PURPOSE: To assess the accuracy of magnetic resonance(MR) arthrography in the diagnosis of anterior labral tear of the shoulder. MATERIALS AND METHODS: Between Semptember 1996 and February 2000, MR arthrography of the shoulder was performed in 281 patients with a history of shoulder pain or instability. Among this total, only 157 shoulders in 154 patients who underwent arthroscopy or open surgery 0 to 230 (average, 20.9) days after MR arthrography were included in this study; the subjects comprised of 150 males and 4 females with an average age of 23.3 years. MR arthrographs of these 154 patients were analyzed for the presence of anterior labral tears, and the findings were correlated with the arthroscopic and surgical findings. Anterior labral tear was classified as A to D according to its location, as determined by arthroscopy and surgery. (A=4 to 6 o 'clock direction, anteroinferior; B=2 to 4 o 'clock direction, central; C=12 to 2 o 'clock direction, anterosuperior; D= SLAP lesions). The retrospective analysis of MR arthrographs showing false-positive and negative findings was also undertaken. RESULTS: In the diagnosis of anterior labral tear, MR arthrography showed a sensitivity of 94%, a specificity of 90% and an accuracy of 91%. Anterior labral tears were confirmed by arthroscopy or surgery in 62 of the 157 shoulders (39%). Among 62 lesions, two (3%) were observed in area A, 32(52%) in area A+B, nine (15%) in area A+B+C, one(2%) in area A+B+D, 13(21%) in area A+B+C+D, two (3%) in area B+C, one(2%) in area B+D, and two(3%) in area C. Among ten false-positive cases, seven were focal lesions (two, three and two lesions in area A, B and C, respectively), and in the remaining three cases, located in area A+B, MR arthrography revealed thickening and deformation. All four false negatives were focal lesions (two in area A and two in area C). CONCLUSION: Other than in focal lesions, in which accuracy was relatively low, MR arthrography showed high sensitivity, specificity and accuracy in the diagnosis of anterior labral tear of the shoulder.

Myocarditis is defined as the myocardial inflammation caused by various infectious agents (such as virus, rickettsia , bacteria, protozoa, fungus and parasites). The clinical manifestations of myocarditis ranges from the asymptomatic state due to focal inflammation to fulminant fatal congestive heart failure secondary to diffuse myocardial involvement. Clinically, in some cases, it may simulate an acute myocardial infarction. We experienced a case of acute fulminant myocarditis that presented as acute myocardial infarction initially, and then progressed into and recovered from congestive heart failure and multiorgan failure.

Spinal cord tumors usually present themselves with severe pain without neurologic deficits during early stages. Cervical or thoracic spinal tumors can evoke anterior neck pain or chest pain without neurologic symptoms. As a result, when chest pain or anterior neck pain occurs initially, many physicians think its origin is a cardiac disorder such as angina. Eventually, during cardiac evaluation and treatment, myelopathy or radiculopathy develops to reveal spinal cord tumor. Even though it is rare, when anterior neck or chest pain is the presenting complaint, the possibility of spinal cord tumors should be considered. We report a case of spinal cord tumor with initial presentations of anginal chest pain and electrocardiographic myocardial ischemic changes (ST segment depression and T-wave inversion) in a 45 year old male patient who had no history of hypertension. It was later discovered that the cause was due to an A-V malformation.

Metal brackets and ceramic brackets were bonded to natural teeth, porcelain crowns and gold crowns. After stored in artificial saliva solution for 72 hours at 37degreesC, the shear bond strengths were measured by Instron and compared with them, the bonding sites and bracket bases were examined by scanning electron microscope and light optical stereomicroscope. The results were as follows: 1. The shear bond strengths of the group which metal brackets were bonded to natural teeth and the groups which ceramic brackets were bonded to natural teeth and porcelain crowns were comparable to each other, the shear bond strength of the group which metal brackets were bonded to gold crowns was significantly low. 2. The bond failed predominantly at the bracket base/adhesive interface with the bulk of adhesive remaining on enamel in the group which metal brackets were bonded to natural teeth. 3. The bond failed consistently at the crown/adhesive interface with all of adhesive remaining on the bracket bases in the group which metal brackets were bonded to gold crowns. 4. The bond failed at the enamel or crown/adhesive interface with the bulk of adhesive remaining on the bracket bases in the groups which cramic brackets were bonded to natural teeth and porcelain crowns. 5. The shear bond strengths of the groups which ceramic brackets were bonded to porcelain crowns were not affected by etching time.

This study was undertaken to observe the histologic changes of surrounding maxillary sutures to the widening of midpalatal suture, using two adult dogs, weighing about 10 kg, for experimental, and one for control group. After widening of the midpalatal suture with expansion screw for ten days was performed, and followed by sacrifice of experimental animals. Tissues were excised from 5 surrounding maxillary suture portions such as, internasal, interfrontal, midsagittal, zygomatrco-temporal, and midpalatal sutures. After that, the specimens were fixed and d calcified in 10% formalin sol. and 5% nitric acid. Embedding in paraffin end, serial sections at a thickness of 5 micron was done, After Hema toxylin Eosin staining and light microscopic examination, Following results were obtained 1. in midpalatal suture area, which showed most prominent histologic change, High degree of fibronlastic and osteoblastic proliferation lining the bony trabeculae with osteoid tissue projection into the fibrous connective tissue were observed. 2. In interfrontal suture area, moderate degree of fibroblastic and osteoblastic proliferation was observed. 3. In internasal suture area, active osteoblastic and osteoclastic proliferation lining the bony trabeculae was observed, and separation of fibrous connective tissue was also observed. 4. In midsagittal and zygomatico-temporal suture areas, no histologic changes can be observed.

The author studied the dental arch widths and lengths height of palates at 2 groups of dentition : mixed dentition, early permanent dentition, young adult, having normal occlusion and dentition. The models of the 336 maxillary and mandibular casts, made from alginate-base hydrocolloid impressions were measured and analyzed statistically. The result as follows; 1. The upper intercanine width increased between the mixed dentition group and early permanent dentition group but there was no change in the young adult group in both sexes. Teh lower intercanine width increased between the mixed dentition group and early permanent dention group in the male. 2. The upper and lower 1st bimolar width increased slightly with age in the male but there was no change in the female. 3. The sex difference found in this study was one of absolute size, the female being slightly smaller than the male in the early permanent dentition group and young adult. 4. The arch length had no notable sexual differences and decreased between the mixed dentition group and early permanent dentition group. There was change in the arch length in the young adult. 5. The height of palate increased gradually with age.
