3.The Study of Serum Free Fatty Acid, Triglycerides and Glycerol in Congestive Heart Failure, Essential Hypertension and Liver Cirrhosis.

Just as hyperbetalipoproteinemia is the most common kind of familiar hyperlipoproteinemia, hyperprebetalipoproteinemia or increased very low density lipoprotein (VLDL) and its associated hypertriglyceridemia is an abnormality frequently found in association with several metabolic and nutritional disorders. This VLDL abnormality is demonstrable in high percentage of insulin dependent diabetic children before they receive treatment. It is also observed in many older, overweight, insulin resistant diabetic, with poor control of diabetes. An elevation of VLDL was also observed after ethanol intake. A mild to moderate degree of VLDL elevation is the most frequently observed from of clinical hyperlipidemia. Many investigators reported that hyperprebetalipoproteinemia (hypertriglyceridemia) is associated with an increasing incidence of coronary artery disease. As opposed to hyperbetalipoproteinemia, hyperprebetalipoproteinemia is prone to influence on the development of atherosclerosis in the middle and older aged persons which indicates the importance of triglyceride determination in these aged groups. There has been relatively rare study reported on the triglyceride metabolism in patients with congestive heart failure, essential hypertension and liver disease in Korea. This study was designed to determine serum triglyceride, free fatty acid and glycerol in the disease mentioned in Busan University Hospital between jan. 1975 and December 1976 and analized the change of lipid profile in each different condition. The results were as follows; 1. The mean of serum FFA, triglyceride and glycerol in the 37 normal were as follows 502.32+/-146.54microEq/L, 111.84+/-40.53mg% and 432.00+/-212.13microM/ml. 2. 23 patients with congestive heart failure showed a significant reduction in serum triglycerides (91.96+/-27.80mg%) and a significant increase in serum free fatty acid (576.77+/-129.67microEqL) and glycerol (432.50+/-212.13microM/ml). 3. In 15 patients with essential hypertension a significant elevation of serum triglycerides (149.38+/-42.28mg%) was noted. 4. In 13 patients with liver cirrhosis, a reduction in serum triglycerides (80.50+/-34.27mg%) was noted.

4.Evaluation of EL-ANA/6 Profiles for Specific Antinuclear Antibodies.

BACKGROUND: Identification of specific antinuclear antibodies is useful for the diagnosis, subclassification and determination of prognosis in autoimmune disorders. In many diseases, multiple autoantibodies are detected, and simultaneous detection of multiple autoantibodies has been shown to be useful. Recently, a commercial kit (EL-ANA/6 profiles, TheraTest Laboratories, USA) losing enzyme-linked immunosorbent assay (ELISA) method for detection of six specific autoantibodies is avallable. In this study, we attempted to compare the results of EL-ANA/6 profiles with those of routinely used methods and evaluated usefulness of EL-ANA/6 profiles. METHODS: EL-ANA/6 profiles were performed with 28 sera which were positive for fluorescent antinuclear antibody (FANA) Simultaneously we tested anti-dsDNA antibodies with immnofluorescent (If) method and anti-Sm, anti-RNP, anti-SSA and anti-SSB antibodies with double immunodiffusion (DID). To evaluate specificity, EL- ANA/6 profiles tests were performed on 10 sera from healthy blood donors. RESULTS: Ten sera of healthly blood donors were all negative for EL-ANA/6 pro biles. In the results of EL-ANA/6 profiles on sera positive for FANA, the concordance rate with IF method for the anti-dsDNA antibodies was 89.3% (25/28) and the con- cordance rates with DID for anti-Sm, anti-Sm/RNP, anti-SSA and anti-SSB antibodies were 85.7% (24/28), 82.1% (23/28), 92.9% (26/28) and 82.1% (23/28), respectively. In 16 discordant settings, thirteen (81.3%) were negative on DID and positive on EL-ANA/6 profiles. CONCLUSIONS: The results of the EL-ANA/6 profiles show good concordance rates with If and DID. EL-ANA/6 profiles showing quantitative profiles for multiple autoantibodies is useful for diagnosis and tool)ow-up of autoimmune disorders.

6.Surgical Treatment for Burst Fracture of the Thoracolumbar Spine: Anterior approach vs posterior approach

We present a prospective, randomized study of acute burst fracture of the thoracolumbar spine. Forty-one patients were treated either by anterior decompression and stabilization with Kaneda device or by posterior distraction instrumentation using the A-O fixateur interne. The mean follow up was 21 months. The result were as follows; 1. The mean preoperative kyphotic angle was 19.2° in those patients treated by anterior surgery and 21.4° in those patients treated by posterior surgery. At last follow-up the mean correction in kyphotic angle was 13.2° in the anterior group and 9.2° in the posterior group. There is no statistically significant difference between those two groups. 2. The mean preoperative midsagittal diameter of the canal compromise 47.4% in anterior group and 49.3% in posterior group. Postoperatively, this was reduced to 2.5% and 7.9%. There is a statistically significant difference between these two groups(P < 0.05). 3. The mean preoperative canal enchroachment 52.)% in anterior group and 47.6% in posterior group. Postoperatively, this was reduced to 3.2% and 6.0%. There is a statistically significant difference between these two group(P < 0.05). 4. Neurologic recovery was 81% in posterior surgery and 85% in anterior surgery. The improvement in Frankel grade was 1 grade in average, and showed no difference between two groups. 5. There was two cases of pedicle screw breadage in posterior group and one case of pyothorax in anterior group but no early or late vascular or neurologic complication.

8.The Diagnostic Value of Segmental Nerve Conduction Study in Diabetics.

Nerve conduction study has been an important part of electrodiagnosis and has been utilized as a clinical diagnostic technique in diabetic neuropathy. But conduction study of the more proximal segment has been a few reports. This study demonstrated segmental method for obtaining orthodromic motor nerve conduction and mixed nerve conduction, and antidromic distal sensory nerve conduction in diabetic group and normal group. The purpose of the study was to determine diagnostic value of segmental nerve conduction in diabetic neuropathy. The results were as follows: 1. Significant difference at P<0.01 between diabetic group and normal group; in median motor terminal latency, median motor nerve conduction velocity at axilla-elbow segment and elbow-wrist segment, median sensory nerve conduction velocity at wrist-elbow segment, ulnar motor and sensory conduction velocity at wrist-elbow segment, posterior tibial motor nerve conduction velocity at popliteal fossa-ankle segment, peroneal motor nerve conduction velocity at fibula head-ankle segment. 2. No significant difference at P>0.05 between diabetic group and normal group; in ulnar motor terminal letency, ulnar sensory nerve conduction velocity at axilla-Erb's point segment and finger-wrist segment, peroneal motor terminal latency and peroneal motor nerve conduction velocity at popliteal fossa-fibula head segment, posterior tibial motor terminal latency, sural sensory nerve conduction velocity. 3. The nerve conduction of proximal segment is more statistically significant than that of distal segment.

9.A study on Purification of Water Works by Multi-filter Bed Method with Aeration: Application of Biological Oxidation by Aerobic Microorganism.

5 bed sand filter, applying biological oxidation, was designed and studied on the treatment of water works. Never using any coagulant agent (drugs), which may cause water pollution in pre-treatment of head water, the author attempt a high rate filtration by the microorganism (nitrofication bacteria) end plant which populate in multi layer sand beds. The result are as follows ; In order to evaluate the oxygen effect on filtration, oxygen was injected in aeration tank attached to each filter tank while filtration, and NH3 was tested as a representative ingredient. It was found out that the aeration method was more effective, with over 33% of NH3 removal capacity, than the anerobic and this 5 bed filter showed double removal capacity of NH3 by comparing with conventional sand bed (2 stage bed). According to the examination of two kind of head water, pre-treated with coagulant agent and activated carben, the filtration capacity was affected by the polluted condition of head water, resulting that lower value of pollution and slower velocity of filtration showed more efficiency of NH3 removal. In this experiment NH3 content tested in treated water had a fairly good correlation with others.
