1.Comparison Studies of SPECT Dopamine Transporter Imaging and Noninvasive Quantification using < Tc-99m > TRODAT-1 and < I-123 > IPT.

The SPECT radiopharmaceuticals labeled with I-123 for dopamine transporter imaging have been used to measure dopamine transporters in patients with movement disorders. However, a cyclotron produced I-123 limits its availiability and ease of use as a radioisotope to be labeled with pharmaceuticals in routine clinical diagnostic procedures. Recently, new radiopharmaceuticals for Tc-99m which has optimal characteristic for SPECT imaging have been developed to overcome the limits of using I-123. The purpose of this study was to compare the quality of [Tc-99m]TRODAT-1 with [I-123]IPT SPECT data and then to evaluate the usefulness of [Tc-99m]TRODAT-I SPECT by using three noninvasive simplified quantitative methods. TRODAT-1 labeled with Tc-99m(15.93+/-0.82 mCi) and IPT labeled with I-123(6.60+/-0.11 mCi) were injected into five normal controls. Dynamic [Tc-99m] TRODAT-I SPECT scans of brain were performed for 10 minutes each over 180 minnutes, and for 20 minutes at 4 hrs and 5 hrs. [I-123IPT SPECT scans were performed for 5 minutes each over 120 minutes. Time activity curves were generated for the left basal ganglia(LBG), right basal ganglia(RBG), and occipital cortex(OCC). Dopamine transporter parameters were obtained using (BG-OCC)/OCC, graphical method(Rv), and area ratio method(RA). TRODAT-1 and IPT SPECT imaging showed high uptake at the level of the basal ganglia. (BG-OCC)/OCC ratios for TRODAT-1 and IPT were 0.80+/-0.14, and 3.22+/-0.81, Rvs were 0.62+/-0.12, and 2.30+/-0.35, and RAs were 0.37+/-0.08 and 1.73+/-0.31, respectively. In conclusion, further improvement of [Tc-99m]TRODAT-I imaging characteris- tics may be required to estimate the dopamine transporter concentrations in human brains although it shows clear BG localization.

2.Orbital floor and infraorbital rim reconstruction with vascularized calvarial bone flap.

A number of methods have been introduced for support the orbital floor following a maxillectomy without orbital exenteration or severe facial trauma. These methods including skin graft and muscular sling provided the unsatisfactory results, like as diplopia, orbital ptosis, enophthalmos and severe facial deformity. Therefore the bone and soft tissue reconstructions using microvascular free flaps were performed recently by many surgeons, but long time operation, donor site morbidity, postoperative large scar, and ptosis of the flap were pointed out as disadvantages of free flap reconstruction. Vascularized calvarial bone flap, a modified method of free calvarial bone graft, was adequate for reconstruction of the orbital floor and the infraorbital rim as a horizontal buttress, especially in case of poor vascular bed and postradiated state. The authors introduced the vascularized calvarial bone flap for the orbital floor and the infraorbital rim reconstruction in 3 cases of maxillectomy, and could be obtained satisfactory results aesthetically and functionally.

3.Clonality of Large Regenerative Nodule Accompanied by Hepatocellular Carcinoma.

In order to clarify the preneoplastic nature of large regenerative nodules without dysplastic change, we analysed the clonality of hepatocellular carcinomas (HCCs) and large nodules, diameter > or =0.5 cm, of cirrhotic liver by X-linked human androgen receptor (HUMARA) gene assay, using the principle of random X chromosome methylation and inactivation in female. Ten cases of HCC and 5 cases of large nodules without dysplasia from 9 female patients were selected. All the tumors, large nodules and paired normal control cells were selectively microdissected from deparaffinized hematoxylin and eosin stained slides. Genomic DNA was isolated and digested with HhaI. Polymerase chain reaction(PCR) amplication of the HUMARA locus was performed using 32P-a-dCTP containing PCR mixtures. The PCR amplified products were separated by gel electrophoresis and analysed by autoradiography. Nine HCCs from 8 patients were monoclonal and 1 case was polyclonal and the remaining 1 case was not polymorphic at the HUMARA locus. The HCC case which showed polyclonality contained many inflammatory cells. All the large nodules were polyclonal by HUMARA assay. These results suggest that all or most of the cells composing the large regenerative nodules without dysplasia are polyclonal. This assay may be informative for the differentiation between regenerative and preneoplastic nodules in cirrhotic liver and the size of nodule may be not important in hepatocarcinogenesis.

4.Clinical Study of Epinephrine Administration to Pediatric Patients under Halothane Aneathesia for Closure of Cleft Palate .

A retrospective review was performed of 72 cases of cleft palate operated at the Nati-onal Medical Center from March 1985 to June, 1986. We evaluated the frequency of cardiac arrythmia and changes in the heart rates when epinephrine is used with halothane anesthesia in pediatric patients for the closure of palate. 21 children (29.2%) developed several types of ventricular dysrrhythmias and one child had ventricular fibrillation. 41 children(65.37) had increase in heart rates, and 23 children had no change after epinephrine injection. All children restored sinus rhythm after 100% oxygen inhalation lidocaine(1-1.5 mg/kg) , and demerol (0.5-1 mg/kg) were injected.

5.Development and Performance Test of Preamplifier and Amplifier for Gamma Probe.

PURPOSE: Preamplifier and amplifier are very important parts for developing a portable counting or imaging gamma probe. They can be used for analyzing pulses containing energy and position information for the emitted radiations. The commercial Nuclear Instrument Modules (NIMs) can be used for processing these pulses. However, it may be improper to use NIMs in developing a portable gamma probe, because of its size and high price. The purpose of this study was to develop both preamplifier and amplifier and measure their performance characteristics. MATERIALS and Methodes: The preamplifier and amplifier were designed as a charge sensitive device and a capacitor resistor-rsistor capacitor (CR-RC) electronic circuit, respectively, and they were mounted on a print circuit board (PCB). We acquired and analyzed energy spectra for Tc-99m and Cs-137 using both PCB and NIMs. Multichannel analyzer (Accuspec/A, Caberra Industries Inc., Meriden Connecticut, U.S.A) and scintillation detectors (EP-047 (Bicron Saint-Gobain/Norton Industrial EP-047 (Ceramics Co., Ohio, U.S.A) with 2"x2" NaI (T1) crystal and R1535 (Hamamatsu Photonics K.K., Electron Tube Center, Shizuoka-ken, Japan) with 1"x1"NaI (T1) crystal) were used for acquiring the energy spectra. RESULTS: Using PCB, energy resolutions or EP-047 detectors for Tc-99m and Cs-137 were 12.92% and 5.01%, respectively, whereas R1535 showed 13.75% and 5.19% of energy resolution. Using the NIM devices, energy resolutions of EP-047 detector for Tc-99m and Cs-137 were measured as 14.6% and 7.58%, respectively. However, reliable energy spectrum of R1535 detector could not be acquired, since its photomultiplier tube (PMT) requires a specific type of preamplifier. CONCLUSION: We developed a special preamplifier and amplifier suitable for a small sized gamma probe that showed good energy resolutions independent of PMT types. The RESULTS indicate that the PCB can be used in developing both counting and imaging gamma probe.

8.Thoracocervicofacial Purpura Occurred After Epileptic Seizure.

The thoracocervicofacial pupura is an obstructive pupura, which occurs in the various conditions with elevated pressure in the thoracic cavity. This pupura can occur as an unusual consequence of epileptic seizure. A 28-year-old man visited due to the syncope and the thoracocervicofacial pupura. His creatine kinase level was elevated. Therefore the cutaneous manifestation is thought to be due to epileptic seizure. Here we report a case of thoracocervicofacial pupura as the unique manifestation of epileptic seizure.

9.A Case of Intradermal Nevus Combined with Epidermal Cyst.

The combination of an intradermal nevus with epidermal cyst or other adnexal tumors has often been described since 1952. Most of these reports have involved the coexistence of common epidermal cysts and melanocytic nevi. Herein, we report a 21-year-old Korean male, who initially presented to our department with a rapidly enlarging preexisting nevus on his back. The mass was surgically excised and histopathologically showed intradermal nevus combined with epidermal cyst.
