sodium concentration was decreased in 24 of the 42 cases and increased in 10 cases and no change in 8 cases. And these change in serum sodium concentration showed no significant correlation with the weight of resected tissue, the amount of irrigating fluid and operation time. 5. Of the 75 patients who had performed TURP, 27(36%) experienced some types of non-fatal complication, the remaining 48(64%) recovered without event. 6. Hypotension during TURP was the most common complication(13.3%). It seemed to be temporary phenomenon due to sympatholytic effect in spinal anesthesia. 7. There was no fatal case in this series.

6.A Clinical Observation on Renal Tuberculosis.

A clinical observation was made on 46 cases of renal tuberculosis who were admitted to the Department of Urology, Eul Ji General Hospital, Seoul, Korea, during the period from January 1978 to December 1982. Following results were obtained. 1. The most common symptoms were urinary frequency (47.8%), gross hematuria (43.5%) flank pain (32.6%) and dysuria (21.7%). 2. Tubercle bacilli were demonstrated in 37% of patients by Ziehl-Neelson staining. The most common findings of urine were hematuria (73.9%), Pyuria (54.4%) and proteinuria (32.6%). 3. Excretory urographic findings in 46 cases of renal tuberculosis were non visualization (45.7%), calyectasis (28.3%), delayed visualization (15.2%), ureteral deformity (10.9%). 4. The most common finding of cystoscopy was hemorrhagic patches (44.4%), followed by sealed off ureteral orifice (s) in 30.6%, ulceration and tubercle formation in 25.0%. 5. Patients were managed by chemotherapy only in 47.8%, chemotherapy with nephrectomy in 43.5%, chemotherapy with reconstructive surgery in 8.7%.

7.Two Cases of Malignant Schwannoma in Association with Neurofibromatosis.

We report two cases of malignant schwannoma of skin in association with non-familial neurofibromatosis. Case 1, a 47 year old man, had a large subcutaneous tumor on the sacral area and case 2, a 62 year old woman, a painful, ulcerating tumor on the posterior aspect of the left arm. Both cases were histopathologically confirmed as malignant schwannomas and immunohistochemical studies showed 5-100 protein in the tumor cells. After surgical excision of the tumors, case 1 was lost to follow up, while case 2 remained without evidence of disease for more than one and half years.

8.Clinical Observation on the Cryptorchidism.

A clinical observation was made on 77 cases of cryptorchidism who had been admitted Eul ji General Hospital in Seoul and Tae Jon, Korea, during the period from January, 1976 to June, 1984 The results were as follows 1. The cryptorchidism had the highest incidence rate among the anomalies of the genitourinary tract (47.8%) . 2. The age distribution was ranging from 3 years to 35 years and the most frequent age group 6 to 10 years old. 3. The bilateral cryptorchidisms were observed in 24 cases (31.2%) , of the unilateral 53 cases, right in 35 cases and left in 18 cases. 4. Among location of cryptorchidisms, the inguinal types were most frequent (56.5%) 5. Hormonal therapy was done on 12 cases of cryptorchidisms but 5 cases were respondent with partial descent and 1 case was responsive with complete descent. 6. Orchiopexy was performed on 92 testes, orchiectomy on 6 testes and 2 cases were confirmed as monoorchism on exploration. 7. On the histopathologic findings of testicular biopsy in 6 cases, there were immature seminiferous tubules, germ cell aplasia, basement membrane thickening and aspermatogenesis.

9.5 cases of the Cryptorchidism Treated with Hormonal Therapy (H.C.G.).

5 cases of cryptorchidisms were treated with H.C.G. during the period from Feb. 1980 to Sept. 1980. Among them, in 3 cases of bilateral cryptorchidism, H.C.G. effect was surprisingly excellent in doses of 5,500 I.U. of puberogen for 7 weeks, 6,000 I.U. for 4 weeks and 4,000 I.U. for 4 weeks respectively. In 2 cases of unilateral cryptorchidisms the H.C.G. effect was rather unsuccessful, but seemed to be meaningful. H.C.G. treatment was thought to be valuable prior to surgery especially in bilateral cryptorchidism.
