2.Phycomycosis Involving Ankle Joint: Report of a case.

Phycomycosis is an uncommon opportunistic fungal infection, involving several organs, such as brain, lungs, gastrointestinal tract and skin. Invasion of joint cavity by phycomycetes, however, has never been reported. We report a case of 33-year-old male who had had prehallux with congenital equinovarus and underwent correction operation on the right ankle joint. There after, joint pain and edema had developed on that site. He received reoperation and underwent excisional biopsy from there. Operation showed hypertrophied yellowish synovium. On microscopic examination, the tissue showed several foci of fibrinoid necrosis and suppurative and granulomatuous inflammation. Periodic acid Schiff stain demonstrated broad. Aseptate hyphae that were wrinkled and folded.

3.A Case of Lichen Planus Treated with Griseofulvin.

Improvement of lichen planus was achieved by 11 weeks of daily oral treatment with griseofulvin. The patient was 59-year-old male, has had hypertension and diabetes mellitus for 9 months, and history of various drug intake to these diseses for 6 months. Four months before first visit, symptome of lichen planus had developed suddenly. Because of treatment failure of oral antihistamine and topical steroid for 6 weeks, we began to use griseofulvin. Praspective studies are needed to better assess the affectiveness of griseafulvin in the treatment of lichen planus.

4.The Distribution of MIC2 Antigen (CD99) Expression on Various Mucosa-Associated Lymphoid Tissue of Human Embryos and Fetuses.

In the present study, we examined to determine the development of various lymphoid tissue including mucosa-associated lymphoid tissue (MALT), thymus, lymph node and liver. In order to investigate the relationship between the morphological events and the expression pattern of MIC2 antigen (CD99) during the development of lymphoid system, we performed the immunohistochemical study using DN16, a monoclonal antibody against MIC2 (CD99), on formalin-fixed and paraffin-embedded lymphoid sections in 68 human embryos and fetuses, between 5 and 39 gestational week (GW). Four neonates, an infant, and 5 adults are also included. CD99 has been expressed along the membrane of hepatocytes and sinusoidal endothelial cells for 10-28 GW, in when the liver the major site of hematopoiesis. In the thymus, CD99 was firstly detected in the presumptive epitheial cells at 10 GW. When the thymus matured and corticomedullary differentiation appeared, CD99 was exclusively expressed in cortical thymocytes. The CD99 expression in epithelial cells of MALT has initiated at 6 GW and 10 GW earlier than that at the onset of MALT development and its expression has been persisted during MALT formation especially 16-25 GW. The finnding that CD99 antigen was expressed in epithelial cells during the development of MALT rnight provide a means to identify a novel epithelial differentiated substance. In addition, endothelial cells that are present in various organs such as liver and small intestine concurrently expressed CD99 antigen and its expression persisted to late fetal period. This point rnight suggest that CD99 antigen regulate the irnigration of lymphocytes from liver, major hematopoietic organ, to thymus or peripheral lyrnphoid organ via the interaction between endothelial cells and lymphocytes.

6.Furosemide-Induced Nephrocalcinosis in Very Low Birth Weight Infants.

Renal calcifications are a recognized complication of furosemide therapy in premature infants. Particularly in infants with chronic lung disease, the use of this potent diuretic is associated with hypercalciuria, which may predispose the infant to medullary nephrocalcinosis and renal calculi, We experienced two cases of nephrocalcinosis in very low birth weight infants. One had bronchopulmonary dysplasia, pneumonia, patent ductus arteriosus, ventricular septal defect and congestive heart failure and the other had systemic cytomegalovirus infection with cytomegalovirus pneumonitis and ricket of prematurity. Both patients received a large amount of furosemide. We presented these cases with brief review of literatures.

7.The Effect of Verapamil on the Specific Activity of Na+-K+-activated Adenosine Triphosphatase in Rabbit Renal Medulla .

The Na+ -K+ -activated ATPase is required to maintain osmotic balance and stabilize cell volume. The Na+ -K+ -ATPase has a more direct role in regulating cell volume; it controls the solute concentrations inside the cell, thereby regulating the osmotic forces that can make a cell swell or shrink. The impotance of the Na+ -K+ -ATPase in controlling cell volume is indicated by the observation that animal cells swell, and may burst, if they are treated with ouabain, which, inhibits the Na+ -K+ -ATPase. The present experiment was designed and carried out to determine the effect of verapamil, a calcium blocker, on the activity of Na+ -K+ -ATPase prepared from renal medulla in the normal rabbit. It was reported that verapamil, a well known coronary vasodilator, possessed negative inotropic effects. The mechanism of action of verapamil was initially thought to be due to coronary vasodilation and blockade of myocardial B-adrenergic receptors. 1t was termed such agent calcium antagonist. A derivative of verapamil, D-600, was subsequently shown to block the movement of calcium through the slow channel and thereby after the plateau phase of the cardiac action potential. Verapamil do not directly antagonize the effects of calcium. Rather, it inhibit the entry of ealcium into cells or its mobilization form intracellular stores and, as such, have been termed a calcium channel blocker.

8.Growth Status in Infants Born Intrauterine Growth Retardation.

PURPOSE:In infants born intrauterine growth retardation(IUGR), there may be persistent short stature in childhood and adulthood, although most IUGR infants show some degree of catch-up growth. The purpose of this study was to describe the postnatal growth status in order to determine the incidence of catch-up growth. METHODS:This study was carried out with the 260 IUGR infants(birth weight<2,500gm) born at Inje University Sanggye Paik Hospital, from October 1989 to March 1995. RESULTS: 1)Mean gestational age was 38.3+/-.6weeks, mean birth weight was 2.3+/-.2kg and mean birth length was 46.1+/-.7cm. 2)Mean height standard deviation score(height SDS) was 0.02+/-.03. Height SDS was -0.002+/-.71 at 1 year of age, 0.03+/-.14 at 2 years of age, -0.03+/-.71 at 3 years of age, 0.01+/-.24 at 4 years of age, 0.16+/-.24 at 5 years of age, -0.08+/-.43 at 6 years of age. 3)Of the entire study group of the 260 children, 16 children (6%) were below 10 percentile in height. 1 of 36 (2.8%) was below 10 percentile in height at 1 year of age, 5 of 86(5.8%) at 2 years of age, none at 3 years of age, 6 of 44 (13.6%) at 4 years of age, 1 of 25 (4.0%) at 5 years of age, 3 of 17 (17.6%) at 6 years of age. 4)Height SDS significantly correlated with gestational age (r=0.57, p<0.01), birth weight (r=0.17, p<0.01) and midparental height (r=0.72, p<0.01). 5)Birth length and midparental height showed significant differences between catch-up group and non-catch-up group (p<0.01). Condusions:In summary, of the infants born IUGR, 94% showed catch-up growth. Birth length and midparental height were significantly lower in non- catch-up group.
