1.A Case of Leukemia Cutis in Myelodysplastic Syndrome Evolving into An Atypical Chronic Myeloid Leukemia.

We report a patient who had been initially diagnosed as a myelodysplastic syndrome in 1998 presenting purpuric patches on the left arm that started to develop about a year prior. The purpuric lesions were diagnosed as leukemia cutis by skin biopsy. Her subsequent bone marrow biopsy showed progression into an atypical chronic myeloid leukemia with increased numbers of leukocytes in the peripheral blood. Leukemia cutis typically is regarded as a sign of progression of disease or a manifestation of recurrent disease in treated patients with an established diagnosis of leukemia. We suggest that the skin lesion in this patient could have been a sign of con-version into atypical chronic myeloid leukemia.

2.Anterior Labral Tear: Diagnostic Value of MR Arthrography of the Shoulder.

PURPOSE: To assess the accuracy of magnetic resonance(MR) arthrography in the diagnosis of anterior labral tear of the shoulder. MATERIALS AND METHODS: Between Semptember 1996 and February 2000, MR arthrography of the shoulder was performed in 281 patients with a history of shoulder pain or instability. Among this total, only 157 shoulders in 154 patients who underwent arthroscopy or open surgery 0 to 230 (average, 20.9) days after MR arthrography were included in this study; the subjects comprised of 150 males and 4 females with an average age of 23.3 years. MR arthrographs of these 154 patients were analyzed for the presence of anterior labral tears, and the findings were correlated with the arthroscopic and surgical findings. Anterior labral tear was classified as A to D according to its location, as determined by arthroscopy and surgery. (A=4 to 6 o 'clock direction, anteroinferior; B=2 to 4 o 'clock direction, central; C=12 to 2 o 'clock direction, anterosuperior; D= SLAP lesions). The retrospective analysis of MR arthrographs showing false-positive and negative findings was also undertaken. RESULTS: In the diagnosis of anterior labral tear, MR arthrography showed a sensitivity of 94%, a specificity of 90% and an accuracy of 91%. Anterior labral tears were confirmed by arthroscopy or surgery in 62 of the 157 shoulders (39%). Among 62 lesions, two (3%) were observed in area A, 32(52%) in area A+B, nine (15%) in area A+B+C, one(2%) in area A+B+D, 13(21%) in area A+B+C+D, two (3%) in area B+C, one(2%) in area B+D, and two(3%) in area C. Among ten false-positive cases, seven were focal lesions (two, three and two lesions in area A, B and C, respectively), and in the remaining three cases, located in area A+B, MR arthrography revealed thickening and deformation. All four false negatives were focal lesions (two in area A and two in area C). CONCLUSION: Other than in focal lesions, in which accuracy was relatively low, MR arthrography showed high sensitivity, specificity and accuracy in the diagnosis of anterior labral tear of the shoulder.

3.Comparative study of radiologic-pathologic findings of experimental clonorchiasis in rabbits.

Radiological investigation in patients with clonorchiasis is very important as this is the only method of evaluating the severity of clonorchiasis. In order to correlate the radiologic and pathologic findings of clonorchiasis, fourteen rabbits infested with Clonorchis sinensis and five control rabbits were examined radiologically by ultrasonography, computed tomography and cholangiography and the results were correlated with pathologic findings. Dilatation of the intrahepatic small bile ducts of the liver was due to obstruction by flukes: oval or elliptical small filling defects or irregular margin of the bile ducts on cholangiogram or intraluminal echoes on sonogram represented flukes per se; periductal thickening on sonogram and periductal enhancement of bile ducts on CT were due to inflammatory cell infiltration, adenomatous hyperplasia and periductal fibrosis: band like enhancement at the periphery of the liver on CT represented proliferated bile ducts, destruction of liver cells and resultant fibrosis. The study confirmed the pathological bases for the radiological findings of clonorchiasis in liver and bile ducts and will, perhaps, serve as a basis for the future radiologic-pathological correlation of clonorchiasis and in further clinical and experimental researches in the biliary tract diseases.

4.Text-Mining Analyses of News Articles on Schizophrenia

Objectives:
In this study, we conducted an exploratory analysis of the current media trends on schizophrenia using text-mining methods.
Methods:
First, web-crawling techniques extracted text data from 575 news articles in10 major newspapers between 2018 and 2019, which were selected by searching “schizophrenia” in the Naver News. We had developed document-term matrix (DTM) and/or term-document matrix (TDM) through pre-processing techniques. Through the use of DTM and TDM, frequency analysis, cooccurrence network analysis, and topic model analysis were conducted.
Results:
Frequency analysis showed that keywords such as “police,” “mental illness,” “admission,” “patient,” “crime,” “apartment,” “lethal weapon,” “treatment,” “Jinju,” and “residents” were frequently mentioned in news articles on schizophrenia. Within the article text, many of these keywords were highly correlated with the term “schizophrenia” and were also interconnected with each other in the co-occurrence network. The latent Dirichlet allocation model presented 10 topics comprising a combination of keywords: “police-Jinju,” “hospital-admission,” “research-finding,” “care-center,” “schizophrenia-symptom,” “society-issue,” “family-mind,” “woman-school,” and “disabled-facilities.”
Conclusion
The results of the present study highlight that in recent years, the media has been reporting violence in patients with schizophrenia, thereby raising an important issue of hospitalization and community management of patients with schizophrenia.

6.Congenital Anomalies of the Spine: Radiologic Findings.

Congenital anomalies of the spine are frequent and variable. Some are restricted to skeletal structures, while others involve combined neural tube defects or are associated with other multi-systemic disorders. Structural spinal anomalies can be classified according to their location: 1) the vertebral body, 2) the articular process, 3) the lamina with spinous process, 4) the pars interarticularis, 5) the facet joint, 6) the pedicle, or 7) other. Because of similarities between these congenital anomalies and (a) secondary changes involving infection or joint disease and (b) deformities resulting from trauma and uncertain tumorous conditions, significant confusion can occur during diagnosis. Moreover, since the anomalies often give rise to both functional impairment and cosmetic problems, appropriate treatment relies crucially on accurate diagnosis. The authors illustrate the pathogenesis and radiologic findings of the relatively common spinal anomalies confined to skeletal structures.

7.Study of Facts and Problems of Reports for Court about Traumatic Mental and Behavioral Disabilities.

Traumatic injury can result not only in physical disability but also in mental and behavioral sequelae. It is very important to properly judge the severity of disability as well as the need for assistance and future medical treatment. It is very difficult to establish objective guidelines for assessing mental and behavioral disabilities due to the inherent peculiarity of the disabilities. This study uses reports generated for court because they need to be more judicious than reports for other types of evaluation. We analyzed reports concerning the facts and problems associated with evaluation of mental and behavioral disabilities. We hope this study will help further our understanding of mental and behavioral disabilities.
8.The Relationships of Internet Addiction, Depression, and Suicidal ideation in Adolescents.

PURPOSE: This study was done to identify the state of internet addiction and to investigate the relationships among internet addiction, depression and suicidal ideation in adolescents. METHOD: Participants were 1,670 high school students in an urban city. The Internet Addiction Scale was used to measure internet addiction, the DISC-MDD-SQ was used to measure depression, and the Suicidal Ideation Questionnaire-JR was used to measure suicidal ideation. RESULT: 38.1% of participants were perceived to be in the early stages of internet addiction and 1.5% reported heavy addiction. Overall the internet addiction scale score was 37.57(+/-12.52). The differences of internet addiction in sample characteristics were found to be significant in rank in class. The differences in DISC-MDD-SQ and IAS scores among the states of internet addiction were statistically significant. The level of internet addiction correlated positively to the level of depression and suicidal ideation. CONCLUSION: Based upon these findings, there were significant positive correlations among Internet addiction, depression, and suicidal ideation in adolescents. As schools are often the first line for the identification of potential life-threatening behaviors, nurses, especially school-based health professionals, need to be cognizant of measures and procedures for the assessment of Internet addiction, depression, and suicidal ideation.

9.A Comparative Study About The Natural Course Of Idiopathic Frozen Shoulder And The Effectiveness Of Conservative Treatment.

PURPOSE: The aim of this study was to investigate about the natural course of idiopathic frozen shoulder and the efficacy of conservative treatment. MATERIALS AND METHODS: Twenty-five cases of unilateral frozen shoulder (study group) treated conservatively by intra-articular steroid injection and staged passive stretching were prospectively followed for average 9.1 months, the outcome was assessed in terms of motion range and the function score derived from patients' self-evaluation, and timing of improvement was searched. Compared with study group, 20 patients (control group) , who were diagnosed as idiopathic frozen shoulder but didn't undertake specific treatment, were checked at average 40 months. RESULTS: Eighteen cases (72%) of the study group had residual symptom, which was mild in most cases. Range of motion less than 80% of normal side was noted in 2 cases. Intra-articular steroid injection was effective for pain relief in 75%. In the control group, one case showed decreased motion and 2 case had mild pain. Comparing with the control group, the study group was improved significantly & statistically faster. CONCLUSION: It is construed that the long term result of idiopathic frozen shoulder is excellent and the combined effect of steroid injection and stretching shortens the duration of disease. Operative treatment for idiopathic frozen shoulder, if indicated, needs to be decided after long term follow-up.

10.Secondary Fish-Odor Syndrome Can be Acquired by Nitric Oxide-mediated Impairment of Flavin-containing Monooxygenase in Hepatitis B Virus-Infected Patients.

Primary fish-odor syndrome (FOS) is a genetic disorder caused by defective flavin-containing monooxygenase 3 gene (FMO3) with deficient N-oxidation of trimethylamine (TMA), causing trimethylaminuria (TMAU). By contrast, secondary FOS can be acquired by decreased FMO activities in patients with chronic liver diseases, but the underlying mechanisms are unknown. In the present study, we examined plasma NOx concentrations and viral DNA contents as well as in vivo FMO activities and their correlations in chronic viral hepatitis (CVH) patients. Plasma concentration of NOx was significantly increased by 2.1 fold (56.2+/-26.5 vs. 26.6+/-5.4micrometer, p< 0.01), and it was positively correlated with plasma hepatitis B virus (HBV) DNA contents (r2=0.2838, p=0.0107). Furthermore, the elevated plasma NOx values were inversely and significantly correlated with in vivo FMO activities detected by ranitidine-challenged test (8.3% vs. 20.0%, r2=0.2109, p=0.0315). TMA N-oxidation activities determined in CVH patients without challenge test were also significantly low (73.6% vs. 95.7%, p< 0.05). In conclusion, these results suggested that secondary FOS could be acquired by the endogenously elevated NO in patients with CVH.
