2.A Comparative Study of the Toxicity Between carboplatin and Cisplatin in VBP Combination Chemotherapy.

Cisplatin has been widely used as chemotherapeutic agent for the treatment of cervical cancer, ovarian cancer, head and neck cancer and squamous cell carcinoma of lung. But cisplatin is highly toxic with nephrotoxicity, gastrointestinal toxicity, myelosuppression and neurotoxicity. The second generation drug of platinum compound, carboplatin was developed in 1980s to reduce side effects. Carboplatin has low nephrotoxicity but its major toxic effect is thrombocytopenia, In this study, the side effects of cisplatin and carboplatin were evaluated on 37 patients of cervical cancer in 169 chemotherpy cycles who were recieved combined VBP chemotherpeutic regimen consisting of cisplatin or carboplatin. Nephrotoxicity of grade 2 or over were 16% in cisplatin group and 1% in carboplatin group. Granulocytopenia of grade 2 or over were 34% in cisplatin group and 10% in carboplatin group. Thrombocytopenia of grade 2 or over were 7% in cisplatin group and 21% in carboplatin group. Gastrointestinal toxicity of grade 2 or over were 11% in cisplatin group and 0% in carboplatin group. This clinical study demonstrated that cisplatin has more toxic effects than carboplatin except thrombocytopenia.

4.MR Findings of Lipold Pneumonia: Report of Two Cases.

Exogenous lipold pneumonia is an unusual disease resulting from aspiration of oil particles. A history of oil ingestion strongly suggests the diagnosis, but the radiological presentation varies from mild perihilar consolidation to diffuse and extensive bilateral involvement, particularly of the posterior basal segment of both lower lung(l). Since magnetic resonance(MR) provides greater contrast resolution than CT, it offers the potential for tissue characterization. In particular, fatty tissue and lipid containing substances are known to have high signal intensities on T1- and T2-weighted spin-echo (SE) images. We report MR findings in two case of exogenous lipold pneumonia caused by ingestion of shark liver oil(Squalene).

6.Effects of Nurses’ Communication Styles and Surgical Patients’ Anxiety and Uncertainty on Patient Satisfaction

Purpose:
: The study was conducted to examine effects of nurses’ communication styles perceived by surgical patients and their anxiety and uncertainty on patient satisfaction.
Methods:
: Data were collected by using a structuralized questionannaire which consists of nurses’ communication style tool, State trail Anxiety Inventory(STAI), Mishel Uncertainty in Illiness scale, and patient satisfaction tool targeting on 276 surgical patients who were hospitalized in general wards from July 24, 2020 to December 18, 2020. The data analysis was done with the correlation analysis and the multiple regression analysis.
Results:
: Nurses’ communication style perceived by surgical patients has found to have the most informative communication styles and the least authoritative communication style. And the variable which has the most effect on patient satisfaction was the affective communication style(β=.35, p<.001), followed by nurse communication importance perceived by surgical patients(β=.15, p=.001). And surgical patients' anxiety (β=-.15, p=.011) and uncertainty(β=-.13, p=.024) has found to have a negative effect on patient satisfaction.
Conclusion
: The patient satisfaction got higher when the surgical patients perceived the Affective & informative communication styles among nurses’ communication styles, and when they perceived the nurses’ communication as important.
7.Agreement of Physical Activity Measured Using Self-Reporting Questionnaires with Those Using Actigraph Devices, Focusing on the Correlation with Psychological State

Purpose:
This study aimed to evaluate the correlation and agreement of physical activity (PA) between data obtained from wearable Actigraph devices and self-reporting questionnaires, and to investigate the relationship between psychological state (depression, anxiety, and fatigue) and PA.
Methods:
A descriptive study was conducted using physical measurements and surveys. PA was measured through both the International Physical Activity Questionnaire (IPAQ) and the Actigraph GT3X+ device. The demographic characteristics of the subjects, as well as their depression, anxiety, and fatigue scores, were collected with structured questionnaires. The Spearman’s rank correlation coefficient and the Bland-Altman plot method were employed.
Results:
Data from 36 healthy adults were analyzed. The overall levels of PA measured using the IPAQ and the Actigraph were 1,891.69 MET min/week and 669.96 MET/ day, respectively. Total levels of PA did not show a significant correlation between the two measurement methodologies. However, the moderate-intensity PA resulting from the IPAQ scores showed a significant positive correlation with the light-intensity PA recorded by the Actigraph. The Bland-Altman plot analysis demonstrated that the levels of PA as measured by the two different methods did not match. In addition, PA measured using the Actigraph showed a significant negative correlation with depression and anxiety whereas PA measured using the IPAQ showed a significant positive correlation with fatigue.
Conclusion
The conclusion of this study is that the data obtained from the subjective self-reporting questionnaire and the wearable Actigraph do not correlate or match in healthy adults. Future research should investigate the relationship between depression and PA intensity through the Actigraph, or other wearable devices equipped with smartphone apps.
8.Factors Affecting Development of Epilepsy and Postoperative Recurrence of Epilepsy in Primary Brain Tumor.

BACKGROUND: A small but significant proportion of patients with brain tumors continued to have seizures postoperatively. All of them could not be explained simply by the failure to adequately resect the tumor mass. We investigated factors influencing seizure recurrence in primary brain tumors. METHODS: We analyzed 435 patients treated with tumor surgery and examined the differences between epileptic seizure group (ESG) and non-epileptic seizure group (NESG). Among ESG, we selected 99 patients confirmed by pathology. We divided patients into chronic epileptic seizure group (CESG; duration of seizure attack >or=1 year) and acute epileptic seizure group (AESG; < 1 year). We also investigated the differences between two groups. RESULTS: Of 435 patients, 104 were ESG and 331 NESG. Among various factors, male, favorable neurological state, fronto-temporal lobe origin, astrocytoma, oligodendroglioma were statistically significant in ESG compared with NESG (p<0.05). Of 99 patients, 43 were CESG and 56 AESG. Among various factors, seizure recurrence rate without residual tumor or tumor recurrences was significantly higher in CESG than in AESG (p<0.05). On the other hand, the laboratory abnormalities, and the rate of residual tumor or tumor recurrences on follow-up MRI were significantly higher in AESG than in CESG (p<0.05). CONCLUSIONS: Among many factors, sex, neurological state, location and pathology of tumors were significantly related to seizure attacks in brain tumor. There were the differences of epileptogenesis between AESG and CESG. We suggest that patients with brain tumor and chronic epilepsy have to be investigated with extensive work-up including invasive electrophyosiologic studies.

9.Association between Migraine with Aura and both Homocysteine and MTHFR C677T Polymorphism.

BACKGROUND: The pathophysiology of migraine is not yet fully understood. Homocysteine acts as an excitatory amino acid in vivo and may influence the threshold of migraine. The previous studies have revealed strong association between the homozygous C677T mutation in the methylenetetrahydrofolate reductase (MTHFR) gene and hyperhomocysteinemia. In this study, we examined the association between both homocysteine and MTHFR C677T polymorphism and headache, such as migraine and tension headache. METHODS: We enrolled 148 headache patients (75 tension headache, 34 migraine with aura [MWA], and 39 migraine without aura [MOA]) and 121 normal controls; checked their fasting homocysteine levels and analyzed for the MTHFR C677T polymorphism. For the multivariate analysis, we used logistic regression to adjust for age and sex. RESULTS: Plasma homocysteine levels were significantly higher in patients with MWA than those in controls (adjusted odds ratio (AOR), 1.12; 95% confidence intervals (CI), 1.02~1.23). The AOR and 95% CI of the MTHFR 677TT genotype was significantly higher in patients with total migraine (AOR, 3.32; 95% CI, 1.30~9.55; p< 0.05) and with MWA (AOR, 4.70; 95% CI, 1.44~15.29; p<0.01) than the controls. However, the AOR was not significant in patients with tension headache (AOR, 0.93; 95% CI, 0.37~2.32) and with MOA (AOR, 2.77; 95% CI, 0.78~9.74). CONCLUSIONS: Our findings suggest that both elevated homocysteine level and the homozygous C677T mutation in the MTHFR gene are associated with migraine with aura.

10.Risk Factors of Musculoskeletal Injuries among the Marine Corps Enlisted Trainees.

OBJECTIVE: The Korean Marine Corps enlistees endure tremendous physical and mental stress during basic military training, resulting in an increased risk of musculoskeletal injuries. The aim of this study was to investigate the incidence and type of the musculoskeletal injury problems and the risk factors associated with injuries among the Marine Corps recruits and to suggest preventive measures. METHODS: The study subjects were the Marine Corps recruits (n=8,231) who were enlisted from July, 2008 to March, 2009 for basic military training. We used a prospective cohort design and collected basic demographic information and subject musculoskeletal injury risk factors through a questionnaire on the first day basic training. The subjects were followed for the subsequent 6-week military training period. RESULTS: The cumulative incidence of musculoskeletal injuries was 13.4% during the 6-week training period, and more than half of the injuries involved the lower limbs at or below the level of the knee. Based on multivariate logistic regression analysis, the following factors were related to an elevated risk for injuries: age, height, educational level, subjective health perception, injury history during the past year, and stress scale after entrance. CONCLUSION: Based on our study results, the application of injury prevention measures such as stress management program and gradual fitness increasing program to the Marine Corps recruit training would contribute to the reduction of musculoskeletal injuries.
