2.Survey on the State of Nutrition Support Team (NST) Activity: Comparison of the Questionnaire Survey 2016 vs. 2005 and the State of NST Activity since the Introduction of the Medical Insurance Fee.

The Korean Society of Health-System Pharmacists (KSHP) and Korean Society for Parenteral and Enteral Nutrition (KSPEN) jointly performed a nation-wide questionnaire survey on the current state of Nutrition Support Team (NST) activity from March to May of 2016. The aim of the survey was to suggest ways to develop NST as well as to improve the relationship between the KSHP and KSPEN. Compared to the results of the 2005 survey, some progress was made in the activity of NST over the last decade. Not only was the activity of NST settled, but quantitative and qualitative growth was also achieved by activating rounds and regular meeting. On the other hand, the ratio of hospitals providing home care services has decreased. Therefore, further effort is needed for their revitalization.

3.Computed tomography of intracranial tuberculosis

CT is a valuable method in determining number, location and extent of lesions, although a definite diagnosisis often not possible on CT. In intracranial tuberculosis, CT was helpful in the diagnosis, assessing the degreeof hydrocephalus and evaluating the effectiveness of antituberculous therapy. Twenty-one cases of clinical lyproven intracranial tuberculosis were studied by CT in our hospital during last 3 years. Of them, eighteen caseswere tuberculous meningitis and the rests were tuberculoma. The results were as follows; 1. Tuberculous meningitis presented the following three patterns of CT findings according to its disease process. a. In early stage of the disease, suspcious multiple isodense small nodules in the cerebral and cerebellar hemispheres showed dense enhancement in postcontrast scan representing miliary tubercles. b. In later stage of the disease, precontrastscan showed partial or total obliteration of the basal and sylvian cisterns with mild dilatation of ventricularsystem. Postcontrast scan showed dense enhancement of basal and sylvian sisterns. This type of finding was themost common in our series. c. Moderate to marked dilatation of ventricle with or without a cluster ofcalcifications in suprsella area on precontrast scan was seen in far later stage or as a sequellae of the disease. No enhancement was noted in postcontrast study. 2. Tuberculoma showed an isodense or slightly hyperdense area inthe cerebral or cerebellar hemisphere with associated minimal edema in precontrast study. Postcontrast scan showeda small ring enhancement with central lucent area.

4.A case of Behcet's syndrome with superior vena cava syndrome.

Behcet's syndrome has been known as a disease which has the triple symptoms such as recurrent orogenital ulceration and eye inflammation. The syndrome is now recogninized as a multisystem disease involving mucocutaneaous, cardiovascular, central nervous system, renal, articular and gastrointestinal system. The vascular involvement in Behcet's disease has been reported since Mischima first described the case in 1961, four types of vascular lesion are recognized ; arterial occlusion ; aneurysm ; venous occlusion ; and varices. Venous occlusion are frequently observed, most commonly in the inferior or superior vena cava. We report a case of Behcet's disease manifested as superior vena cava syndrome due to thrombus on both brachiocephalic veins.

6.Two cases of collodion Baby.

Two case of a rare from of congenital ichthyosis in infants born to the same parents are presented. Typical features revealed of parchment like skin, ectropion of upper eyelids, and fixed semiflexion attitude of the upper limb. The skin lesion were followed by desquamation and appearance of normal looking skin from the the second day of life to a month. Diagnosis of collodion baby was established by clinical features, histopathological and laboratory studies. A brief feview of literatures were presented.

7.Cornelia de Lange Syndrome.

The Cornelia de Lange syndrome is characterized by severe growth and mental retardations and a cluster of minor malformations, the facial appearance being most characteristic. In the present paper, we shall report I case of this syndrome in Korean male infant and the variability of de Lange syndrome is discussed. The bady showed hirsutism, low forehead coved with lanugo-like hair, bushy eyebrows that meet in the midline, long curely eyelashes as well as low pitched, growling cry and skeletal abnormalities of hand bones. There is no positive family history and the karyotype was normal. Although the de Lange syndrome has received more interest there is no agreement as to the possible cause.

8.The Changes of Percutaneous Oxygen Saturation Following Obstructive Apnea in Infants.

Preoxygenation is a standard anesthetic technique which prevents significant hypoxemia during the induction of anesthesia. Complete oxygenation is especially important in clinical situations of difficult intubation or in patients with decreased FRC, and in siturations where oxygen saturation is critical. During the induction of anesthesia in children, airway obstruction and apnea are associated with rapid development of hypoxemia. The decreasing speed of oxyhemoglobin saturation was faster in smaller infants than bigger infants. The most important factor determining the speed with which hypoxemia develops in healthy children is probably the oxygen reserve contained in the lungs and its relation to the oxygen consumption of the child. With deaeasing age, the arterial oxygen consumption increases and the ratio of FRC to body weight decreases. Due to the anatomical structure of an infant's upper airway, it is more difficult to obtaine patient airway in infants than in children. During repeated atttempts to intubate the trachea or while waiting for recovery from laryngeal spasms hypoxia can occur easily resulting in visible cyanosis in infants. This study was carried out to measure the time permissible for apnea before occurance of hypoxia following full oxygenation. The subjects consisted of 6 randomly selected infants 1-2 month of age, 4.6+/-0.6 Kg of body weight with no abnormalities of cardiorespiratory functions. After the intramuscular injection of atropine, patients were anesthetized through mask using oxygen and halothane. SpO2 and pulse rates were recorded throughout the study. After the patients were intubated, a plug was placed on the distal end of the tube to induce obstructive apnea. As soon as SpO2 decreased to just below 90%, the patients were ventilated again. In 2 of the infants, the time required to obtaine 90% saturation was 60 seconds. Within less than 70 seconds, four out of 6 infants had SpO2 below 90% and SpO2 below 80% were noticed in 3 cases. After the reestablishment of ventilation, SpO2 returned to the preapneic value within 10 second in all subjects. There was no evidence of increasing pulse rate as SpO2 levels decreased. However, pulse rate decreased in all subjects thoughout the study. In summary, maximum time permissible for apnea in neonate and young infant is approximately one minute. Furthermore, tachycardia should not be used as a sign for the onset of hypoxia.

9.Impact of the Learning Curve on the Survival of Abdominal or Minimally Invasive Radical Hysterectomy for Early-Stage Cervical Cancer

Purpose:
The objective of this study was to define the learning curve required to attain satisfactory oncologic outcomes of cervical cancer patients who were undergoing open or minimally invasive surgery for radical hysterectomy, and to analyze the correlation between the learning curve and tumor size.
Materials and Methods:
Cervical cancer patients (stage IA-IIA) who underwent open radical hysterectomy (n=280) or minimal invasive radical hysterectomy (n=282) were retrospectively reviewed. The learning curve was evaluated using cumulative sum of 5-year recurrence rates. Survival outcomes were analyzed based on the operation period (“learning period,” P1 vs. “skilled period,” P2), operation mode, and tumor size.
Results:
The 5-year disease-free and overall survival rates between open and minimally invasive groups were 91.8% and 89.0% (p=0.098) and 96.1% and 97.2% (p=0.944), respectively. The number of surgeries for learning period was 30 and 60 in open and minimally invasive group, respectively. P2 had better 5-year disease-free survival than P1 after adjusting for risk factors (hazard ratio, 0.392; 95% confidence interval, 0.210 to 0.734; p=0.003). All patients with tumors < 2 cm had similar 5-year disease-free survival regardless of operation mode or learning curve. Minimally invasive group presented lower survival rates than open group when tumors ≥ 2 cm in P2. Preoperative conization improved disease-free survival in patients with tumors ≥ 2 cm, especially in minimally invasive group.
Conclusion
Minimally invasive radical hysterectomy required more cases than open group to achieve acceptable 5-year disease-free survival. When tumors ≥ 2 cm, the surgeon’s proficiency affected survival outcomes in both groups.
10.Annual Report on the External Quality Assessment Scheme for Immunoassay Tests in Korea (2015).

Two trials of external quality assessment were performed in 2015, with 13 test items grouped into four test categories. The first trial materials were sent on May 19, 2015 and the second trial was performed on November 9, 2015 with 13 items including tumor markers, thyroid hormones, cardiac marker troponin (troponin T or troponin I), and procalcitonin (PCT) as biomarkers by immunoassay methods. The bone marker, carboxy-terminal collagen crosslinks (CTX) was replaced by procalcitonin since 2014, because a limited number of institutions performed assays with CTX. External quality surveys of 13 immunoassay test items with 16 control materials were conducted, as scheduled. In total, 13 control materials were used, which consisted of six tumor markers, namely alpha-fetoprotein (AFP), carcinoembryonic antigen (CEA), carcinoma antigen (CA) 125, carbohydrate antigen (CA) 19-9, human chorionic gonadotrophin (HCG), and prostate specific antigen (PSA). In addition to tumor markers, 5 thyroid markers, namely thyroid hormone (T)3, T4, thyroid stimulating hormone (TSH), free T4, and thyroglobulin (TG) were included. Furthermore, troponin, as a cardiac marker, and procalcitonin, as a new biomarker, have been adopted since 2014. Five home-made pooled sera and 3 commercial control sera were used as survey materials. MAS Tri-point Liquimmune level 3 (Medical Analysis Systems Inc., USA) was used for thyroid hormones. Procalcitonin and troponin control materials were from Elecsys Precis Control Varia and Elecsys Precis Control Troponin (Roche, Germany), respectively. The number of laboratories participating in the external quality assessment for Immunoassay Subcommittee was 719 institutions in the first trial survey (response rate 98.7%) and 730 institutions in the second survey (94.9%). The test items most frequently used in immunoassays were TSH (93.2%, 93.1%), free T4 (90.3%, 90.2%), and AFP (89.4%, 89.0%), whereas recently adopted biomarkers were less frequently used: troponin I (36.6%, 37.1%), procalcitonin (24.1%, 26.7%), and thyroglobulin (10.3%, 10.7%). The quality of the laboratories participating in the survey seems to be continuously improving, according to their peer group results.
