Korean Journal of Perinatology 1998;9(1):26-30
PURPOSE: The purpose of this study was to evaluate the effects of theophylline in preterm infants with apnea on glucose homeostasis and insulin values. METHOD: In this prospective study, level of glucose and insulin were measured from peripheral blood of 8 neonates(1,450+/-114gm, 31+/-2.1week), who were admitted from April 1, 1997 to July 30, 1997 in Neonatal Intensive Care Unit of Wonkwang University Hospital, for apnea of prematurity(> 20 sec with bradycardia and/or cyanosis) were given aminophylline intravenously. Blood samples were collected at pretreatment, posttreatment 2hours, 1-2days, 3-4days, 5-7days and posttreatment 48hours, and compare to those of the 8 control neonates(1,711+/-232gm, 32+/-1.7week). RESULTS: The results were as follows: 1) Plasma glucose values were significantly higher in the treatment group than those of the control group at 1-2days(104.67+/-20.39mg/dL vs 83.43+/-15.86mg/dL) and 3-4days(111.0+/-32.39mg/dL vs 79.25+/-14.03mg/dL)(p<0.05). 2) Plasma glucose values which were increased at 1-2days(104.67+/-20.39mg/dL)and 3-4days(111.0+-32.39mg/dL), were significantly higher than pretreatment values(66.33+/-31.19mg/ dL)(p<0.02), but were not increased to the level of hyperglycemia(> 125mg/dL). 3) The mean posttreatment glucose levels drawn at 48hours after discontinuation of theophylline was significantly decreased to the values of pretreatment values compared to those of the 1-2days and 3-4days(p<0.01). 4) Insulin concentrations were also increased insignificantly when blood glucose were increased in the treatment group compared with control group. CONCLUSION: In conclusion, intravenous administration of theophylline produces hyperglycemia significantly, including an increase in a serum insulin. But, clinically significant hyperglycemia(> 125mg/dL) was not noted. So, plasma glucose may not need to be monitored in preterm apneic infants receiving theophylline. But, further studies are need to elucidate the effect of theophylline considering the serum toxic level of theophylline.

8.Effects of Adjuvant Immunochemotherapy in Stage III Gastric Cancer.
Jeong Kyun LEE ; Jung Taeck OH
Journal of the Korean Surgical Society 2000;58(1):72-78
BACKGROUND: To evaluate the effectiveness of early postoperative adjuvant immunochemotherapy in a stage III (UICC, 1997) primary gastric carcinomas we analyzed cases histories of 140 patients retro spectively who had undergone curative gastrectomy at Wonkwang University Hospital from November 1988 to November 1995. METHODS: For immunotherapy, OK-432 intramuscularly or oral PSK was used for 2 months, and for chemotherapy, FAM (8 week cycle) or oral 5-FU derivatives and MMC were used for 6 months or longer. Immunotherapy was started at the 5th postoperative day and chemotherapy at the 7th to 10th postoperative day. Sixty-eight (68) patients received immunochemotherapy (therapy group; TG) and 72 patients did not (nontherapy group; NTG). Statistical analysis were carried out with Anova, Kaplan-Meier, and Log rank test. RESULTS: One hundred eight (108) patients were male, and 84 patients were younger than 60 years. Eighty-one (81) cases involves the lower stomach and 52 the middle stomach. Lymphatic invasion was seen in 60 cases. Twenty-five (25) cases were T2, 112 T3, 42 N1, 90 N2, 75 stage IIIa, and 65 stage IIIb. The overall 5-year survival rate was 48.6% (stage IIIa 58.7%, stage IIIb 36.9%, p<0.05). The 5-year survival rates for the TG and the NTG were 52.9% and 44.4%, respectively (p=0.10). The 5-year survival rates were 85.7% (n=18) and 85.7% (n=7) in T2, 54.0% (n=50) and 40.3% (n=62) in T3 (p<0.05), 69.6% (n=23) and 50.0% (n=24) in N1, 44.4% (n=45) and 42.2% (n=45) in N2 (p=0.14), 61.0% (n=41) and 55.9% (n=34) in stage IIIa, 40.7% (n=27) and 34.2% (n=38) in stage IIIb (p=0.16), 59.5% (n=37) and 48.8% (n=43) in the non-lymphatic invasion group, and 45.2% (n=31) and 37.9% (n=29) in the lymphatic invasion group (p=0.09). There was no significant difference in the 5-year survival rates of the other parameters, such as age, sex, tumor location, size, gross finding, tumor differentiation, between the TG and the NTG. CONCLUSION: The survival rate in the early postoperative immunochemotherapy group was not signi ficantly increased compared to that in the nontherapy group for stage III gastric cancer, but the immuno chemotherapy group showed a tendency for a higher 5-year survival than the nontherapy group did.

10.The Study for the Outflow Of Aqueous Humor after Injection of Dye into the Anterior Chamber.
Jung Hyub OH ; Sang Joon PARK ; Sang Kyun KIM
Journal of the Korean Ophthalmological Society 1999;40(3):779-783
The aqueous bumor makes an important role to maintain intraocular pressure. The production and outflow of aqueous humor controls the intraocular pressure. The accurate understanding of the pathway of aqueous outflow and the rate of outflow is essential to the study of glaucoma. The purpose of this study is to investigate the pathway of aqueous outflow after injection of dye into the anterior chamber, the duration of staining and the degree of delay of aqueous outflow after the use of viscoelastics. Three dyes. Gentian Violet, Methylene Blue and Viscoelastics mixed with Gentian Violer were injected into the anterior chambers of 20 eyes of 10 rabbits. The eyes were enucleated at 1 hour, 2 hour, 3 hour, 4 hour after injection of Gentian Violer and Methylene Blue. After injiection of viscoelastics mixed with Gentian violet, the eyes were enucleated at 2 hour, 4 hour, 6 hour, 8 hour, and 9 hour. The enucleated eyes were performed for frozen section and the fragments were observed by light microscopee. Gentian violet and Merhylene Blue were stained in the pathway of the pectinate ligaments, trabecular meshwork, intrascleral plexus and the pathway of the capillary network of iris and suprachoroidal space. The disappearance of injected dye was ended between 3 hour and 4 hour. And the viscoelastics mixed with dye was removed between 8 hour and 9 hour. This means that viscoelastics make the outflow of aqueous delay.
