3.Histopathologic Observation on Hydrofluoric Acid Burn Using Guinea Pigs.

BACKGROUND: Hydrofluoric acid(HF) is one of the strong stirritating, corrosive and poisonous inorganic acid, The HF burns are occuring with ever-increa firifrequency due to the wide use of this acid in industrial fields, such as electronics and semicor tor industries. OBJECTIVE: The purpie of this study is to quantiate theound depths produced by varying concentrations of HF aiud to suggest the treatment guideline. METHODS: Uarious cinicentrations of HF(5%, 25%, 50%),were applied to two pairs of test sites on a guinea pig's back in a uniform fashion with cotton-tip applicators and then the histopathologic changes of guinea pig skin were observed. RESULTS: 1, In the 5% HF applied group, intracellular edema vaobserved in the epidermis after 12 hours. After 3days, epidermal cell necrosis was observed. 2. In the 25% HF applied group, intraepidermal vesice was observed after 6hours. After 12hours, inflammatory cell infiltrations were observed in the apper dermis and then after 3days, dermal necrosis was oberved. 3. In the 50% HF applied group, epidermal cell necros After 6hours, liquefaction degeneration in the epidermis an was observed focally after 3hours. iflhmmatory cell infiltrations in the upper dermis were observed. After 2days, dermalar.ecrosis was observed and then after days, the reaction was at its peak and subcutaneous tissuc. necrosis was observed. CONCLUSION: This study showed that a higher concentrator HF exposure enhances The penetration of HF. For treatmi not alcohloic solution of a quarternary ammonium for avintralesional injection of calcium gluconate within 24 hours of HF and a longer period after we suggest wet dressing with an concentration of HF burn and high concentration of HF burn.

4.Treatment of Moderate Hypercholesterolemia with Lovastatin(Mevacor(R)).

To evaluate the lipid-lowering effect of lovastatin(Mevacor(R)), lovastatin was administered to 38 patients with non-familial hypertcholesterolemia(>220mg/dl). The analysis of the effect was made with 25 patients(58.2+/-7.5 years ; 13 male, 12 female)who had received lovastatin more than 12 weeks. The drug was administered as a single dose with evening meal 20mg at the begining and adding another 20mg if the total cholesterol level was persistently higher than 200mg/dl at the end of each 4 week-period. 1) Total cholesterol level was decreased from 256.6+/-36.9mg/dl to 20932+/-50.1mg/dl at the end of the 4th week, 201.9+/-44.2mg/dl the 8th week and 203.6+/-39.6mg/dl the 12th week (p<0.001), respectively). 2) Triglyceride level was decreased from 196.4+/-104.1mg/dl to 163.4+/-74.4mg/dl at the end of the 4th week(p<0.05) but no significant change at the end of the 8th week showing 169.8+/-73.2mg/dl and 162.7+/-54.8mg/dl the 12th week(p<0.05). 3) High density lipoprotein cholesterol(HDL-C) level was not significantly changed with the drug during the 12 week treatment period. 4) Low density lipoprotein cholesterol(LDL-C) level was decreased remarkably similar to that of total cholesterol. 5) Total cholesterol/HDL-C ratio was decreased from 5.05+/-0.92 to 4.06+/-1.40 at the end of the 4th week(p<0.05), 3.89+/-0.99 the 8th week(p<0.001). 4.20+/-1.10 the 12th week(p<0.01). 6) LDL-C/HDL-C ratio was decreased from 3.24+/-0.94 to 2.43+/-1.21 at the end of the 4th week(p<0.05), 2.23+/-0.86 the 8th week(p<0.001) and 2.54+/-0.98 the 12th week(p<0.05). 7) There was no significant side effect on lovastatin therapy of 12 weeks duration. 8) The laboratory findings including liver function test, uric acid, creatinine, creatine phosphokinase and blood glucose were not changed significantly. From above results we concluded that lovastatin is safe and effective hypocholesterolemic agent in its clinical use.

5.Cystic Adventitial Disease of the Popliteal Artery: A case report.

Localized cystic degeneration of peripheral arteries represents and unusual cause of arterial insufficiency. It frequently occurs in patient without generalized arteriosclerosis. It has been reported in patients from age 11 to 62 years. Cystic adventitial disease is most common in the popliteal artery. At least 115 cases have been reported worldwide, but none in Korea. We report a case of cystic adventitial disease involving the left popliteal artery. This 64-year-old man presented with an 18-month history of cramping pain of sudden onset in the left calf and claudication. Angiographic findings showed a 6 cm length of luminal obliteration of the popliteal artery. Segmentally resected popliteal artery showed two longitudinally directed cystic masses measuring 3.5x1.5 cm and 2.5x1.5 cm in the adventitia. Microscopic examination revealed cystic space in the arterial adventitia compressing arterial lumen. There were a number of foamy histiocytes collected along the cystic lumen.

6.Changes of hematological references depends on storage period and temperature conditions in rats and dogs.

Because changes in rat and dog hematological parameters according to storage conditions have been poorly documented, we sought to examine such changes. Blood analysis was performed using two hematology analyzers (ADVIA 2120i and Sysmex XN-V) after storage at room temperature and in cold storage for 5, 24, and 48 h, respectively. Interassay coefficients of variation for hematological parameters analyzed with the ADVIA 2120i and the XN-V showed similar. The levels of hematocrit, mean corpuscular volume, mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration, and platelet (PLT) showed significant variations with time in blood samples of rats and dogs. The leukocyte subpopulation showed high variation with storage conditions. The data for leukocyte differential counts obtained using the ADVIA 2120i, XN-V, and a manual differential counting procedure showed good agreement for neutrophils and lymphocyte counts, but monocytes, eosinophils, and basophils showed differences between the procedures. In conclusions, most rat and dog hematological parameters showed minimal changes; however, some showed high variation with storage time and temperature, especially PLT and leukocyte subpopulations. In conclusion, when performing hematological analysis in dogs and rats, it will be exactitude to analyze blood samples in fresh condition and at least within 24 h in the cold storage.

7.Three Cases of Hereditary Angioedema in One Family.

Hereditary angioedema is a rare genetic disease transmitted with a autosomal dominant trait result of a quantitative or functional defect of Cl inhibitor. We report three cases of heriditary angioedema from the same famly respectively the 35-year-old sister, the younger bn)ther of 32, and the younger sister of 20 The older sister died due to her airway being obstructel by acute laryngeal edema, and her you igirbrother and sister experienced recurrent episodes oswelling on the hand and forearm. The ore we examined the two patients serum complement levils to confirm diagnosis. In both, the r;ults showed decreased levels of Cl inhibitor and C4 which are the evidence of hereditary a giedema. The younger sister has been administered danazol (600mg/day) to prevent angioedem or 8 weeks, and we have not yet found a recurrenc of he symptoms. 8.Hyperkeratosis of the nipple and areola surgical excision and skin graft.

Hyperkeratosis of the nipple and areola is a rare skin disordr characterized clinically by thickening and discoloration of the skin of the areola and/or nipple and pathoiog cally by papillomaosis or acanthosis-nigricans like changes. We report a case of hyperkeratosis of the nipple and areola occuring in a woman with no underlying endocrinopathy and no other additional hyperkeratotic conditions. Ttie iresent case seems to be the nevoid form by the Levy-Franckel classification. Treatment with topical medications including steroids and keratolytic agents produced no improvement. Therefore we recommeneded surgical excision because the paient was greatly disturbed by the appearance of the lesions. Surgical excision and full thickness skin graft were performed sucessfully.

9.Distal Posterior Interosseous Nerve Syndrome.

PURPOSE: Chronic dorsal wrist pain limits hand function and is related to several diseases. Especially, pain and tenderness over the fourth extensor compartment with repetitive wrist dorsiflexion was described as distal posterior interosseous nerve syndrome. This article details the history, symptoms, physical findings, and treatment of this syndrome. MATERIALS AND METHODS: This study includes six patients seen between 1996 and 1998. Five men and one woman (ages 17 to 57 years) presented with localized, nonradiating, dull dorsal wrist pain, ulnar and distal to Lister's tubercle. Presumptive diagnostic test was an injection of selective lido cane 2 cm proximal to the wrist joint. RESULTS: Six patients were diagnosed of having distal posterior interosseous nerve syndrome. The five patients who recurred symptoms after injection underwent surgical exploration. Histologic evaluations revealed abnormal perineural fibrosis and edema. Patients were reexamined at least 12 months after surgery and have returned to full asymptomatic activity. CONCLUSIONS: Initially, non-operative treatment seems to be effective. If conservative treatment fails, operative excision of distal posterior interosseous nerve may be required
