1.A Case of Bednar Tumor.

A Bednar tumor is a rare pigmented subtype of dermatofibrosarcoma protuberans (DFSP). The clinical and histopathological findings of Bednar tumors are identical to DFSP, except for the presence of melanin-containing cells within the lesion. We present a case of a Bednar tumor on the left anterior chest of a 58-years-old woman. It was presented as an asymptomatic, firm, hyperpigmented 1.8x2.7cm sized nodule with surrounding erythematous patch which had been present for over 35 years. The tumor was treated by wide local excision of the lesion. There has been no evidence of recurrence during a 1 year follow-up period.

2.Efficacy of Interferon-alpha2b in Treatment of Warts.

BACKGOUND: Interferon-alpha2b has already proven to be effective in the clinical treatment of virus-originated diseases such as hairy cell leukemia, condyloma acuminatum, and AIDS-related Kaposi's sarcoma. The use of recombinant interferon-alpha2b may allow various types of wart to be treated relatively atraumatically and with less incidence of recurrence. OBJECTIVE: We tried to compare the effectiveness and safety of intralesional injections of recombinant interferon-alpha2b with natural interferon-alpha2b in the treatment of patients with various types of wart. METHOD: Patients with more than two warts were treated by injecting the different warts with 0.5 to 1.0X105 IU/1mm3 of recombinant and natural interferon-alpha2b, twice per week for 4 to 20 weeks. The response to treatment was followed up at 36 weeks. RESULTS: At the end of treatment, clearing of the treated warts had occurred in 83.3% of the recombinant interferon-alpha2b group and 91.6% of the natural interferon-alpha2b group. A more rapid cure rate was observed in the natural interferon-alpha2b group than in the recombinant interferon-alpha2b group. The rest showed partial improvement. With evaluation for relapse up to 16 weeks after treatment, warts were found to relapse in 11.1% of both the recombinant and natural interferon groups. CONCLUSION: Intralesional natural interferon-alpha2b has a better therapeutic effect than recombinant interferon-alpha2b, and may be considered as a therapeutic modality of recalcitrant verruca or when it can be anticipated that destructive techniques or blistering agents will not be tolerated.

3.A Case of Anagen Effluvium Induced by INH (Isonicotinic Acid Hydrazide).

We report a case of a 30 year-old woman who presented with acute scalp hair loss induced by isonicotinic acid hydrazide gap (INH). Considerable hair loss started within 4 weeks of INH administration. There was no evidence of dermatitis, allergic reaction, or any other cause for the hair loss. INH was discontinued, and the hair loss stopped within 4 weeks, with new hair growth seen. There was complete recovery of hair loss after 12 weeks of alopecia. Medication-induced hair loss is an occasional adverse effect of many drugs, however hair loss induced by INH has been reported in only 1 case. The complete recovery from anagen effluvium is difficult to explain, but it could have been due to the early discontinuance of INH.

4.Initial Experiences using Robot-assisted Distal Pancreatectomy: A Single Institution Study of 28 Patients.

PURPOSE: Surgical procedures using robot-assisted surgery, including pancreatic surgery, have recently become popular. This study aimed to report our initial experiences with distal pancreatectomy procedures using the robot-assisted surgery system. METHODS: The clinical records of 28 patients who underwent robot-assisted distal pancreatectomy (RDP) between July 2012 and January 2016 were reviewed. RESULTS: Of the 28 patients, 5 (17.9%) were male and 23 (82.1%) were female. Their ages ranged from 11 to 78 years, with a median age of 44.5 years. The mean diameter of the pancreatic tumors was 3.6 cm. The median operative time was 192.5 (range, 100~390) minutes, and the median blood loss was 200 (range, 50~1,900) ml. All of the 28 RDPs were successfully completed. Spleen preservation was achieved in 16 (57.1%) patients. Clinically significant postoperative pancreatic fistula was detected in 4 (14.3%) patients. Postoperative complications were evident in 5 (17.9%) of the 28 patients. CONCLUSION: Our experiences suggest that RDP is feasible for patients with benign or borderline tumors at the body or tail of the pancreas. It may be considered as an effective surgical procedure for benign or borderline tumors of the pancreas in the future; however, further studies to confirm this are warranted.

5.Anthropometric analysis of maxillary anterior buccal bone of Korean adults using cone-beam CT.

PURPOSE: The aim of this study was to evaluate the thickness of buccal and palatal alveolar bone and buccal bony curvature below root apex in maxillary anterior teeth of Korean adults using Cone-beam CT images. MATERIAL AND METHODS: The 3D image was reconstructed with dicom file obtained through CBCT from 20 - 39 year old Korean subjects (n = 20). The thickness of buccal and palatal plate, root diameter, the buccal bony curvature angle below root apex and the distance from root apex to the deepest point of buccal bony curvature were measured on maxillary anterior teeth area using OnDemand3D program. RESULTS: Mean thickness of buccal plate 3 mm below CEJ was 0.68 +/- 0.29 mm at central incisor, 0.76 +/- 0.59 mm at lateral incisor, and 1.07 +/- 0.80 mm at canine. Mean thickness of palatal plate 3 mm below CEJ was 1.53 +/- 0.55 mm of central incisor, 1.18 +/- 0.66 mm of lateral incisor, 1.42 +/- 0.77 mm of canine. Bucco-lingual diameter 3 mm below CEJ was 5.13 +/- 0.37 mm of central incisor, 4.58 +/- 0.46 mm of lateral incisor, and 5.93 +/- 0.47 mm of canine. Buccal bony curvature angle below root apex was 134.7 +/- 17.5degrees at central incisor, 151.0 +/- 13.9degrees at lateral incisor, 153.0 +/- 9.5degrees at canine. Distance between root apex and the deepest point of buccal bony curvature of central incisor was 3.67 +/- 1.28 mm at central incisor, 3.90 +/- 1.51 mm at lateral incisor, and 5.13 +/- 1.70 mm at canine. CONCLUSION: Within the limitation of this study in Korean adults, the thickness of maxillary anterior buccal plate was very thin within 1mm and the thickness of palatal plate was thick, relatively. The buccal bony curvature below root apex of maxillary central incisor was higher than that of lateral incisor and canine and it seems that the buccal bony plate below root apex of central incisor is most curved.

6.The Treatement of the Unstable Proximal Humerus Fracture Using Bifurcate Angled Blade Plate with Tension Band Wiring.

PURPOSE: To evaluate the results of internal fixation with bifurcate angled blade plate and tension band wiring of greater tuberosity or rotator cuff for the unstable proximal humerus fracture. MATERIALS AND METHODS: Nineteen cases of unstable proximal humerus fracture who were followed up for more than 1 year were treated using bifurcate angled blade plate and tension band wiring between March 2003 and March 2005. Average age of the patients was 46.5 years (range, 24 to 69), and there were 9 male and 10 female patients. The radiological results were evaluated by bony union and neck shaft angle. In addition, the clinical results were evaluated by Neer's evaluation criteria. RESULTS: In 18 out of 19 cases, bony union was obtained in average 12.5 weeks after operation. The neck shaft angles of sixteen cases were between 120 and 140 degrees by Paavolainen method. According to Neer's evaluation criteria, 17 cases showed excellent or good results. CONCLUSION: The patients treated using bifurcate angled blade plate and tension band wiring could exercise earlier because of good initial stability. Also, the radiological and clinical results were relatively good. The treatment of unstable proximal humerus fracture with bifurcate angled blade plate and tension band wiring was considered as a good method.

7.A Case of Diabetic Thick Skin Syndrome.

The diabetic thick skin syndrome is a common disease in the children of diabetes mellitus. But, it has not been reported in Korea. It is characterized by the stiff joints of hands and the indurated skin of both hands and arms. The simple screening test is prayer sign that patient is unable to bring fingers together because of contractures of proximal and distal interphalangeal joints. We present an 11-year-old Korean child with stiff joints of both hands and waxy, indurated skin of both hands and forearms that had been increasing in extent for the last 4 years.

8.Two-stage revision of infected total knee arthroplasty.

PURPOSE: To evaluate the result of treatment of the infected total knee arthroplasty by two-stage revision using antibiotics-impregnated cement. MATERIALS AND METHODS: We observed 2 staged revision cases of 11 infected total knee arthroplasty from March 1988 and March 2001. Average follow-up was 39.4 (28-60) months. At the time of infection management, the average age was 64.0 years. Local inflammatory finding, WBC with differential count, erythrocyte sedimentation ratio, C-reactive protein, joint aspiration and intraoperative frozen section (x400) were used to diagnose infection. We assessed knee function according to the knee rating system of the Hospital for Special Surgery (HSS). RESULTS: Prior to revision, the average HSS score was 50.8 points, and the average range of motion was 73.4 degrees. At. final follow-up, the average HSS score was 79.1 points, and the average range of motion was 90.2 degrees. At the last follow up, there were no sign of recurrence of infection clinically and radiologically. CONCLUSION: At least 2 year follow up, the results of two-stage revision using antibiotics-impregnated cement were good.

9.Expression of peroxisome proliferator-activated receptor (PPAR)-alpha and PPAR-gamma in the lung tissue of obese mice and the effect of rosiglitazone on proinflammatory cytokine expressions in the lung tissue.

PURPOSE: We investigated the mRNA levels of peroxisome proliferator-activated receptor (PPAR)-alpha, PPAR-gamma, adipokines, and cytokines in the lung tissue of lean and obese mice with and without ovalbumin (OVA) challenge, and the effect of rosiglitazone, a PPAR-gamma agonist. METHODS: We developed 6 mice models: OVA-challenged lean mice with and without rosiglitazone; obese mice with and without rosiglitazone; and OVA-challenged obese mice with and without rosiglitazone. We performed real-time polymerase chain reaction for leptin, leptin receptor, adiponectin, vascular endothelial growth factor (VEGF), tumor necrosis factor (TNF)-alpha, transforming growth factor (TGF)-beta, PPAR-alpha and PPAR-gamma from the lung tissue and determined the cell counts and cytokine levels in the bronchoalveolar lavage fluid. RESULTS: Mice with OVA challenge showed airway hyperresponsiveness. The lung mRNA levels of PPARalpha and PPAR-gamma increased significantly in obese mice with OVA challenge compared to that in other types of mice and decreased after rosiglitazone administeration. Leptin and leptin receptor expression increased in obese mice with and without OVA challenge and decreased following rosiglitazone treatment. Adiponectin mRNA level increased in lean mice with OVA challenge. Lung VEGF, TNF-alpha, and TGF-beta mRNA levels increased in obese mice with and without OVA challenge compared to that in the control mice. However, rosiglitazone reduced only TGF-beta expression in obese mice, and even augmented VEGF expression in all types of mice. Rosiglitazone treatment did not reduce airway responsiveness, but increased neutrophils and macrophages in the bronchoalveolar lavage fluid. CONCLUSION: PPAR-alpha and PPAR-gamma expressions were upregulated in the lung tissue of OVA-challenged obese mice however, rosiglitazone treatment did not downregulate airway inflammation in these mice.

10.A Case of Toxic Hepatitis by Weight-Loss Herbal Supplement Containing Garcinia cambogia.

We report a case of patient with acute toxic hepatitis induced by weight-loss herbal supplement. A patient took natural extract supplement containing Garcinia cambogia for only two days and visited Kyung Hee University Hospital at Gangdong complaining of abdominal discomfort and jaundice. Serum ceruloplasmin level was decreased and hepatic copper levels on liver biopsy specimen were increased. Although, those were mimicking Wilson's disease, pathological findings and clinical manifestations were consistent with drug-induced acute hepatitis. Genetic test was negative for Wilson's disease. After one month of hospitalization, the patient improved and was discharged home in good condition. She was followed up for four months and her symptoms and signs of hepatitis continued to improve steadily with normal liver function tests. Our case emphasizes the importance of taking a caution when taking herbal supplements for the purpose of weight loss. This case also demonstrates the importance of suspicion of medication-related adverse events, even though the duration of taking the materials seems to be too short to affect the patients.
