2.A Study on the Angle Between the Abdominal Aorta and the Superior Mesenteric Artery by 3D Image Reconstruction.

SMAS(Superior Mesenteric Artery Syndrome) is a disease caused by a chronic obstruction of the duodenum(transverse portion), which is hardly detectable. However, it is known that when the superior mesenteric artery and abdominal aorta form a narrow angle, that the transverse portion of the duodenum is pressed down between the superior mesenteric artery and the abdominal aorta, and that this can lead to obstruction of the duodenum. Measuring this angle is a complicated job using conventional angiography, and results often turns out to be inaccurate. In addition, no attempt has been made to determine the value of this angle in Koreans. In this study, we conducted abdominal CT angiography using MIP(maximum intensity projection) on patients with no clinical evidence of SMAS in order to determine the angle at which the superior mesenteric artery branches from the abdominal aorta by using PC based software(Rapidia ver. 1.2) for the image reconstruction. Accordingly, we found that the mean angle between the abdominal aorta and the superior mesenteric artery was 50.05+/-15.87 degrees on average, and that the angle in men(53.64+/-16.57 degrees) is higher than in women(46.46+/-14.98 degrees). We hope that the angles determined by our study will serve as an important indicator for detecting SMAS.

5.Comparison of anticancer drug efficacy using the short-term microplate culture and MTT dye reduction assay.

Individual tumors, even those of the same histologic type, show varying sensitivity to specific cytotoxic agent. Therefore, sensitivity testing assume an increasingly important as an orientational aid in planning chemotherapy. In the past decade there have been many attempts to develop a chemosensitivity test that would predict the clinical effectiveness of various chemostherapeutic agents against human neoplasms. In the United States National Institue's anticancer drug screening program, a colorimetric assey based on the ability of live cells to reduce a tetrazolium-base compound(MTT) to a blue formazan product was used. There has been an increase in reports of a chemosensitivity assay that use tetrazolium dyes and current the assay is in use in our country. The efficacy of several anticancer drug (vincristine sulfate, Etoposide, doxorubicin CDDP) were evaluated using the in vitro chemosensitivity of MTT assay with two cancer cell lines (MOLT-4, KHOS/NP). The follows obtained. 1) CI50 on MOLT-4 are 0.55ng/ml and 0.81ng/ml for vincristine and oncovin, 142.30ng/ml and 78.75ng/ml for lastet and vepesid, and 19.75ng/ml, 20.43ng/ml and 8.66ng/ml for ADR, ADM and adriblastin, respectively. 2) CI50 on KHOS/NP are 691.35ng/ml, 873.73ng/ml, 1,205.22ng/ml, 768.81ng/ml and 672.19ng/ml for cisplan, cisplatin, cispatin, platinol and cisplatin G, and 9.22ng/ml, 11.46ng/ml and 4.28ng/ml for ADR, ADM and adriblastin, respectively. In conclusion the MTT dye reduction assay to anticancer drug sensitivity using short-term microplate culture might serve as a reliable tool for the selection of effective chemotherapeutic agents in patients with cancers.

6.c-erbB-2 Oncoprotein Expression in Ductal Carcinoma in situ and Paget's Disease of the Breast.

A clinico-pathologic study with an immunohistochemical examination for c-erbB-2 expression in 54 cases of ductal carcinoma in situ and 16 cases of Paget's disease of the breast was performed. c-erbB-2 oncoprotein overexpression was observed in 45% (24/54) and 88% (14/16) of ductal carcinoma in situ and Paget's disease, respectively. The overexpression of c-erbB-2 oncoprotein was significantly correlated with the nuclear grade of tumors and inversely with the status of the estrogen receptor. c-erbB-2 was positive in 4 out of 5 patients with metastasis to axillary lymph nodes and 3 out of 4 patients who died of the disease. Prognostic significance of c-erbB-2 oncoprotein in ductal carcinoma in situ was highly suggested. The expression of c-erbB-2 oncoprotein in Paget's disease was well correlated with coexisting infiltrating or in situ ductal carcinoma. The high positive rate of c-erbB-2 oncoprotein in ductal carcinoma with Paget's disease could be understood with a recent hypothesis that c-erbB-2 oncoprotein is involved in promotion of cell motility and the spread of carcinoma cells.

Ankylosing spondylitis is a chronic inflammatory disease of unknown etiology that affects spine, sacroiliac joint, and the large joints of the extremities, and lead to fibrous or bony ankylosis and deformity. The rigid ankylosed spine of ankylosing spondylitis is prone to fracture by minimal trauma, and the resulting fractures likely to be unstable and damage to spinal cord or cauda equina. The authors report a case of paraplegia in ankylosing spondylitis after open reduction of the femoral trohanteric fracture.

9.A Case of Recurrent Spontaneous Abortion Successfully Delivered by Using Preimplantation Genetic Diagnosis.

OBJECTIVE: To report the successful delivery in a patient of recurrent spontaneous abortion caused by chromosomal abnormality. MATERIAL AND METHOD: Case report. RESULTS: Twelve oocytes were obtained by in vitro fertilization. Of eleven oocytes fertilized, two embryos turned out to be normal by using fluorescent in situ hybridization on blastomere biopsy. The patient succeeded in pregnancy and the result of amniocentesis was found to be normal. She delivered the healthy female baby by cesarean section. CONCLUSIONS: The successful delivery is possible in recurrent spontaneous abortion related with reciprocal translocation by using preimplantation genetic diagnosis.

10.Brodie's Abscess Caused by Salmonella enteritica serovar Senftenberg in a Healthy Child.

Salmonella enteritica serovar Senftenberg is a rare pathogen in osteomyelitis, and is not usually encountered in healthy individuals. Here we report radiological and microbiological findings of a case of Brodie's abscess caused by S. enteritica serovar Senftenberg in the left tibia of an otherwise healthy child.
