1.Clinical Observation on Weight Bearing of the Tibial Fractures with Functional Below the Knee Cast

The early weight bearing in treatment for tibial fractures was reporded first by Gurd(1940) and in recent by Weissman(1966), Sarmiento(1967), and Brown(1969). 1. One hundred and fifty cases of fractures of the tibia treated at the Orthopedic Department f the 17 th Army Hospital during period of January 1968 to June 1970 were subjected to analysis. Of three groups, first group was applied functional below the knee cast after mannual reduction and started weight bearing on the 5 th weeks from injury, second group was treated with same method as first group except the reduction was achievd surgically, and third group was applied toe-to-groin cast after manipulation with no weight bearing until union. 2. Casting method; The toe-to-groin cast was applied on the day of injury after manipulation and functional below the knee cast was applied tightly after swelling was subsided. Supporting the leg on the table, we could obtain satisfactory alignment in all cases. Especial firm molding over the medial flare of the tibia, Patellar tendon and popliteal space was the most important step in applying this type of the cast. Finally rubber walking heel was held in place throught the tibial axis. 3. Traffic accident was the most common cause of the fractures in this series and it was 78 cases out of 150. 4. The average healing time of the first group was 15.7 weeks, of second group was 19.4 weeks, and of third group was 23.5 weeks. 5. The open and/or comminuted fractures would delay healing time, and prolonged convalescent period. 6. Though slight shortening and angulation was observed in the early weight bearing groups, the final function was excellent.

3.Structural Modeling of Quality, Satisfaction, Value and Purchase Intention in Health Care Service.

OBJECTIVES: To determine the relationships among quality, satisfaction, value and purchase intention in health care service. METHODS: The data were gathered from out-patients who had used hospital services. They were asked to assess service quality, satisfaction, service value, and purchase intention. A total of 557 usable questionnaires were gathered. The data were analyzed using SAS version 6.12. The analytic methods employed in the study were confirmatory analysis and covariance structural analysis. RESULTS: Service quality exhibited a significant and positive relationship with satisfaction, service value, and purchase intention. Furthermore, satisfaction had a significant and positive relationship with purchase intention. And finally, service value had a significant and positive relationship with both satisfaction and purchase intention. Based on these findings, it is evident that satisfaction was a mediator between service quality and purchase intention. Also service value played a mediating role between service quality and satisfaction. CONCLUSIONS: These results suggest that service quality is an antecedent of satisfaction and service value, and exerts a stronger influence on purchase intentions than satisfaction and service value do. Thus, managers may need to emphasize service quality in health care.

4.The Clinical Effect of Recombinant Human Granulocyte-Colony Stimulating Factor to the Leukopenia During Chemotherapy in the Patients with Gynecologic Malignancies.

An increase in the dose of chemotherapy enhances the response of many experimental and clinical cancers, but the extent of chemotherapy dose escalation and repeated use is often limited by myelosuppression. The side effects of chemotherapy including bleeding and infection due to myelosuppression have resulted in delayed therapy and a reduction in the therapeutic dose, therefore it is necessary to overcome myelosuppression especially leukopenia in patients with gynecologic malignancies who recieved chemotherapy. This study is undertaken to investigate the clinical effects of rhG-CSF(recombinant human Granulocyte-colony stimulating factor) in 29 patients with gynecologic malignancy who recieved chemotherapy. It was given at a dose of 100 microgram bid/day subcutaneously until significantly increase of leukocyte count in leukopenic patient. The results showed, the rhG-CSF has significantly increased the number and function of leukocyte. The use of rhG-CSF was effective and useful to treat chemotherapy induced leukopenia and to accelerate the recovery from this complications.

7.Fluorescence-guided colorectal surgery: applications, clinical results, and protocols

In recent years, the rise of minimally invasive surgery has driven the development of surgical devices. Indocyanine green (ICG) fluorescence imaging is receiving increased attention in colorectal surgery for improved intraoperative visualization and decision-making. ICG, approved by the U.S. Food and Drug Administration in 1959, rapidly binds to plasma proteins and is primarily intravascular. ICG absorption of near-infrared light (750–800 nm) and emission as fluorescence (830 nm) when bound to tissue proteins enhances deep tissue visualization. Applications include assessing anastomotic perfusion, identifying sentinel lymph nodes, and detecting colorectal cancer metastasis. However, standardized protocols and research on clinical outcomes remain limited. This study explores ICG’s role, advantages, disadvantages, and potential clinical impact in colorectal surgery.
8.Endoscopic Local Injection of Hypertionic Saline Epinephrine Solution for Arrest Hemorrhage from Upper Gastrointestinal Tract: Hemostatic and diagnostic rate according to the time interval.

A prospective randomised trial was performed to assess the diagnostic accuracy according to the time interval and the efficacy of endoscopic injection of hypertonic saline-epinephrine(HS- E)solution, consisting of 3.5% sodium chloride with 0.0045% epinephrine, for actively bleeding peptic ulcers, exposed vessel or blood clot on ulcer bed, or Mallory-Weiss tear. Over 24 month, emergency endoscopy in 180 patients admitted for upper gastrointestinal hemorrhage identified 51 patients with nonvariceal hemorrhage. The causes of bleeding were; gastric ulcer in 32; duodenal ulcer in 13; gastric cancer in 4; Mallory-Weiss tear in 2. With this method, the hemostatic effect was permanent in 40 cases(84.3%), temporary in 9 cases(11.8%), and failed in 2 cases(3.9%). By applying this method, the rate of emergency operation for patients with bleeding from the upper gastrointestinal tract was significantly reduced from 20.0% (8/40)to 3.9%(2/51)(p<0.05). Emergency endoscopy in acute UGI bleeding increases the accuracy of detection of actual bleeding sites(p<0.05), but if the endoscopic procedure was performed within 48 hours, the hemostatic rate was not affected(p<0.05). We concluded that hypertonic saline-epinephrine injection method could provide a simple maneuver with reasonable cost, high safety, and satisfactory hemostatic efficacy in the treatment of nonvariceal upper gastrointestinal bleeding.

10.Clinical Study of Gastrointestinal perforation in the Newborn.

A clinical study was made on 10 neonates with gastrointestinal perforation admitted to the department of pediatrics and surgery of Severance hospital from January, 1965, through Jane, 1978. The following results were obtained; 1) Males predominate (7M : 3F). 2) 7 cases of perforations occur in the first 2 days of life. 3) The most common site of perforations is stomach (3 cases) and terminal lieum (3 cases). The lesion showes inflammation, submucosal hemorrhage, atresia and muscular defect. 4) The clinical picture is abdominal distension developing repidly, cyanosis, vomiting, poor sucking, etc. 5) Abdominal X-rays are diagnostic, Massive pneumoperitoneum is visible in 5 cases. 6) Seven patients in 9 operated neonates died.
