1.A Clinical Analysis of Bone and Joint Tuberculosis Complicated with Mixed Infection

200 cases of bone and joint tuberculosis complicated with mixed infection treated at the Department of Orthopedic Surgery, National Medical Center through the year of 1958 to 1971 were analyzed and reported in this paper. 1. All the mixed infections complicated in bone and joint tuberculosis occurred secondarily as a complication of the formation of fistula in this series. 2. The incidence of the fistula formation as a complication of bone and joint tuberculosis was considerably higher as 117 out of 200 cases in the age group of 10–30 years compared with other age groups. Accordingly the incidence of mixed infection was higher in this age group than other groups in same proportion in this seiies. 3. Mixed infection occurred relatively earlier after the fistuIar was formed. 46 out of 200cases (23%) had mixed infection one month after the fistula was formed. 4. Majority of the fistula developed spontaneously as 129 out of 200 cases (64.5%) 5. 135 cases of fistula developed during the treatment with PAS. and INH, and 11 cases developed fistula during triple antituberculosis therapy with PAS. INH. and Streptomycin. 6. Majority of the causative microorganism was Staphylococcus aureus as 210 out 360 cultures. (58.3%). 7. The result of the treatment was evaluated, and the result of radical operation gave remarkably higher incidence of healing, 110 out of 130 cases, than that of medical treatment, 33 healing out of 64 cases.

3.Outcomes of Treatment for Squamous Cell Carcinoma Originating as a Marjolin's Ulcer.

PURPOSE: The purpose of this study was to analyze the results of treatment and prognosis of Marjolin's ulcer compared with primary squamous cell carcinoma. MATERIALS AND METHODS: Fourteen patients treated for Marjolin's ulcer were analyzed. Twenty patients with primary squamous cell carcinoma treated during the same time period was the control group. Mean age was 61.2 years. There were 24 males and 10 females. The locations, TNM stages, histological grades, recurrence, metastasis, and survival rate were analyzed and compared between two groups. RESULTS: The mean follow-up period was 54.8 months (range, 12-168 months). Local recurrences were found in 6 cases, 5 ones in Marjolin's ulcer patients, and one case in primary squamous cell carcinoma patients. The mean time interval between the initial presentation and occurrence of local recurrences was 9 months (range, 2-20 months). There were 6 metastases. 2 (14.3%) metastases were found in Marjolin's ulcer patients, and 4 (20.0%) metastases in primary squamous cell carcinoma patients. Total events (metastasis or local recurrence) were found in 10 pateients, 6 of them in Marjolin's ulcer group, and the remaining four in primary group. 5-year disease-free survival rate was 64.3% in Marjolin's ulcer group and 95.0% in primary squamous cell carcinoma group. CONCLUSION: Squamous cell carcinomas originating as Marjolin's ulcers revealed higher recurrence rate and lower survival rate despite of aggressive treatment. Therefore, new treatment modalities should be developed for improving outcomes.

5.Induction of Dendritic Cell and Cytokine Gene Expression by In situ Delivery of Flt3 Ligand Plasmid.

Dendritic cell (DC)s are protessional antigen presenting cells and they have been used for antitumor immunotherapy or cell vaccines. However therapy using DC is restricted because the number of DC available from tissue is very low. Flt3 ligand (FL) has been known as a hematopoietic growth factor that increases proliferation of hematopoietic stem cells and progenitor cells, and recently it showed inducibility of dendritic cell (DC)s and signiticant antitumor effects in vivo. Thus FL will be frequently used for DC induction and antitumor immunotherapy in future. Here we constructed FL plasmid and studied its in vivo effect. FL plasmids were made by cloning of partial FL cDNA into pcDNA3 plasmid, and gene expression and protein producibility of FL plasmid were confirmed in Renca cells transfected with FL plasmid. Mice were injected with FL plasmid (100ug/mouse) three times and 20 days later mouse spleens were harvested for staining and RT-PCR. There were lots of blastogenic cells in the spleen of mice treated with FL plasmid. FL plasmid also induced DEC205, IL-12 and GM-CSF gene expression in mouse splenocyte. All these data suggest FL plasmid may be used for induction of DC and antitumor therapy as DNA adjuvant.

9.Tratment of the Nail Bed Avulsion Injury with Split-thickness Nail Bed Graft

The primary goal in the treatment of the nail bed avulsion injury is complete regrowing of the nail plate without any deformity. There are many procedures for treatment of the nail bed avulsion injury as a split-thickness skin graft, full-thickness nail bed graft, reverse dermal graft or split-thickness nail bed graft. From February, 1984 to June, 1985 at In-Chon Gill Hospital, 13 cases of nail bed avulsion injury were treated with split-thickness nail bed grafts. The intact residual part of the injured nail bed or the toe nail bed was served as a donor site. The results were as followings; 1. The incidence of nail bed avulsion injury of all hand injuries was 9.7%. 2. The main cause of injury was the industrial accident and more prevalent in the left hand. 3. The good result was obtained in the split-thickness nail bed graft for the nail bed avulsion injury, but in nail root portion, the poor result was noticed. 4. The average time for normal nail appearance is six months.
