Subcutaneous tissue calcification in rheumatic diseases usually occurs in connective tissue diseases, such as systemic lupus erythematosus, scleroderma, and dermatomyositis. Domestic cases of calcification in rheumatoid arthritis have not been reported. The mechanism of subcutaneous tissue calcification may differ depending on the cause and it can develop on all parts of the body. Calcification occurring in rheumatic diseases is a major mechanism of tissue damage caused by chronic inflammation. No standard therapy for calcification has been established; however, many studies have reported on medical and surgical treatment. We report on subcutaneous tissue calcification in a rheumatoid arthritis patient tissue calcification on both sides of the buttocks, the upper limbs, and the lower limbs.

2.Ankylosing spondylitis associated with balanced reciprocal X-1 translocation.

A number of research papers have reported more frequent occurrence of rheumatic/autoimmune disease among patients with hypogonadism or a chromosomal anomaly with potential X-chromosome defects. A 30-year-old female patient came to the hospital with a main cause of bilateral buttock pain, which began two years ago and worsened seven days ago. Ankylosing spondylitis with invasion of both sacral-iliac joints was observed. On magnetic resonance imaging, although the uterus was observed normally, an ovary was not observed. In a chromosome test, balanced reciprocal X-1 translocation of 46,X,t(X;1)(p10;q10) was diagnosed. Here, we report on the first case involving ankylosing spondylitis accompanied by balanced reciprocal X-1 translocation.

3.Delayed recovery of neuromuscular blockade by rocuronium in a hypothermic patient during a laparoscopic procedure: A case report.

Perioperative hypothermia can occur frequently and lead to various complications. Laparoscopic procedures do not require a large incision or exposure of the viscera, but a prolonged and high pressure procedure can lead to unexpected hypothermia. Hypothermia prolongs the time-course of non-depolarizing neuromuscular blocking agents by reducing the plasma clearance significantly. Rocuronium, a new aminosteroidal neuromuscular blocking agent, can be influenced by reductions in plasma clearance and prolong the duration of action. Therefore, patients must receive ventilator care in the ICU. Aggressive measures were taken for hypothermia, and the patient was discharged without any complications. We report a case of delayed of neuromuscular blockade by rocuronium in a hypothermic patient during a laparoscopic procedure with a brief review of literatures.

5.Sclerotial Development of Grifola umbellata.

Sclerotial development of Grifola umbellata (Pers. : Fr.) Donk was investigated through microscopic examinations. The sclerotium of G. umbellata was bumpy and rugged, multi-branched, and dark-brown to black in color. The sclerotial development of G. umbellata was categorized into three stages such as sclerotial initial, development and maturation. Sclerotium development was initiated as the white fungal mass. The superficial part of white sclerotium changed into gray, light brown and then black as its development proceeded further. As a distinctive characteristic of this fungus, a large number of crystals were observed in the medulla layer of sclerotium during its maturation. For development of new sclerotium, G. umbellata formed a white sclerotial primordium on the matured sclerotium. Development of sclerotium in G. umbellata was intimately associated with rhizomorphs of Armillariella mellea and the developing sclerotia were often penetrated by rhizomorphs of A. mellea into medulla layer.

Frozen shoulder is characterized by pain in the shoulder and limitation of glenohumeral movement. The underlying pathologic changes in frozen shoulder are synovial inflammation with subsequent reactive capsular fibrosis. Capsular fibrosis is the ultimate course of frozen shoulder suffering from various causes irrespectively. Therefore, adhesions that are affecting the joint movement must be released. Most important diagnostic method is thoughtful history taking and physical examination, and ultrasonography is very valuable device of diagnosis and treatment. Interventional microadhesiolysis and nerve stimulation (IMNS) is effective to manage frozen shoulder patients. IMNS for frozen shoulder is composed of three release approaches. These are (1) subacromial, (2) posterior inferior capsular, and (3) subdeltoidal & subcoracoidal release and round needle can be used for each release. We report this experiences and recommand this technique for the treatment of frozen shoulder patients.

9.Subconjuctival Loa loa with Calabar Swelling.

Loa loa is unique among the human filariae in that adult worms are occasionally visible during subconjuntival migration. A 29-yr-old African female student, living in Korea for the past 5 yr without ever visiting her home country, presented with acute eyelid swelling and a sensation of motion on the left eyeball. Her symptoms started one day earlier and became worse over time. Examination revealed a threadlike worm beneath the left upper bulbar conjunctiva with mild eyelid swelling as well as painless swelling of the right forearm. Upon exposure to slit-lamp illumination, a sudden movement of the worm toward the fornix was noted. After surgical extraction, parasitologic analysis confirmed the worm to be a female adult Loa loa with the vulva at the extreme anterior end. On blood smear, the microfilariae had characteristic features of Loa loa, including sheath and body nuclei up to the tip of the tail. The patient also showed eosinophilia (37%) measuring 4,100/microliter. She took ivermectin (200 microgram/kg) as a single dose and suffered from a mild fever and chills for one day. This patient, to the best of our knowledge, is the first case of subconjunctival loiasis with Calabar swelling in Korea.

10.Relationship between manual cephalo-anthropometric analysis and degree of snoring in Korean adult.

BACKGROUND AND PURPOSE: This purpose of this research is to study the prevalence of habitual snoring and the association between snoring and cephalo-anthropometric analysis in Korean population. MATERIAL AND METHOD: Four thousand and two hundred twenty four subjects (male: 2277, female: 1947), aged between 18~85 (mean age male: 45.3, female: 44.0), were recruited through home visits and interviewed by well-trained investigators. RESULTS: Three hundred fifty two male subjects (15.5%) and one hundred sixty two female subjects (8.32%) exhibited habitual snoring. Age, weight, and body mass index (BMI) were significantly higher in the snoring group compared to those who dont snore habitually. Using a stepwise variable selection method of multiple logistic regression analysis, we found independent factors as being significant for males and females of the snoring groups. For males, the factors were BMI (p<0.001, OR=1.18 CI: 1.12~1.25), mandibular angle (p<0.001, OR=0.97 CI: 0.96~0.98), and abdominal girth (p<0.001, OR=1.04, CI: 1.02~1.06. For females, they were abdominal girth (p=0.031, OR=1.03, CI: 1.00~1.05), neck girth (p=0.003, OR=1.12, CI: 1.04~1.21), age (p=0.0002, OR=1.02 CI: 1.01~10.3), and BMI (p=0.0007, OR=1.11 CI: 1.05~1.18). CONCLUSION: We found that BMI, gonion angle and abdomen girth in male and abdominal girth, neck girth, age, BMI in female were the significant factors affecting the severity of snoring in Korean population.
